Board certified physicians and allied providers have participated in a voluntary process that involves
evaluation of their knowledge and skills beyond what is required for them to become licensed
physicians. The standards for board certification, such as the type of evaluation, and whether
additional education and training are required, vary depending on the certifying board.
The BC symbol in this directory refers to specialties recognized by one of the following organizations:
• Academy of Chiropractic Orthopedists (ACO)
• Advisory Board of Medical Specialties (ABMS)
• American Association of Physician Specialists (AAPS)
• American Board of Optometry (ABO)
may undertake after completing a specialty training program, are known as fellows. Fellows are
capable of acting as an attending physician or consultant physician in the generalist field in which
they were trained. After completing a fellowship in the relevant sub-specialty, the physician is
permitted to practice without direct supervision by other physicians in that sub-specialty.
Physicians or allied providers that have passed evaluations specific to an area of expertise but are
not represented by one of the organizations listed above.
Partners in the Independent Practice
of Patient-Centered Care
Physicians’ Health Network is an affiliation of physicians and allied health
professionals serving Sheboygan, Calumet, Fond du Lac, Manitowoc, and Ozaukee
Counties. Since its inception in 1988, PHN has grown to include over 150 independent,
local primary and specialty care physicians and other healthcare professionals.
PHN emphasizes primary care—family practice, internal medicine, pediatrics, and
obstetrics and gynecology. PHN providers believe that the best medical care depends
on the fundamental bond developed between patient and doctor. This relationship
allows PHN professionals to know and meet the needs of patients as unique
individuals, as whole persons.
PHN is committed to maintaining the highest standards of practice and is working
continuously to improve quality. On-going quality activities include credentialing,
physician and staff education, consumer surveys, and the development of improved
information systems.
PHN offers medical practice that works—emphasizing preventive medicine and
patient education, optimizing appropriate referrals to other specialties of medicine,
and utilizing the best acute care and tertiary care facilities and resources available.
Mission
Faithful to the spirit of the Hospital
Sisters of St. Francis, St. Nicholas Hospital
is dedicated to suffering humanity
for His sake.
s T . n i c H o l a s H o s p i T a l
St. Nicholas Hospital is a full-service community hospital committed to providing
patients a choice for their healthcare provider. The Hospital understands the needs
of the community. Programs and services are built to serve those needs while staying
true to our founding Sisters’ mission, vision, and values.
We continue to expand our services through innovative thinking and technology to
provide high quality, family-centered care to all our patients. We stay on the cusp
of high-tech in order to provide “big city” care close to home. We upgrade our
technology in a way that is fiscally sound to keep healthcare costs down.
St. Nicholas Hospital provides on-going treatment for acute illness, as well as
numerous other inpatient and outpatient health services. We wisely invest in our
facility and bring new equipment to the community. Our care delivery models bring
our care out into the community to make it more convenient for our patients.
We have the only CARF-accredited inpatient rehabilitation unit in Sheboygan
County so you can return to your community for intensive rehabilitation care close
to your family and friends. The St. Nicholas Hospital Comprehensive Wound Care
Center uses advanced treatment techniques to help heal wounds when wounds won’t
heal. Two hyperbaric oxygen chambers make this a full-service center of healing.
We have a well earned reputation for quality healthcare and are known for
displaying concern for the total needs of the community.
Our mission of healing and philosophy of caring continue to revolve around treating
the whole person – spiritually, physically, and emotionally.
This deep connection to the community makes choosing St. Nicholas Hospital –
The Right Choice.
Vision
We will bring Christ’s healing presence
to all who come to us by providing
family-centered, compassionate care
without regard for race, creed,
or ability to pay.
31
Values
Our core values of respect, care,
competence, and joy will be lived
by all who work here and felt by all
who use our services.
For more than 115 years, St. Nicholas Hospital has
represented innovation, vision, and commitment to the
health and well-being of Sheboygan County.
Four members of the Hospital Sisters of St. Francis
arrived in Sheboygan in the spring of 1890, and the first
hospital began a short time later when the Sisters founded
St. Nicholas Hospital in a two-story building built for
them by the community. St. Nicholas Hospital continues
to be a healthcare leader in this community and is the
right choice for your healthcare needs.
The Hospital has always been committed to investing in
its facilities and equipment to provide the highest quality
services with modern technology at a reasonable cost.
St. Nicholas Hospital and its physician partners invest
wisely in innovative solutions:
• Our emergency physicians are all board-certified in
Emergency Medicine.
• A 16-slice CT scanner which is able to perform
cutting-edge tests such as noninvasive cardiac imaging.
• Unique diagnostic technology combining Positron
Emission Tomography (PET) and Computed
Tomography (CT) technologies into one and used to
determine the location and extent of tumor growth.
This addition of PET/CT equipment eliminates the
need for patients to travel to Milwaukee.
• The introduction of progressive orthopedic solutions
such as Birmingham hip resurfacing and a minimally
invasive hip replacement surgery.
• The area’s most advanced magnetic resonance
imaging (MRI), the Signa HDx 1.5T, provides
accelerated scanning and consistent high quality
images for body, vascular, cardiac, neuro, and breast.
• Use of the intra-aortic balloon pump for the
treatment of heart disease.
