Fellowship trained physicians have completed a fellowship, the period of medical training a physician<br />

may undertake after completing a specialty training program, are known as fellows. Fellows are<br />

capable of acting as an attending physician or consultant physician in the generalist field in which<br />

they were trained. After completing a fellowship in the relevant sub-specialty, the physician is<br />

permitted to practice without direct supervision by other physicians in that sub-specialty.<br />

Partners in the Independent Practice<br />

of Patient-Centered Care<br />

Physicians’ Health Network is an affiliation of physicians and allied health<br />

professionals serving Sheboygan, Calumet, Fond du Lac, Manitowoc, and Ozaukee<br />

Counties. Since its inception in 1988, PHN has grown to include over 150 independent,<br />

local primary and specialty care physicians and other healthcare professionals.<br />

PHN emphasizes primary care—family practice, internal medicine, pediatrics, and<br />

obstetrics and gynecology. PHN providers believe that the best medical care depends<br />

on the fundamental bond developed between patient and doctor. This relationship<br />

allows PHN professionals to know and meet the needs of patients as unique<br />

individuals, as whole persons.<br />

PHN is committed to maintaining the highest standards of practice and is working<br />

continuously to improve quality. On-going quality activities include credentialing,<br />

physician and staff education, consumer surveys, and the development of improved<br />

For more than 115 years, <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> has<br />

Four members of the <strong>Hospital</strong> Sisters of <strong>St</strong>. Francis<br />

arrived in Sheboygan in the spring of 1890, and the first<br />

hospital began a short time later when the Sisters founded<br />

agile ultrasound and featuring 3D/4D imaging.<br />

• An addition of two hyperbaric oxygen chambers to<br />

the Comprehensive Wound Care Center. Hyperbaric<br />

oxygen has been scientifically proven to aid in healing<br />

wounds that may otherwise lead to amputation.<br />

more than 1,700 participants, the Project is the largest<br />

comparative analysis database of its type in the United<br />

<strong>St</strong>ates. Project indicators measure facility-level clinical<br />

outcomes and focus on areas of care that are universally<br />

important to healthcare organizations (e.g., mortality rates,<br />

unscheduled readmissions, use of restraints, infection rates).<br />

The Project contains data from more than 20 percent of all<br />

U.S. hospitals.<br />

Heart Association in its “Get with the Guidelines” program<br />

for continuous quality improvement for prevention of<br />

Anesthesiology<br />

The Anesthesiology Department uses state of the art<br />

equipment to provide a complete spectrum of care<br />

including general, spinal, epidural, and regional nerve<br />

blocks. The department is also involved in the management<br />

of acute and chronic pain through the Pain Management<br />

Clinic (see page 43). The department is comprised of<br />

board-certified anesthesiologists (see Surgery, page 46).<br />

Cancer Services<br />

<strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> offers patients battling cancer and<br />

their families a source of strength, care, support, and hope<br />

through comprehensive, integrated oncology (cancer)<br />

services. The cancer care program has been approved by<br />

the Commission on Cancer of the American College of<br />

Surgeons. The <strong>Hospital</strong> is a member of the Association<br />

of Community Cancer Centers, the leading national<br />

interdisciplinary organization that defines quality care for<br />

patients with cancer and influences change to continually<br />

improve oncology care. In addition, <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong><br />

participates in the Regional Cancer Collaborative, a<br />

cooperative effort that brings together top medical<br />

specialists, the latest medical technology and highly-<br />

integrated treatment options. For specialized treatments,<br />

patients may be referred to <strong>St</strong>. Vincent <strong>Hospital</strong>, Green Bay,<br />

