2. NV Roche SA, by means of the Spontaneous Adverse Event fax cover sheet and
Adverse Event form provided in the CU file.
This form should be sent by fax to NV Roche SA Drug Safety Brussels (fax:
02/525.84.66).
Details on requirements and timelines of safety reporting to NV Roche SA can be
found in the section 'Physician's commitments (§ 6, 7, 8)', which is part of the
Physician Request Form.
Oseltamivir IV is restricted to medical prescription on a compassionate use basis.