• Holmium laser for the treatment of kidney stones.
• Timely administration of “clot-busting” medication
to patients in the Emergency Department to reduce
damage caused by heart attacks and strokes.
• A Chest Pain Evaluation Center to better evaluate—
and treat—patients with all types of chest pain.
• A Hospital-based radiation therapy program,
improving outcomes for patients with cancer.
St. Nicholas Hospital also has a partnership with the
s T . n i c H o l a s H o s p i T a l
Make the Right Choice
32
Northeast Wisconsin Regional Cancer Collaborative
for ground-breaking cancer research and results.
• A renal dialysis unit, which saves patients traveling
to Milwaukee several times a week to receive
lifesaving treatments.
• The Center for Pain and Work Rehabilitation,
providing relief for people with chronic pain.
• A Hospital-based angiography laboratory, providing
cardiologists and radiologists with a diagnostic tool
for the most timely and accurate detection of heart
and vascular disease.
• St. Nicholas Hospital has the only CARF-accredited
inpatient rehabilitation unit in Sheboygan County
so patients can return back to the community for
intensive rehabilitation care close to family and friends.
• St. Nicholas Hospital uses SpyGlass, an innovative
system which is a fiber optic probe that attaches to a
camera and provides direct visual access into a
patient’s biliary duct to improve diagnosis by helping
to identify stones, strictures or mass causing
obstructions of the bile duct.
• St. Nicholas Hospital also acquired three, GE
LOGIQ E9, a high-tech ultrasound system using
agile ultrasound and featuring 3D/4D imaging.
• An addition of two hyperbaric oxygen chambers to
the Comprehensive Wound Care Center. Hyperbaric
oxygen has been scientifically proven to aid in healing
wounds that may otherwise lead to amputation.
While our preference is to help you stay healthy, our
mission is to ensure that when you need healthcare you’ll
choose St. Nicholas Hospital, the area’s innovative leader in
healthcare excellence. St. Nicholas Hospital is committed in
assuring that all its programs and services are of the highest
quality. As a strategic initiative, the Hospital incorporates a
wide variety of performance improvement methodologies
through its operations, constantly evaluating the efficacy
of its work.
External benchmarking efforts include participation in a
number of comparative databases, including the Maryland
Hospital Association Quality Indicator Project ® . With
more than 1,700 participants, the Project is the largest
comparative analysis database of its type in the United
States. Project indicators measure facility-level clinical
outcomes and focus on areas of care that are universally
important to healthcare organizations (e.g., mortality rates,
unscheduled readmissions, use of restraints, infection rates).
The Project contains data from more than 20 percent of all
U.S. hospitals.
St. Nicholas Hospital also participates with the American
Heart Association in its “Get with the Guidelines” program
for continuous quality improvement for prevention of
coronary artery disease. St. Nicholas Hospital has achieved
Get with the Guidelines – Coronary Artery Disease Level
1 Hospital recognition for its efforts in implementing
standing orders and protocols that are in accordance with
the current Secondary Prevention Guidelines for Coronary
Artery Disease.
Additional external benchmarks include data compilation
and comparison and improvement initiatives from the
Wisconsin Hospital Association’s CheckPoint Quality
initiative, and the American Hospital Association’s
Hospital Quality Alliance and the Institute for Healthcare
Improvement (IHI).
The Hospital also utilizes internal benchmarks in its
ongoing efforts to monitor and improve its quality. Patient
satisfaction studies are regularly conducted projects for all
inpatient admissions as well as for outpatient services, the
Emergency Department, Ambulatory Surgery, and the
Comprehensive Rehabilitation Inpatient Unit. The
Hospital also conducts Consumer Surveys, Patient Waiting
Time Studies, Physician Credentialing, and Physician Peer
Review Analysis. The Hospital’s Performance Improvement
and Quality Care Committees and the Board of Directors
receive reports from these data and trends benchmarking
efforts for review and action when appropriate.
Choosing St. Nicholas Hospital means that you will be
provided with compassionate, quality care with state of the
art technology, a full continuum of programs and services,
and highly trained healthcare professionals skilled in all of
their specialty areas. St. Nicholas Hospital continues to be
founding Sisters over a century ago. It continues under the
sole sponsorship of the Hospital Sisters of St. Francis as
one of 13 hospitals in Illinois and Wisconsin affiliated with
Hospital Sisters Health System. Its mission of healing and
philosophy of caring continues to revolve around treating
the whole individual spiritually and emotionally as well
as physically.
St. Nicholas Hospital is a Catholic non-profit, acute care,
The Anesthesiology Department uses state of the art
equipment to provide a complete spectrum of care
including general, spinal, epidural, and regional nerve
blocks. The department is also involved in the management
of acute and chronic pain through the Pain Management
Clinic (see page 43). The department is comprised of
board-certified anesthesiologists (see Surgery, page 46).