which serves as the base of the Collaborative, providing highend,<br />

high-tech services not available in every community.<br />

The oncology care team includes board-certified medical<br />

and radiation oncologists, oncology-certified nurses,<br />

radiation therapists, dietitians, spiritual and pastoral<br />

support staff, and other highly skilled individuals from a<br />

variety of specialized fields of medicine needed to treat the<br />

complex disease processes of cancer. The team works with<br />

the patient and family, the family physician, and doctors<br />

specializing in surgery, pathology, and radiation and<br />

medical oncology to provide complete care.<br />

Radiation therapy is often an important facet of cancer<br />

care on either an inpatient or outpatient basis. With a<br />

state-of-the-art, dual-energy linear accelerator, highlytrained<br />

radiation therapists are able to treat cancer patients<br />

with both photons and electrons, as well as with IMRT<br />

(Intensity Modulated Radiation Therapy). IMRT is another<br />

form of radiation therapy that uses three-dimensional<br />

imaging and a specialized treatment planning technique to<br />

target cancer cells with high doses of radiation. While at<br />

the same time minimizing the radiation received by nearby<br />

healthy tissue.<br />

Radiation therapy destroys tumors or reduces their size<br />

through exposure to high doses of ionizing radiation which<br />

kills or damages tumor cells. The therapy is usually<br />

delivered in a series of small treatment doses over a period<br />

of several weeks to allow healthy tissue surrounding the<br />

tumor the opportunity to repair itself between doses. In the<br />

case of prostate cancer, brachytherapy, or radioactive seed<br />

implantation, offers a therapeutic treatment option.<br />

Radiation therapy can be used for curative purposes in<br />

combination with surgery, chemotherapy, or by itself. It<br />

can also be used in a palliative manner to control bleeding<br />

or pain or be used to shrink tumors, making them easier to<br />

remove surgically. Services are provided by board-certified<br />

radiation oncologists and registered radiation therapists<br />

and physicists.<br />

Chemotherapy, which is also given on an inpatient or<br />

outpatient basis, is the use of drugs to treat cancer. These<br />

drugs destroy cancer cells by stopping them from growing<br />

or multiplying at one or more points in their cell cycle.<br />

Because some drugs work better together than alone,<br />

chemotherapy may consist of more than one drug. This is<br />

called combination chemotherapy. Chemotherapy may be<br />

used in combination with surgery and/or radiation therapy<br />

and may be palliative or curative. Registered nurses who<br />

administer chemotherapy complete the Oncology Nursing<br />

Society’s Chemotherapy and Biotherapy Course. This<br />

specialized education and preparation validates that the<br />

nurses have the knowledge needed to provide a safe level<br />

of care for the individuals receiving these agents.<br />

Prevention and early detection of cancer are encouraged<br />

through a variety of programs such as skin cancer<br />

screenings and the Wisconsin Well Woman Program,<br />

which provides free mammography for under-insured or<br />

uninsured women.<br />

A lending library of books and videos, support groups<br />

(see page 52), and other services provide other support for<br />

patients and families. In addition, wigs, hats, turbans, and<br />

other head wear items are available for women who<br />

experience hair loss as a result of cancer treatment.<br />

Cardiac Services<br />

<strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> Cardiac Services offer comprehensive<br />

preventive, diagnostic, and primary care for patients<br />

with all types of heart disease. Care is tailored to each<br />

individual’s needs and is provided by a multidisciplinary<br />

team of physicians, nurses, therapists, dietitians, and other<br />

healthcare professionals.<br />

In a state-of-the-art Chest Pain Evaluation Center,<br />

individuals experiencing chest pain are quickly evaluated<br />

for appropriate treatment which may include thrombolytics<br />

(clot busters) to minimize damage to the heart.<br />

Some individuals may be referred for diagnostic procedures<br />

such as stress tests, transesophageal echocardiograms,<br />

electrophysiology testing, tilt table studies, continuous<br />

blood pressure monitoring, or cardiac catheterization. Such<br />

tests allow physicians to identify heart rhythm aberrations<br />

and determine valve function and the condition of heart<br />

muscle and arteries.<br />

Partnerships with tertiary care facilities (see page 50)<br />

provide an option for seamless transition to advanced<br />

cardiovascular medicine, cardiac and vascular surgery, and<br />

vascular radiology care for patients who need these services.<br />

A Cardiac Rehabilitation program assists in recovery<br />

(see page 46).<br />

Care Management and Social Services<br />

Care management focuses on outcomes to enhance the<br />

quality of patient care through effective and efficient use of<br />

the <strong>Hospital</strong>’s resources. <strong>St</strong>rategies utilized include guidelines,<br />

protocols, standardized order sets, patient education,<br />

communication with insurance companies to certify the<br />

medical necessity of hospital care, and discharge planning.<br />

Care management services are available to assist patients<br />

and their families as they deal with problems which often<br />

accompany illness. Services may include counseling with<br />

the patient and family in areas such as financial problems,<br />

marriage or family problems arising out of the patient’s<br />

illness, and discharge planning. <strong>St</strong>aff assists patients and<br />

families with arranging services (i.e., Lifeline, nursing<br />

home, and homecare nursing) which may be necessary<br />

before or after discharge either at the patient’s home or at<br />

another facility.<br />

Care Management services are provided by social workers<br />

with bachelor’s or master’s degrees in social service and<br />

registered nurses with specialized education and training in<br />

discharge planning and utilization management. Services<br />

are available to address social work and discharge planning<br />

needs of patients ranging from newborns to geriatric<br />

patients and are also available to those patients/families<br />

utilizing the facility on an outpatient basis (including<br />

Emergency Department usage). Services are also available<br />

to individuals in the community requesting assistance<br />

regardless of status as prior, current, or future patients.<br />

A Patient Advocate, a registered nurse with specialized<br />

training, investigates patient complaints/referrals regarding<br />

services provided and ensures appropriate organizational<br />

follow-up, including explanation of <strong>Hospital</strong> policies and<br />

routines.<br />

The Patient Advocate and Care Management staff provides<br />

other special services to patients/families, including<br />

explanation and assistance with Advance Directives,<br />

assisting patients and families with end-of-life decision<br />

making, facilitating family meetings, and arranging for<br />

assistance with interpretive services.<br />

CarePages are free, private websites that help people<br />

stay connected and easily communicate with friends<br />

and family during a health event. CarePages also<br />

provides stories, resources, and tools to meet people in<br />

similar situations.


Central Scheduling<br />

Many hospital visits are comprised of a combination of<br />

services, a number of which can be scheduled with one call<br />

from your physician to the <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> Central<br />

Scheduling Department. Following the call, department<br />

staff will make all the arrangements necessary for your visit.<br />

Community Education<br />

Helping you maintain good health is of the utmost<br />

importance to <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong>. With that in mind,<br />

our Community Education Department presents a variety<br />

of health education classes and outreach activities which<br />

help you learn more about health. From blood pressure<br />

screenings to stroke awareness, from summer sports fitness<br />

to winter health hazards, <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> wants to<br />

make sure you have the best information available to you.<br />

Programs are conducted by physicians, nurses, therapists,<br />

and other <strong>Hospital</strong> staff. Many are offered free or at a<br />

minimal charge. The HealthExpress van travels to<br />

community events, providing health information and<br />

screenings to the public. Health fairs familiarize the public<br />

with <strong>Hospital</strong> services and health issues. Child safety seat<br />

clinics and multi-sport and extreme skateboard helmet<br />

fittings are offered both on-site and in community locations.<br />

Comprehensive Wound Care Center<br />

Outpatient wound services are offered in the newly<br />

renovated Comprehensive Wound Care Center. The Center<br />

is dedicated to providing “best practice” advanced wound<br />

care interventions. Goals of treatment in the Center include<br />

providing acute and chronic wound care management,<br />

disease/condition specific patient and caregiver education,<br />

access to the most cost effective wound care technology<br />

and specialty trained and certified physicians and staff, and<br />

access to a wide referral base within the hospital system<br />

and campus.<br />

The Center offers specialized care for leg and foot ulcers<br />

and other wounds often caused by diabetes, poor<br />

circulation, and unhealed surgical incisions. A wound<br />

care specialist and trained nursing staff provide patients’<br />

wound care and work with the physician to coordinate<br />

their multidisciplinary care needs.<br />

The Center and the <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> Home Health<br />

