Welcome to the 2015 IBI Annual Forum. We are pleased to host the Forum again this year at the Fairmont Hotel, one of<br />

San Francisco’s grandest. Enjoy our beautiful city. We have a variety of exciting new sessions, resources and events for our<br />

attendees, so I’d like to highlight a few items not to be missed.<br />

This year’s Forum will focus on a critical issue for employers and their supplier partners: given the rapidly changing healthcare<br />

environment, how can employers create and maintain a healthy, high-performing, at-work and productive workforce? We are<br />

especially fortunate to have such a wide variety of speakers joining us this year to address these critical issues. We’ve emphasized<br />

practical solutions that can be taken back to your offices and implemented, while also exploring innovative strategies that are<br />

pushing the envelope and helping the field evolve.<br />

In 2015, IBI is expanding ways to reach and engage with our members and we want the Forum to serve as a launching pad for our<br />

additional member initiatives. In addition to the Forum, members can look forward to seeing regional conferences, webinars with<br />

leading professionals, diverse case studies, industry-focused benchmarking groups, and research driven by member needs.<br />

Today, there is more uncertainty than ever about how employers will manage workforce<br />

health and productivity. IBI’s Board of Directors met last summer to discuss the volatility<br />

of the health and productivity landscape and the diversity of employers’ options. This<br />

preconference workshop will focus on three aspects of the Board’s discussion: (1) forces<br />

influencing how employers manage workforce health and productivity, (2) implications<br />

for how employers with a more “traditional” approach to healthcare will proceed and (3)<br />

implications for how employers with a more “consumer-centric” approach will proceed.<br />

Former IBI Board Chairman Chris McSwain will moderate the panel; panel members will<br />

include Kathy McAlpine from Kraft; Gary Earl from Anthem-Wellpoint; Tammy Green from<br />

Providence Health Systems and Brad Johnson from Sedgwick CMS. The audience is invited<br />

private exchanges may play in supporting workforce health management efforts. Each of the<br />

panelists has a unique perspective, and together will provide an insightful look at the role of<br />

With the implementation of Affordable Care Act reforms and the release of ADAAA final<br />

regulations adopted by the EEOC in the past two years, the absence management field<br />

has changed greatly. This session will present findings from the 2015 Guardian Absence<br />

Management Activity Index℠ and Study, a Spring Consulting Group commissioned survey,<br />

as well as an accompanying metric developed in 2012 to monitor changes in employer<br />

engagement and absence management-related activities. By providing insight into the current<br />

absence management landscape, this presentation will explain the series of steps employers<br />

of all sizes can take to get started on their absence management journey and build an<br />

effective, long-standing program over time.<br />

Workshop B: Using Consumer Activation to Better Understand Consumer Behavior<br />

and Outcomes<br />

• Craig Kurtzweil, Senior Vice President, Analytic Innovations, UnitedHealth Group<br />

Recognizing the impact consumer behavior has on health care and related costs, Unum and<br />

UnitedHealth Group took aim at consumer health activation with their Consumer Activation<br />

Index (CAI), a tool used for measuring decision making and short-term disability exposure.<br />

In a study of 17 customers with 270k members, the CAI was used to determine if individuals<br />

make an optimal clinical, financial or service choice when faced with a health care decision.<br />

The CAI navigates individuals through 51 decision points to complete its assessment. By<br />

better understanding consumer behavior, members can then seek to modify behavior to<br />

achieve better health. The discussion will highlight how CAI works and its results in relation<br />

to short term disability and productivity.<br />

Workshop C: Breaking Through: Communicating for Engagement and Behavior Change<br />

In this session, a leading employer will discuss how they are using targeted communication<br />

techniques and campaigns to drive engagement with corporate leadership as well as<br />

embrace the impact of workforce health on productivity as well as provide insights about<br />

why employee and family engagement in health and benefits programs is integral for<br />

success. This session will focus on utilizing communications to influence the subcultures of<br />

Workshop D: Effective Employee Engagement and Support for Obesity Management<br />

Obesity is a top health and productivity concern as well as a complex and challenging<br />

condition that most employers have found difficult to address effectively. Employers that<br />

want to move their employee populations to healthier weights are looking for proven weight<br />

management strategies that go beyond the standard approaches to employee engagement<br />

and support. This session will present key findings of an employee engagement research<br />

study focused on obesity management, along with a case study of a leading employer—<br />

Johnson & Johnson—that has been a pioneer in implementing new integrated, value-based<br />

solutions, measuring their results, and deploying them across their organization.<br />

Workshop E: Fit for Work in a Physically Demanding Environment: A Comprehensive<br />

Approach to Wellness at Goldcorp’s Wharf Mine<br />

In the fall of 2011, Wharf Mine partnered with 3:1 Corporate HPM Solutions to launch a new<br />

wellness program for its employees. The project aimed for a comprehensive approach to<br />

wellness by integrating traditional occupational safety, prevention, and health protection<br />

efforts with health promotion. Key components of the program included individual health<br />

assessments, health and safety education, nutrition, core strengthening, stress and sleep<br />

management, wearable devices for tracking progress, and follow-up sessions. Speakers<br />

will discuss how these components led to measurable successes in heightened employee<br />

efficiency, reductions in serious health conditions, improved health risk status, and reduced<br />

worker’s compensation costs related to slips, trips, falls and overexertion.<br />

Workshop A: Telemedicine: Innovative Approaches to Health Care<br />

explore a telemedicine care option. Telemedicine offers hard-to-reach patients an alternate<br />

channel of care from any location without the delay of scheduling or taking time off from<br />

work. Speakers will discuss the value based plan design of Penske’s telemedicine program as<br />

well as its results due to productivity savings and employee satisfaction. This presentation<br />

will address innovative remote approaches to health care as well as the key elements of an<br />

effective deployment strategy that achieves maximal adoption and impact.<br />

Workshop B: FMLA & ADA: Avoiding Pitfalls and Reducing Absences<br />

Increasing overlap between FMLA and ADA has presented employers with the question<br />

of how to address new regulations while maintaining compliance and managing absence.<br />

ComPsych will present a case study illustrating how employers can reduce absences by as<br />

much as 30 percent by focusing on important changes in FMLA law, interpreting what the<br />

EEOC’s “rigorous enforcement” of ADA laws means to employers, proactively identifying<br />

FMLA absences and other applicable leaves that may require ADA analysis, integrating<br />

FMLA and ADA processes to improve efficiency and reduce risk, and instituting new<br />

corporate policies and procedures that will help reduce overall absence.<br />

Workshop C: Work-Focused Interventions for Value-Based Care around Depression<br />

Depression is a major issue affecting millions of Americans, inhibiting their ability to lead<br />

productive lives at home and at work. OptumHealth partnered with Tufts Medical Center<br />

and 19 national employers to create a work-focused intervention aimed at improving the<br />

productivity of middle- aged and older adults with depression and work limitations. The<br />

program utilized telephone-based counseling with care coordination, cognitive behavioral<br />

therapy strategy development, and work coaching/modification. Speakers will share results<br />

including a 44% work productivity improvement and discuss increasing efforts to provide<br />

more value-based care with a work-focused intervention strategy.<br />

Workshop B: Partnership and Prevention: Utilizing Different Health Networks to Prevent<br />

Approximately 50,000 adults die each year in the US from vaccine-preventable diseases<br />

such as influenza and pneumonia. To confront this problem, Pfizer collaborated with the<br />

University of South Florida (USFHealth) and Tampa General Hospital to review the use of<br />

vaccines as a method of improving workforce health. The first step in this collaboration<br />

was to model the direct and indirect benefit costs associated with Community Acquired<br />

