A.7. Eligible bargaining unit members who have a spouse who is both employed andeligible for coverage with another Consortium member district shall either each obtaina single coverage policy from their respective districts or may select family coveragefrom the district where the employee with the earliest birth date in the year isemployed (i.e., the birthday rule). Neither spouse is eligible to participate in the “optout”provision below.A.8. Eligible bargaining unit members who choose to forgo their right to coverage shallreceive an “opt-out” benefit in the amount of twelve (12%) percent of the annualpremium costs of the most expensive PPO alternative for which they were eligibleand/or twelve (12%) percent of the annual premium costs of dental and/or visioncoverage (according to which coverages the employee chooses to “opt-out”) duringthe month of June of the school year of non-participation. Bargaining unit memberswho forego their right to coverage but are receiving health insurance coverage fromthe Trumbull County Schools Insurance Consortium as part of family coverage are noteligible for the “opt-out”.A.9. An eligible bargaining unit member that has declined the available coverages underthis contract who suffers a significant life change (e.g., divorce, birth of a child, etc)shall thereafter have the right to select and receive coverage from among the PPO’sfor which they are eligible under the previous provisions of this Section and in accordwith the requirements of the insurer. Any “opt-out” payments shall be reducedproportionately to the time that coverage is thereafter extended; and, if anoverpayment has occurred, the bargaining unit member shall remit such overpaymentto the Treasurer within thirty (30) days of extension of coverage to him/her.A.10. If the Consortium adopts health risk assessment programs as provided by andrecommended by the insurance carrier, each bargaining unit member and theirdependents must participate by completing assessment questionnaires and/orparticipate in routine testing. Although bargaining unit members would beencouraged, based upon their assessment/testing results to participate in healthimprovement programs and/or disease management programs as part of this healthcoverage package, they would not be required to do so.A.11. All employees will be required to provide (spouse and child) eligibility documentationas required by the Trumbull County Schools Insurance Consortium.A.12. Bargaining unit members shall be provided dental insurance with a maximum benefitper person each calendar year of $1,500.00 with opt out payment rights as describedabove.A.13. Bargaining unit members shall receive vision insurance comparable to that offered inthe 2006-2007 year with opt out payment rights as described above.25