Department have also collaborated to provide a wound<br />

telehealth program in managing and healing wounds of<br />

home health clients (see Home Health Services, page 39).<br />

The Center also recently added two hyperbaric oxygen<br />

chambers to the Comprehensive Wound Care Center. As<br />

the only facility to offer hyperbaric oxygen therapy in<br />

Sheboygan County, residents now have access to state of<br />

the art wound care for acute and chronic wounds.<br />

The Center has added advanced wound care and hyperbaric<br />

medicine trained physicians from Infinity Healthcare.<br />

Wound care services include:<br />

• Assessment Following a comprehensive examination<br />

and detailed workup for wound healing risk factors,<br />

a comprehensive treatment plan is developed.<br />

• Photo Documentation As part of the ongoing<br />

evaluation process, digital photographs are used to<br />

document the progress of healing.<br />

• Treatment Plan A comprehensive treatment plan is<br />

developed for each patient. Plans may include<br />

medications, dressings, and patient education.<br />

• Follow-up Care Progress and treatment needs are<br />

monitored and assessed throughout wound healing.<br />

The Wound Care team works with the patients’<br />

primary physician to develop an evidence-based<br />

clinical plan to promote healing.<br />

Continence Services<br />

Over ten million Americans have a bladder control<br />

problem, but many are too embarrassed to talk about it—<br />

even to their own doctors. Inpatient and outpatient<br />

continence services are available for these individuals.<br />

Both new and continuing patients with stress or urge<br />

incontinence benefit from the comprehensive assessments,<br />

advanced treatments, and compassionate counseling<br />

provided by occupational therapists and a Continence<br />

Specialist.<br />

Incontinence services include:<br />

• Patient/family education<br />

• Prevention of skin breakdown<br />

• Complex skin care<br />

• Product selection<br />

• Behavior training<br />

• Pelvic muscle re-education<br />

Deaf Communications Superphone<br />

A Deaf Communications Superphone is located in the<br />

<strong>Hospital</strong> Emergency Department. This special equipment<br />

gives hearing impaired persons the ability to contact the<br />

<strong>Hospital</strong> in the event of an emergency. The service is<br />

available 24 hours a day. The telephone number is<br />

The Center’s self-management education program has been<br />

awarded recognition by the American Diabetes Association<br />

in accordance with the National <strong>St</strong>andards for Diabetes<br />

Self-Management Education Programs.<br />

Patients receive education regarding the importance of<br />

proper exercise, diet, stress management, and medications,<br />

including insulin. Newly diagnosed diabetes patients are<br />

taught the skills to become more confident in their diabetes<br />

management and allay any fears. For those whose diabetes<br />

management requires some “tuning up,” staff can help find<br />

a plan that best works for the individual and help develop<br />

the skills needed to succeed. Family members are welcome<br />

to take part in the education process. The Center also<br />

provides a 72-hour blood sugar monitor for patients by<br />

physician order. Community Forums are offered for Type 1<br />

and Type 2 diabetes, including those with insulin pumps.<br />

Diabetes Center services are offered in Sheboygan and<br />

Plymouth with appointments to accommodate most<br />

schedules. Foot clinics and free diabetes (blood sugar)<br />

screenings offer preventive care while the Center offers<br />

group and individual education classes that:<br />

• Define diabetes and its effects<br />

• Monitors blood sugar<br />

• Provides insulin management including insulin<br />

pump and Continuous Glucose Monitoring System<br />

• Manages oral medication<br />

• Offers nutrition counseling and meal planning<br />

Services are accredited by the American College of Radiology.<br />

Diagnostic Imaging Services is staffed by board-certified<br />

diagnostic radiologists of Green Bay Radiology, several<br />

of whom have added qualifications in vascular and<br />

interventional radiology. Green Bay Radiology physicians<br />

include fellowship trained radiologists in Neuroradiology,<br />

Vascular Interventional Radiology, Musculoskeletal<br />

Radiology, Nuclear Medicine, MRI, and Body Imaging<br />

(CT/US/MRI fellowship). The department staff also<br />

includes technologists registered in mammography,<br />

diagnostic ultrasound, certified nuclear medicine, CT,<br />

MRI, angiography, and support staff. X-ray and<br />

mammography screening services are also available at<br />

the Plymouth Medical Plaza.<br />

Dialysis Services<br />

Patients with end-stage renal disease can benefit from<br />

Dialysis Services at <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong>. Individuals who<br />