Pneumonia (CAP). This session will reveal the economic burden of CAP and the advantages<br />

of partnerships to deter CAP incidents. Presenters will discuss the health system’s<br />

direct and indirect benefit costs associated with CAP and annual cost estimates for the<br />

organization’s health, sick leave/absence, short-term disability (STD), and group life<br />

insurance benefit programs. Speakers will then explore how the expansion of employee<br />

coverage for vaccines in partnership with pharmacy networks worked to improve the overall<br />

health and productivity of affected workforces.<br />

Workshop C: Health and Productivity Management Guided by a Workforce’s<br />

Professional, Social, and Emotional Wellbeing<br />

Well brand and developed custom campaigns for each ministry to maximize employee<br />

engagement and participation in the Healthways Well-Being Assessment. This survey<br />

measured more than just the physical health of employees, it also identified the impact that<br />

work environment and social, emotional and community well-being had on their overall<br />

well-being as well. Through robust data analytics, Healthways identified key priorities for<br />

improvement and this session will discuss the program’s approach, implementation, and<br />

navigate the landscape of the new ADAAA regulations and EEOC enforced employer<br />

will discuss available data and the results of a fully outsourced ADA administration program<br />

12 months post-implementation. They will focus on how this program aimed to ensure<br />

compliance, improve productivity, and reduce overall exposure.<br />

Workshop E: Tying Educational Outcomes to Good Health<br />

their medical home. Speakers from MNPS, CHS, and Vanderbilt will discuss the program’s<br />

integration of medical, pharmacy, dental, vision, and worker’s compensation services, as well<br />

as their plans to expand the scope of care to include health coaching and behavioral health.<br />

Presenting three years of results, speakers will discuss how they’re using data on student<br />

claims, the organization reduced the total number of annual days lost by 34%. Presenters<br />

will discuss how interventions made by on-site EAP counselors and behavioral health<br />

specialists helped employees find professional mental health services and achieve a timely<br />

means of accepting leadership as an employer and healthcare provider to enhance the health<br />

and performance of its workforce. In launching several innovative initiatives, UCLA Health<br />

demonstrated material results: reducing healthcare service utilization through the Primary<br />

Care Innovation Model (PCIM), increasing employee engagement through a Value-based<br />

Care, Person Journey Model for employees in the UCLA health plan (in partnership with U.S.<br />

Preventive Medicine), and decreasing workers’ comp costs through its WorkStrong program.<br />

Speakers will discuss the health system redesign as well as integration, capacity growth, preprimary<br />

care, telemedicine, and home services.<br />

Workshop D: Low Back Pain: A Major Employer Uses Guidelines to Manage Quality,<br />

Costs and Outcomes (continued)<br />

Up to 84% of the general population has been found to report low back pain (LBP) at some<br />

point during their lifetime. This session will focus on a recent study by a major self-insured<br />

employer to cultivate evidence on issues key to a successful program focusing on illness/<br />

injury prevention, reduction of workers’ compensation claims, and disability and healthcare<br />

cost savings. This study longitudinally tracked episodes of three types of ICD-9 diagnosis<br />

code-identified back problems from 2001-2009 and found that care consistent with 10 of<br />

11 guidelines was associated with lower total costs. Speakers will review how approaches<br />

marked by more intensive resource use and lower guideline consistency were linked to poorer<br />

LBP quality care, worse patient/employee outcomes, and greater direct and indirect costs.<br />

Implications for employer involvement in guideline cost effectiveness will be discussed.<br />

Workshop E: Redesigning to Better Manage Absence and Disability<br />

Fraser Health, a Canadian organization with 24,000 employees, worked with StaffRelay to create<br />

a program to better approach and manage absence and disability. Through an extensive redesign<br />

process, a comprehensive electronic employee health record, an Enhanced Disability Management<br />

Program (EDMP), and an automated attendance reporting solution, Fraser Health has produced a<br />

return on investment of $28 million in decreased LTD and other disability costs. This session will<br />

detail positive impacts on absence, sick leave, STD and WC, as well as higher participation rates.<br />

remain among employers for whom this is still new and unchartered territory. In an attempt<br />

to discover and define the underlying patterns framing successful working environments<br />

for those with disabilities, The Hartford has partnered with the research team at the<br />

nationally recognized Beach Center on Disability at The University of Kansas. This session<br />

discusses how to impact the future workforce based on recent independent research by The<br />

University of Kansas. The session will focus on motivational factors affecting performance,<br />

development, and retention. Speakers will discuss their successful program results related<br />

to recruiting, developing, and retaining abled and disabled employees. Speakers will share<br />

their findings on self-determination as a predictor of success in the workplace as well as<br />

