DEAN’S LETTERWheels in MotionMert N. Aksu, DeanLike most functioning mechanisms, theindividual components support the greaterstructure. As I reflect back on the last fiveyears as Dean, I realize how thankful the Schoolof Dentistry is for the support from alumni andfriends. In an intertwined-fashion, our lives areenriched because of the infrastructure ofencouragement cultivated within the University. For me, the infrastructure resembles a bicyclerim, or a complex interweaving of multiplespokes, which on their own are too delicate, butwhen assembled together in an organizedfashion, support the weight of the profession. Our lives are enrichedbecause of the infrastructureof encouragement cultivatedwithin the University. As implied in the analogy above, when workingtogether, we are at our strongest. With thecentralized hub acting as the School, alumni andfriends continue to be connected to the missionof the administration and help support thevalues of the University. As spokes, or graduates of the School, you arenever far from that centralized hub. A strongalma mater brings value long after graduation. As a School, we have an unwaveringcommitment to providing our students apractical, relevant, and evidence-basededucation. This charge, in addition to the focuson clinically contemporary practices, frames theessence of our programs and educationalexperiences. In this edition of the Leading Edge, the UDMoutreach efforts will showcase clinicalexperiences that allow for personal andprofessional development. The development ofhumanistic professionals is what distinguishesUDM as a dental school with a purpose. The community-based clinics have evolved overthe years. Whether you remember the linesoutside of the Jefferson Avenue clinic, or theopening of the University Health Center (UHC),public health has been at the heart of theSchool’s mission. For today’s graduates, it’s theopportunity to serve the surroundingneighborhoods by rotating through communityclinics. The exposure is an important part of theprocess and creates a sense of self-awareness.In addition to the educational rotations, theSchool makes an effort to expand ourcommunity footprint through various organizedactivities. We look to our friends and supportersto be a part of the movement, and to help ussustain momentum in providing these positiveprograms to the students. Ultimately, we wantour students to have the technical ability todeliver clinical results, the analytical ability tounderstand the issues, and the compassion tomake a difference. In looking back, I hope that you can share thesame sense of pride that I have for our expertfaculty, dedicated staff, and engaged students. Please congratulate yourselves for being part ofa School that is here today, and is planning fortomorrow because of your involvement. The development ofhumanistic professionals iswhat distinguishes UDM as adental school with a purpose. As stated earlier, it is the individual componentsthat make up the greater structure.Thank you for your friendship and supportin this great ride.Mert N. At the initial appointment, it was found that the fracture for #8 was onlythrough dentin, while the fracture for #9 involved the pulpal tissues. To helpalleviate the pain, a gross pulpal debridement was completed. To repair her anterior teeth, she had a Class IV composite planned for #8 and anRCT followed by a composite core on #9. A pin was also placed on #9 to aidwith retention of the core. Over the next few appointments, these procedureswere completed. The end result is that #8 and #9 were built up with composite, thus repairing her front teeth and restoring her smile.Mr. Art Foster is leading the wayto a “greener clean” for UDMSDUDMSDGoes Green!Under the direction of Art Foster,Custodial Manager for the School ofDentistry, the Corktown Campus istaking strides to go green. Nick DeBoer, a 2013 UDM Dentalgraduate, says he wasn’t surewhat to expect when he beganhis off-site rotations last year. “Ithought I’d seen it all,” says DeBoer, referring to the patients he had treatedon campus. But those experiences did not preparehim for the population he saw duringhis two-week rotation in Detroit, as aparticipant in UDM Dental’sCommunity Based Dental Education(CBDE) program. DeBoer says that heand his fellow students often struggledto meet the needs of patients who hadmultiple, complex dental issues—whichwere frequently the result of a prolongedlack of access to regular, preventativedental care. His two-week rotation in Grand Rapidswas even more eye-opening. The clinicwhere he served was about a mile fromdowntown, and the population waspredominantly Hispanic. Most of thepatients there were pediatric, and heneeded a translator to talk with many ofthem, and their families.