do not yet require dialysis treatment are eligible for<br />

“preteaching,” which assists patients and their families<br />

with treatment choices. Twelve hemodialysis stations are<br />

available weekdays and Saturdays as well as some evenings<br />

for patients requiring dialysis treatments. A physician and<br />

nurse are available for emergency treatments outside of the<br />

regularly scheduled hours.<br />

Fistula and peritoneal catheter placement is done on an<br />

outpatient basis and training for continuous ambulatory<br />

peritoneal dialysis and continuous cycling peritoneal<br />

dialysis is given to patients who have shown a desire and<br />

aptitude to learn home dialysis. In addition to treating its<br />

local dialysis patients, Dialysis Services provide transient<br />

dialysis treatment to people traveling through the area. All<br />

patients are evaluated for eligibility for a kidney transplant.<br />

A highly skilled team of registered nurses provides care<br />

and education for dialysis patients. A registered dietitian<br />

assesses and personalizes nutritional plans, and social<br />

services staff provides assistance for home healthcare<br />

needs and/or financial concerns. The staff is assisted by<br />

nephrologists and internists with special training in the<br />

care of patients with end-stage renal disease.<br />

Electrodiagnosis Services<br />

Eectrodiagnosis refers to diagnostic testing of the<br />

cardiovascular system and brain. Among the procedures<br />

available are electrocardiograph (EKG), cardiac event<br />

recorders, Holter monitors, electroencephalograph (EEG)<br />

testing, graded exercise and pharmacological stress testing,<br />

and 24-hour blood pressure monitoring. (See Cardiac<br />

Services, page 35.)<br />

Emergency Services<br />

In an emergency, time is of the essence, and the hospital<br />

and its team of emergency specialists you choose is critical.<br />

So when minutes count, make the <strong>Right</strong> <strong>Choice</strong>.<br />

Open 24 hours a day, 7 days a week, the Emergency<br />

Services Department is staffed by board-certified emergency<br />

medicine physicians and emergency nurses with support<br />

from every area of the <strong>Hospital</strong>. All <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong><br />

Emergency Service physicians are members of Infinity<br />

HealthCare (see page 49).<br />

<strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> has been designated by the <strong>St</strong>ate of<br />

Wisconsin as a Level III Trauma Care Facility. Committed<br />

to providing optimal care to trauma patients, this<br />

designation indicates that <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> is capable<br />

of providing assessment, resuscitation, stabilization,<br />

emergency surgery, and, if necessary, arranging transfer to<br />

a Level I or II facility for definitive surgical and intensive<br />

care as necessary.<br />

The Emergency Department has the distinction of earning<br />

numerous awards during the past several years, including<br />

the PCR 5-star award for excellent overall quality of care<br />

in 2009.<br />

The Emergency Department features: two large, dedicated<br />

trauma rooms; three large treatment rooms; five general<br />

exam rooms; a dedicated obstetrical/gynecological exam<br />

room; a room dedicated to specialized treatment situations;<br />

a bereavement room; a decontamination area; a double<br />

ambulance garage; and its own entrance.<br />

<strong>St</strong>aff members are certified in advanced cardiac life support,<br />

pediatric advanced life support, and trauma. The most<br />

advanced treatments for stroke and chest pain begin here.<br />

The department also performs some outpatient services<br />

and procedures.<br />

Poison control information is available 24 hours a day by<br />

calling The Poison Control Center at 1-800-222-1222.<br />

<strong>St</strong>affed by a toxicologist at all times, the Center provides<br />

information and advice to callers.<br />

Orange Cross Ambulance, Inc. operates a Sheboygan<br />

County station from the Emergency Department at<br />

<strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong>. Paramedic staff who provide<br />

advanced prehospital medical care are stationed at the<br />

<strong>Hospital</strong>. Orange Cross offers emergency intercept services<br />

to other outlying EMS units at their request.<br />

<strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong>’s close proximity to the I-43/Highway<br />

23 interchange means rapid emergency treatment is only<br />

minutes away—wherever you may be in our service area.<br />

In addition, medical air transport services provide<br />

transportation to tertiary care facilities when the situation<br />

requires more extensive care than can be provided on-site.<br />

Endocrinology Services<br />

Prevea Sheboygan Endocrinology provides services at<br />

<strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> for adults with endocrine disease,<br />

which involves disorders of the hormone-producing glands<br />

of the body. Diabetes (see page 37) is the most common of<br />

these, followed by thyroid conditions, including under- and<br />

overactive thyroid conditions and osteoporosis. In addition<br />

endocrinologists treat disorders of calcium metabolism,<br />

including disorders of the pituitary and adrenal disease.<br />

Friends Outreach<br />

Friends Outreach offers a variety of health services that<br />

include holding health education sessions and screenings,<br />

promoting wellness and healthy lifestyles, raising awareness<br />

in the community on the issue of domestic violence, and<br />

broadening programs that help underserved and minority<br />

individuals and families. Friends Outreach collaborates with<br />

numerous community organizations including the Hmong<br />

Mutual Assistance Association, Hispanic/Latino Task Force,<br />

Dental Access Committee, Domestic Violence Education<br />

for Faith Leaders Committee, the Alcohol and Other Drugs<br />

Abuse Committee, and the Sheboygan County Tobacco<br />

Free Coalition.<br />

The Hope After Loss Organization (HALO) (see page 52)<br />

is also another function of Friends Outreach.<br />

The Friends of <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> has provided Friends<br />

Outreach with financial support since its inception in 2001.<br />

Holmium Laser Treatment<br />

A noninvasive, nonsurgical treatment, the holmium laser<br />

service is available 24-hours-a-day for patients with kidney<br />

stones. This treatment helps patients avoid major surgery.<br />

The holmium laser provides precisely controlled incisions<br />

and can be used in areas of the body other lasers cannot<br />

reach. It may also be used to vaporize kidney tumors, treat<br />

strictures, remove scar tissue, and resolve obstructions.<br />

Home Health Services<br />

Home Health is a state-licensed, Medicare certified, and<br />

Joint Commission accredited department of <strong>St</strong>. <strong>Nicholas</strong><br />