Dr. Tyler Amell is a well-known and respected<br />

management consultant specializing in<br />

evidence-informed workplace health. One of Dr. Amell’s<br />

specialty areas is human capital solutions, particularly the<br />

subject of workplace health and productivity. Currently,<br />

Dr. Amell serves as Chief Executive Officer of StaffRelay, a<br />

company focused on helping organizations maximize the<br />

potential of their workforce through strategic absence and<br />

disability management. Dr. Amell has significant experience<br />

in, and has conducted extensive university research on<br />

topics such as: work-related musculoskeletal injuries and<br />

illnesses, return to work, disability management, and mental<br />

health in the workplace. Dr. Amell has authored over 20<br />

peer-reviewed publications, as well as book chapters. He has<br />

also given seminars and presentations at over 75 national<br />

and international conferences in areas of his expertise. He<br />

is a former instructor at the Universities of Calgary, Alberta,<br />

Ottawa and Queen’s University in physical therapy, anatomy,<br />

kinesiology, and occupational health. Dr. Amell currently<br />

serves on the Executive Board of Directors of the Canadian<br />

Association for Research on Work and Health (CARWH),<br />

and has worked extensively with the Institute for Work and<br />

Health (IWH). He is passionate about optimizing technology<br />

to improve integrated workplace health and benefit support<br />

systems and processes.<br />

Francisca Azocar, Ph.D.<br />

Behavioral Health Sciences<br />

Dr. Azocar is Vice President of Research<br />

and Evaluation of Behavioral Health Sciences at Optum. Dr.<br />

Azocar has been at Optum for 15 years, and she specializes<br />

in creating research partnerships between academia,<br />

employers and health plans. She has conducted research on<br />

the dissemination of practice guidelines for the treatment of<br />

depression within managed behavioral healthcare, particularly<br />

on workplace depression, telephonic care management<br />

and outreach to depressed chronically-ill medical patients,<br />

development of patient-reported outcome measures and<br />

impact of provider feedback reports, development of<br />

evidence-based psychotherapy measures, and impact of<br />

clinical interventions on service utilization and costs. In<br />

collaboration with academic partners, Dr. Azocar has been<br />

involved in several National Institute of Health-funded studies<br />

including studies on depression in the workplace, autism<br />

spectrum disorders, and the impact of the Mental Health and<br />

Addiction Equity act on access, utilization and costs. Prior<br />

to joining Optum Dr. Azocar was a faculty member in the<br />

Department of Psychiatry at the University of California, San<br />

Francisco. There her research and clinical work focused on the<br />

treatment of depression in disadvantaged general medicine<br />

and ob/gyn patients, particularly with Spanish-speaking<br />

young women. Dr. Azocar received her doctorate from UC<br />

Berkeley and a National Institute of Mental Health Clinical<br />

Services Research Post-Doctoral Fellowship at the UCSF. She<br />

is a licensed clinical psychologist in the state of California.<br />

Larry Bakke, Training Coordinator at Goldcorp<br />

Wharf Mine, has over 32 years’ experience<br />

in the mining industry in Wyoming, Colorado, North Dakota,<br />

and South Dakota. His experience includes operating Heavy<br />

Equipment, Open Pit Operations Supervisor, Gold Recovery<br />

Specialist, and currently Training and Recruitment Specialist.<br />

In Coordinating Wharf Mine’s Training, Larry incorporates<br />

health and wellness as an integral part of the MSHA Safety<br />

Training for the well being and safety of the miners. This<br />

training includes instruction in the company’s 10 Golden<br />

Rules, one of which is being “Fit For Work”. He has been<br />

trained to be a facilitator for Goldcorp’s leadership and safety<br />

programs including the Supervisor Leadership Development<br />

Program, Dare To Grow, DELTA Safety, Coach the Coach,<br />

Safety Leadership Training, Goldcorp’s StepUP Program, as<br />

Colleen Broersma is a proud mother<br />

Dr. Bunn recently retired as Vice President<br />

originally International Harvester and he now serves as<br />

an appointment as Professor, Clinical Preventive Medicine at<br />

Northwestern University School of Medicine. He received the<br />

Institute for Health and Productivity Management President’s<br />

Award in 2010, DMAA: The Care Continuum Alliance<br />

Leadership Award for Outstanding Employer Program in<br />

2010, the Institute for Health and Productivity Management<br />

Corporate Award in 2001 and the ACOEM Knudsen Award<br />

in 2002. He has served on numerous advisory committees<br />

a fellow, board member and former officer of the American<br />

dozens of professional awards. Jan’s clients have included<br />

Management, Disease Management and Safety programs.<br />

Previously, he was Corporate Medical Director for JPMorgan<br />

Chase and its legacy banks for more than 25 years. He is a<br />

member to several Boards and previously was on the Board<br />

of the National Business Group on Health and was Chairman<br />

of the Board for the Midwest Business Group on Health. He<br />

has been the recipient of several industry awards including<br />

the Health Achievement Award from the American College<br />

of Occupational and Environmental Medicine (ACOEM),<br />

the Jonas Salk Health Leadership Award from the March of<br />

Dimes, the Innovation Award from Mental Health America and<br />

Innovation in Health and Productivity Award from National<br />

Business Group on Health (NBGH). In 2009, he received the<br />

first Global Health Leadership Award from NBGH/ACOEM.<br />

The following year, he was honored with the Meritorious<br />

Service award and the President’s Award from the American<br />

College of Occupational and Environmental Medicine. Since<br />

1978, Burton has been an associate professor of clinical<br />

medicine at Northwestern University Medical School. He’s also<br />

an adjunct professor of Environmental & Occupational Sciences<br />

Joseph G. Cacchione, M.D., F.A.C.C., is an<br />

interventional cardiologist at the Cleveland Clinic; Robert and<br />

Suzanne Tomsich Department of Cardiovascular Medicine;<br />

Arnold and Sydell Miller Heart and Vascular Institute. In<br />

addition to his clinical responsibilities, Dr. Cacchione is the<br />

Chairman of Operations and Strategy, Cleveland Clinic Heart<br />

and Vascular Institute. As a leader in health care quality<br />

initiatives, he heads up Network Growth Strategy for the<br />

Cleveland Clinic Foundation. Previous to his tenure at the<br />

Cleveland Clinic, he was in a private practice for over nineteen<br />

years and served as executive vice president and chief<br />

quality officer at Saint Vincent Health System in Erie, PA. Dr.<br />

Cacchione is currently a member of the American College of<br />

Cardiology’s (ACC) Board of Trustees. He chairs the ACC’s<br />

Partners in Quality Subcommittee and is an active member<br />

of several committees, including the Payment Reform Task<br />

force. He serves on the AQA Steering Committee, co-chairs<br />

the AQA Reporting Workgroup and has participated in the<br />

Brooking Institution’s High-Value Health Care Project Data Set<br />

Workgroup. He also served as President, American College<br />

of Cardiology Pennsylvania Chapter, Western. Recently, he<br />

was named to the National Priorities Partnership Advisory<br />

Council. Dr. Cacchione’s interests include payment reform,<br />

Denise Callari, M.D.<br />

Dr. Callari joined UnitedHealthcare in 2005<br />

as Vice President of Health Care Strategies; as<br />

a member of a team of physicians working with large national<br />

employers. This team focuses on providing a portfolio of<br />

clinical services, including consultation, account management,<br />

care management program design and implementation and<br />

sales support. In addition, the team acts as a liaison to clinical<br />

leaders. Dr. Callari is also involved with strategic clinical product<br />

development for UnitedHealthcare National Accounts. She is<br />

involved in leading the UnitedHealthcare effort in developing<br />

disease management programs aimed at improving the health of<br />

our members, and reducing an employer’s health trend. She is also<br />

focused on enhancing the value proposition of UnitedHealthcare<br />

to consumers and employers. Formerly, Dr. Callari practiced<br />

internal medicine at the Buffalo Medical Group, one of the largest<br />

multi-specialty groups in New York State. Over her 13 year tenure,<br />

she was the Associate Medical Director for the Group, served as<br />

the Medical Director of their Urgent Care Center and implemented<br />

an on-site Diabetes Disease Management Clinic. Dr. Callari<br />

received her medical degree from the State University of New York<br />

at Buffalo where she trained in internal medicine.<br />

Chris Chan is an Innovation Imagineer at<br />

LABS has him focused on developing strategies and products<br />

Jennifer is the Executive Director,<br />

designed to improve workforce health & productivity<br />

