Case #32: - Reported to NPDB 08/06/2010Complainant: WV <strong>Board</strong> <strong>of</strong> <strong>Speech</strong> and AudiologyDate: June 24, 2010Name: Lisa Ann Bosley, P/SLP-0225Nature <strong>of</strong> complaint: Discovered through the standard verification process, Ms.Bosley had been practicing without a license since January, 2002. She hadforged license documents and ASHA certification cards since January 4, 2002when her provisional license expired. She did not apply or receive a pr<strong>of</strong>essionallicense to practice <strong>Speech</strong>-<strong>Language</strong> <strong>Pathology</strong> in the State <strong>of</strong> WV.Disciplinary Action: Since Ms. Bosley was practicing without a valid license, theWV <strong>Board</strong> does not have jurisdiction. A Cease and Desist letter was sent to Ms.Bosley on June 25, 2010 and all documents relating to the case were turned overto the WV State Police for investigation.U.S. Attorney’s Office prosecuted. Plea agreement reached – sentenced onApril 27, 2012. 5 Years Probation - $240,000 in restitution to GenesisRehabilitation Corp. @ $50 per month.Case #33 – Reported to NPDB 09/16/2011Complainant: WV <strong>Board</strong> <strong>of</strong> <strong>Speech</strong> and AudiologyNature <strong>of</strong> complaint: Unlicensed provisional (CFY) practiceName: Holly Hitt, SLP-1281The <strong>Board</strong> found Ms. Hitt to be in violation <strong>of</strong> WV Code and WV Code Rules bypracticing without a Provisional SLP License from June 21, 2010 thru September8, 2010.Ms. Hitt is in violation <strong>of</strong> WV Code §§30-32-17 (b)(4), (b)(9) and (b)(13), and WVCode Rule §§29-5-2.6.a.1 and 2.7.b.2Some leniency was given to Ms. Hitt for the following reasons:- Completed her Clinical Fellowship Year in the Kanawha County, WVschool system; WV licensure is not a requirement in the school system- Even though, she advised her employer that she did not have apr<strong>of</strong>essional license and was waiting for her CCC and would thenapply, the employer told her she could see patients and they wouldhave a licensed SLP co-sign her evaluations, treatments, etc.-Disciplinary Action: August 26, 2011 – Consent Decree SignedLicense # SLP-1281 – Letter <strong>of</strong> Reprimand valid for one (1) year and will beexpunged after the designated timeframe has expired. The following additionalrequirements also apply:- Complete a <strong>Board</strong> approved Ethics Seminar/Course in addition to thetwenty (20) Continuing Education Requirements for license renewal in2012. Completion <strong>of</strong> the course will be completed within ninety (90)days.- Write an article regarding experiences with the <strong>West</strong> <strong>Virginia</strong> licensingprocess; to include the importance <strong>of</strong> knowing the Legislative Lawsand Rules that regulate the <strong>Speech</strong>-<strong>Language</strong> <strong>Pathology</strong> pr<strong>of</strong>essionaland the likely repercussions if ignored.- Pay a fine <strong>of</strong> $250.00
