<strong>of</strong> particular <strong>in</strong>terest. Other symptoms <strong>of</strong> acute systemic toxicity occur, but<br />

with 2,4-D exposure has been extensively described. It has an early onset,<br />

causes prolonged disability <strong>of</strong> variable degree, and recovery has been <strong>in</strong>complete<br />

<strong>in</strong> many cases. Electromyography <strong>in</strong> some patients has demonstrated denervation,<br />

and some studies have detected decreases <strong>in</strong> nerve conduction velocities. One<br />

autopsy study demonstrated a demyel<strong>in</strong>ation process with<strong>in</strong> the bra<strong>in</strong> <strong>of</strong> a 76-<br />

year-old male who committed suicide by <strong>in</strong>gest<strong>in</strong>g 2,4-D <strong>in</strong> kerosene.<br />

(2) 2.4.5-T/TCDD<br />

The human effects <strong>of</strong> 2,4,5-T are difficult to evaluate s<strong>in</strong>ce the<br />

chemical is contam<strong>in</strong>ated with TCDD <strong>in</strong> the manufactur<strong>in</strong>g process. The effects <strong>of</strong><br />

TCDD itself have been determ<strong>in</strong>ed from studies <strong>of</strong> trichlorophenol workers, and<br />

from laboratory workers us<strong>in</strong>g TCDD. Symptom/sign complexes attributable to<br />

exposure to 2,4,5-T and TCDD are listed <strong>in</strong> Tables A-2 and A-3 <strong>of</strong> the Appendix.<br />

Chloracne usually beg<strong>in</strong>s <strong>in</strong> the zygomatic/temporal region and is <strong>of</strong>ten found on<br />

and beh<strong>in</strong>d the p<strong>in</strong>na <strong>of</strong> the ear. This is an oily acne-like sk<strong>in</strong> condition characterized<br />

by comedones and <strong>in</strong>clusion cysts which may result <strong>in</strong> extensive scarr<strong>in</strong>g.<br />

In severe cases follow<strong>in</strong>g heavy exposure, spread <strong>of</strong> lesions to the<br />

throat, back and <strong>in</strong>gu<strong>in</strong>al areas has been noted. This sk<strong>in</strong> condition is frequently<br />

preceded by erythema and blepharoconjunctivitis. Active lesions usually<br />

disappear with<strong>in</strong> two years, but have been found 30 years after exposure. Porphyria<br />

cutanea tarda and hypothyroidism have also been l<strong>in</strong>ked to 2,4,5-T/TCDD<br />

exposure. Other symptoms such as asthenia, liver and re'nal dysfunction, neuropathy,<br />

and gastro<strong>in</strong>test<strong>in</strong>al and cardiac disturbances are probably due to mechanisms<br />

similar or identical to those o f 2,4-D. With the exception <strong>of</strong> chloracne<br />

and possible disorders <strong>of</strong> porphyr<strong>in</strong> metabolism, all <strong>of</strong> these effects have been<br />

acute or subacute <strong>in</strong> nature.<br />

Numerous <strong>in</strong>stances <strong>of</strong> alleged disease due to 2,4-D/2,4,5-T exposure<br />

have been the subject <strong>of</strong> heavy media attention, particularly an episode <strong>of</strong><br />

alleged 2,4,5-T exposure <strong>in</strong> Globe, Arizona, <strong>in</strong> 1969. Despite extensive scientific<br />

review and analysis with negative f<strong>in</strong>d<strong>in</strong>gs, the Globe <strong>in</strong>cident cont<strong>in</strong>ues to<br />

be cited <strong>in</strong> news media presentations. An <strong>in</strong>cident <strong>in</strong> Missouri <strong>in</strong> 1971 <strong>in</strong> which<br />

six children, two adults and numerous animals were exposed to TCDD-contam<strong>in</strong>ated<br />

oil is frequently described as well. Many <strong>of</strong> the animals died and the humans<br />

developed chloracne and other ocute toxic effects; however, all humans were<br />

healthy after five years <strong>of</strong> follow-up study. A f<strong>in</strong>al prospective assessment <strong>of</strong><br />

fertility, teratogenesis and carc<strong>in</strong>ogenesis, <strong>in</strong> these <strong>in</strong>dividuals will probably<br />

be made <strong>in</strong> the future.<br />

F. Vete r a n Concerns<br />

The Veterans Adm<strong>in</strong>istration provided the USAF with data on 46,771<br />

patients participat<strong>in</strong>g <strong>in</strong> the Herbicide Registry. Numerous media presentations<br />

emphasiz<strong>in</strong>g both military and civilian herbicide exposures have described a<br />

remarkably wide spectrum <strong>of</strong> health effects be<strong>in</strong>g claimed by the veterans. Three<br />

compensation claims have been allowed for service-connected acneiform sk<strong>in</strong><br />

lesions (but not chloracne), 16 claims for other sk<strong>in</strong> conditions, and an additional<br />

three claims for other diagnoses. A direct causal relationship between a<br />

disease and a specific exposure is not necessary to receive compensation.<br />