Cancer Services
St. Nicholas Hospital offers patients battling cancer and
their families a source of strength, care, support, and hope
through comprehensive, integrated oncology (cancer)
services. The cancer care program has been approved by
the Commission on Cancer of the American College of
Surgeons. The Hospital is a member of the Association
of Community Cancer Centers, the leading national
interdisciplinary organization that defines quality care for
patients with cancer and influences change to continually
improve oncology care. In addition, St. Nicholas Hospital
participates in the Regional Cancer Collaborative, a
cooperative effort that brings together top medical
specialists, the latest medical technology and highly-
integrated treatment options. For specialized treatments,
patients may be referred to St. Vincent Hospital, Green Bay,
which serves as the base of the Collaborative, providing highend,
high-tech services not available in every community.
The oncology care team includes board-certified medical
and radiation oncologists, oncology-certified nurses,
radiation therapists, dietitians, spiritual and pastoral
support staff, and other highly skilled individuals from a
variety of specialized fields of medicine needed to treat the
34
complex disease processes of cancer. The team works with
the patient and family, the family physician, and doctors
specializing in surgery, pathology, and radiation and
medical oncology to provide complete care.
Radiation therapy is often an important facet of cancer
care on either an inpatient or outpatient basis. With a
state-of-the-art, dual-energy linear accelerator, highlytrained
radiation therapists are able to treat cancer patients
with both photons and electrons, as well as with IMRT
(Intensity Modulated Radiation Therapy). IMRT is another
form of radiation therapy that uses three-dimensional
imaging and a specialized treatment planning technique to
target cancer cells with high doses of radiation. While at
the same time minimizing the radiation received by nearby
healthy tissue.
Radiation therapy destroys tumors or reduces their size
through exposure to high doses of ionizing radiation which
kills or damages tumor cells. The therapy is usually
delivered in a series of small treatment doses over a period
of several weeks to allow healthy tissue surrounding the
tumor the opportunity to repair itself between doses. In the
case of prostate cancer, brachytherapy, or radioactive seed
implantation, offers a therapeutic treatment option.
Radiation therapy can be used for curative purposes in
combination with surgery, chemotherapy, or by itself. It
can also be used in a palliative manner to control bleeding
or pain or be used to shrink tumors, making them easier to
remove surgically. Services are provided by board-certified
radiation oncologists and registered radiation therapists
and physicists.
Chemotherapy, which is also given on an inpatient or
outpatient basis, is the use of drugs to treat cancer. These
drugs destroy cancer cells by stopping them from growing
or multiplying at one or more points in their cell cycle.
Because some drugs work better together than alone,
chemotherapy may consist of more than one drug. This is
called combination chemotherapy. Chemotherapy may be
used in combination with surgery and/or radiation therapy
and may be palliative or curative. Registered nurses who
administer chemotherapy complete the Oncology Nursing
Society’s Chemotherapy and Biotherapy Course. This
specialized education and preparation validates that the
nurses have the knowledge needed to provide a safe level
of care for the individuals receiving these agents.
Prevention and early detection of cancer are encouraged
through a variety of programs such as skin cancer
screenings and the Wisconsin Well Woman Program,
which provides free mammography for under-insured or
uninsured women.
A lending library of books and videos, support groups
(see page 52), and other services provide other support for
patients and families. In addition, wigs, hats, turbans, and
other head wear items are available for women who
experience hair loss as a result of cancer treatment.
Medical Director: H. Marshall Matthews, MD
Cardiac Services
St. Nicholas Hospital Cardiac Services offer comprehensive
preventive, diagnostic, and primary care for patients
with all types of heart disease. Care is tailored to each
individual’s needs and is provided by a multidisciplinary
team of physicians, nurses, therapists, dietitians, and other
healthcare professionals.
In a state-of-the-art Chest Pain Evaluation Center,
individuals experiencing chest pain are quickly evaluated
for appropriate treatment which may include thrombolytics
(clot busters) to minimize damage to the heart.
Some individuals may be referred for diagnostic procedures
such as stress tests, transesophageal echocardiograms,
electrophysiology testing, tilt table studies, continuous
blood pressure monitoring, or cardiac catheterization. Such
tests allow physicians to identify heart rhythm aberrations
and determine valve function and the condition of heart
muscle and arteries.
Partnerships with tertiary care facilities (see page 50)
provide an option for seamless transition to advanced
cardiovascular medicine, cardiac and vascular surgery, and
vascular radiology care for patients who need these services.
A Cardiac Rehabilitation program assists in recovery
(see page 46).
Medical Director: Louie Coulis, MD
Care Management and Social Services
Care management focuses on outcomes to enhance the
quality of patient care through effective and efficient use of
the Hospital’s resources. Strategies utilized include guidelines,
protocols, standardized order sets, patient education,
communication with insurance companies to certify the
medical necessity of hospital care, and discharge planning.
Care management services are available to assist patients
and their families as they deal with problems which often
accompany illness. Services may include counseling with
the patient and family in areas such as financial problems,
marriage or family problems arising out of the patient’s
illness, and discharge planning. Staff assists patients and
families with arranging services (i.e., Lifeline, nursing
home, and homecare nursing) which may be necessary
before or after discharge either at the patient’s home or at
another facility.
Care Management services are provided by social workers
with bachelor’s or master’s degrees in social service and
registered nurses with specialized education and training in
discharge planning and utilization management. Services
Central Scheduling
Many hospital visits are comprised of a combination of
services, a number of which can be scheduled with one call
from your physician to the St. Nicholas Hospital Central
Scheduling Department. Following the call, department
staff will make all the arrangements necessary for your visit.