<strong>Hospital</strong>. Home Health offers services to individuals in<br />

their home who require the skills of a nurse or therapist, are<br />

homebound, and are under the care of a physician. Home<br />

Health services focus on improving the health and<br />

independence of those in the community while allowing<br />

individuals to receive healthcare services in the comfort of<br />

their own home. Home Health offers a variety of services<br />

at a significant cost savings and Home Health is covered by<br />

Medicare, Medicaid and most insurance plans.<br />

Home Health is available across the life span for individuals<br />

recovering from a serious illness or injury to those suffering<br />

from a chronic condition. From newborns with sleep<br />

apnea to an adolescent or adult diagnosed with diabetes,<br />

or an elderly person recovering after a fractured hip, just<br />

to name a few, home health is available to assist individuals<br />

in maintaining or returning to an optimal level of health<br />

and functioning.<br />

The <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> Home Health Department is<br />

also the first home care agency in Sheboygan County to<br />

offer electronic and digital oversight of wound care patients<br />

by a satellite site and credentialed specialists.<br />

Telehealth is the delivery of health-related services and<br />

information via telecommunications technologies. It is as<br />

simple as two health professionals discussing a case over the<br />

telephone, or as sophisticated as using videoconferencing<br />

between providers at facilities in two countries.<br />

Telehealth encompasses preventive, promotive and curative<br />

aspects. The format of telehealth that <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong><br />

will employ is called store-and-forward telehealth. Digital<br />

images, video, audio and clinical data are captured and<br />

“stored” on the field nurse’s computer; then transmitted<br />

securely to the <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> Comprehensive<br />

Wound Care Center (see page 36) where they are studied<br />

by specialists and their input is provided.


Home Health services through <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> Home<br />

Health and Hospice are available within a 30 mile radius<br />

of <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong>. Referrals to Home Health can be<br />

made by physicians, families, or a prospective client him/<br />

herself. A specialized home health Liaison will work with<br />

individuals to determine eligibility and arrange for services.<br />

Care is available on an intermittent basis, 24 hours a day,<br />

seven days a week and a Registered Nurse is only a phone<br />

call away around the clock for those admitted into services.<br />

Home Health services are supported in part by the United<br />

Way and in keeping with the mission, vision, and values of<br />

<strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong>; no one will be turned away regardless<br />

of ability to pay.<br />

Hospice Services<br />

The Hospice program at <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> Home<br />

Health and Hospice is also a state-licensed, Medicare<br />

certified, and Joint Commission accredited program that<br />

offers care to those individuals in our community that are<br />

facing a life limiting illness. Hospice services are available to<br />

anyone who has been diagnosed with a terminal illness, has<br />

a limited life expectancy and are no longer seeking curative<br />

treatment. Hospice services are appropriate for those<br />

suffering from cancer, end stage cardiac or respiratory<br />

conditions, AIDS, end stage renal disease, and other<br />

illnesses that are life limiting.<br />

Hospice recognizes dying as part of the normal process of<br />

living and focuses on maintaining the quality of life<br />

remaining. Hospice affirms life and neither hastens nor<br />

postpones death. It exists in the hope and belief that,<br />

through appropriate care and the promotion of a caring<br />

community sensitive to personal needs, patients and their<br />

families may be free to attain a degree of mental and<br />

spiritual preparation for death that is satisfactory to them.<br />

The Hospice team, consisting of the patient’s own physician,<br />

a Medical Director, nurses, social workers, counselors and<br />

spiritual care representative, works with the patient, family<br />

and caregivers to develop a plan of care that meets the<br />

needs of everyone involved. Hospice care seeks to relieve<br />

symptoms, provide emotional support, and most<br />

importantly, allow a person to live their remaining days<br />

with dignity in the setting of their choosing. Hospice is<br />

provided wherever the patient calls home.<br />

Palliative Care<br />

The Palliative Care program at <strong>St</strong>. <strong>Nicholas</strong> Home Health<br />

and Hospice is available to those who have been diagnosed<br />

with a life limiting or chronic illness but are still seeking<br />

curative treatment and may have a life expectancy that is<br />

greater than six months. Palliative Care focuses on symptom<br />

management and retention of independence with the goal<br />

of preventing re-hospitalization. Palliative Care services<br />

include the availability of nursing visits, home health aide<br />

visits for personal care, social worker consultation for future<br />

planning or assistance in accessing community resources,<br />

pastoral support, homemaking services and volunteer<br />

services. Palliative Care is typically a private pay service as<br />

it is not reimburseable by Medicare, Medicaid, or most<br />

private insurances. A member of the Palliative Care team is<br />

available for consultation to discuss payment arrangements<br />

and services available.<br />

Intensive/Cardiac Care Unit (ICCU)<br />

The Intensive/Cardiac Care Unit is designed for patients<br />

who require very close observation and intensive nursing<br />

care around-the-clock. All staff members are certified in<br />

advanced cardiac life support; some are also certified in<br />

critical care nursing, and pediatric advanced life support.<br />

Intensive monitoring is provided for all patients so that staff<br />

may quickly assess and respond to any changes. In addition,<br />

other forms of hemodynamic monitoring may be utilized,<br />

depending on the patient’s condition. Special procedures such<br />

as medical and electrical cardioversions, tilt table studies,<br />

intra-aortic balloon pumps, temporary pacemakers, and<br />

transesophageal echocardiograms are performed in ICCU.<br />

Patients who are having cardiac catheterization and<br />

electrophysiology studies are also cared for here.<br />

blood glucose, and lipid tests for the public are available<br />

7:00 a.m.-4:30 p.m. Monday-Friday through the<br />

Nutrition Services<br />

Nutrition is an important part of care at <strong>St</strong>. <strong>Nicholas</strong><br />