through sustained behavior change. Jennifer joined Kaiser<br />

Permanente in September 2010. Prior to that, she was<br />

National Vice President, Integrated Health Improvement at<br />

Health Net for 5 years and was Regional Director of Clinical<br />

Research for CIGNA for 5 years. She has held roles at UCLA’s<br />

Neuropsychiatric Institute and Department of Psychology.<br />

She has her Ph.D. in Clinical Psychology (SUNY-Stony Brook)<br />

Robert Cleary is Senior Human Resources<br />

Inc. in Concord, Mass. Cleary has managed the benefits<br />

programs for Welch’s for the past eight years. During that<br />

time Welch’s has been able to maintain core benefit programs<br />

while implementing various outsourced relationships with<br />

key vendors for retirement programs, disability programs,<br />

and FMLA and ADA compliance. Bert worked for actuarial<br />

consulting firms for half of his career where he helped<br />

various large multi-national companies manage their benefits<br />

program design, cost, and administration with a particular<br />

interest in retirement programs including pension plans and<br />

retiree medical programs. Over the years as a consultant,<br />

Mr. Cleary gained a wide breadth of financial, accounting,<br />

and program design experience working with a variety of<br />

different organizations and plans. Bert has been able to<br />

apply those skills in designing and implementing program<br />

efficiencies as a Human Resources team member for two<br />

well-established organizations over the past 14 years. Mr.<br />

Cleary has a Master of Business Administration and Master<br />

of Healthcare Administration from the Warrington College<br />

of Business Administration at the University of Florida. Bert<br />

also has a Bachelor of Science degree in Mathematics with<br />

a concentration in Applied Sciences from the University of<br />

Massachusetts Amherst.<br />

Alyssa Coleman<br />

Alyssa oversees various benefit programs for<br />

NextEra Energy, Inc., a leading clean energy<br />

company with more than 15,000 employees in 28 states<br />

and internationally in Canada and Spain. She joined NextEra<br />

Energy in 1998, serving in various other positions including<br />

accounting and financial reporting until beginning her benefits<br />

consultant role. For the past 9 years, Alyssa has served as<br />

a subject matter expert surrounding employee benefits.<br />

She is responsible for overseeing the overall administration<br />

of various programs including disability benefits, FMLA,<br />

and time off programs. Alyssa interacts with employees<br />

and supervisors at all levels throughout the organization. In<br />

addition, Alyssa is CIGNA’s liaison with NextEra Energy.<br />

Mike Dunst<br />

Management<br />

Mike Dunst is a nationally known absence<br />

management expert. He joined The Hartford in 2013 to oversee<br />

product and strategy for absence management. In his role,<br />

Mike is accountable for our absence and self-funded Short-term<br />

Disability product capabilities, including profit and loss. He<br />

is responsible for ensuring that we deliver maximum value to<br />

our customers, helping them manage the productivity of their<br />

workforce. Prior to joining The Hartford, Mike was the national<br />

practice leader for absence management at The Standard where<br />

he was responsible for both operations and product strategy.<br />

He joined Hewitt Associates (now Aon/Hewitt) in 2008 as the<br />

operations absence leader when his third party administration<br />

(TPA) firm was acquired by Hewitt. Mike owned and operated<br />

this absence management TPA, which served more than 2<br />

million covered lives before being acquired. Mike has direct<br />

experience managing, implementing and servicing Fortune 500<br />

companies for absence management. In addition to significant<br />

operational expertise, he has experience building state-of-the<br />

art leave systems. Mike earned a Bachelor of Science degree in<br />

accounting from Villanova University and a Master of Business<br />

Administration in finance from St. Joseph’s University.<br />

He has been recognized and rewarded for providing leadingedge<br />

and health service companies, and in the role of employer. He<br />

has been recognized for his work in imaginative design and<br />

health ecosystem strategy for Caesars Entertainment and was<br />

honored as Benefit Professional of the Year 2003 by Employee<br />

Benefits Magazine based on the missions innovative, effective<br />

and positive results. Gary was founder and Executive Director<br />

of Health, Hope & Purpose, Inc., a nationally recognized health<br />

information non-profit and is the creator and operator of<br />

both the Health Awareness Tour 2005 and the 2013 Journey<br />

for Health Tour; both cross-America bicycle tours were<br />

established by Gary in order to promote health awareness<br />

and instigate health improvement by the delivery of an<br />

inspirational, educational message to companies, individuals<br />

Dr. Richard A. Feifer is the national medical<br />

director for National Accounts at Aetna<br />

Inc. He develops and implements strategies to help some<br />

of the nation’s largest employers improve the health and<br />

productivity of their members. Prior to joining Aetna, Dr.<br />

Feifer served as vice president of Clinical Program Innovation<br />

and Evaluation at Medco, where he was responsible for<br />

clinical program development, effectiveness, and medical<br />

integrity, across the company’s portfolio of health and care<br />

improvement programs. Dr. Feifer participated in the creation<br />

and deployment of clinical guidelines and care pathways<br />

during his years at Harvard Community Health Plan and<br />

Fallon, and continued those efforts at the InterQual Products<br />

Group of McKesson, where he developed clinical decision<br />

support criteria. Dr. Feifer’s main professional focus is the<br />

design, implementation, evaluation, and communication of<br />

strategies to promote the adoption of “best practices” for<br />

improving the quality and efficiency of health care. A graduate<br />

of Brown University and the University of Pennsylvania<br />

School of Medicine, Dr. Feifer trained in primary care internal<br />

A. Mark Fendrick, M.D. is a Professor of<br />

Internal Medicine in the School of Medicine<br />

and a Professor of Health Management and Policy in the<br />

School of Public Health at the University of Michigan.<br />

Dr. Fendrick received a bachelor’s degree in economics<br />

and chemistry from the University of Pennsylvania and his<br />

medical degree from Harvard Medical School. He completed<br />

his residency in internal medicine at the University of<br />

Pennsylvania where he was a fellow in the Robert Wood<br />

Johnson Foundation Clinical Scholars Program. Dr. Fendrick<br />

conceptualized and coined the term Value-Based Insurance<br />

Design (V-BID) and currently directs the V-BID Center at the<br />

advocate for development, implementation, and evaluation<br />

Ms. George is a senior vice president,<br />

corporate development, M&A, healthcare at Sedgwick.<br />

In this new position, Kimberly will explore and work to<br />

improve Sedgwick’s understanding of how health care<br />

reform affects its business models and product and service<br />

offerings. Kimberly previously served as Sedgwick’s<br />

managed care practice lead. She joined Sedgwick in 2001<br />

overseeing integrated disability medical programs and<br />

later case management services. Prior to joining Sedgwick,<br />

Kimberly worked as a consultant within the 24-hour health<br />

and integrated disability management arena. She also held<br />

a leadership position with a large national managed care<br />

organization’s case management division. Her career has<br />

focused on creating health and productivity programs for<br />

employers, impacting quality and cost of risk. Kimberly has<br />

been a registered nurse for 20 years. Although she began her<br />

career as a neuro-trauma nurse, she quickly transitioned to<br />

the insurance and benefits arena. Her experience in the cost<br />

containment field spans nearly 20 years. Kimberly’s roles in<br />

the property and casualty field include product development,<br />

national telephonic case management and utilization review<br />

oversight, and integrated disability management as well<br />

Tammy Green has spent her career<br />

developing innovative approaches to population health<br />

management, utilizing a unique combination of public and<br />

private-sector perspective and experience. Most recently,<br />

she has worked as Director of Well-being for Providence<br />

Health & Services, developing long-term strategies toward<br />

improving the health and well-being of more than 80,000<br />

employees, as well as their families, across the Pacific<br />

Northwest. In this role, Tammy oversees and provides<br />

leadership to a range of employee well-being services,<br />

including health incentive design and integration of wellbeing<br />

into organizational strategy and development. Prior to<br />

assuming her current position, Tammy oversaw employee<br />

health and well-being services in Providence’s Alaska region,<br />

since 2007. Her initiative and leadership were instrumental<br />