Community Education
Helping you maintain good health is of the utmost
importance to St. Nicholas Hospital. With that in mind,
our Community Education Department presents a variety
of health education classes and outreach activities which
help you learn more about health. From blood pressure
screenings to stroke awareness, from summer sports fitness
to winter health hazards, St. Nicholas Hospital wants to
make sure you have the best information available to you.
Programs are conducted by physicians, nurses, therapists,
and other Hospital staff. Many are offered free or at a
minimal charge. The HealthExpress van travels to
community events, providing health information and
screenings to the public. Health fairs familiarize the public
with Hospital services and health issues. Child safety seat
clinics and multi-sport and extreme skateboard helmet
fittings are offered both on-site and in community locations.
Comprehensive Wound Care Center
Outpatient wound services are offered in the newly
renovated Comprehensive Wound Care Center. The Center
The Center offers specialized care for leg and foot ulcers
and other wounds often caused by diabetes, poor
circulation, and unhealed surgical incisions. A wound
care specialist and trained nursing staff provide patients’
wound care and work with the physician to coordinate
their multidisciplinary care needs.
The Center and the St. Nicholas Hospital Home Health
Department have also collaborated to provide a wound
telehealth program in managing and healing wounds of
home health clients (see Home Health Services, page 39).
The Center also recently added two hyperbaric oxygen
chambers to the Comprehensive Wound Care Center. As
the only facility to offer hyperbaric oxygen therapy in
Sheboygan County, residents now have access to state of
the art wound care for acute and chronic wounds.
The Center has added advanced wound care and hyperbaric
medicine trained physicians from Infinity Healthcare.
Wound care services include:
• Assessment Following a comprehensive examination
and detailed workup for wound healing risk factors,
a comprehensive treatment plan is developed.
• Photo Documentation As part of the ongoing
evaluation process, digital photographs are used to
document the progress of healing.
• Treatment Plan A comprehensive treatment plan is
developed for each patient. Plans may include
medications, dressings, and patient education.
• Follow-up Care Progress and treatment needs are
monitored and assessed throughout wound healing.
The Wound Care team works with the patients’
primary physician to develop an evidence-based
clinical plan to promote healing.
Dialysis Services
Patients with end-stage renal disease can benefit from
Dialysis Services at St. Nicholas Hospital. Individuals who
do not yet require dialysis treatment are eligible for
“preteaching,” which assists patients and their families
with treatment choices. Twelve hemodialysis stations are
available weekdays and Saturdays as well as some evenings
for patients requiring dialysis treatments. A physician and
nurse are available for emergency treatments outside of the
regularly scheduled hours.
Fistula and peritoneal catheter placement is done on an
outpatient basis and training for continuous ambulatory
peritoneal dialysis and continuous cycling peritoneal
dialysis is given to patients who have shown a desire and
strictures, remove scar tissue, and resolve obstructions.
Home Health Services
Home Health is a state-licensed, Medicare certified, and
Joint Commission accredited department of St. Nicholas
Hospital. Home Health offers services to individuals in
their home who require the skills of a nurse or therapist, are
homebound, and are under the care of a physician. Home
Health services focus on improving the health and
independence of those in the community while allowing
individuals to receive healthcare services in the comfort of
their own home. Home Health offers a variety of services
at a significant cost savings and Home Health is covered by
Medicare, Medicaid and most insurance plans.
Home Health is available across the life span for individuals
recovering from a serious illness or injury to those suffering
from a chronic condition. From newborns with sleep
apnea to an adolescent or adult diagnosed with diabetes,
or an elderly person recovering after a fractured hip, just
to name a few, home health is available to assist individuals
in maintaining or returning to an optimal level of health
and functioning.
The St. Nicholas Hospital Home Health Department is
also the first home care agency in Sheboygan County to
offer electronic and digital oversight of wound care patients
by a satellite site and credentialed specialists.
Telehealth is the delivery of health-related services and
information via telecommunications technologies. It is as
simple as two health professionals discussing a case over the
telephone, or as sophisticated as using videoconferencing
between providers at facilities in two countries.
Telehealth encompasses preventive, promotive and curative
aspects. The format of telehealth that St. Nicholas Hospital
will employ is called store-and-forward telehealth. Digital
images, video, audio and clinical data are captured and
“stored” on the field nurse’s computer; then transmitted
securely to the St. Nicholas Hospital Comprehensive
Wound Care Center (see page 36) where they are studied
by specialists and their input is provided.
Home Health services through St. Nicholas Hospital Home
Health and Hospice are available within a 30 mile radius
of St. Nicholas Hospital. Referrals to Home Health can be
made by physicians, families, or a prospective client him/
herself. A specialized home health Liaison will work with
individuals to determine eligibility and arrange for services.
Care is available on an intermittent basis, 24 hours a day,
seven days a week and a Registered Nurse is only a phone
call away around the clock for those admitted into services.
Home Health services are supported in part by the United
Way and in keeping with the mission, vision, and values of
St. Nicholas Hospital; no one will be turned away regardless
of ability to pay.
