Appendix G: Screening Instruments forMental DisordersSeveral mental health screening instruments are reviewed in this appendix.Without use of these instruments, mental disorders are often undetected incriminal justice settings. As a result, staff are less likely to anticipate suicidalbehavior and other mental health problems, and the effectiveness of treatment isreduced.Beck Depression Inventory-II (BDI-II)The BDI-II (Beck, Steer, & Brown, 1996) is a 21-item self-report instrument thatexamines the intensity of depressive symptoms and suicidality. This instrument isone of the most widely used measures of depression. The BDI-II was developed tocorrespond to DSM-IV criteria of depression, and reviews key symptoms includingagitation, difficulty in concentration, feelings of worthlessness, and loss of energy.Elevated scores on items related to suicidal ideation and hopelessness should beattended to carefully, since these items are the most highly predictive of suicidalbehavior. Despite its usefulness in screening for depression and suicide, the BDI-IIshould not be used in diagnosing depression (as reported for the BDI-I; Sundberg,1987), which requires a more intensive assessment process.Positive Features•y The BDI-II requires little training to administer or score•y The wording of the BDI-II is clear and concise, and the measure can becompleted in 5–10 minutes•y Only a fifth grade reading level is required to complete the BDI-II•y The instrument has frequently been used with substance users and hasbeen found to be useful in screening and assessment of depression amongthis population (Buckley, Parker, & Heggie, 2001). For example, the BDIhas been found to be among the most effective instruments in detectingdepression among people who abuse alcohol (Weiss & Mirin, 1989)•y The BDI has excellent content, convergent, and divergent validity (Steer,Beck, & Garrison, 1986), and scores from the BDI are significantlycorrelated with other indices of depression, including the Hamilton RatingScale for Depression (r = .71) and the Beck Hopelessness Scale (r = .68)•y The BDI has moderately good sensitivity (67%) and moderately goodspecificity (69%) in diagnosing depression among individuals with alcoholproblems (Willenbring, 1986)•y The BDI has higher sensitivity (94%) and specificity (59%) than theRaskin Depression Scale, the HAM-D, and the SCL-90-R (Rounsaville,Weissman, Rosenberger, Wilber, & Kleber, 1979)62