“Grand Rapids is my home town,” saysDeBoer, “and I couldn’t believe howmany underserved people there were inmy own community.”Combatting the “Dental Divide”DeBoer’s experiences point to atroubling nationwide trend: The rapidlygrowing number of people who lackaccess to regular dental care. Accordingto the U.S. Department of Health andHuman Services, that number jumpedfrom 33 million to 49 million from 2011to 2012—an increase of more than 48percent in just one year. The problem isn’t a shortage of dentists, says Dr. Bill Calnon, president of theAmerican Dental Association (ADA),but rather—an uneven distribution ofdentists, and the relatively small numberof dentists who participate in Medicaidand other state and federal programsthat serve mainly low-income families. This combination of factors is creatingwhat the ADA is calling a “dentaldivide” that is affecting many poorcommunities, and in particular, those communities that havepredominantly African American,Hispanic or Native Americanpopulations. This is especially true inMichigan—which has significantpockets of both urban and rural poverty,a growing immigrant population, andwhere, according to the AmericanAcademy of Pediatrics, approximately41 percent of children are enrolled inMedicaid. In the late 1990’s, the Robert WoodJohnson Foundation (RWJF) conductedresearch aimed at better understandingthe causes of this growing discrepancyin access to care—and focused ondeveloping ways to address it. Theirresearch demonstrated that studentswho had opportunities to work withpatients in underserved communities notonly had greater satisfaction, andgreater confidence in dealing with thetreatment needs of these patients—but also a greater willingness to pursueDr. Lyn Eickholt (‘08) and Dr. Lauren Jefferson (‘11) bothchose to pursue careers at Covenant Community Care Clinic20 LEADING EDGE SUMMER 2013

career opportunities in underservedareas upon graduation.Consequently, in 2001, the RWJFfounded a “dental pipeline” program, which encouraged dental schools acrossthe nation to establish community-baseddental education programs, integratecommunity-based service learningopportunities into their educationalprograms, strengthen course offerings incultural content, and implementprograms to increase the recruitmentand retention of underrepresentedminority and low-income students.Impressed by the success of thisapproach, UDM Dental decided toestablish its own program in 2007.UDM Dental student Brent Hazelwood (‘14) and Covenant Community Care dental assistantJacqueline Spencer work together to help a patientcontinuedCommunity Based DentalEducation—a “win-win” approachUDM Dental’s Community BasedDental Education (CBDE) programbegan as a modest effort, incollaboration with HamiltonCommunity Health Network, in Flint.Since then, the CBDE program hasgrown dramatically, and now operatesin collaboration with seven independent, private, clinics across the state, including:Detroit Community Health Center,DetroitCovenant Community Care, DetroitMichigan Community Dental Clinics,Samaritan Dental Clinic, DetroitHerman Keifer Pediatric Clinic,DetroitFamily Health Center, KalamazooHamilton Community HealthNetwork, FlintCherry Street Health Services,Grand RapidsAs the program has grown, it has alsobecome a more established part of theUDM Dental student experience, andcurriculum. In 2010, Dr. DiveshByrappagari was hired as UDM Dental’sDirector of Community Programs, tooversee the School’s community-basededucational activities. Since assumingleadership for the program,Byrappagari has developed a newcourse (DSD 8470-Clinical ServiceLearning) that is based on the CBDEprogram model. That course is nowrequired for all UDM Dental students, prior to graduation.“This course, and the experiences itprovides, represent a ‘win-win’approach,” says Byrappagari. “In theshort term, more underserved patientsgain better access to care. In the longerterm, this positive communityexperience may influence graduates totreat vulnerable populations in theirprivate practices, or to work inunderserved areas.”WWW.DENTAL.UDMERCY.EDU 21