Hospice Services
The Hospice program at St. Nicholas Hospital Home
Health and Hospice is also a state-licensed, Medicare
certified, and Joint Commission accredited program that
offers care to those individuals in our community that are
facing a life limiting illness. Hospice services are available to
anyone who has been diagnosed with a terminal illness, has
a limited life expectancy and are no longer seeking curative
treatment. Hospice services are appropriate for those
suffering from cancer, end stage cardiac or respiratory
conditions, AIDS, end stage renal disease, and other
illnesses that are life limiting.
Hospice recognizes dying as part of the normal process of
living and focuses on maintaining the quality of life
remaining. Hospice affirms life and neither hastens nor
postpones death. It exists in the hope and belief that,
through appropriate care and the promotion of a caring
community sensitive to personal needs, patients and their
families may be free to attain a degree of mental and
spiritual preparation for death that is satisfactory to them.
The Hospice team, consisting of the patient’s own physician,
a Medical Director, nurses, social workers, counselors and
spiritual care representative, works with the patient, family
and caregivers to develop a plan of care that meets the
needs of everyone involved. Hospice care seeks to relieve
symptoms, provide emotional support, and most
importantly, allow a person to live their remaining days
with dignity in the setting of their choosing. Hospice is
provided wherever the patient calls home.
Palliative Care
The Palliative Care program at St. Nicholas Home Health
and Hospice is available to those who have been diagnosed
with a life limiting or chronic illness but are still seeking
s T . n i c H o l a s H o s p i T a l s e r V i c e s
40
curative treatment and may have a life expectancy that is
greater than six months. Palliative Care focuses on symptom
management and retention of independence with the goal
of preventing re-hospitalization. Palliative Care services
include the availability of nursing visits, home health aide
visits for personal care, social worker consultation for future
planning or assistance in accessing community resources,
pastoral support, homemaking services and volunteer
services. Palliative Care is typically a private pay service as
it is not reimburseable by Medicare, Medicaid, or most
private insurances. A member of the Palliative Care team is
available for consultation to discuss payment arrangements
and services available.
Intensive/Cardiac Care Unit (ICCU)
The Intensive/Cardiac Care Unit is designed for patients
who require very close observation and intensive nursing
care around-the-clock. All staff members are certified in
advanced cardiac life support; some are also certified in
critical care nursing, and pediatric advanced life support.
Intensive monitoring is provided for all patients so that staff
may quickly assess and respond to any changes. In addition,
other forms of hemodynamic monitoring may be utilized,
depending on the patient’s condition. Special procedures such
as medical and electrical cardioversions, tilt table studies,
intra-aortic balloon pumps, temporary pacemakers, and
transesophageal echocardiograms are performed in ICCU.
Patients who are having cardiac catheterization and
electrophysiology studies are also cared for here.
easy to use.
Pain Management
Center for Pain and Work Rehabilitation
The Center for Pain and Work Rehabilitation is the area’s
most comprehensive source for the management of chronic
pain. Accredited by the Commission on Accreditation of
Rehabilitation Facilities (CARF), the Center’s goal is to
improve the quality of life for those who suffer from pain
and help them return to a productive life at work or their
homes. The Center works in cooperation with physicians
and other services of the Hospital, including the Interventional
Pain Clinic and Occupational Health Services
(see page 41).
The goals of the program include:
• Using medications appropriately
• Establishing normal sleep patterns
• Decreasing depression and anxiety
• Increasing strength, endurance, and flexibility
• Resuming former activities
The Center for Pain and Work Rehabilitation draws on
the expertise of all of the rehabilitation disciplines at
the Hospital.
Medical Director: Sridhar V. Vasudevan, MD
Interventional Pain Clinic
Pain management is an important part of patient care. In
fact, up to 80% of all health insurance claims are related to
pain, particularly back pain. However, today, more methods
of treating pain are available than ever before, and, under
the care of a qualified physician, almost every patient’s pain
can be treated effectively. This includes sufferers of chronic
back pain and cancer-related pain.
Board-certified anesthesiologists at St. Nicholas Hospital
manage aspects of both acute and/or chronic pain. The
Palliative Care Services
Being faced with a serious, chronic, or life limiting illness
can affect you or your loved one in many ways. St. Nicholas
Hospital offers a Palliative Care Program that can assist you
or your loved one in dealing with a serious, chronic, or life
limiting illness by:
• Addressing your unique symptom management needs.
• Focusing on improving your quality of life.
• Managing your symptoms to keep you at home, resulting
in fewer hospital visits.
The philosophy of palliative care is to focus on quality of
life issues, symptom management, communication, and
emotional and spiritual wellness while continuing some
form of therapy for the chronic illness itself. Treatment
decisions are made in the context of maximizing quality of
life while understanding that the ultimate disease outcome
may not change.
The Palliative Care Team, an interdisciplinary group of
healthcare professionals, works with the patient’s primary
care physician to establish a plan of care based on the
patient’s individual needs. The patient and family are
always a part of the care planning process through regular
scheduled family meetings.
Palliative Care does not replace active treatment of your
disease nor is it hospice. Palliative Care is beneficial at any
stage of illness.
