In itsdarkened depths, a pipe suddenly burst. Water flowed<strong>in</strong>to the cl<strong>in</strong>ic and towards the corridor outside. It wouldhave cont<strong>in</strong>ued all night if not for a sharp-eyed staff onhis rounds.Mohd Dali B<strong>in</strong> Khalid has been a staff member at MountElizabeth Hospital for 22 years, work<strong>in</strong>g his way up fromLaundry Attendant to become a ‘Specialist’ <strong>in</strong> clean<strong>in</strong>g.Now Senior Attendant for Environmental Services, hechecks every level of the 18 floors the Hospital occupies.That night Dali was work<strong>in</strong>g the second shift, from2 to 9.30pm. He had completed his stock check and justbegun <strong>in</strong>spect<strong>in</strong>g every floor. “My job is to check all thepublic areas to see that everyth<strong>in</strong>g is <strong>in</strong> work<strong>in</strong>g order. Forexample, I look out for lights that have blown their fuse.I also check the toilets to make sure that noth<strong>in</strong>g is spoilt,and no toilet bowl is choked. If I f<strong>in</strong>d anyth<strong>in</strong>g wrong,I will issue a work order for the ma<strong>in</strong>tenance departmentto fix it,” he says.As Dali passed by one of the closed cl<strong>in</strong>ics, he noticedwater flow<strong>in</strong>g out from under its door. He quickly calledthe rest of his team to help, and made an urgent call tothe plumber. A humandynamo who cycles 20 kilometres to and from work daily,treks <strong>in</strong> Nepal, Tibet and Africa, and backpacks <strong>in</strong> Turkeyand India, there is very little that fazes Mohd Dali.ConnectJanet Sze: Spick and Span FreshnessIf no one notices anyth<strong>in</strong>g amiss, that means Janet Sze’steam has done a good job.And every day, Janet is there first to make sure thatnoth<strong>in</strong>g is out of place, and that there is neither dust,dirt nor mess <strong>in</strong> Mount Elizabeth Hospital.As Senior Operations Assistant, Janet checks everyward and all the public areas. “Cleanl<strong>in</strong>ess is veryimportant,” she emphasises, “especially <strong>in</strong> a hospitalenvironment.” She br<strong>in</strong>gs to bear the housekeep<strong>in</strong>gexperience she has accumulated after several years <strong>in</strong>the hotel <strong>in</strong>dustry. INNOvation14ConnectINNOvation 15ConnectMobilis<strong>in</strong>gStem CellsA newly approved drug is set to changethe way stem cells are collected, spell<strong>in</strong>gnew hope for patients.“You won’t even lose one hair becauseof me and Mozobil” — Dr Teoh“Doctor, did you get enough?” Accord<strong>in</strong>g to haemotologistDr Gerrard Teoh, that was the most frequent question thatpatients would ask him whenever he collected their stemcells for treatments.All that will be a th<strong>in</strong>g of the past with a new drug thatDr Teoh has <strong>in</strong>troduced at Gleneagles Hospital’s Cl<strong>in</strong>ic forBlood Disorders and Research, where he is Medical Director.The hospital is the first <strong>in</strong> Asia to use Mozobil, a drug justapproved by the U.S. Food and Drug Adm<strong>in</strong>istration <strong>in</strong>December 2008.Stem Cell Harvest<strong>in</strong>g TodayTypically, patients with cancers such as multiple myelomaor non-Hodgk<strong>in</strong>’s lymphomas would have their stemcells harvested from their blood or bone marrow beforeundergo<strong>in</strong>g chemotherapy or radiotherapy — treatmentswhich normally destroy stem cells and bone marrow. Theharvested stem cells are later transplanted back <strong>in</strong>to thepatient where they will differentiate <strong>in</strong>to mature red bloodcells, white blood cells and platelets to br<strong>in</strong>g the patientback to health.But there are drawbacks to the exist<strong>in</strong>g methods ofcollect<strong>in</strong>g stem cells. For example, <strong>in</strong> the traditional methodof harvest<strong>in</strong>g stem cells from bone marrow, Dr Teohexpla<strong>in</strong>s, “It requires surgery under General Anaesthesia,and that carries some surgical risk. Not only that, somestudies have shown that bone marrow stem cells don’tfunction as well as peripheral blood stem cells. So there isa trend towards gett<strong>in</strong>g stem cells from peripheral bloodrather than traditional harvest<strong>in</strong>g.”However, the stem cells would first have to be mobilisedfrom the bone marrow <strong>in</strong>to the bloodstream before it can becollected. The exist<strong>in</strong>g methods used for such mobilisationrequire either chemotherapy or a growth factor. But evenafter that, the patient goes through the anxiety of severalrounds of stem cell collection before enough is obta<strong>in</strong>ed fora satisfactory transplant. “The problem is that it takes abouttwo weeks to get enough stem cells for the transplant,”Dr Teoh says.Harvest<strong>in</strong>g with MozobilDr Teoh illustrates how Mozobil makes a difference forsuch patients, “No chemotherapy, no growth factors, it’sjust an unstick<strong>in</strong>g agent which moves stem cells from thebone marrow to