• High School Graduates ONLY - please submit your equivalent U.S. high school diploma and/or transcripts. High School transcripts must clearly indicate<br />
that you have successfully completed your secondary education.<br />
• College or University Students ONLY - please submit <strong>of</strong>ficial transcript.<br />
4. Official Vocational Transcripts - if applicable<br />
Applicants must complete the International Student Previous Education/Training History Form IN-501A, if studies are related to the cosmetology, barber, or<br />
spa industry. All documents must be translated by a certified English translator. The academic transcript must include the number <strong>of</strong> hours attempted and<br />
completed, services completed, and the name <strong>of</strong> the exams with the grades obtained. If submitting this form it will take an additional two (2) weeks from<br />
the time <strong>of</strong> the transcripts are received.<br />
5. Official Bank Letter <strong>of</strong> Financial Resources (for you and/or sponsor)<br />
Documentary evidence <strong>of</strong> means <strong>of</strong> financial support must be provided to the Institution for admission purposes and issuance <strong>of</strong> the Certificate <strong>of</strong><br />
Eligibility ( I-20). The <strong>of</strong>ficial bank letter (refer to the sample provided) must:<br />
• Be written on bank letterhead and addressed to <strong>Beauty</strong> <strong>Schools</strong> <strong>of</strong> <strong>America</strong> ® to the address <strong>of</strong> the campus you plan to attend.<br />
• Indicate that you and/or your sponsor have a bank account with sufficient funds to cover your expenses while you study at <strong>Beauty</strong> <strong>Schools</strong> <strong>of</strong> <strong>America</strong> ® .<br />
• Reflect the required amount for the program chosen (please refer to the Tuition and Fees for International Students).<br />
• Be certified authentic (notarized) by a bank <strong>of</strong>ficial, in English and with the equivalent <strong>of</strong> U.S. dollars (USD) noted in the letter.<br />
• Be dated no more than three (3) months from the program start date.<br />
• Include the holder account number.<br />
• Reference the name <strong>of</strong> the applicant applying for the Certificate <strong>of</strong> Eligibility (I-20); otherwise a Sponsor Statement Form (IN-503) will be required.<br />
• Clearly state the sponsor’s intent to use the funds for applicants’ expenses at <strong>Beauty</strong> <strong>Schools</strong> <strong>of</strong> <strong>America</strong> ® .<br />
• Ensure the signature and address <strong>of</strong> the sponsor is notarized with an <strong>of</strong>ficial seal.<br />
• Have acceptable financial sources, which are savings, checking, money market accounts and certificates <strong>of</strong> deposit.<br />
6. Living Expense Letter<br />
Applicants must provide a letter including the estimated monthly living expenses. Please use the Living Expense Letter Form (IN-502) which will include<br />
the following:<br />
• Housing (rent) and Utilities<br />
• Transportation<br />
• Food<br />
• Miscellaneous<br />
The average cost <strong>of</strong> living expenses (rent, food, transportation and any miscellaneous costs) is estimated at $2,000.00 per month. Applicants with dependents<br />
need to add an additional $500.00 per month, per dependent. To calculate the total amount required on your bank letter, please refer to the Estimated Costs <strong>of</strong><br />
Living Expenses (Sample) Information Sheet. Remember that it is very important to demonstrate that you have adequate funds to study in the United States, as<br />
well as sufficient funds to pay for living expenses and medical, if needed.<br />
7. U.S. Department <strong>of</strong> Homeland Security (DHS) Regulations BSA Form IN-504<br />
Applicants must complete the DHS Regulations BSA Form (IN-504). The following are important regulations from DHS. All International M-1 applicants must:<br />
• Prove that the intention to enter the United States is temporary, attend an approved vocational program and engage in a full course <strong>of</strong> study.<br />
• Not work without proper authorization according to the United States Citizenship and Immigration Services (USCIS) regulations.<br />
• Not transfer schools or work without the authorization <strong>of</strong> USCIS.<br />
• A passport valid for travel to the United States and with a validity date <strong>of</strong> at least six months beyond the applicant’s intended period <strong>of</strong> stay in the<br />
United States.<br />
• Keep a valid Certificate <strong>of</strong> Eligibility (I-20) for the duration <strong>of</strong> the program <strong>of</strong> study.<br />
• Notify USCIS within 10 days <strong>of</strong> an address change.<br />
• Maintain Satisfactory Academic Progress (SAP). Please refer to the Attendance and Satisfactory Academic Policy section <strong>of</strong> this catalog.<br />
• Adhere to all DHS regulations, otherwise he/she may be reported as being out <strong>of</strong> status. For additional information refer to<br />

8. Statement <strong>of</strong> Non-Social Security Form IN-505<br />
Applicants must complete the Statement <strong>of</strong> Non-Social Security Form (IN-505) and send it along with the aforementioned documents. This form notifies<br />
the school as to whether you have ever been issued a social security number in the United States. Please refer to section “Arrival to the U.S. (Item 7)” for<br />
more information regarding this subject.<br />
9. Additional Documentation<br />
Additional suggested documentation is strongly recommended that you submit along with your documentation for the interview at the Embassy. Please<br />
refer to the Additional Suggested Documentation form for detail information. Please find the embassy nearest you by either calling your country directory<br />
or searching on the World Wide Web.<br />