Palliative Care is a proven means to meet the needs of
those suffering from serious, chronic, or life limiting
illness. Some of the ways that Palliative Care can meet
your needs include: Treatment of complex symptoms
including but not limited to:
• Pain • Insomnia
• Nausea • Anxiety
• Constipation • Depression
Palliative Care also:
• Facilitates communication between the patient,
family members, and staff about treatment.
• Provides spiritual and emotional support to patients
and families.
• Addresses concerns as the patient’s condition changes.
• Assists with hospital discharge and continuation of
Palliative Care Services in the home setting.
Who Could Benefit from Palliative Care?
• Those with serious, chronic or life limiting illness that
are having difficulty with symptom management.
• Those experiencing recurrent hospital admissions.
• Those with changing care goals.
Your physician or Hospital staff may suggest Palliative
Care for you. You may also inquire about it yourself and a
staff member will meet with you to discuss your questions
and determine if you or your loved one qualify for
Palliative Care.
Pediatric Services
Having a child in the hospital is never easy. However, our
Pediatric Services staff tries to make this as stress free for
both patient and family as possible. Parents are encouraged
to spend as much time in their child’s room as they wish
and to become part of their child’s patient care team.
Beds for parents are available for overnight stays.
Pharmaceutical Services (Inpatient)
Medications are dispensed to St. Nicholas Hospital patients
after a registered pharmacist reviews each medication order
for accuracy and safety, including correct dose for age and
status of illnesses,
potential drug
interactions, allergy
screening, therapeutic
duplication, and
appropriate directions
for administration.
Medications are stored
in and dispensed from
an automated Pyxis
system which ensures
accuracy and enhances
timeliness of medication distribution. Sterile injectable
medications and intravenous solutions (IVs) are prepared
by skilled technicians under the supervision of a pharmacist.
The pharmacist assists physicians by continuously screening
medication profiles and associated laboratory results,
providing recommendations as indicated. Upon physician
order, drug therapy responses are monitored for antibiotics
and anticoagulants. Pharmacists team with dietitians to
recommend formulas for optimal intravenous nutrition
when needed. Pharmacists respond to medication questions
utilizing up-to-date drug information resources. A
pharmacist is available for consultation 24 hours a day.
Rehabilitation Services
Often after illness or injury, an individual finds him/herself
in need of some form of rehabilitation services on either an
inpatient or outpatient basis. The goal of rehabilitation is
to improve the functional capabilities of the patient so that
he/she may participate in and enjoy life’s activities as fully
as possible.
Rehabilitation services can take many forms: physical
therapy, occupational therapy, speech therapy, recreational
therapy, rehabilitation nursing, and psychological services.
Rehabilitation may be done on an inpatient basis or as an
outpatient. Services are provided by a specially trained staff.
Services available include:
Physical Therapy
The goal of physical
therapists is to restore
normal movement and
function while attempting
to resolve problems such
as pain, weakness, loss
of motion, or lack of
coordination. Treatment
begins with an evaluation
to determine the structures
involved as well as the
severity and nature of the
problem. The plan of care
may include exercises to restore function, training in the
use of proper body mechanics, gait training, prosthetic
training, and muscle reeducation. Physical therapy services
are available at both the St. Nicholas Hospital and the
Plymouth Medical Plaza.
Occupational Therapy
Occupational therapists work with individuals who have
physical, developmental, emotional, or social deficits which
affect their ability to participate in daily activities. After
evaluating a patient’s daily living skills, i.e., the ability to get
dressed, walk, or perform routine functions, the therapist
helps the patient “relearn” those skills.
Speech Therapy
Through careful evaluation, planning, and treatment
programs, speech-language pathologists work at improving
a patient’s communication skills. Services include direct
therapeutic treatment, staff consultation, and family
counseling. Disorders treated by speech-language
pathologists may include aphasia, swallowing disorders,
speech and language impairment, oral-motor dysfunction,
Alzheimer’s, and hearing impairment.
Recreational Therapy
Recreational therapists facilitate the development,
maintenance, and expression of an appropriate leisure
lifestyle for individuals with physical, mental, emotional,
or social limitations. While developing leisure skills, the
patient is assisted in eliminating these barriers.
Comprehensive Rehabilitation Inpatient Unit
The Comprehensive Rehabilitation Inpatient Unit of
St. Nicholas Hospital offers an interdisciplinary approach
to restoring living skills, delivering care in a complete range
of services which treat the whole person—body, mind, and
spirit. The care team is composed of a physician medical
director; nurse practitioner; physical, occupational,
recreational, and speech therapists; primary care physician;
psychologist; social worker; pastoral care representative;
dietitian; and discharge planning nurse. Other medical
professionals are brought in as needed with everyone—
including the patient and family—working together toward
a common goal—the patient’s independence in mobility,
self-care, safety, cognition, and communication and return
to safe community living and function. The program is
accredited by the Commission on Accreditation of
Rehabilitation Facilities.
Sleep Disorders Center
Sleep disorders can leave you with a feeling of constant
sleepiness that is both frustrating and hazardous. The Sleep
Disorders Center can help your physician identify your
condition and begin a proper course of treatment. Under
the medical direction of a physician board-certified in
sleep medicine, the Sleep Disorders Center offers complete,
state-of-the-art diagnostic testing for sleep apnea, narcolepsy,
night terrors, insomnia, sleep-related movement disorders,
circadian rhythm disturbances, and many other sleep
disorders. Most sleep studies are conducted overnight at
the Hospital.
