We, the undersigned do hereby state that all of the Rules of Eligibility for burial in Military Memorial Division
of Riverside Cemetery have been met, that the deceased,_______________________ is eligible for burial on
said tract.
It is further understood and agreed that if said deceased meets with the residential requirements of the agreement,
there is to be no charge for the grave space, and opening and closing of the grave. If the said deceased does not
meet the residential requirements as per the Military Cemetery Agreement dated 7-26-83 a charge for the grave
space and a charge for opening and closing the grave will be made at current rates established by the City
Commission. This cost is to be paid to the City Clerk before permission is granted for the burial.
