Welcome to the <strong>Lebanon</strong> <strong>VA</strong> <strong>Medical</strong> <strong>Center</strong>. We are committed to providing you with quality<br />
health care. We want to meet your health care needs by helping you to avoid illness through<br />
prevention and wellness programs and services.<br />
This handbook covers many of the health care services available to you. It explains what you<br />
are eligible <strong>for</strong>, how to apply <strong>for</strong> and use your health care benefits, and what to do if you have a<br />
problem. This in<strong>for</strong>mation is current as of the time of publication, but may change as a result of<br />
law and/or regulation. <strong>Lebanon</strong> <strong>VA</strong> staff will do their best to keep you in<strong>for</strong>med of these changes.<br />
<strong>Veterans</strong> Identification Card<br />
Your <strong>VA</strong> card is your passport to <strong>VA</strong> health services. Carry it in your wallet and bring it each time<br />
you come to the clinic.<br />

8771, Ext. 6000) so that we can make all of the<br />
necessary changes to update your electronic<br />
medical record if you have changed your:<br />
• Address<br />
• Telephone number<br />
• <strong>Healthcare</strong> insurance<br />
• Any other demographic in<strong>for</strong>mation<br />
If you have not been seen by your Primary Care Provider within 12 months, you will be contacted<br />
asking if you wish to continue your medical care with us.<br />