Medical Director: Philip M. DeTrana, MD, FCCP
Spiritual and Pastoral Support Services
Spiritual and emotional support are important aspects of
holistic health and healing. Chaplains, nuns, priests and
Holy Communion ministers visit patients and their loved
ones on a regular basis. Upon the request of patients or
their families, their churches or synagogues can be notified
of their current hospitalization.
Roman Catholic Mass is celebrated weekly in the Hospital
Chapel and telecast to patient rooms over closed-circuit TV.
Protestant services are also telecast weekly. Sacraments are
available daily or upon request. A chaplain can also forward
any special requests on to a patient’s pastor or rabbi.
Surgery
Comprehensive inpatient and outpatient perioperative
services, including a variety of gastrointestinal (GI) services,
are available in all
areas of care with
the exception of
neurosurgery and
open heart surgery.
Many surgeries are
performed on an
outpatient basis,
allowing patients
the benefit of recovering in their own homes. Holmium
laser services are also available for the treatment of kidney
stones (see Holium Laser Treatment, page 39) and
brachytherapy for the treatment of prostate cancer.
Procedures are generally scheduled from 8:00 a.m.-3:15 p.m.
Surgical services are available for emergencies at all other
times. Board-certified anesthesiologists provide anesthesia
care (see Anesthesiology, page 34).
Therapeutic Exercise
Regular, progressive exercise and disease management can
be extremely beneficial in recovery from illness or injury,
in managing chronic health conditions, and as a goal in
improving overall fitness and conditioning levels. The
St. Nicholas Hospital Therapeutic Exercise Department
encompasses a variety of therapeutic exercise programs
for patients with chronic health conditions, such as
cardiovascular disease, pulmonary disease, and diabetes, as
well as for individuals recovering from illness or injury.
Cardiac Rehabilitation
Cardiac rehabilitation is a specialty service designed to assist
individuals to recover fully following a heart attack or other
heart problem. The nationally certified program is based
on standards established by the American Association of
Cardiovascular and Pulmonary Rehabilitation. It is offered
in three phases to meet the needs of new cardiac patients as
well as individuals maintaining their cardiac health on an
outpatient basis. All phases are designed to help individuals
lead productive, active, and healthy lives. Fitness training,
strengthening, and cardiovascular risk factor modification
are a part of each phase. Cardiac rehabilitation is provided
by a multidisciplinary team trained in cardiac care. The team
includes a registered nurse with advanced cardiac rehabilitation
training and certification, a registered dietitian, exercise
physiologists, other registered nurses, and technicians.
The program is staffed by Exercise Physiologists and
registered nurses and includes blood pressure and heart
rate monitoring, supervised exercise conditioning and
strengthening, and health and medication counseling.
Pulmonary Rehabilitation
Patients with chronic lung disease (COPD) such as asthma,
emphysema, pulmonary fibrosis, and bronchitis as well as
individuals with other pulmonary diseases and conditions
may be referred by their physician to the St. Nicholas Hospital
Pulmonary Rehabilitation Program. This nationally
certified program is based on standards from the American
Association of Cardiovascular and Pulmonary Rehabilitation.
It features supervised exercise and education designed to
help individuals with chronic lung disease break through
the cycle of breathing difficulties, fatigue, and depression.
A multidisciplinary group of pulmonary rehabilitation
professionals, including a board-certified pulmonologist,
nurses, respiratory therapists, dietitians, and exercise
physiologists, work together to help participants improve
their quality of life and enhance their ability to perform
daily living activities.
opportunity for individuals with a variety of acute and
chronic medical conditions to benefit from a medically
supervised exercise regimen. RESTORE is designed for
those who are weakened or deconditioned by a variety of
conditions, including arthritis, congestive heart failure,
diabetes, neuromuscular disease, obesity, metabolic
syndrome, and more. Participants follow an individualized
treatment plan for exercise and education in a group
setting, using exercise equipment specifically designed to
meet the needs of patients with physical challenges. The
program is safe and enjoyable with an abundance of
emotional support, guidance, and encouragement. It
includes supervised exercise training to improve fitness,
strength, stamina, and flexibility; condition-specific
education; home exercise and diet instruction; risk factor
modification; instruction on proper use of medications; and
smoking cessation assistance. Participants may be
self-referred or physician referred. The program may be
covered by insurance.
Isernhagen Work System
A team approach to the treatment of work-related injuries
involves physicians, occupational health professionals,
rehabilitation specialists, the injured worker, and
employers and assists in and enhances the rehabilitation
process. The St. Nicholas Hospital Center for Pain and
Work Rehabilitation (see page 43) and Occupational
Health Services (see page 41) use the nationally recognized
Isernhagen Work System to bring these facets of treatment
and recovery together. Isernhagen is a total work
rehabilitation and work injury prevention program that
incorporates each part of a work-related injury—from
prevention to rehabilitation to a safe and rapid return
to work.
Isernhagen’s goals are safe, efficient work and productive,
healthy workers. Therefore, its programs may be put into
place on the job site before an injury happens to reduce the
chance of on-the-job injuries and ensure that employers
and workers receive the best outcomes for both the
individual and the company. Isernhagen may be used alone
or in conjunction with the St. Nicholas Hospital Workplace
Health Program (see page 48).
