rendering its service<br />

since 1994 and right<br />

now is reaching out to<br />

over 440 such persons<br />

through 18 centres – 9<br />

special schools and 9<br />

post school units located<br />

in and around Chennai.<br />

Maithree provides an array<br />

of services encompassing<br />

Summer at its peak, kidney stone disease also in their peak.<br />

Throughout the world the<br />

incidences of kidney stone<br />

disease also increasing,<br />

mainly due climate and<br />

the dietary changes of the<br />

individuals.one of the main<br />

reason for kidney stones<br />

are not drinking enough<br />

water and eating more junk<br />

foods, majority of the time<br />

we may not be able to find<br />

the aetiology of the stone<br />

disease, less frequently<br />

we may find the calcium,<br />

Uric acid, and oxalate<br />

levels in our body will have<br />

an abnormal levels which<br />

may predisposes for stone<br />

disease.<br />

Symptoms and signs<br />

The patients with stone in<br />

the urinary tract present<br />

with severe pain in the<br />

flanks and they will be<br />

colicky in nature. The<br />

symptoms are due to<br />

the size or the site of<br />

the stone in the urinary<br />

tract. The sizes may vary<br />

from 2-3mm to 5-6cm<br />

and sometime the whole<br />

kidney may be replaced by<br />

the stones called stag horn<br />

calculus. The stones in the<br />

kidney can cause urinary<br />

tract infection,blood in the<br />

urine ,if it is not detected<br />

or treated may end up in<br />

kidney failure. How to detect<br />

the entire life span of the<br />

child/person with special<br />

needs.<br />

Maithree celebrated its<br />

21st Annual day on Friday<br />

the 6th of March, 2015<br />

from 4:00 p.m onwards<br />

at Kamarajar Arangam,<br />

Teynampet.<br />

“Varnajalam” being the<br />

theme of the Annual Day,<br />

a colourful presentation of<br />

Usually very common<br />

in middle age group<br />

colours by the 9 special<br />

schools of Maithree. over<br />

300 children with special<br />

skills of weaving, stitching,<br />

jewellery making and there<br />

was also a sale of the<br />

urine .With the classic<br />

symptoms and doing tests<br />

like ultrasonogram and an<br />

simple X ray majority (60-<br />

80%)of the stones in the<br />

products made by them.<br />

the X-ray may not show<br />

the stones like Uric acid,<br />

cysteine.We may need CT<br />

scan to detect the stones.<br />

With good CT scan almost<br />

we can detect 99% of<br />

stones in the urinary tract.<br />

The advantage of CT<br />

scan is that it can give the<br />

exact location, size and<br />

density (Hounsfield unit)<br />

of the stone which is the<br />

main factors to decide the<br />

treatment options.<br />

How to treat?<br />

Once diagnosed as stones<br />

in the urinary tract, it has<br />

to be treated otherwise<br />

end up in renal damage by<br />

blocking the urinary tract<br />

or the source of recurrent<br />

urinary tract infection.<br />

As mentioned earlier the<br />

treatment options depends<br />

on the factors like site,<br />

size, density ,presence of<br />

obstruction and infection.<br />

In majority of times smaller<br />

stones of 4-6 mm will pass<br />

out from the urinary tract<br />

by nature with simple<br />

conservative treatments<br />

with medication(70-80%)<br />

Even the smaller stones<br />

will not come out and<br />

produce severe pain and<br />

infection needs surgical<br />

treatment.<br />

Treatment options are<br />

1 observation<br />

2 Extracorporeal<br />

lithotripsy(ESWL)<br />

3 Intracorporeal lithotripsy<br />

(Rigid and Flexible<br />

Ureteroscopy with<br />

pneumatic or LASER<br />

energy)<br />

4 Percutaneous lithotripsy<br />

(PCNL)<br />

5 Laparoscopic stone<br />

removal<br />

6 Open surgery<br />

Traditionally open surgery<br />

(pyelolithotomy or<br />

ureterolithotomy)has been<br />

considered a treatment<br />

for kidney stones (>3cm)<br />

,with the advancement in<br />

technology in endoscopy<br />

and the lithotripsy technics<br />

the open surgery has<br />

been obsolete from the<br />

treatment options and<br />

almost all stones and<br />

any sizes can be treated<br />

endoscopically .since<br />

the stone disease are<br />

recurrent disease we<br />

cannot do open surgery<br />

every time,it has to<br />

be treated endoscopy<br />

methods without damage<br />

to the kidneys<br />

How to prevent?<br />

Stones in the kidneys are<br />

likely to recur and it needs<br />

regular attention otherwise<br />

end up in permanent<br />

kidney damage. If the<br />

person is producing<br />

stones again and again<br />

needs metabolic work up<br />

to analyse the blood for<br />

calcium, phosphorous,<br />

Uric acid and oxalates,<br />

citrates level in our body<br />

and if any abnormality<br />

detected can be corrected<br />

with medications.<br />

Another important factor<br />

in stone formation is not<br />

drinking enough water<br />

and taking too much junk<br />

food. By taking adequate<br />

water (3-4 litre) per day<br />

and avoiding or reducing<br />

certain foods contain more<br />

calcium and oxalates ,<br />

regular visit to the urologist<br />

the recurrence of the<br />

stones in the kidney can<br />

be prevented there by<br />

reducing the damage to<br />

the kidney and prevent<br />

kidney failures.<br />

Removable Partial Denture<br />

A denture is a removable<br />

replacement for missing<br />

teeth and surrounding<br />

tissues. Two types of<br />

dentures are available<br />

complete and partial<br />

dentures. Complete<br />

dentures are used when<br />

all the teeth are missing,<br />

while partial dentures are<br />

used when some natural<br />

teeth remain.<br />

A removable partial<br />

denture usually consists<br />

of replacement teeth<br />

attached to a pink or<br />

gum-colored plastic<br />

base, which is sometimes<br />

connected by metal<br />

framework that holds the<br />

denture in place in the<br />

mouth. Not only does<br />

a partial denture fill in<br />

the spaces created by<br />

missing teeth, it prevents<br />

other teeth from changing<br />

position.<br />

New dentures may feel<br />

a little odd or loose for<br />

a few weeks until the<br />

muscles of the cheeks<br />

and tongue learn to keep<br />

them in place and you get<br />

comfortable inserting and<br />

removing them. Also, it<br />

is not unusual for minor<br />

irritation or soreness to<br />

occur and for saliva flow<br />

to increase when you first<br />

start wearing dentures,<br />

but these problems will<br />

diminish as the mouth<br />

adjusts.<br />

Eating with new<br />

dentures will take a little<br />

practice and may be<br />

uncomfortable for some<br />

wearers for a few weeks.<br />

To get used to the new<br />

denture, start with soft<br />

foods cut into small<br />

pieces. Chew slowly<br />

using both sides of your<br />

mouth. As you get used to<br />

new dentures, add other<br />

foods until you return to a<br />

normal diet. Be cautious<br />

with hot or hard foods and<br />

sharp-edged bones or<br />

shells. And, avoid foods<br />

that are extremely sticky<br />

or hard. You should also<br />

avoid chewing gum while<br />

you wear the denture.<br />

Also, don't use toothpicks<br />

while wearing dentures.<br />