<strong>Hospital</strong>. Registered dietitians assess and monitor the<br />

nutritional status of all patients. Following physicians’ orders<br />

and in collaboration with a pharmacist, they recommend and<br />

implement changes in patient diets, including tube feedings.<br />

Registered dietitians also educate patients with special<br />

nutritional needs, for example, those with diabetes, renal,<br />

pulmonary, or cardiac conditions. Individual outpatient<br />

nutrition consultations are also available to assist individuals<br />

in achieving healthy eating behaviors. The Center offers a<br />

year-long weight management program called CHANGE<br />

and a quarterly Gastric Bypass Support Group.<br />

Ostomy Services<br />

Patients undergoing ostomy surgery, whether temporary or<br />

permanent, require intensive physical and emotional care<br />

and support to return to their normal daily lives. Outpatient<br />

ostomy services are offered through the Comprehensive<br />

Wound Care Center. Both new and continuing patients<br />

benefit from comprehensive assessments, advanced<br />

treatments, and compassionate counseling provided by a<br />

Wound, Ostomy, and Continence Specialist.<br />

The Ostomy Center is designed to meet the needs of the<br />

active person with a new or existing colostomy, ileostomy<br />

or urostomy. A Wound, Ostomy, and Continence Specialist<br />

meets with individuals and their families before and after<br />

surgery and continue to provide ongoing education and<br />

support. Long-time ostomy patients benefit from counseling,<br />

fittings, updated appliances, and other services.<br />

Ostomy care services include:<br />

• Pre- and postoperative counseling to help prepare and<br />

assist patients in adjusting to life with an ostomy.<br />

• Patients are fitted or refitted with ostomy appliances<br />

suited to their individual needs. Newer appliances<br />

have been designed to be more comfortable and<br />

Pain Management<br />

Center for Pain and Work Rehabilitation<br />

The Center for Pain and Work Rehabilitation is the area’s<br />

most comprehensive source for the management of chronic<br />

pain. Accredited by the Commission on Accreditation of<br />

Rehabilitation Facilities (CARF), the Center’s goal is to<br />

improve the quality of life for those who suffer from pain<br />

and help them return to a productive life at work or their<br />

homes. The Center works in cooperation with physicians<br />

and other services of the <strong>Hospital</strong>, including the Interventional<br />

Pain Clinic and Occupational Health Services<br />

(see page 41).<br />

Interventional Pain Clinic<br />

Pain management is an important part of patient care. In<br />

fact, up to 80% of all health insurance claims are related to<br />

pain, particularly back pain. However, today, more methods<br />

of treating pain are available than ever before, and, under<br />

the care of a qualified physician, almost every patient’s pain<br />

can be treated effectively. This includes sufferers of chronic<br />

back pain and cancer-related pain.<br />

Board-certified anesthesiologists at <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong><br />

manage aspects of both acute and/or chronic pain. The<br />

Pain Clinic works cooperatively with the Center for Pain<br />

physicians in that effort. The team is concerned with<br />

The philosophy of palliative care is to focus on quality of<br />

life issues, symptom management, communication, and<br />

emotional and spiritual wellness while continuing some<br />

form of therapy for the chronic illness itself. Treatment<br />

decisions are made in the context of maximizing quality of<br />

life while understanding that the ultimate disease outcome<br />

may not change.<br />

The Palliative Care Team, an interdisciplinary group of<br />

healthcare professionals, works with the patient’s primary<br />

care physician to establish a plan of care based on the<br />

patient’s individual needs. The patient and family are<br />

always a part of the care planning process through regular<br />

scheduled family meetings.<br />

Palliative Care does not replace active treatment of your<br />

disease nor is it hospice. Palliative Care is beneficial at any<br />

stage of illness.<br />

Palliative Care is a proven means to meet the needs of<br />

those suffering from serious, chronic, or life limiting<br />

medications and intravenous solutions (IVs) are prepared<br />

by skilled technicians under the supervision of a pharmacist.<br />

The pharmacist assists physicians by continuously screening<br />

medication profiles and associated laboratory results,<br />

providing recommendations as indicated. Upon physician<br />

order, drug therapy responses are monitored for antibiotics<br />

and anticoagulants. Pharmacists team with dietitians to<br />

recommend formulas for optimal intravenous nutrition<br />

when needed. Pharmacists respond to medication questions<br />

utilizing up-to-date drug information resources. A<br />

pharmacist is available for consultation 24 hours a day.<br />

Rehabilitation Services<br />

Often after illness or injury, an individual finds him/herself<br />

in need of some form of rehabilitation services on either an<br />

inpatient or outpatient basis. The goal of rehabilitation is<br />

to improve the functional capabilities of the patient so that<br />

he/she may participate in and enjoy life’s activities as fully<br />

as possible.<br />

Rehabilitation services can take many forms: physical<br />

therapy, occupational therapy, speech therapy, recreational<br />

therapy, rehabilitation nursing, and psychological services.<br />

Rehabilitation may be done on an inpatient basis or as an<br />

outpatient. Services are provided by a specially trained staff.<br />

Occupational Therapy<br />

Occupational therapists work with individuals who have<br />

physical, developmental, emotional, or social deficits which<br />

affect their ability to participate in daily activities. After<br />

evaluating a patient’s daily living skills, i.e., the ability to get<br />

dressed, walk, or perform routine functions, the therapist<br />

helps the patient “relearn” those skills.<br />

Speech Therapy<br />

Through careful evaluation, planning, and treatment<br />

programs, speech-language pathologists work at improving<br />

a patient’s communication skills. Services include direct<br />

therapeutic treatment, staff consultation, and family<br />

counseling. Disorders treated by speech-language<br />

pathologists may include aphasia, swallowing disorders,<br />

speech and language impairment, oral-motor dysfunction,<br />

Alzheimer’s, and hearing impairment.<br />

Recreational Therapy<br />

Recreational therapists facilitate the development,<br />

maintenance, and expression of an appropriate leisure<br />

lifestyle for individuals with physical, mental, emotional,<br />

or social limitations. While developing leisure skills, the<br />

patient is assisted in eliminating these barriers.<br />

Comprehensive Rehabilitation Inpatient Unit<br />

The Comprehensive Rehabilitation Inpatient Unit of<br />

<strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> offers an interdisciplinary approach<br />