in the implementation of results-driven health and wellness<br />

programs for the region’s 4,400 employees. Tammy has more<br />

than twenty years of experience in health education, disease<br />

prevention, health program management & implementation.<br />

Prior to her time with Providence, Tammy led multiple<br />

initiatives with the Alaska State Department of Public Health,<br />

culminating in her position as Alaska’s Section Chief for<br />

Chronic Disease Prevention and Health Promotion. Tammy<br />

decision-making across the product cycle. Tony has over<br />

Ann Kuhnen, M.D.<br />

Ann Kuhnen, M.D., MPH is currently Medical<br />

Director, Disability Programs at the Hartford. Ann worked<br />

previously at Lockheed Martin and GlaxoSmithKline where<br />

she held a variety of positions responsible for employee health<br />

and productivity, including leave and disability services and<br />

wellness. Dr. Kuhnen earned her medical degree at Boston<br />

University School of Medicine and is a graduate of Boston<br />

University College of Arts and Sciences. She is board certified<br />

in both family medicine and occupational medicine. She<br />

completed a residency in family medicine at the University<br />

of Wisconsin-Madison and a fellowship in occupational<br />

medicine at Johns Hopkins Bloomberg School of Public Health<br />

where she also earned a Masters of Public Health degree. Ann<br />

served 7 years in the Army National Guard during medical<br />

school and residency training. After that she served 2 years<br />

active duty Air Force as a Captain in the Medical Corps,<br />

including an assignment at Riyadh Air Base during Operation<br />

Desert Storm.<br />

From 2005 to 2013, Craig joined<br />

UnitedHealthcare to begin forming a team of<br />

strategic customer analytic specialists. That team is focused<br />

on leveraging clinical, financial and consumer behavior<br />

healthcare data in a strategic customer dialog. Our overall<br />

goal is to partner with employers helping them to maintain<br />

healthcare affordability and increase their competitive<br />

advantage with a healthy and productive workforce. He<br />

services many customers himself and many more through<br />

his team. He also is responsible for the continual innovation<br />

of analytic tools nationally supporting commercial, public<br />

sector, and government programs. Those innovations focus<br />

on the ability to hot spot subsets of the population to focus<br />

on the communities in need of interventions. Starting in 2013,<br />

Craig assumed the role of Senior VP of Analytic Innovations<br />

for UnitedHealth Group. In this role he is focused on analytic<br />

innovations, community health analytics, and complex<br />

customer support for UHG. He is leveraging the breadth of<br />

UHG’s data to create actionable insights across a wide array<br />

of audiences ranging from customers to communities to<br />

policy makers. He is focused on spreading the best practice<br />

innovations across the UHG enterprise to impact all of the<br />

members that it serves. Craig also serves as an Innovation<br />

Champion for the enterprise.<br />

Deborah LaBonar, Ph.D.<br />

Deborah is the North American Analytics<br />

leader for Aon Hewitt’s 360° Absence<br />

Solutions. With responsibility for analytic consulting and<br />

measurement, she helps employers understand their program<br />

outcomes and the associated impact to their business. She<br />

uses data analytic tools to identify clients’ key drivers of<br />

absence in order to support fully informed decision making<br />

with regards to their programs and strategies. With 15 years<br />

of experience in absence management, Deborah is an expert<br />

in call center environments with a high incidence of leaves<br />

and disability. Her skills in uncovering the organizational<br />

factors driving employee behavior have resulted in savings<br />

of 20–30% of protected and unprotected lost time. Deborah<br />

has presented her work at the annual conferences of the<br />

Disability Management Employer’s Coalition (DMEC), the<br />

National Business Group on Health (NBGH) and the Society<br />

of Industrial Psychology (SIOP). She has also co-authored<br />

several peer reviewed journal articles and white papers,<br />

and will be a recurring columnist for the DMEC @Work<br />

publication in 2015. She currently serves on the Integrated<br />

Benefits Institute (IBI) Research Committee. Deborah<br />

received her master’s and doctorate degrees in industrial and<br />

organizational psychology from Purdue University in West<br />

within Guardian Life’s Group Benefits business. Mr. Lanzoni<br />

David Lea is a Disability and Absence<br />

Management practice leader in the western<br />

U.S. He is responsible for new business development and<br />

client retention for our absence business (FMLA, ADA, STD,<br />

LTD) in the region and assists our sales staff in working with<br />

brokers, consultants and prospective clients. Dave has worked<br />

in the industry for over 25 years, primarily with a focus on<br />

disability and absence management in the national accounts<br />

market. He lives on Bainbridge Island, WA with his wife and<br />

three sons and is responsible for all markets from the midwest<br />

to the Pacific Coast.<br />

Debra Lerner, Ph.D.<br />

Work and Productivity<br />

Debra Lerner, MS, Ph.D. is a Senior Scientist<br />

within the Tufts Medical Center Institute for Clinical Research<br />

and Health Policy Studies (ICRHPS), where she is founder and<br />

director of the Program on Health, Work and Productivity.<br />

She is a Professor within the Departments of Medicine and<br />

Psychiatry within the Tufts University School of Medicine<br />

and the Sackler Graduate School of Biomedical Sciences. Dr.<br />

Lerner had dedicated her career to improving the health and<br />

well-being of working people and enabling them to participate<br />

fully and productively in the workplace. Under her leadership,<br />

Dr. Lerner and her colleagues developed the Work Limitations<br />

Questionnaire (WLQ). The WLQ is used throughout the world<br />

and has become a standard of “presenteeism” measurement.<br />

In addition, Dr. Lerner’s team has developed innovative<br />

intervention programs aimed at improving employee<br />

functional performance and work productivity. Sponsorship<br />

for this research has been provided by the National Institute<br />

on Aging, the National Institute of Mental Health and the<br />

Centers for Disease Control and Prevention. Dr. Lerner<br />

has published numerous articles on the topic of health and<br />

productivity improvement. She has also served on many<br />

scientific panels and advisory boards addressing health in the<br />

workplace including the Health Project, which is responsible<br />

for administering the C. Everett Koop Award to an employer<br />

that exemplifies excellence in workplace health promotion<br />

and wellness.<br />

Ron Loeppke, M.D.<br />

Ron Loeppke, M.D., MPH, FACOEM, FACPM<br />

is Vice-Chairman of U.S. Preventive Medicine<br />

(USPM), and serves as co-Chair of the company’s International<br />

Advisory Board. Dr. Loeppke has over 30 years of experience<br />

in healthcare and is Board Certified in Preventive Medicine;<br />

and is a Fellow of both the American College of Preventive<br />

Medicine (ACPM) and the American College of Occupational<br />

and Environmental Medicine (ACOEM). Dr. Loeppke is<br />

Immediate-past President of ACOEM as well as a member of<br />

the Board of Directors of ACOEM. He is also the Co-Chair of<br />

the International Occupational Medical Society Collaborative<br />

(IOMSC) a global initiative involving over 24 countries. He also<br />

Chairs the CDC Diabetes at Work group. He was the Founder,<br />

CEO and President of the Health and Productivity Corporation<br />

of America (HPCA). HPCA was acquired by CorSolutions<br />

where Dr. Loeppke became Chief Health Officer. CorSolutions<br />

was then acquired by Matria where he became Chief Strategy<br />

Officer. Matria was then acquired by Alere. Dr. Loeppke was<br />

also previously the Chief Medical Officer of PhyCor, Inc., with<br />

over 25,000 affiliated physicians and $1.3 billion in annual<br />

revenue. Dr. Loeppke also founded and was President of<br />

Corporate Health and Medical Programs, Inc., which contracted<br />

with over 2000 employers (including Kodak, Hewlett Packard,<br />

Teledyne Waterpik, Anheuser Busch, ConAgra) for onsite<br />

occupational health and preventive medicine services as well as<br />

consulting Medical Director services.<br />

Michelle Martin<br />

Michelle Martin is a Human Resources<br />

professional with corporate level experience.<br />

She is Vice President, Human Resources Specialty Services at<br />

CBS Corporation. In this role she is responsible for the strategy,<br />

design and performance of CBS’s health and welfare plans,<br />

employee engagement and the development of HR policies.<br />

As a human resources professional with broad experience<br />

Ms. Martin taught a CEBS course at Hofstra University which<br />

placed emphasis on key economic, accounting and financial<br />

concepts essential in the administration of employee benefit<br />

plans. Ms. Martin is a Certified Employee Benefits Specialist<br />