dental care—but also being able toafford it.Brian Roy (’13) participated in rotationsat three public health clinics during hisfourth year of dental school, anddescribes treating an 11 year-old girlwho presented with significant decay onher maxillary left first molar, which hadbecome symptomatic to the point ofdisturbing the girl’s sleep.UDM Dental student Michael Liu (‘14), and Dr. Heidi Hudson of Covenant Community Care, review their patient’s scansChanging attitudes (and lives)For many dental students, especiallythose who come from relativelycomfortable, middle-class backgrounds, caring for patients who are oftenstruggling with multiple, complexissues—including extreme poverty, homelessness, legal problems, and evenaddiction—can initially be somewhatdistressing. However, initialawkwardness frequently turns tounderstanding, and even empathy, asstudents begin to see their patients notjust as “cases” to be solved, but asindividuals—each with his or her ownlife story, including their own uniqueopportunities and challenges. In many cases, these interactions forcestudents to challenge their own attitudesand assumptions about the “poor” andthe “underserved.”Lisa Sontag (’13) describes a patient shetreated while participating in a CBDEprogram rotation at the Cherry Streetclinic in Grand Rapids. Whenever theman arrived, he was always wellgroomed, and very well dressed, so shejust assumed he was a middle-classprofessional. But, after making smalltalk with the man, she eventuallydiscovered he had been unemployed—and desperately searching for work—forseveral years. While participating in a rotation inDetroit, Sontag says she alsoencountered a man who showed up foran appointment over an hour late—bundled up, head-to-toe in a snowsuit.“He cheerfully responded to ourquestioning glances by saying ‘It’s coldoutside! I had to walk!’… as it turns outthe man had walked three miles inbone-chilling cold in order to make hisappointment,” says Sontag. “Thecommon assumption is that people whoattend community dental clinics don’ttake responsibility for their health, orfeel they are ‘entitled’ to care. However,my program experiences showed thisoften is not the case.”For other students, participating in theCBDE program illuminates thetremendous challenges many patientsface with regard to not only accessing“The tooth was restorable, but itrequired endodontic treatment,” saysRoy. “The patient was enrolled inMedicaid, which covers endodontictreatment for children—but there are noproviders in the Grand Rapids area thataccept Medicaid payment for rootcanals. I suggested to her father that wecould perform a root canal, and that itwould cost around $800…but he toldme that he simply could not afford it,and asked me to extract the tooth. Thatcase made me really sad, and I felt veryguilty extracting a restorable molar onsuch a young patient,” he says.New experiences leadto new career pathsFor some students, having suchexperiences leads not only to greaterawareness and better understanding, but also a greater willingness toconsider practicing in underserved areas, after graduation.Kyle Blau (’13) says that as he began hisfourth year of dental school and beganto consider his post-graduationemployment options, his primary focuswas on finding a job that would allowhim to follow his passion of practicingdentistry while still being able to “paythe bills.” However, following a CBDErotation in Kalamazoo, and two inDetroit, he began to think differently.“After finding out that almost all publichealth dentistry jobs offer some form ofloan repayment assistance, it becamemore obvious to me that this might besomething worth looking into,” says22 LEADING EDGE SUMMER 2013

Blau. Eventually, I had opportunitiesto work with doctors at multipleclinics who all told me how much theyenjoyed their work, and howrewarding it was to be able to providedental services to underservedpopulations. Many of them alsoexpressed how important they felt itwas to be able to continue to providethese services to many people whoneed them so badly…and I started tobecome more and more convinced.“Based on these experiences, I’vedecided that public health dentistry isdefinitely the route I want to take afterdental school,” says Blau. From modest beginnings…many promising futuresUDM Dental student John Yurkovich (‘14) spends a few minutes chattingwith a nervous patient at Gateway ClinicFrom its modest beginnings only fiveyears ago, the CBDE has come toprovide hundreds of UDM Dentalstudents with extraordinaryopportunities to help—and to betterunderstand the complex challengesthat many patients, and communities, face with regard to being able toaccess (and afford) regular, preventivedental care. In so doing, the programhas also allowed many students tobetter appreciate, and perhaps evenconsider career opportunities inpublic health dentistry. In the process, the CBDE program hasbecome an integral part of both theUDM