FROM THE OFFICE OF: Nurse Health Examinations Each pupil in the East Penn School District shall receive a comprehensive medical examination upon entry into sixth grade and the eleventh grade. These medical examinations will be conducted by a school physician, contracted through the district. In lieu of the medical examination conducted by a school physician, the parents/ guardians may submit a medical report of examination conducted by a physician, at their own expense, provided that the examination is made and the report furnished no more than one year prior to the opening of the school term during which the regular examination is scheduled. Each pupil shall receive a comprehensive dental examination while in third grade and seventh grade. These examinations will be conducted by a school dentist. In lieu of a dental examination conducted by a school dentist, a parents/ guardians may submit a dental report of the examination, at their own expense, provided that the examination is made and the report furnished no more than one year prior to the opening of the school term during which the regular examination is scheduled. Parents/Guardians of children who are to be examined by the school physician or dentist shall be notified of such examinations. The notice shall include the date and location of the examination and encouragement that the parents/guardians attend. If the parents/guardians choose to have medical or dental examinations conducted privately at their expense and the reports of the examinations are not timely furnished, the following notification procedures shall occur: The first notice to comply with the policy of medical or dental examinations will be mailed on or around October 1. The second notice to comply with the policy of medical or dental examinations will be mailed on or around December 1. If the medical or dental examination, as requested through the Department of Health and/or the East Penn School District, is not completed and proof submitted to the appropriate school nurse on February 5, 2010 of the school year in question, the student will be excluded from school. Readmission will be contingent upon presenting evidence that the required examination had been completed. IF YOU WISH YOUR CHILD TO HAVE THESE MEDICATIONS, YOU MUST PROVIDE WRITTEN PERMISSION AND A PARENT OR GUARDIAN MUST BRING THE MEDICA- TION INTO THE HEALTH ROOM. DOSAGE WILL FOLLOW THE PRINTED GUIDELINES ON THE BOTTLE. ANY DEVIATION FROM THIS WILL REQUIRE A PHYSICIAN’S NOTE. Page 6
Counseling FROM THE OFFICE OF: Attendance PSAT/NMSQT INFORMATION The PSAT/NMSQT will be offered at Emmaus High School on Saturday, October 16, 2010. The PSAT/NMSQT or Preliminary SAT/National Merit Scholarship Qualifying Test, measures the verbal, math and writing skills that students have developed over the course of their education. Typically, college bound students will take the PSAT in their junior year of high school. Students can register for the PSAT in the Counseling Office. The fee to take this test is $20.00. (Checks should be made payable to Emmaus High School). Upon payment of the registration fee, students will receive an important practice booklet. Registration begins September 20, 2010. The deadline to register for the PSAT is Tuesday, October 12, 2010. SATs The SAT and SAT Subject Tests will be administered at Emmaus High School on Saturday, October 9, 2010. REMINDERS - Procedures for Early Dismissal In an effort to ensure the continued safety and security of our students, we follow strict guidelines for the early dismissal of students. A student must bring a note to the Attendance Office prior to homeroom for an early dismissal. This note should contain the student’s full name, student’s ID number, date and time of early dismissal, and reason for early dismissal. The note must be signed by the parent/ guardian. Please make arrangements with your child as to where to meet for pick up. If this procedure is followed, it’s not necessary for the parent/guardian to come to the Attendance Office to sign out the student. We understand that at times an appointment may be made last minute by the parent/ guardian, or there may be a family emergency resulting in the student needing to be dismissed early. Should emergencies occur, the parent/guardian must come to the Attendance Office to sign out the student. To ensure the safety of our students, we will ask for photo identification. Please be advised that this method for early dismissal should only be used in last minute and emergency cases. Please note that phone calls for early dismissals are not accepted.