with a certification in Global Benefits Management. She<br />

holds an MBA from Adelphi University. Michelle is an active<br />

member of the human resources community. She is on<br />

the board of directors of the New York Business Group on<br />

Health, HealthPass New York and The Leapfrog Group. She is<br />

Renee has over 27 years of experience in the<br />

field of health and productivity management.<br />

She has her Master’s in Vocational Rehabilitation Counseling<br />

from Michigan State University. Renee has worked in the<br />

insurance industry, managing departments in both workers<br />

compensation and disability. In addition, she owned a<br />

consulting firm assisting corporations with the development<br />

and implementation of absence management programs.<br />

Renee has also served as the Manager of Integrated<br />

Disability Management Programs for a major Midwest-based<br />

pharmaceutical and manufacturing company. She has been<br />

in the consulting industry for over 15 years with extensive<br />

experience in the healthcare, manufacturing, gaming, and<br />

financial and service industries. As a VP for Laurus Strategies<br />

(A HUB International Company), Renee assists our clients<br />

address health and productivity issues within their operation<br />

related to vendor management and coordination, review and<br />

enhancement of internal process and procedure to enhance<br />

productivity. Her command of this discipline has gained her<br />

largest food and beverage company headquartered in North<br />

America. Kathy is responsible for all health and welfare,<br />

retirement and savings plans at Kraft. Previously Kathy had<br />

responsibility for Kraft Foods U.S. benefits, global employee<br />

health and wellness as well as benefits communication and<br />

training. Prior to joining Kraft Foods, Kathy was the Director of<br />

Compensation & Benefits for The NutraSweet Company and<br />

Patti McCarver graduated with her Diploma<br />

and her Master’s in Nursing from the University of Mobile.<br />

She earned her Doctorate in Nursing Science from Vanderbilt<br />

University. Patti has worked as an emergency room nurse,<br />

emergency room director, and as a family nurse practitioner in<br />

private practice and urgent care. Currently Patti is the Director<br />

of the Vanderbilt Health at MNPS Employee and Family Health<br />

Care Centers. She has managed the Health Care Centers since<br />

they opened and played a vital role in designing and opening<br />

the centers as well as in developing the disease management<br />

programs for the centers. Patti is married to Lewis McCarver,<br />

a pediatric cardiac nurse anesthetist, and has two daughters,<br />

one of whom is a pediatric cardiac intensive care nurse<br />

and the other is a teacher who is the mother of Patti’s two<br />

granddaughters. If not working or with her granddaughters you<br />

can usually find Patti on the golf course.<br />

from 2010-2014 as Vice President, U.S. Benefits, supporting<br />

1.4M associates. Pursuits outside his corporate roles<br />

(IBI) Board Chairman. Chris co-founded the “World Wide<br />

Wellbeing” initiative, bringing thought leaders together<br />

globally and presenting results to the Organization for<br />

Economic Co-operation and Development’s leadership in<br />

Paris. As a long-standing member of the Global Knowledge<br />

Exchange Network (GKEN), he has served leadership roles in<br />

support of the “Global Healthy Workplace Awards” in London<br />

and Shanghai. In 2007, he joined Whirlpool Corporation as<br />

Sr. Director, Global Benefits where his five-year strategic plan<br />

helped drive business, employee and financial performance<br />

improvements over all sub-functional areas of global<br />

benefits. Business Insurance Magazine named Chris 1 of 20<br />

“2008 People to Watch” along with Senator Hillary Rodham<br />

Oswald “Oz” Mondejar is the Senior Vice<br />

President of Mission and Advocacy for Partners<br />

division of Partners HealthCare. Oz works at the intersection<br />

of policy, advocacy and media, defining PCC’s core values and<br />

leveraging the organization’s resources in order to maximize<br />

Dr. Mora holds a Ph.D. in mathematical psychology from<br />

the University of Michigan. He specializes in the theory<br />

settings, particularly its uses for decision making and program<br />

evaluation. Dr. Mora has developed measurement models<br />

for schools, hospitals, and manufacturing organizations. Dr.<br />

Mora is the president of Determinant, a consulting firm in Ann<br />

Arbor, Michigan, that provides statistical analysis services to<br />

public and private organizations. He is a Research Scientist<br />

with the Center for Positive Organizational Scholarship at the<br />

University of Michigan. He is involved in a comprehensive<br />

indicator system of well-being.<br />

Matt Morris<br />

As Vice President of AbsenceResources, Matt<br />

is responsible for ComPsych’s FMLA and ADA<br />

compliance programs. An employment lawyer by background,<br />

Matt has more than 10 years of experience working with<br />

employers’ FMLA and ADA administration. Matt is a frequent<br />

speaker on FMLA, state leaves, and the ADA and compliance<br />

challenges regarding leaves of absence. He was invited by the<br />

U.S. Department of Labor (DOL) to participate in a roundtable<br />

discussion about FMLA and authored a response to the DOL’s<br />

proposed FMLA regulations which was cited 38 times by the<br />

DOL in the final rule. Prior to his work in FMLA administration,<br />

Matt practiced employment law representing both employees<br />

and management in employment litigation, and counseling<br />

clients on compliance with federal, state and local laws. He<br />

received his B.A., magna cum laude, from Miami University<br />

in Oxford, Ohio and his J.D. from The University of Michigan<br />

Law School in Ann Arbor, Michigan.<br />

Sri Mummaneni, M.D.<br />

Dr. Sri Mummaneni, M.D., MPH has served as<br />

the medical director for Aerojet Rocketdyne<br />

in Los Angeles, for 9 years. He has set up or maintained<br />

onsite medical clinics for several large employers in the<br />

Los Angeles, including Hospital Corporation of America,<br />

AIG and Boeing. He has helped companies save millions<br />

in worker’s compensation costs through improving patient<br />

communication, medical care, and the administrative process.<br />

Currently, he leads the medical team at Opus Telehealth,<br />

a technology company focused on increasing physicianpatient<br />

communications using mobile platforms. The<br />

company is developing new technology as well as adopting<br />

current devices to improve patient outcomes in worker’s<br />

compensation and reduce costs of claims. Dr. Mummaneni<br />

completed his residency in Occupational Medicine and<br />

received his Master’s in Public Health from UCLA. He is also<br />

currently finishing a fellowship in Integrative and Holistic<br />

Jack Nightingale is Vice President of the<br />

Healthcare Practice for Benfield – Arthur J.<br />

Gallagher & Co. He is based in the Washington, D.C. area.<br />

Nightingale provides consulting services to healthcare<br />

clients in market research, strategy development and<br />

marketing communications. He helps leading pharmaceutical<br />

companies, healthcare delivery systems and other healthcare<br />

stakeholders to develop and implement innovative market<br />

strategies that deliver value to employers. Nightingale has<br />

specialized in business strategy and innovation for over 25<br />

years. After launching his career as a management consultant<br />

with PricewaterhouseCoopers, he served in leadership roles<br />

in strategy and business development for Honeywell and<br />

Monsanto. He moved into the healthcare field in 2000 when<br />

he became President of a start-up company that developed<br />

and licensed technologies for antiviral drug development. He<br />

returned to consulting in 2004 and joined Benfield in 2009,<br />

where he has contributed to the firm’s thought leadership<br />

related to healthcare transformation. Nightingale co-authored<br />

the 2011 book, Bridging the Value Gap: Collaborating<br />

Along the Health Care Supply Chain to Improve Health and<br />

Control Costs, One Community at a Time. In 2013, he was<br />

the lead author for a GE Healthymagination publication,<br />

“Building Better Healthcare in Cincinnati: How Employers are<br />

Collaborating with Other Healthcare Stakeholders to Improve<br />

Health and Reduce Costs in the Queen City.” More recently,<br />

he has led a joint research project with the American Medical<br />

Group Foundation to study emerging trends in collaboration<br />

Joe O’Neill is Vice President Compensation<br />

and Benefits for Penske Truck Leasing and<br />

has been employed with Penske for 25 years. In this role Joe<br />

is responsible for strategic planning and day-to-day operation<br />

compensation programs, evaluation of company acquisitions<br />

Thomas Parry, Ph.D.<br />

President<br />

Dr. Thomas Parry directs IBI’s activities and<br />

stewards its research agenda. Before cofounding<br />

IBI, he served 11 years as Research Director at the<br />

California Workers’ Compensation Institute. His research<br />