Through Isernhagen, the employee/employer/
healthcare professional team can take advantage of:
• Functional capacity evaluation (demonstration of an
individual’s capabilities for function tasks of the job)
• Work conditioning/hardening
• Functional job analysis (identification and evaluation
of the physical relationships of work through an
analysis of work, worker, and work site)
• Job-site rehabilitation and coaching
• Prework screening (review of functional tasks of
the job as compared to the medical and physical
capabilities and aptitudes of the worker)
Prevea Health and St. Vincent Hospital in Green Bay offer
a variety of medical specialty services to Sheboygan area
patients. The Green Bay location provides high-quality
alternatives for transferring patients who are currently
transferred to Milwaukee-area facilities—often at a
lower cost.
Prevea Health is a group of primary care and specialty care
physicians from 16 clinics providing medical care throughout
Green Bay and northeastern Wisconsin. With more than 200
providers, they provide a wide range of specialty services.
The Prevea physician group provides care in these
specialty areas:
• Cardiothoracic surgery
• Neurosurgery
• Otolaryngology (ENT)
• Pediatric cardiology
• Pediatric critical care
• Pediatric hematology and oncology
• Pediatric rheumatology
• Trauma surgery
The newest addition to the St. Nicholas Hospital medical
campus is the Sheboygan Surgery Center. Sheboygan
Surgery Center is an outpatient facility, known in the
health care industry as an Ambulatory Surgery Center.
The Sheboygan Surgery Center offers patients the
opportunity to have selected surgical and procedural
services, which do not require an overnight stay, performed
outside the hospital setting.
The most common types of procedures performed at
Sheboygan Surgery Center include:
• GI (Gastrointestinal) • Ophthalmological
• Pain Management • Plastic/Cosmetic
• Urological • Foot
• General Surgery • Ear, Nose and Throat (ENT)
• Orthopedic • Gynecological
• Hand • Oral
If your surgeon or physician needs to schedule an
outpatient surgery or procedure, his/her office will call the
Center to schedule your procedure. Surgery Center will
notify you with all of your pre-op instructions such as time
of the surgery and the time you need to arrive.
Not only does Sheboygan Surgery Center focus on
providing the best surgical experience possible for its
patients, but also the care provided is more affordable.
Sheboygan Surgery Center is located at 3141 Saemann
Avenue, Sheboygan. The phone number is (920) 783-5000.
Sheboygan Surgery Center is open 6:30 a.m.-5:00 p.m
Tertiary Care
Sometimes certain health conditions, such as those
requiring open heart surgery, neurosurgery, or neonatal
intensive care, are best treated at tertiary care facilities.
These facilities offer healthcare services provided by
specialized practitioners and which often require highly
sophisticated technologies and facilities.
St. Nicholas Hospital and Physicians’ Health Network
partner with a number of tertiary care facilities in our
region to best meet the needs of patients and employers.
These facilities include but may not be limited to:
• Children’s Hospital of Wisconsin
• Froedtert Memorial Lutheran Hospital
• Mayo Clinics and Hospitals
• St. Vincent Hospital
The tertiary care provider(s) available to you are determined
by your insurance plan. Check with your healthcare
insurance provider or your employer’s Human Resources
Department for information about the tertiary care
facilities included in your health plan.
Climbing the Mountain of Grief:
Youth Support Group
Young people who experience the loss of a loved one
often feel alone. Because children often experience grief
differently than adults, the Youth Grief Support Group
helps children follow their own natural grieving patterns.
Through age-appropriate activities, bereavement facilitators
help youth, work through their grief, learn to accept the
reality of their loss, express their feelings in appropriate
ways, commemorate the life that is finished, and learn to
go on with life and loving.
Diabetes/Nutrition Community Forums
Like any chronic condition, diabetes presents challenges to
individuals coping with it. Diabetes Community Forums
provide education and social activities for individuals with
diabetes. Speakers and activities help people cope with
various aspects of their condition, while opportunities for
sharing and questions assure participants that they are not
alone. Participation is free; however, registration is required.
Experiencing Grief Support Group
the loss of a loved one...the loss of a job...loss of a lifelong
dream...and more. Our grief group is not limited to
anyone for any reason, but more inclusive of anyone who
experiences any type of grief.
HALO
Hope After Loss Organization
When babies come into our lives and are too quickly and
quietly gone, they leave lasting footprints on our hearts.
At St. Nicholas Hospital, we believe those lives are precious
and should be honored and remembered no matter how
brief their life was. HALO is a Pregnancy Loss and Newborn
Death Support Program created to support parents and
their families in whatever stage of pregnancy loss they find
themselves; miscarriage, ectopic, stillbirth, or early newborn
death. Caring support is offered whether the loss is sudden
and unexpected, or needed because the baby has an anomaly
not compatible with life outside the womb. If the mother
wants to carry the baby to term, however long that may
be, support and help is provided to her as she continues to
provide a hospice for her baby. For more information about
this program, please contact Cori Salchert RN, Certified
Pregnancy Loss Counselor at 920-451-7572.
