Quality Care<br />

You are encouraged/expected to seek help from your treatment team and/or the Patient Advocate<br />

if you have concerns. You will be given understandable in<strong>for</strong>mation about the complaint process.<br />

You may express your concerns verbally or in writing, without fear of retaliation.<br />

Patient Advocate<br />

The <strong>Lebanon</strong> <strong>VA</strong> <strong>Medical</strong> <strong>Center</strong> seeks to treat our patients with fairness and concern, recognizing<br />

their needs and satisfying them to the fullest extent possible. The Patient Advocacy Office was<br />

established <strong>for</strong> you, the patient. If you have any questions or problems that have not been answered<br />

to your satisfaction or if you have a special need, you may call the Patient Advocate <strong>for</strong> further<br />

assistance at 1-717-272-6621, ext. 5918 or 1-800-409-8771, ext. 5918.<br />

Confidentiality<br />

The health care and financial in<strong>for</strong>mation included in your <strong>VA</strong> records will remain confidential.<br />

Release of In<strong>for</strong>mation (ROI)<br />

You may request a copy of your medical in<strong>for</strong>mation <strong>for</strong> yourself or someone else that you choose.<br />

You must sign a consent <strong>for</strong>m that will allow <strong>VA</strong> to release this in<strong>for</strong>mation.<br />

The ROI office can also assist you with the following:<br />

• Obtaining your DD214<br />

• Having a non-<strong>VA</strong> <strong>for</strong>m completed by your provider<br />

• Requesting documents through the Freedom of In<strong>for</strong>mation Act (FOIA)<br />