at CWCI encompassed a wide variety of topics in workers’<br />

compensation, including medical treatment patterns, vocational<br />

rehabilitation costs and effectiveness, legal costs and trends,<br />

integrated benefits. Parry speaks on integrated benefits and<br />

the United States and internationally. He served for five years<br />

as research advisor to the Roadway Express Inc. Medical Board.<br />

Parry received his bachelor’s, master’s and Ph.D. degrees from<br />

Jim Purvis<br />

Jim Purvis joined Healthways in January<br />

2007 and currently oversees the Healthways<br />

Well-Being Improvement design team where he coordinates<br />

the advancement of innovations proven concept learnings into<br />

Healthways core offerings as well as leading the design teams<br />

for Healthways product suite. Most recently, Jim successfully<br />

led a new strategic initiative to design and bring to production<br />

the Healthways Financial Wellness Program, in conjunction<br />

with a strategic business partnership with Dave Ramsey and<br />

the Lampo Organization. Prior work at Healthways includes the<br />

development of Healthways’ Blue Zones Project communitybased<br />

change design and the QuitNet tobacco cessation<br />

program. Jim also leads pilot design and implementations,<br />

including a recent pilot testing a new approach to behavior<br />

change blending purpose, mindfulness and neuroscience’s<br />

brain rewiring theories. Prior to joining the Healthways team,<br />

Jim spent 20 years at Mayo Clinic in Rochester Minnesota<br />

where he initially was a clinical social worker and social work<br />

supervisor, then served as the Director in the Organ Transplant<br />

Center so patients came to one location for care and all<br />

the pregnancy care program, 24-hour nurse line, and tobacco<br />

cessation programs, Jim led the initial development of Mayo’s<br />

Lifestyle Coaching product. Jim holds Bachelor’s degrees in<br />

Social Work & Psychology as well as a Master’s degree in Social<br />

Work from the University of Wisconsin-Madison.<br />

Scott Rabin<br />

Scott Rabin is a Principal at Buck Consultants,<br />

based in Los Angeles and a member of the<br />

national Health and Productivity practice. He is the General<br />

Manager of Buck’s RightOpt Health Exchange Solutions,<br />

where he works on all aspects of the solution, including<br />

vendor evaluation and integration, data analytics, and platform<br />

sustainability. In addition to his work on RightOpt, Scott works<br />

closely with the sister companies within Xerox to leverage<br />

our deep experience across the healthcare ecosystem. This<br />

work focuses primarily in the area of emerging delivery<br />

system strategies, where he has worked with employers,<br />

health systems, and health plans to define and develop quality<br />

and outcomes based network designs. Scott is a nationally<br />

recognized leader in healthcare data analytics, having held<br />

leadership roles in this area for two large consulting firms.<br />

Scott has over twenty years of healthcare experience, the<br />

last fourteen in consulting. He has a degree in Economics<br />

Al Rapp is the recently retired VP of Corporate<br />

Benefits at United Parcel Service (UPS) after<br />

37 years of service. He started at UPS in Cleveland, Ohio in<br />

1977 as a part time employee while attending college. Over the<br />

next 20 years, Al progressed at UPS and served many Human<br />

Resource responsibilities while working and living in Cleveland,<br />

Michigan, Washington, Idaho and Alaska. In 1998, Al relocated<br />

to Louisville, Kentucky and assumed the role of Vice President<br />

of Human Resources for the Worldwide UPS Air Operation,<br />

Airline and Global Network. In 2004, Al and family relocated<br />

to the UPS Corporate Headquarters in Atlanta, Georgia, as<br />

he was named the Vice President of Corporate Benefits for<br />

UPS and their more than 400,000 domestic employees. In<br />

that role, he was responsible for more than 50 different UPS<br />

administered and multi-employer health care plans, to include<br />

both pre and post 65 retiree health care. In addition to medical,<br />

dental and vision benefits at UPS, Al was responsible for all<br />

union and nonunion STD, LTD, Life Insurance, AD&D, Disease<br />

Management, Behavioral Health, EAP, overall Plan Design<br />

and Wellness. Prior to his recent retirement, Al directed the<br />

UPS Healthcare Strategy around Direct Contracting, Narrow<br />

Networks, ACO’s, Centers of Excellence, Exchanges and<br />

overall ACA Compliance. Following his retirement, Al has been<br />

providing guidance, direction and input around the future of<br />

Healthcare, all from a large employer’s perspective.<br />

Jane Ryan<br />

Jane Ryan is Return to Work Section Head<br />

with over 25 years of experience in disability<br />

management with Mayo Clinic. She was instrumental<br />

in the development of return to work practices in the<br />

organization, a program that has received national awards<br />

for best practice. Previous to working with Mayo, Ms. Ryan<br />

had several years of related disability and psychiatric case<br />

management experience in vocational and medical fields,<br />

working as a Qualified Rehabilitation Consultant (QRC)<br />

in the private rehabilitation sector, program coordinator<br />

of a pain management program, nurse therapist in both<br />

inpatient chemical dependency and psychiatric programs,<br />

and counselor in a private marriage counseling practice. Ms.<br />

Ryan is on the board of the Minnesota Chapter of Disability<br />

Management Employer Coalition (DMEC) where she is Past<br />

President and current Program Chair. She is a member of the<br />

planning committee for Impairment Without Disability, an<br />

annual, national continuing medical education conference<br />

now in its 8th year. Ms. Ryan is a member of the US<br />

Department of Labor’s Office of Disability and Employment<br />

Policy return to work (RTW) Community of Practice (CoP).<br />

Stephen Shaya, M.D. graduated from<br />

Birmingham Brother Rice High School Summa<br />

Cum Laude. He was awarded a Merit Scholarship to attend<br />

Wayne State University. During his time at WSU, Dr. Shaya<br />

won many leadership awards including the Outstanding<br />

Student Leader in 1991 and Howard Hughes Medical Institute<br />

Research Grant Awardee. He was selected to serve on the<br />

Wayne State University Board of Governors from 1991 to<br />

1993. He attended Wayne State University Medical School<br />

and graduated in 1996. Dr. Shaya subsequently finished<br />

training in Family Practice at North Oakland Medical Center<br />

and a fellowship at Wayne State University for Occupational<br />

and Environmental Health. Since 2004, Dr. Shaya has been<br />

the Chief Medical Officer of J & B Medical, a global healthcare<br />

solutions company. J & B is a privately held global health care<br />

solutions company based in Wixom, Michigan and started<br />

in 1996. We have sixteen different business enterprises<br />

and currently has 16 business enterprises and customers in<br />

48 states and 28 countries. This innovative company has<br />

brought many new ideas and solutions forward to the delivery<br />

care and continues to strive to improve patient access and<br />

quality. His role oversees all strategy, innovation and growth<br />

in our company. By leading the planning and development<br />

of our new enterprises, many describe Dr. Shaya as an<br />

Dr. Bruce Sherman, M.D., FCCP, FACOEM,<br />

serves as Medical Director of the Ohio-based<br />

Employers Health Coalition, where he brings health and valuebased<br />

purchasing strategies to employer members. Similarly,<br />

he serves as the consulting Corporate Medical Director for<br />

Wal-Mart Stores, Inc., wherein he advices the development of<br />

Karrie A. Shogren, Ph.D. is an Associate Professor in the<br />

the Kansas University Center on Developmental Disabilities and<br />

the Beach Center on Disability, all at the University of Kansas. Dr.<br />

Shogren’s research focuses on self-determination and systems of<br />

support for youth and adults with disabilities. She is specifically<br />

interested in how we can implement interventions that promote<br />

self-determination by teaching self-regulated goal setting<br />

and problem solving skills across contexts (e.g., education,<br />

employment, health) to promote valued outcomes. Dr. Shogren<br />

has published over 65 articles in peer-reviewed journals and<br />

is the author or co-author of 5 books, and has received grant<br />

funding from several sources, public and private.<br />

John Sinnott, M.D.<br />

Dr. John Sinnott received his M.D. from<br />

the University of South Alabama. After<br />

completing an Internal Medicine Residency and Infectious<br />

Disease Fellowship at the University of South Florida<br />

(USF) he joined the faculty at USF. He is board certified in<br />

Internal Medicine and Infectious Disease and is a Fellow<br />

of the American College of Physicians and the Infectious<br />

Disease Society of America. In 1992, Dr. Sinnott was named<br />

to the James A. Cullison Endowed Professorship for his<br />

distinguished contributions to medical science. In 2012, Dr.<br />