Dental curriculum, as well theSchool’s ongoing commitment toproviding expanded services to thecommunity — including providingdental sealants in school-basedprograms, oral cancer screenings athealth fairs, and prevention programstargeting certain populations in groupcare settings, such as nursing homes. As such, the CBDE program is yetanother outstanding example ofUDM’s mission to promote Jesuitand Mercy education. Here’s HowYou Can HelpNo matter where youlive or practice, thereare many ways you canhelp to to closethe “dental divide,”by providing direct,or indirect, assistanceto individuals who needregular, preventivedental care:Work in a “safety-net” clinic, such as a Federally QualifiedHealth Center (FQHC) or acommunity clinic located ina Dental Health ProfessionShortage Area (D-HPSA).Help to sponsor, start, orparticipate in a school-basedsealant program.Agree to take more Medicaidpatients in your practice. I am a holistic practitioner, which means I dealwith supplements and herbs to heal the body. My experience and views are fromthe Eastern side of medicine, which can often put me at odds with the Westernworld of medicine that is prevalent in the United States. That being said, thereare no Eastern cures for a tooth that is in need of a crown, and I have beencoming to U of D for about 10 years. I am also a yoga instructor that practicesthe power of breath. The “conflict” between East and West particularly showsup when I deny use of any type of sedative for any drilling. I have to admit, it isfun to see the horror on the new dental student’s face when they read my chart.I imagine they think I will be flinching and jerking while they attempt to drill mtooth, but that is never the case. To me the experience is a little uncomfortable, but not at all painful.Recently, I was diagnosed with an abscessed tooth. It gave me no painwhatsoever, so my inclination was to try and treat the infection that I believecould be systemic in my body-which for me, means the tooth is only a symptom.Kyle was very respectful of my views, but showed great concern as well asmonumental patience for me. I repeatedly asked him to explain the entireprocess, almost at times calling upon him to put me through 4 years of dentalschool in a ½ hour explanation. He never faltered in his attempt to care for mydental health, always showing what I would call excellent “chair side manner.”Such is my trust in him, that I will attempt to follow his care to the letter. Thistype of person with such dedication and genuine care for his patients is rareindeed and an excellent reflection upon your facility. Thank you.Sincerely,Lyn ScharretThis issue’s letter was submitted by Dr. Joel Grand24 LEADING EDGE SUMMER 2013

DIVERSITYDental Imprint ProgramExplore a career in dentistry! The Dental ImprintProgram was established in 2009 to serve as a pipelineprogram for local middle school and high schoolstudents interested in the field. The goal of the program is toexpose Detroit-area students to careers in dentistry, while alsopromoting excellence in oral health. Under the direction ofDr. Deirdre Young, over 800 students from public, privateand charter school sectors have been exposed to the healthprofession. Each year, ten schools participate. The program starts with an informational presentation ateach school. Participants are then able to tour the School ofDentistry and observe the gross anatomy lab, while alsopartaking in the simulation lab with hands-on activities. The program is made possible through funding from theU.S. Health Resources and Services Administration, theW.K. In Memoriam of Dr. Joseph IngraoOnce behind the doors at theUHC, a similar sentiment wasechoed through the halls fromthe UDM faculty and staff regarding alate colleague. The man was a “peopleperson, clinic cheerleader, a true Sicilian, and a Rodney Dangerfield fan.” Themissed colleague they were describingwas Dr. Joseph Ingrao.Dr. Ingrao (’57) was an AffiliateAssociate Professor in the Departmentof Patient Management at the Schoolof Dentistry. Known for his infectioussmile and pleasant temperament,Dr. Ingrao practiced in St. Clair Shoresfor 40 years before joining the Schoolin 2000.Dr. Ingrao always madetime for the students, joked with the students, and made everyonearound him feelcomfortable and at ease.Often selected by his students to be the“Faculty Hooder” at commencement,Dr. Ingrao made it a point to seek-outthe students who may have struggledwith a procedure or needed the extrabit of encouragement. Dr. DianeHoelsher, Chair of the Departmentof Patient Management noted that,“Dr. Ingrao