to restoring living skills, delivering care in a complete range<br />

of services which treat the whole person—body, mind, and<br />

spirit. The care team is composed of a physician medical<br />

director; nurse practitioner; physical, occupational,<br />

recreational, and speech therapists; primary care physician;<br />

psychologist; social worker; pastoral care representative;<br />

dietitian; and discharge planning nurse. Other medical<br />

professionals are brought in as needed with everyone—<br />

including the patient and family—working together toward<br />

a common goal—the patient’s independence in mobility,<br />

self-care, safety, cognition, and communication and return<br />

to safe community living and function. The program is<br />

accredited by the Commission on Accreditation of<br />

Rehabilitation Facilities.<br />

Respiratory Care Services<br />

The <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> Respiratory Care staff works<br />

closely with physicians to provide a comprehensive<br />

approach to treating patients with respiratory disorders.<br />

Services provided include assessment, diagnostic testing,<br />

therapeutics, monitoring, education, and rehabilitation.<br />

Patients with respiratory-related sleep disorders receive<br />

additional evaluation through the <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong><br />

Sleep Disorders Center (see page 45). A Pulmonary<br />

Rehabilitation program aids patients in improving quality<br />

of life and functional abilites.<br />

Sleep disorders can leave you with a feeling of constant<br />

sleepiness that is both frustrating and hazardous. The Sleep<br />

Disorders Center can help your physician identify your<br />

condition and begin a proper course of treatment. Under<br />

the medical direction of a physician board-certified in<br />

sleep medicine, the Sleep Disorders Center offers complete,<br />

state-of-the-art diagnostic testing for sleep apnea, narcolepsy,<br />

night terrors, insomnia, sleep-related movement disorders,<br />

circadian rhythm disturbances, and many other sleep<br />

disorders. Most sleep studies are conducted overnight at<br />

the <strong>Hospital</strong>.<br />

Spiritual and Pastoral Support Services<br />

Spiritual and emotional support are important aspects of<br />

holistic health and healing. Chaplains, nuns, priests and<br />

Holy Communion ministers visit patients and their loved<br />

ones on a regular basis. Upon the request of patients or<br />

their families, their churches or synagogues can be notified<br />

of their current hospitalization.<br />

Roman Catholic Mass is celebrated weekly in the <strong>Hospital</strong><br />

Chapel and telecast to patient rooms over closed-circuit TV.<br />

Protestant services are also telecast weekly. Sacraments are<br />

available daily or upon request. A chaplain can also forward<br />

any special requests on to a patient’s pastor or rabbi.


Surgery<br />

Comprehensive inpatient and outpatient perioperative<br />

services, including a variety of gastrointestinal (GI) services,<br />

are available in all<br />

areas of care with<br />

the exception of<br />

neurosurgery and<br />

open heart surgery.<br />

Many surgeries are<br />

performed on an<br />

outpatient basis,<br />

allowing patients<br />

the benefit of recovering in their own homes. Holmium<br />

laser services are also available for the treatment of kidney<br />

stones (see Holium Laser Treatment, page 39) and<br />

brachytherapy for the treatment of prostate cancer.<br />

Procedures are generally scheduled from 8:00 a.m.-3:15 p.m.<br />

Surgical services are available for emergencies at all other<br />

times. Board-certified anesthesiologists provide anesthesia<br />

care (see Anesthesiology, page 34).<br />

Therapeutic Exercise<br />

Regular, progressive exercise and disease management can<br />

be extremely beneficial in recovery from illness or injury,<br />

in managing chronic health conditions, and as a goal in<br />

improving overall fitness and conditioning levels. The<br />

<strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> Therapeutic Exercise Department<br />

encompasses a variety of therapeutic exercise programs<br />

for patients with chronic health conditions, such as<br />

cardiovascular disease, pulmonary disease, and diabetes, as<br />

well as for individuals recovering from illness or injury.<br />

Cardiac Rehabilitation<br />

Cardiac rehabilitation is a specialty service designed to assist<br />

individuals to recover fully following a heart attack or other<br />

heart problem. The nationally certified program is based<br />

on standards established by the American Association of<br />

Cardiovascular and Pulmonary Rehabilitation. It is offered<br />

in three phases to meet the needs of new cardiac patients as<br />

well as individuals maintaining their cardiac health on an<br />

outpatient basis. All phases are designed to help individuals<br />

lead productive, active, and healthy lives. Fitness training,<br />

strengthening, and cardiovascular risk factor modification<br />

are a part of each phase. Cardiac rehabilitation is provided<br />

by a multidisciplinary team trained in cardiac care. The team<br />

includes a registered nurse with advanced cardiac rehabilitation<br />

training and certification, a registered dietitian, exercise<br />

physiologists, other registered nurses, and technicians.<br />

Lifetime Exercise Program<br />

The Lifetime Exercise Program is a maintenance-level<br />

program designed to provide exercise and education to help<br />

the patient continue on the road to recovery. Tailored to<br />

the needs of each individual, the goal of the program is to<br />

encourage the continual development of a healthy lifestyle.<br />

The program is staffed by Exercise Physiologists and<br />

registered nurses and includes blood pressure and heart<br />

rate monitoring, supervised exercise conditioning and<br />

strengthening, and health and medication counseling.<br />

Pulmonary Rehabilitation<br />

Patients with chronic lung disease (COPD) such as asthma,<br />

emphysema, pulmonary fibrosis, and bronchitis as well as<br />

individuals with other pulmonary diseases and conditions<br />

may be referred by their physician to the <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong><br />