Sinnott was selected as Chairman of the Department of<br />

Internal Medicine at the USF Morsani College of Medicine.<br />

Prior to this, he served as the Director of the Division of<br />

Infectious Disease and International Medicine from 1991-<br />

2012, and additionally, was Associate Dean of International<br />

Affairs for the Morsani College of Medicine from 2005-<br />

2012. During this time he built the ID faculty from 3 to 18<br />

members and founded Medicine International. Dr. Sinnott<br />

has contributed to the ongoing evolution of medicine by<br />

serving on over 40 local, regional, and national health care<br />

committees, and publishing over 200 abstracts, articles and<br />

textbook chapters. Recently, he developed an app, MDstats to<br />

teach clinicians about medical statistics. Dr. Sinnott has been<br />

inducted into the Leon G. Smith Infectious Disease Institute<br />

Hall of Fame.<br />

defense industries in the Washington DC area. Ms. Snider<br />

has a BS in Human Resources Management from Michigan<br />

State University and a MA in Organizational Training from<br />

University of Maryland and has earned her CEBS and CCP<br />

designations. She is currently Director of Health and Welfare<br />

Benefits for BAE Systems Inc where she designs and manages<br />

benefit plans for 40,000 employees and retirees.<br />

Jamie Stalker, M.D.<br />

Wellness Division<br />

Dr. Stalker joined Argonne National Laboratory<br />

in 2001 and became the director of medical programming there<br />

in 2005. A board certified internist and advocate for disease<br />

prevention, she was convinced that the Department of Energy<br />

(DOE) contractor mandated workplace occupational health<br />

clinics had immense potential both for impacting employee<br />

health and helping slow the increasing costs of medical benefits.<br />

In her role as the director, she started planning accordingly<br />

to expand the occupational health clinic services to include<br />

wellness and disability management services using the<br />

integrated human capital management concept as her model.<br />

Now in 2015, Argonne has a robust award winning Wellness<br />

program in addition to their occupational health clinic. The<br />

on-site physical therapy program alone demonstrated deferred<br />

costs of over 1 million dollars in its first year. Prior to her career at<br />

Argonne Dr. Stalker was a practicing internal medicine physician<br />

and clinical instructor at Northwestern Memorial Hospital and<br />

Rush University Medical Center in Chicago. At both institutions<br />

she was also actively involved in managed care and executive<br />

steering committees. Dr. Stalker served four years as national<br />

chair for the DOE EFCOG Occupational Medicine Subgroup<br />

and currently is on the Board of Directors for NAWHC. She is<br />

a regular lecturer on safety, leadership, and preventive health<br />

care topics. Dr. Stalker received her B.S. in Biology from Indiana<br />

University, M.D. from Rush Medical College, and did her<br />

residency training at the University of Michigan.<br />

Douglas R. Stover is a Senior Managing<br />

Consultant at Gallup. He is responsible for<br />

the overall success of the joint venture between Gallup and<br />

organizations, communities, and countries measure and<br />

manage well-being. As a member of the leadership team for<br />

this groundbreaking venture, he partners with Healthways<br />

to transition businesses from a narrow focus on employees’<br />

physical health to a multidimensional emphasis on their<br />

customers’ and employees’ overall well-being. Mr. Stover<br />

also serves as a strategic advisor to Gallup’s leading clients<br />

on their most pressing employee and customer opportunities<br />

and challenges. His industry expertise includes the healthcare,<br />

pharmaceutical, and hospitality sectors. Before joining Gallup,<br />

Mr. Stover was an associate director at The Advisory Board<br />

Company, a healthcare research organization, developing<br />

workplace improvement and customer/patient loyalty<br />

strategies. Prior to his work with The Advisory Board Company,<br />

Mr. Stover was a senior consultant with OgilvyOne, charged<br />

with building strategic marketing solutions for pharmaceutical,<br />

consumer packaged goods, and retail finance organizations.<br />

Mr. Stover has also served as a consultant with ICG Commerce,<br />

specializing in the development of supply chain solutions for<br />

pharmaceutical clients. Mr. Stover received his bachelor’s<br />

Dave Taylor is the Director of Integrated<br />

Disability Management at Reyes Holdings, a<br />

products. He has a Bachelor’s Degree in Criminal Justice and is<br />

working on completing his Masters in the same field of study.<br />

Dave has over 18 years’ experience in workers’ compensation<br />

and disability claims management, including roles as a claims<br />

investigator, adjuster, and administrator He has served as<br />

IDM program manager for Navistar and other employers<br />

saving millions through implementation of integrated program<br />

strategies. In his current role, Dave directs the Integrated<br />

Disability Management department responsible for Workers<br />

Compensation, Disability, FMLA and related state leaves, Auto<br />

and General Liability Claims, and transitional duty over multiple<br />

jurisdictions for Reyes Holdings and its affiliates, overseeing<br />

all claims handling processes, protocols, and procedures and<br />

Tamela Thomas is Wellness Manager at the<br />

Washington Athletic Club, Seattle’s premier<br />

executive club and four-time winner of the 5 Star Platinum Club<br />

of America. She initiated the WAC’s Wellness Center, which<br />

now serves 4300 client visits annually. Tamela brings handson<br />

management experience from her 27 year career, including<br />

capital improvement projects, directing diverse teams of fitness<br />

and healthcare professionals as well as overseeing a variety<br />

of service areas from physical therapy, nutrition, naturopathic<br />

medicine, acupuncture, group fitness, personal trainers and<br />

spa. As Program Director of REV, the new executive fitness<br />

program at the WAC, Tamela created a customized solution for<br />

health concerns that follow executives into the corner office.<br />

This comprehensive, progressive, high-touch approach reduces<br />

catastrophic risk and creates health for enhanced productivity<br />

and innovation. Tamela will be addressing practicalities within<br />

Michael L. Wehmeyer, Ph.D. is Professor of Special Education;<br />

Director, Kansas University Center on Developmental<br />

Disabilities; and Senior Scientist and Co-Director, Beach<br />

Center on Disability, all at the University of Kansas.<br />

Dr. Wehmeyer has directed externally funded projects<br />

totaling almost $30 million conducting research and model<br />

development activities and personnel and leadership<br />

preparation activities pertaining to the education and support<br />

of youth and adults with intellectual and developmental<br />

disabilities. He is the author or co-author of over 300 peerreviewed<br />

journal articles or book chapters and has authored,<br />

co-authored, edited, or co-edited 32 books on disability and<br />

education related issues, including issues pertaining to selfdetermination,<br />

positive psychology and disability, transition<br />

to adulthood, the education and inclusion of students with<br />

severe disabilities, and technology use by people with<br />

cognitive disabilities.<br />

then to Johns Hopkins for qualification in Pediatric Surgery.<br />

Following 10 years on staff at Johns Hopkins, he was<br />

Professor of Surgery and Pediatrics successively at Albany<br />

Medical College, Loma Linda, and Mercer Medical College.<br />

He is the author and coauthor of over 175 peer reviewed<br />

paper and book chapters, and coeditor of 5 textbooks.<br />

He was an active member of 18 scientific organizations.<br />

Following retirement from academic medicine, Dr. White<br />

joined Diagnos-Techs Laboratory as a medical consultant and<br />

educator. He now is the Chief Medical Advisor and Chairman<br />

of the Medical Board. In this position, he is a liaison to the<br />

medical community, particularly M.D.’s and to the public. He<br />

represents on-site the Board of Scientific Advisor to Diagnos-<br />

Techs, and spearheads its R&D and academic activities.<br />

department at the M.D. Anderson Cancer Center, followed<br />

by training in occupational/preventive medicine at the<br />

University of Cincinnati Medical Center. Dr. Yarborough is<br />

on the medical staff of Johns Hopkins hospital in Bethesda<br />

and is a voluntary physician for a community health center.<br />

He is an Associate in the Department of Health Behavior and<br />

Society at Johns Hopkins University Bloomberg School of<br />

Public Health. Dr. Yarborough is an elected member of Delta<br />

Omega (the National Public Health Honors Society), is the<br />

honorary chairman for the non-profit International Corporate<br />

Health Leadership Council, and served on a health advisory<br />

committee for U.S. Environmental Protection Agency.<br />

Dr. Yarborough is serving a third elected term as a member<br />

of the board of directors for the American College of<br />
