When you come to register, bring proof of residency, e.g.: a current driver’s licenseor a recent utility bill showing your name and current address.NOTE: Web Registration requires that payment be made in full.Early Bird RegistrationSave with early registration rates through Sunday, 5/19. Regular rates begin on Monday, 5/20.Summer Camp Refund Policy• 90% refund for all requests received and date stamped on or before May 19, 2013.• 75% refund for all requests received and date stamped by the first day of the registered camp session.• All refunds received after the first day of a session are prorated from the time the request is receivedand date stamped by our office; all refunds are assessed a 25% processing fee.Assistance<strong>Park</strong> <strong>District</strong> staff is not capable of providing bathroom assistance to those individuals who cannot care for themselves. If you are in need of assistance from our Maine Niles SpecialRecreation Association please check the Americans with Disabilities Act Need Accommodation box on the <strong>Des</strong> <strong>Plaines</strong> <strong>Park</strong><strong>District</strong> program registration form.Please note that all requests for M-NASR assistance in our programs must be made at least one week in advance of the programstart date. Unfortunately, we may not be able to fulfill requests made with less than one week’s notice.Parents are responsible for informing the <strong>Park</strong> <strong>District</strong> of any physical or emotional limitations that may limit or impair theirchildren’s participation in activities that are conducted in the camp session for which they are registering.Children enrolled in <strong>Park</strong> <strong>District</strong> programs are not released to anyone other than a parent or guardian. Persons desiring adifferent procedure must inform the camp supervisor, in writing, prior to camp.Parent Packets, with detailed information about camps,can be mailed to your house upon request, or you candownload them from our website: DP<strong>Park</strong>s.org<strong>Park</strong> <strong>District</strong> Scholarship InformationProgram scholarships are available for individualsunable to afford program fees. Scholarship requestsshould be submitted at least three weeks prior toall program registration.
