This Addendum is an integral part of the contract documents and shall be treated as such.<br />

Contractor shall acknowledge receipt of this Addendum on the Bid Form.<br />

Please note that Exhibit K to the contract has been revised as noted in the attached Exhibit K<br />

sample. Also, the Owner furnished Out to Bid Default Documents have been revised to include the<br />

below list of documents which are also included as part of this <strong>addendum</strong>.<br />

Procedures To Prepare Change Requests For Submittal And Approval<br />

1. Change Requests are to be submitted via the on-line Change Request tool provided by Evoco.<br />

2. The General Contractor must review all Change Request documents submitted by Subcontractors<br />

and “Sub-subcontractors,” including suppliers and third party vendors, including a copy of the notice,<br />

cost, quantities, overall accuracy, fairness and scope of work. Include all support documentation and<br />

calculations.<br />

3. Change Requests must be submitted per the Construction Agreement between Owner and<br />

Contractor Article 3.3, Special Conditions for Walmart Stores, Inc. Construction Paragraph 16.A, and<br />

Articles 1, 3 and 16 in the General Conditions.<br />

4. For all projects with a base bid contract of $1 Million or greater, the crew labor rate package will be<br />

required. See the attached Mixed Crew Hourly Labor Rate Workbook for instructions.<br />

5. All Change Requests must be properly itemized and supported by sufficient substantiating data to<br />

permit evaluation. Specifically, this requires the change proposal estimates to be submitted in the<br />

format specified in the document titled “Change Request Instructions”.<br />

6. Per Article 3.1.3 of the General Conditions, Walmart may request additional information to permit<br />

complete evaluation of a submitted Change Request. After a 14 day period, Walmart will make a<br />

determination of validity and final pricing based on the merits of all information provided.<br />

7. The Change Request will be routed and approved electronically.<br />

On – Line Change Request Instructions<br />

Change Requests are to be submitted in such a way as to allow a reasonable person to<br />

understand the validity and value of the requested changed amount based on the attached<br />

documentation alone. As such, all Change Requests must be properly itemized and supported by<br />

sufficient substantiating data to permit evaluation.<br />

The Change Request submission must clearly show how the described scope of work exceeds<br />

the original bid scope. The Change Requests should be valued as if the Contractor was placing a<br />

bid for the scope of work included in the Change Request.<br />

Specifically, the Change Requests must include the following information:<br />

1. Online Change Request Form: The scope of work must be summarized in the space<br />

provided. Detail what event caused the change in the scope of work, such as a CCD, or<br />

discovery of any unforeseen condition. Reference all applicable documents and drawings,<br />

and include all relevant dates. Include the description of the work. Providing the CCD<br />

number will not be sufficient without a description.<br />

2. On-Line Change Request Attachments: The on-line Evoco Change Request tool allows the<br />

Contractor to upload documentation to support the Change Request. In order to facilitate the<br />

quickest processing of your Change Request, upload all documents in the same format in<br />

which it was provided (e.g. An EXCEL sheet as an EXCEL sheet). Submitting documents in<br />

a different format delays the review significantly. The supporting documentation is as follows:<br />

a. Change Request Scope Narrative Description: This letter is a separate, detailed narrative<br />

to fully explain all work elements in the Change Request. It must summarize the cost of<br />

the change and give a thorough explanation of the scope of work. It can be in paragraph<br />

form or in bullet points. This letter must clearly define how the scope of work to be<br />

performed exceeds the scope of work as defined in the original bid and any previous<br />

Change Requests. This letter must be a stand alone document in that it explains the<br />

basis of the Change Request without relying on the reader having any prior knowledge of<br />

the project.<br />

b. Preliminary Change Request Budget (PCR): This document is housed in the Evoco<br />

Change Request tool and does not need to be submitted separately.<br />

c. Authorization to Proceed: This document gives authorization to proceed with the scope of<br />

work detailed in the PCR. It is housed in the Evoco Change Request tool and does not<br />

need to be submitted separately. The General Contractor is required to provide any<br />

amendments made to the original ATP via email or other forms under this item.<br />

d. RFI and RFI response: (If applicable). All unforeseen condition Change Requests must<br />

include an RFI response pertaining to the treatment of the unforeseen condition that<br />

exceeds the original scope of work.<br />

e. CCD/SCD Narrative (If applicable). Attach the narrative, excluding any attachments. It is<br />

good practice to provide a date stamp on the cover sheet for reference of date when the<br />