always made time for thestudents, joked with the students, andmade everyone around him feelcomfortable and at ease.”The same theme was true in regards tohis interactions with the faculty andstaff. “From the front desk to thecollections office in the very backhallway of the clinic, Dr. Ingraoconnected and checked in witheveryone. He would go out of his wayto make sure everyone was doing ok,”said Dr. Melanie Mayberry.Recently while planning a graduationget-together, it was suggested thatthe event take place at Roma’s Café,a favorite of the late dentist’s. “Wewanted to feel like he was a part of thecelebration,” said Dr. Mayberry. Whatwas originally a small after workouting turned into an opportunity topay homage to Dr. Ingrao’scontributions to the School. His labcoat was turned into a UDM relic, asstudents, faculty and staff signed thegarment. Index cards were passedaround the clinic, while everyone wasasked to write down a word thatdescribed him best. Both the notes andthe lab coat were presented to theevening’s guest of honor, his wife Patricia.Upon leaving the clinic, you had a sensethat although not physically inattendance, his presence was still felton the floor. As a memento from hisclosest friends, a photo hangs by the“instructor’s bench” with the latedoctor’s warm and welcoming smile. The photo serves as a simple reminderthat Dr. Ingrao is still watching over hiscolleagues. In closing, Dr. Steven Faller summedit up with, “Dr. Ingrao was one of themost caring, genuine people I have hadthe good fortune to work with atUniversity of Detroit Mercy Schoolof Dentistry.”This photo of Dr. Ingrao proudly hangs overthe “instructor’s bench,” topDr. Ingrao “hooding” a graduating dentalstudent, bottomWWW.DENTAL.UDMERCY.EDU 29

ALUMNI BOARD PRESIDENT’S LETTERFamily is ForeverDr. Fares Elias ’78My dear friends and colleagues, this ismy last message to you as Presidentof the Alumni Association. At thistime, I would like to thank the Alumni Board, the Dean of the School of Dentistry and thestaff for all of their support. While serving as President of the Alumni Board,I attended numerous School-related functions. However, my greatest pleasure was meeting allof you. Throughout my time on the AlumniBoard, I led many discussions on the centraltheme of loyalty and support for the School. Both words should resonate with everygraduate, new and old.You are and will always be apart of this School. It is yourfamily now. You are part ofthe UDM legacy. Propagateit, support it, nurture it, andrevel in those years and thefriendships you have made. One of the greatest challenges faced, was theever-changing environment of dentistry. Privateand solo practices are shrinking, while corporatedentistry is expanding. Unfortunately students arealso being impacted with larger debt in part dueto diminishing state and federal funds. To staycurrent, universities have had to adapt quickly. Iam proud to say, the School of Dentistry identifiedthe concerns early and implemented changes toaccommodate the current trends in decreasingsupport from the state. Without question, thechanges implemented concerned many of thealumni. However, changes had to be made inorder to survive and that is an indisputable fact.As mentioned earlier, this is the time to showyour loyalty and support for the School. This isthe time to get involved and reach out to localalumni. In concluding my letter, I would like toleave you with a few words from my 2013commencement address:“You are and will always be a part of thisSchool. It is your family now. You are part ofthe UDM legacy. Propagate it, support it,nurture it, and revel in those years and thefriendships you have made.Stand tall and take pride in yourself. Takepride in your alma mater and the educationyou received. It is incomparable. Stay true toyour education, to the principles and codes ofhumanists and healers. Keep an open mind andan open heart, to participate, to share, toquestion, and to stay informed.Be a joiner and a team player. Let your voices beheard individually and collectively within dentalassociations and academies. Sound your viewsconstructively. You owe it to yourself and yourprofession. You also owe it to your alma mater. Our UDM alumni welcome you with openarms. So seek us out. In us you will findlifelong friends and advocates. So take a moment, close your eyes, and retraceyour route. The Alumni Association is herefor you and will always be here for you.You are a member of the UDM familyand family is forever.”Sincerely,Dr. Fares Elias (’78)fares_elias@hotmail.com248-709-1916WWW.DENTAL.UDMERCY.EDU 33
