These goals reflect the Hospital System's dedication to developing andmaintaining the capacity to improve quality and do so efficiently, effectively, and in a way thatmaximizes the utilization <strong>of</strong> its resources.The Hospital System is working to maintain and improve quality as it assists in DBHDDsstrategic direction toward building community-based services while reducing its dependence onstate hospitals. As the System's hospitals are reduced in size, closed and/or repurposed, it isessential that an effective quality management system is maintained so that those transitions aremanaged in a way that assures the consumers receive the quality <strong>of</strong> service that they deserve. Atthe time <strong>of</strong> this report, the progress, with regard to the identified goals was consistent with theplan.CBH QM Work PlanThe Community BH work plan was discussed at the July 2013 PQC meeting and is currently indraft form. Even though the work plan is still in draft form, the first task <strong>of</strong> the first goal relatedto distribution <strong>of</strong> the PMET to the CBH membership has been met. The first task <strong>of</strong> the secondgoal related to QM training for CBH State and Regional Office staff has begun with thedistribution <strong>of</strong> a memo outlining expectations related to the first QM web based training module.The progress towards the remainder <strong>of</strong> the goals is consistent with the plan. See appendix D forthe CBH QM Work Plan.DD QM Work PlanThe <strong>Developmental</strong> Disabilities system quality management work plan supports the DBHDDQM work plan and addresses the need to ensure that individual with DD who transition out <strong>of</strong>state hospitals receive the highest quality <strong>of</strong> services and achieve their goals once in thecommunity. Progress toward meeting the goals <strong>of</strong> the DD work plan is consistent with the plan’stargeted timelines. See Appendix E.Key Performance Indicators and OutcomesData Collection Plan/Data Definition DocumentA DBHDD data definition document was developed for the KPIs, for use by each <strong>of</strong> the threefunctional QM areas within the <strong>Department</strong>. The data definition document provides guidance onhow each element and attribute should be used. It gives details about the structure <strong>of</strong> theelements and format <strong>of</strong> the data. Additionally this document was used as the basis to develop atool (called the Performance Measure Evaluation Tool) which provides guidance on developingnew and evaluating existing KPIs.DashboardsThe KPI dashboard format was redesigned to incorporate the KPI data in table and graph form,measure definition & explanation, numerator & denominator explanation and an analysis <strong>of</strong> theKPI for the time period. The KPI dashboards can be found in Appendices F, G and H.7