General Contractor received.<br />

f. Original Schedule: This schedule was approved by the Construction Manager at the<br />

beginning of the project and must be included with any time related Change Request or<br />

claim.<br />

g. Revised/Updated/Acceleration Schedule: This schedule was approved by the<br />

Construction Manager at the time that the PCR was submitted and must be included with<br />

any time related Change Request or claim.<br />

h. Change Request Cost Summary: This form is a part of the Change Request Submittal<br />

Workbook. This three page Excel spreadsheet summarizes all the costs by<br />

Subcontractors, Sub-Subcontractors and Suppliers, which are submitted under this<br />

change. It must be attached as an Excel document, in the format in which it was<br />

provided.<br />

i. General Contractor’s and Subcontractor’s Work Summary: This form is a part of the<br />

Change Request Submittal Workbook. The General Contractor will require the<br />

Subcontractor to complete this form and attach back-up. The General Contractor must<br />

complete this form for all self-performed work. No cost itemized in the summary will be<br />

allowed without full and complete back-up. The form must be attached as an Excel<br />

document, in the format in which it was provided. Provide reference to plans, details,<br />

narrative, etc to better breakout and help indicate portions of work.<br />

i. Must summarize the cost of the change and give a thorough, detailed explanation of<br />

the scope of work. It can be in paragraph form or in bullet points.<br />

ii.<br />

This includes current market pricing on material, quotes on supplier letterhead for<br />

major items are required, labor hours for each item of work performed, approved<br />

crew labor rates or labor rates with complete back-up, and owned equipment costs<br />

based on the Equipment Watch Rental Rate Blue Book. No lump sum pricing is<br />

allowed on this form, and all materials shall be itemized to show UOM, quantity, and<br />

cost per unit.<br />

Price work by drawings and details. This is especially necessary for electricians and<br />

HVAC, plumbing and sprinkler Subcontractors and will make it easier for Walmart to<br />

relate the cost to a specific piece of work. Highlight the drawings to show the specific<br />

change.<br />

j. Unforeseen Conditions or Time and Material Form: Include only for an unforeseen<br />

condition or time and material Change Request. This report identifies the daily resources<br />

used to remedy an unforeseen condition. All actual costs in this unforeseen conditions or<br />

Time and Materials form must be substantiated with regards to material cost, labor cost,<br />

and equipment cost. All unit rates or costs agreed to in the Authorization to Proceed letter<br />

must be adhered to. This report rolls up to the Subcontractor’s Work Summary. The form<br />

must be attached as an Excel document, in the format in which it was provided.<br />

k. Labor Rate Sheets:<br />

i. For all projects with a base bid contract of $1 Million or greater, the crew labor rate<br />

package will be required. See the attached Mixed Crew Hourly Labor Rate<br />

Workbook for instructions.<br />

If the crew labor rate package was reviewed by Walmart and approved at the<br />

beginning of the project, provide only the crew rate summary sheet signed by the<br />

General Contractor and Estimating Manager. This form is a part of the Change<br />

Request Submittal Workbook.<br />

If the crew labor rate package was not used, the Subcontractor must complete the<br />

Walmart labor rate form for all trades associated with a change. This form is a part of<br />

the Change Request Submittal Workbook. Additionally, the Subcontractor must<br />

provide back-up that proves the rate to be correct. For union Contractors, the union<br />

labor agreement must be attached and for non-union Contractors, a certified payroll<br />

must be attached. Additional documents verifying state insurance rates (SUTA) and<br />

worker’s compensation insurance rates must also be attached. This must be<br />

submitted with each Change Request even if it has been previously submitted with<br />

another Change Request on the same project. No per diems, tools, trucks, or travel<br />

costs are allowed in the hourly wage calculation. Refer to Mixed Crew Hourly Labor<br />

Rate Workbook Requirements tab or Remodel Projects Labor Rate Workbook<br />

Requirements tab for additional explanation on acceptable benefits and burdens.<br />

l. Material Quotes: All major materials that are included in this Change Request must be<br />

accompanied by a material quote on the supplier's letterhead.<br />

m. Walmart Consultant Reports for unforeseen conditions must be collected by the General<br />

Contractor and submitted with the Change Request. The required reports will be outlined<br />

in the Authorization to Proceed letter. Place these in the section with the applicable<br />

