Please read and sign this form.<br />
In this usability test:<br />
• We will record you performing the test, with both audio and video.<br />
• We will request that you fill out a post-test questionnaire and answer post-test interview<br />
questions.<br />
• Your responses will be used in data analysis.<br />
• Your personal information will not be disclosed at any point.<br />
Participation in this usability study is voluntary. All information will remain strictly confidential. The<br />
descriptions and findings gathered from this study may be used in the report(s) we provide to our<br />
client, however, at no time will your name or any other identification be used. You can withdraw<br />
your consent to the test and stop participation at any time.<br />
