community nurses and providing an environment which<br />

facilitates use <strong>of</strong> the <strong>Standards</strong> would produce such a result.<br />

From these examples, it can be seen that long-term and<br />

short-term represent points on a continuum rather than a<br />

defined period <strong>of</strong> time. When there is research evidence<br />

linking the various levels <strong>of</strong> outcome, then there is good reason<br />

to believe that a short-term outcome will lead to the long-term<br />

outcome, and this assumption is <strong>of</strong>ten made (Israel et al.,<br />

1995).<br />

The activities may be further specified in the logic model to<br />

indicate the type and frequency <strong>of</strong> each activity that will be<br />

required to achieve the desired outcome. The activities provide<br />

a measure <strong>of</strong> output. For example, we might specify that 80%<br />

<strong>of</strong> nurses should be oriented to the <strong>CCHN</strong> <strong>Standards</strong> in the<br />

first year to make a change in practice. By constructing the<br />

logic model up front, the outputs and outcome indicators are<br />

explicit from the outset, which makes it easier to set up<br />

systems to gather critical operational data.<br />