Subcontractor.<br />

n. Superintendent Daily Reports must be submitted for any unforeseen condition, or as<br />

agreed to in the Authorization to Proceed letter. Organize the reports so that they are<br />

included in the appropriate Subcontractor’s documentation.<br />

o. Equipment Watch Rental Rate Blue Book spreadsheet form gives the format showing the<br />

acceptable calculations for specific pieces of equipment. These must be submitted for<br />

any Contractor owned piece of equipment anticipated for use in the Change Request. Do<br />

not include labor (operators) in equipment rates. For leased equipment, include rental<br />

agreement, invoice, or quote.<br />

p. Drawings and Sketches: Any drawings, illustrations, photographs, or edited copies of the<br />

plans or specifications that may assist in explaining the basis of the change should be<br />

included. Assume that the reviewer of the Change Request knows nothing about the<br />

project or the Change Request; thus, the General Contractor and Subcontractor must<br />

submit drawings and sketches (as necessary) that highlight the specific changes. Make<br />

notes on the drawings and sketches that easily identify the changed work.<br />

q. Color Photographs are necessary if a drawing or sketch does not adequately convey the<br />

scope or complexity of the change. Photos should include the time and date as part of<br />

the image. Identify the location of all photos.<br />

r. Note: The General Contractor and Subcontractor are encouraged to submit all of their<br />

calculations in a format that is easy to follow. The easier it is to follow how the estimate<br />

was developed, the easier it will be to approve the Change Request.<br />

It is the General Contractor’s responsibility to review all aspects of the Change Request (to<br />

include the Sub-contractors’ documentation) for compliance with the above noted guidelines. The<br />

GC shall confirm scope of work, labor hours and rates, material quantities and rates, and<br />

equipment used and rates.<br />

Walmart Construction and Estimating reserves the right to request any reasonable, additional<br />

information, which will assist in the review of each change.<br />

Change Requests must be treated and priced in the same manner as a bid – that is, they must be<br />

an estimated cost based on the work that is added or deleted from the original scope of work. All<br />

estimates must be justified as detailed above. As an exception, the Construction Manager may<br />

stipulate that a change be priced on a unit cost basis or on a time and material basis. This will be<br />

indicated on the Authorization to Proceed document. Unit costs submitted at bid time shall include<br />

all overhead and profit, fees, and additional costs associated with scope of work and Contractor,<br />

Subcontractors and Sub-subcontractors are not entitled to any other overhead, profit or fees or<br />

additional costs. Owner’s acceptance of a unit price including, without limitation, material prices,<br />

labor rates and equipment rates, shall not be deemed an acceptance of that unit price on other<br />

Change Requests and Owner reserves its right to reject that unit price on other Change<br />

Requests.<br />

The bid form, that the General Contractor used to submit his bid, states the overhead and profit<br />

allowed on Change Requests. Contractor and Subcontractor mark-up on the final charge costs<br />

associated with a Change Request are only permitted as specifically set forth in Section 3.1.8 of<br />

the Construction Contract. Negotiated projects may differ per the agreement.<br />

A Change Request may be initiated by one of the following conditions:<br />

1. One of Walmart’s consultants issues a Construction Change Directive (CCD) or a Site<br />

Change Directive (SCD) that alters the scope of work of the contract, or<br />

2. The Contractor identifies an unforeseen condition and verifies its validity with the Walmart<br />

Construction Manager (CM) (The Contractor must generate and RFI for direction on how<br />

to proceed) or,<br />

3. The local authorities including building departments, inspectors, fire departments, etc,<br />

identify a code or other issue and requests work to be performed which was not included<br />

in the original scope of work, (The Contractor must generate an RFI detailing the results of<br />

the inspection, and receive a response from the engineer confirming that additional work<br />

must be performed) or,<br />

4. Other as indicated in writing from and approved by the Wal-Mart Construction Manager.<br />

After the discovery of a condition or receipt of instruction that change the original scope of work,<br />

the Contractor has 7 days to submit a Preliminary Change Request budget (PCR). The PCR is a<br />

best, quick, rough order of magnitude estimate of the probable cost of the change; it is not<br />

binding on either party. The primary purpose of the PCR is to alert Walmart that a Change<br />

Request is coming and that there is a cost associated with the change.<br />

Following receipt of the PCR, Walmart will either issue the Contractor a form giving Authorization<br />

to proceed with the scope of work outlined in the PCR, or will issue the Contractor a form giving<br />

