ANIMALS, EXHIBITIONS & PETTING ZOOSAnimals may be allowed on <strong>Park</strong> <strong>District</strong> property if the following conditions are satisfied: (1) proof of insurance and endorsement, (2) proof ofownership, (3) vaccination records, and (4) all documentation and payment must comply with the Fees and Deadlines section on page 8 or late feesand/or denial of the request may result. All petting zoos must be registered with the State of Illinois and certified by the UnitedStates Department of Agriculture. All animal exhibitions must be licensed by the Department of Business Affairs andConsumer Protection. Animals/petting zoos may be added to any <strong>event</strong> <strong>permit</strong> level at a cost of $150. All requests are subject to the approval ofthe <strong>Park</strong> <strong>District</strong> and must comply with section 4-384-010 through 4-384-170 of the <strong>Chicago</strong> Municipal Code.Does your <strong>event</strong> plan to provide animals and/or an exhibition or petting zoo?(Please check NO or YES below. Additionally, the <strong>Park</strong> <strong>District</strong> is not responsible for any costs associated withthe denial of a proposed route.