the bloodstream.” He is also pleased withthe added benefit. “In three days we had enough stem cellsfor six transplants, which is amaz<strong>in</strong>g! In the past, you couldtry for five days and collect just enough for one transplant.So the efficiency has gone up tremendously.” This easesthe patient’s worry that he would have to endure severalrounds of collection. “It gives the patient confidence thatyou only need to come <strong>in</strong> for a week to get enough stemcells to last for as long as you need,” says Dr Teoh.Beyond Cancer TreatmentDr Teoh extrapolates that Mozobil can be used for otherdiseases <strong>in</strong> future. “Stem cells are cells that can becomeother cells. So they have the potential to make tissue forrepair, whether the tissue has been damaged by cancer,disease or <strong>in</strong>jury; even for an <strong>in</strong>jured immune system. Wecall it ‘differentiation’ where we take the stem cell and makeit become someth<strong>in</strong>g different like blood cells or immunesystem cells, or others. The future for stem cells is great.“We want to extend the stem cell mobilisation to noncancerpatients, and make it available for other diseasesand conditions such as stroke, orthopaedic <strong>in</strong>jury, heartattacks and sp<strong>in</strong>e <strong>in</strong>jury.” This is where Dr Teoh hopeseven patients without cancer can potentially benefit fromMozobil as it allows stem cells to be mobilised from thebone marrow without chemotherapy or growth factors.“You obviously cannot give chemotherapy to healthypeople. And there have been records of growth factors<strong>in</strong>duc<strong>in</strong>g cancer when given to healthy people, so there’sa little danger <strong>in</strong>volved if non-cancer patients were toreceive growth factors for mobilisation.”The Future With<strong>in</strong> SightIn anticipation of the day when even non-cancer patientswould harvest stem cells to cure other diseases, Dr Teoh isconduct<strong>in</strong>g his current study on Mozobil with “a specialpatient”, as he calls him. The patient was prepared for thisday from the time he began treatment for cancer.“When we first heard about Mozobil be<strong>in</strong>g developed,we had the idea to create a cl<strong>in</strong>ical trial by treat<strong>in</strong>g thispatient’s cancer without chemotherapy, so as to preserveall his stem cells. Once the patient was <strong>in</strong> remission andMozobil became available, we would then give him Mozobiland collect his stem cells. He has s<strong>in</strong>ce been given Mozobil.The stem cells we collected from him were found to be asgood as a healthy person’s. So we can extrapolate his case tothat of a healthy person’s. This means Mozobil can be usedfor healthy people.”The results have been promis<strong>in</strong>g. “The patient isdelighted. His doctor back home said it would have taken15 to 28 days to collect so much stem cells, yet we did it <strong>in</strong>three days!” says Dr Teoh.Dr Teoh is currently conduct<strong>in</strong>g his study on the secondpatient and assures, “You won’t even lose one hair becauseof me and Mozobil.”

We talkto Mrs Kang Kee Chee and Ms Tang Yuet Lan about its early beg<strong>in</strong>n<strong>in</strong>gs.Some of the older doctors are now <strong>in</strong> private practice.Whenever they come back to visit, they still call out‘Uncle Sham, you are the best!’How it All BeganBefore Mount Elizabeth Hospital became one of Asia’smost renowned healthcare and medical centres, it startedlife, as all build<strong>in</strong>gs do, on paper. The hospital had beenconceived as one that would cater to a large foreign,notably Indonesian, clientele. Its location — at that time,it was the only hospital located near the Central Bus<strong>in</strong>essDistrict — would see to its popularity with expats and wellheeledS<strong>in</strong>gaporeans alike.Construction of the hospital began <strong>in</strong> earnest <strong>in</strong> early1978 and it was a process that <strong>in</strong>volved not just topmanagement, but also staff like Mrs Kang Kee Chee, whowas the hospital’s first employee. The spry 68-year-old,Mrs Kang Kee Chee

It is stated <strong>in</strong> the sales contract thatthey are never to be removed.who had earlier expressed astonishment that someone<strong>in</strong> the hospital still recognised her after 30 years, laughsas she recalls her first day at work. “I started on the firstof April and I remember I was so shocked that I was theonly one start<strong>in</strong>g work that day. I thought it was an AprilFool’s joke!”Build<strong>in</strong>g from ScratchFor Mrs Kang, who had been hired as an Assistant Matron,and the nurs<strong>in</strong>g team, be<strong>in</strong>g <strong>in</strong>volved <strong>in</strong> the plann<strong>in</strong>g ofthe hospital meant more than just attend<strong>in</strong>g meet<strong>in</strong>gs <strong>in</strong>an air-conditioned office. At the time when she jo<strong>in</strong>ed, thehospital was still <strong>in</strong> its plann<strong>in</strong>g stage. “We used to havemeet<strong>in</strong>gs with the architects