Notice Not to Proceed with the scope of work outlined in the PCR. The Authorization to Proceed<br />

form only validates that Walmart wishes the Contractor to perform the scope of work outlined in<br />

the PCR. It does not authorize any change in the contract sum. The Authorization to Proceed<br />

(ATP) must be signed by the Construction Manager and/or Construction Director before work is<br />

started. Upon receipt of the ATP verify what scope was authorized and how the Change Request<br />

is to be priced.<br />

Regardless of when the Contractor receives the Authorization to Proceed, the Contractor has an<br />

additional 30 days from the date the PCR is signed to submit a fully documented Change<br />

Request to Wal-Mart for evaluation. If the change request is not submitted within the 30 day<br />

period, the on-line Change Request tool will close out that PCR. The Contractor may request an<br />

extension be granted by the Construction Manager. Please review the “Construction Agreement<br />

between Owner and Contractor” Article 3, Clause 3.3 for further information on this.<br />

Computation of Mixed Crew Hourly Labor Rate<br />

1 Identify the exact trade(s) to perform the task. Crew Labor Rate requirement applies to all General Contractors and<br />

Subcontractors performing labor on the project. At this time, only submit rates for trades listed in this workbook.<br />

2 PROJECT INFORMATION TAB:<br />

Fill in the name of the person who will manage the labor rate process for your company.<br />

Enter the subcontractors you will use for each trade. Select the backup type you will provide for each trade - payroll history<br />

or union agreement.<br />

3 TRADE SPECIFIC TABS:<br />

Under Classification, input the crew you are utilizing for base contract work.<br />

The Number of Craft and Basic Hourly Rate are to be completed based on base contract work crew. Enter No. of Craft<br />

quantity only, and productive hours field will populate.<br />

Foreman No. of Craft stays at .33 with Productive Hours at 1.<br />

The Base Crew Rate or Total Crew Rate per Hour will automatically calculate once the information is entered correctly.<br />

Fringe Benefits shall be entered as Percentage (%) or as Hourly Rates according to the backup. The Total Fringe Benefits<br />

will be automatically calculated. Refer to Fringe Benefits on the Requirements tab for more details.<br />

FICA and FUTA will automatically adjust to the calculated Base Crew Rate. Please do not adjust. SUTA and Worker’s<br />

Compensation must be entered as a percentage for each rate. The rates are required to be supported with documentation.<br />

Refer to Fixed Benefits on the Requirements tab for more details.<br />

Once the Base Crew Rate, Fringe Benefits, and Fixed Burdens are all entered correctly, a Total Hourly Rate will be<br />

calculated automatically to include Straight time, Time & One Half, and Double time rates. Total Hourly Rate is the rate to<br />

be charged per man hour spent on change order work.<br />

Each Sub-Contractor will sign and date their own crew labor rate sheet. General Contractor will submit the crew labor rate<br />

sheet along with all backup documentation to Walmart for final approval no later than two weeks after award.<br />

Crew Labor Rate Package Submittal and Review Process:<br />

1 It is the General Contractor's responsibility to validate the completeness and accuracy of the submittal before forwarding to<br />

Walmart. Once all portions are correctly documented, a Principal for General Contractor will submit via email all Crew Labor<br />

Rates and backup documentation to Walmart for final approval no later than two weeks after award.<br />

Walmart Estimating will review the initial submittal and give feedback to the General Contractor on the CSI tab of the Crew<br />

Labor Rate workbook. This will be the only feedback Walmart Estimating will issue, and after the second submittal, Walmart<br />

Estimating will finalize crew labor rates based on all available valid backup documentation submitted to date. Once all crew<br />

labor rates are correctly documented, a Walmart Regional Estimating Manager will review the entire package, and if no<br />

further questions/concerns, Walmart will submit a Final Accepted Rates Sheet to the General Contractor for approval. Final<br />

Accepted Rates Sheet will then be approved by Walmart Regional Estimating Manager to be utilized for change order<br />

processing.<br />

2 If a change order is submitted and it includes labor performed by the subcontractor for which the crew labor rate has not been<br />

approved, Walmart Regional Estimating Manager and Construction Manager may at their sole discretion either assign a fair<br />

and reasonable rate based on historical data available or request crew labor rate workbook to be submitted by the General<br />

Contractor at that time. It is the General Contractor's responsibility to ensure crew labor rates are submitted and approved for<br />

all trades performing work for the project no later than two weeks after award.<br />

3 Providing proper backup does not guarantee acceptance. Walmart Estimating reserves the right to reject rates not deemed<br />

fair and reasonable for region of work.<br />

The labor rates set forth herein shall not be subject to any increase for any reason whatsoever.<br />

Computation of Mixed Crew Hourly Labor Rate<br />

Base Rate:<br />

A base rate is truly a base rate. No extra costs, such as small tools, travel, vehicles, vacation, liability, per diem, etc. will be<br />

allowed to be added into this rate. The base rate will be accepted based on supporting documentationfrom Union agreements<br />

or certified payroll reports only. Davis Bacon rates and state prevailing wage documentation is not allowed.<br />

Fringe Benefits:<br />

The US Department of Labor form can be utilized to submit a certified payroll report. In addition submit copies of month's<br />

worth of payroll stubs and company's payroll report as backup to certified payroll. Payroll is required to be certified by a third<br />

party such as a CPA. If a company belongs to a Union(s), a letter(s) of good standing from the Union shall be required, along<br />

with the Union agreement indicating labor rates and benefits. The Union letter should state that its agreement governs the<br />

Walmart project where the subcontractor is employed. Please ensure that the submitted Union agreement supports the<br />

foreman's rate. Contractor shall support each rate and benefits listed on a crew labor rate sheet with a Union agreement.<br />

Fringe Benefits are personal benefits provided to the employee by the employer. Such benefits include: Health Insurance,<br />

Dental, Vision, Pension, Training, and 401K. All benefits must be verified through proper documentation such as local Union<br />

agreements, certified payroll reports, and invoices from insurance companies, investment companies, etc. Identify<br />

deductions for benefits on payroll reports and pay stubs.<br />

For non-union companies, only the Employer's side of fringe benefits will be included in crew labor rate calculation.Vacation,<br />

Small tools, Travel, Vehicles, Uniform, Holiday Pay, Bonus pay, Sick pay, Bereavement pay, Per Diem, cell phones,Overhead<br />

& Profit, etc. will not be included as a part of the crew labor rate sheet as these benefits have already been funded for the<br />

base contract work. These costs would be entertained if the project encounters extended contract schedule.<br />

Fixed Burdens:<br />

The fixed burdens are:<br />

FICA (Federal Insurance Contribution Act), which consists of 6.2% Social Security FICA Tax and 1.45% Medicare FICA tax,<br />

will be assessed at 7.65% and shall not be changed.<br />

FUTA (Federal Unemployment Tax) will be assessed at 0.8% and shall not be changed.<br />

The SUTA (State Unemployment Tax) will require verification by submittal of documentation from the state’s tax commission.<br />

Please ensure the appropriate SUTA rate is being submitted based on the location of work and that state's tax<br />

requirements.<br />

Worker’s Compensation will require verification of documentation from the company's insurance agency. Worker's<br />

Compensation shall be submitted in the form of Comprehensive Schedule of Premium including payroll, manual rates,<br />

premiums, and premium adjustments. (see Workers Comp Example tab)<br />

Selecting a rate belonging to one classification is not acceptable.<br />

Do not include General Liability or Excess Liability Insurance as it is funded as a part of the Change Order fee.<br />

Time & One Half and Double time:<br />

Time & One Half and Double time shall only be paid with written approval by authorized personnel from Wal-Mart. Fringe<br />

Benefits will remain constant through the labor rate sheet. They should not be differentfrom Straight time through Double<br />

time. The Fixed Benefits shall be adjusted as required, from Straight time through Double time.<br />

Crew Rate:<br />

A Mixed Crew Hourly Rate will include each trades crew make-up and size that is utilized for all Base Contract Work.<br />

Note in the attached labor rate file all labor rates by trade will include a recommended make-up of a crew.<br />

Only crew members doing 100% of the base contract work are allowed on the crew labor rate sheet.<br />

Composite rates do not apply to superintendents or any other level of supervision.<br />

Foreman time will be automatically adjusted to 1/3 of an hour under the No. Craft section on the Mixed Crew Hourly Labor<br />

Rate Sheet. Follow Union agreement requirements for Apprentices and crew ratios to be used on a project.<br />