and hospital consultants togive them our <strong>in</strong>put. Although there was already a plan,we had to let them know our requirements as well. So onepart of our duty was to study the workflow and the plansand see what was required <strong>in</strong> the rooms, such as howmuch space was required for the bed, locker and chair <strong>in</strong> aroom. Initially our office was at Well<strong>in</strong>gton Build<strong>in</strong>g. Later,the office was shifted onsite and we were go<strong>in</strong>g aroundwear<strong>in</strong>g helmets and check<strong>in</strong>g the services.”Workflow and plann<strong>in</strong>g aside, the other aspect of theirjob <strong>in</strong>volved study<strong>in</strong>g the furniture and medical equipmentsuch as suction mach<strong>in</strong>es and X-rays. This would pose oneof their biggest challenges. “Their delivery had to co<strong>in</strong>cidewith the construction schedule. If they arrived too early,there wouldn’t be any place to put them. If they arrivedtoo late, they might not fit <strong>in</strong>to the door. So this plann<strong>in</strong>grequired a lot of our time. We had to study the manualsand liaise with suppliers to make sure they knew whatwe wanted. We gave them our specifications — like theheight and width — and asked the suppliers to put thesespecifications <strong>in</strong> their tender. We were very prim andproper; we didn’t favour one company over another.”Another concern was staff<strong>in</strong>g. “We thought s<strong>in</strong>ce thehospital was so big (there were 250 beds), we wouldn’tbe able to get all the staff that we would need. But wewere lucky <strong>in</strong> the sense that when we were ready to open,there were a lot of people com<strong>in</strong>g back from abroad. WeMs Tang Yuet Lan

There is also a checklist togo through before surgery the way a pilot would do beforetake-off. Through countless hours of data collation,nurtur<strong>in</strong>g of supplier relationships, creativity and a lotof hard work, they will save over $570,000 <strong>in</strong> 2009 — theequivalent of $47,000 <strong>in</strong> sav<strong>in</strong>gseach month.Chian Huey credits strongpharmacy-supplier relationshipsas key <strong>in</strong> ma<strong>in</strong>ta<strong>in</strong><strong>in</strong>g availabilityand affordability of medications.“We work really hard to buildgood relationships with oursuppliers — and vice versa,” saysChian Huey.The hospital-supplier relationship be<strong>in</strong>g a mutuallybeneficial one — suppliers depend<strong>in</strong>g on the bus<strong>in</strong>ess ofhospitals like <strong>Parkway</strong>’s and <strong>Parkway</strong> on suppliers forpharmaceuticals — cooperation not only makes sense butis also crucial for survival. This is especially true <strong>in</strong> timesof economic hardship, with both sides rely<strong>in</strong>g more andmore on the “goodwill”, as Chian Huey puts it, of the other.Accord<strong>in</strong>g to Chian Huey, this show of goodwill goes asfar as suppliers work<strong>in</strong>g around <strong>in</strong>dustry practices, such asannual drug cost <strong>in</strong>creases, to aid hospitals.“Traditionally, [the price of] pharmaceuticals goesup every year — partly because of proprietary drugswhich are prohibited from be<strong>in</strong>g substituted with lessexpensive (generic) drug alternatives,” expla<strong>in</strong>s ChianHuey. “Although this year has been especially difficult forus, most suppliers have been very supportive, especiallyconsider<strong>in</strong>g <strong>in</strong>dustry practice.”Such support doesn’t come easy and is the result ofthe tireless dedication of people like Chian Huey, SookHan and Choon Yuen to develop mean<strong>in</strong>gful relationshipswith suppliers.So how do purchas<strong>in</strong>g pharmacists deal with theoverwhelm<strong>in</strong>g task of cutt<strong>in</strong>g costs while still ma<strong>in</strong>ta<strong>in</strong><strong>in</strong>gavailability and affordability of medications?“We all know it has to be done, so we set goals forourselves and do our best to meet them. It may seemimpossible at first but youhave to believe you can doit and be will<strong>in</strong>g to exploreany and all avenues,” admitsChian Huey.And what motivates themto keep go<strong>in</strong>g?“There are three importantgroups of people we arealways th<strong>in</strong>k<strong>in</strong>g about – ourpatients, doctors and shareholders. For our patients anddoctors, we try our hardest to get them exactly whatthey need when they need it, while still keep<strong>in</strong>g priceslow. At the same time, we must keep our bottom-l<strong>in</strong>ef<strong>in</strong>ancially viable for shareholders. It’s a difficult balanceto ma<strong>in</strong>ta<strong>in</strong>, but there’s much satisfaction <strong>in</strong> be<strong>in</strong>gable to make everyone happy.”The cost-sav<strong>in</strong>g achievements of ChianHuey, Sook Han and Choon Yuen are proofthat even the smallest of efforts or, <strong>in</strong>this case, the smallest of teams, cangreatly impact the whole.