1. Identify the exact piece of equipment. Include the make, model, and year.<br />

2. The calculation shall be based on the listed Equipment Watch Blue Book (EWBB) monthly rental rate for each<br />

piece of equipment.<br />

3. The monthly rate will be adjusted based on age and location.<br />

4. Adjustment factors are found in tables in the EWBB.<br />

5. The adjusted monthly rate shall be divided by 176 hours per month to derive the hourly rental rate.<br />

6. The operating rate per hour (including fuel, oil, maintenance, filters, etc.) is the last column in the Blue Book in<br />

the same row as the monthly rental rate.<br />

7. The hourly rental rate for each piece of equipment includes the hourly rate (#5) and the operation hourly rate (#6).<br />

GC and Sub-Summary Instructions<br />

1. All Change Requests need to be completed per “Change Request Procedures to Prepare for Submittal" and "Change Request Instructions."<br />

2. All material costs for major items must have a quote or invoice attached.<br />

3. All labor rates must be accompanied by Labor Wage Breakdown sheet and / or union agreement as applicable.<br />

4. All Owned Equipment rates must be calculated by using the Equipment Worksheet or the Equipment Watch website.<br />

5. All Rented Equipment rates shall utilize the monthly rental rate supported by the specific and reasonable vendor quote / purchase order and attached to the change request.<br />

6. Calculation of the Unit per Manhour, manhours divided by quantities, example #8 wire, 1,000 LF, 8 manhours, 8/1000 = .008 MHRS/LF.<br />

7. Overhead and Profit of % includes all items identified in the General Conditions Article 3.1.2, "fee includes all overhead, profit, and operational items such as taxes, bonds, and insurance..."<br />

Unforeseen Conditions Changes to the Work<br />

the costs for approval.<br />

Unforeseen changes shall be submitted based on the following criteria and the attached form.<br />

Change Daily Report<br />

The Contractor shall maintain a separate change daily report that includes but is not limited to the following<br />

data:<br />

1. Description of Work – Changed / unforeseen work performed for the specific day.<br />

2. Employees – Identify by name on the daily report.<br />

3. Classification – Identify each employees craft responsibility (laborer, operator, etc.).<br />

4. Time – Identify each employee hour worked, straight & overtime, all overtime work to be authorized by<br />

Walmart Construction Manager prior to execution of the work. Attach authorization to the daily time &<br />

material report.<br />

5. Hourly Rate – Rate to be based on actual Contractor booked costs (payroll) – Use Labor Rate Sheet as<br />

provided in the Walmart Change Request package.<br />

6. Equipment – Identify each piece of equipment used for this Work (to include model no., manufacturer,<br />

number of piece, size) – Use Blue Book Equipment Rate Form as provide in the Walmart Change Request<br />

package.<br />

7. Equipment Hours – Submit hours actually worked for each piece of equipment.<br />

8. Equipment Rates – Submit rate for each piece of equipment as previously agreed and attach the equipment<br />

rate sheet. If not previously agreed to submit the rental rate and Purchase Order for the specific date of<br />

the Work. If Owned equipment submits the rate based on the Contract requirements per the Blue Book and<br />

forms in the Walmart Change Request package.<br />

9. Materials – All materials expended for Unforeseen Condition Work shall be quantified and supported with a<br />

Purchase Order, Invoice, quote and receiving reports to the job site.<br />

10. Other field quantities to include excavation, backfill, reinforcement, forming, piping, etc. shall be identified<br />

on the change daily report.<br />

11. All materials drawn from Contractor’s stock, Contractor shall obtain prior approval from the Walmart<br />

Construction Manager for pricing. In no instance shall the price of Contractor’s material exceed the<br />

prevailing price that Walmart could obtain for comparable quantities and types of material from commercial<br />

suppliers.<br />

12. Approvals – The Contractor’s Superintendent overseeing the Work and the Project Manager shall sign off<br />

on all Daily Change Reports. The General Contractor's review will verify the craft by name, man-hours,<br />

equipment model and hours and material for each daily change report.<br />

13. Weekly Summary – A weekly summary of the change shall be submitted to Walmart Construction Manager.<br />

This report will include daily man-hours, daily labor costs, daily equipment hours, daily equipment costs,<br />

daily material costs and total daily and to date quantities and costs.<br />

14. Drawings / Sketches – At the completion of the work, drawings and sketches shall be submitted as part of<br />

the claim that details the location and quantity of work which was performed.<br />