Pulmonary Rehabilitation Program. This nationally<br />

certified program is based on standards from the American<br />

Association of Cardiovascular and Pulmonary Rehabilitation.<br />

It features supervised exercise and education designed to<br />

help individuals with chronic lung disease break through<br />

the cycle of breathing difficulties, fatigue, and depression.<br />

A multidisciplinary group of pulmonary rehabilitation<br />

professionals, including a board-certified pulmonologist,<br />

nurses, respiratory therapists, dietitians, and exercise<br />

physiologists, work together to help participants improve<br />

their quality of life and enhance their ability to perform<br />

daily living activities.<br />

RESTORE<br />

This exercise and education program is a unique<br />

opportunity for individuals with a variety of acute and<br />

chronic medical conditions to benefit from a medically<br />

supervised exercise regimen. RESTORE is designed for<br />

those who are weakened or deconditioned by a variety of<br />

conditions, including arthritis, congestive heart failure,<br />

diabetes, neuromuscular disease, obesity, metabolic<br />

syndrome, and more. Participants follow an individualized<br />

treatment plan for exercise and education in a group<br />

setting, using exercise equipment specifically designed to<br />

meet the needs of patients with physical challenges. The<br />

program is safe and enjoyable with an abundance of<br />

emotional support, guidance, and encouragement. It<br />

includes supervised exercise training to improve fitness,<br />

strength, stamina, and flexibility; condition-specific<br />

education; home exercise and diet instruction; risk factor<br />

modification; instruction on proper use of medications; and<br />

smoking cessation assistance. Participants may be<br />

self-referred or physician referred. The program may be<br />

covered by insurance.<br />

Volunteers<br />

<strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> relies on its volunteers to provide a<br />

number of important services from internal patient<br />

transportation to visitor reception to hospice support. We<br />

invite you to become one of the growing numbers of people<br />

who give their time and talents to help others at the<br />

<strong>Hospital</strong>. The Volunteer Services staff takes special care<br />

to match volunteers with activities they enjoy. The typical<br />

volunteer commitment is one day a week for four hours.<br />

Women’s and Infants’ Center<br />

The birth of a baby is a special time. Each experience is<br />

unique and brings its own special needs. Recognizing that<br />

no two births are alike, the Women’s and Infants’ Center<br />

offers highly personalized care and the support needed<br />

throughout pregnancy, labor, delivery, and the postpartum<br />

period. Women who designate The Women’s and Infants’<br />

Center as their choice for delivery receive information on<br />

all aspects of the birth experience. Our nurses are available<br />

before delivery and remain available throughout the<br />

postpartum period to answer your questions on the birth<br />

experience and new parenting issues.<br />

The Women’s and Infants’ Center itself contains LDRP<br />

(labor, delivery, recovery, postpartum) suites which are<br />

private, independently equipped, and self-contained to<br />

allow new moms to enjoy maximum comfort in the same<br />

room for their entire stay. A whirlpool offers expectant<br />

mothers the option of hydrotherapy relaxation during<br />

labor. A high-tech, yet homey, nursery gives infants<br />

needing special care close attention and assures security.<br />

The Women’s and Infants’ Center staff of registered nurses<br />

was the first in Sheboygan County to attain Neonatal<br />

Resuscitation Provider status.<br />

Classes for both new and experienced parents are offered<br />

throughout the year (see Community Education, page 36).<br />

In addition, the Women’s and Infants’ Center provides<br />

breastfeeding support and counseling for the new moms.<br />

HALO, (see page 52) a bereavement program offers compassionate,<br />

individual care in times of pregnancy loss or<br />

fetal death.<br />

Worker’s Compensation<br />

<strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> is committed to providing quality<br />

occupational health services to meet the unique needs of<br />

each business and its injured and ill workers. When an<br />

employee is injured or becomes ill at the worksite, he/she<br />

is evaluated and treated through the <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong><br />

Emergency Services (see page 38) and Occupational Health<br />

Services (see page 41) on a 24-hour-a-day basis. After this<br />

initial evaluation and treatment, appropriate follow-up care<br />

is coordinated with the injured worker and the employer. A<br />

safe and seamless transition from injury to return to work<br />

is planned. Return to work or light duty placement options<br />

are recommended as appropriate. Suggestions regarding<br />

worksite accommodations are available through consultation.<br />

Isernhagen Work System<br />

A team approach to the treatment of work-related injuries<br />

involves physicians, occupational health professionals,<br />

rehabilitation specialists, the injured worker, and<br />

employers and assists in and enhances the rehabilitation<br />

process. The <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> Center for Pain and<br />

Work Rehabilitation (see page 43) and Occupational<br />

Health Services (see page 41) use the nationally recognized<br />

Isernhagen Work System to bring these facets of treatment<br />

and recovery together. Isernhagen is a total work<br />

rehabilitation and work injury prevention program that<br />

incorporates each part of a work-related injury—from<br />

prevention to rehabilitation to a safe and rapid return<br />

to work.<br />

Isernhagen’s goals are safe, efficient work and productive,<br />

healthy workers. Therefore, its programs may be put into<br />

place on the job site before an injury happens to reduce the<br />

chance of on-the-job injuries and ensure that employers<br />

and workers receive the best outcomes for both the<br />

individual and the company. Isernhagen may be used alone<br />

or in conjunction with the <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> Workplace<br />

Health Program (see page 48).<br />

Department.<br />

Infinity HealthCare, established in 1976, is the oldest and<br />

largest emergency medicine staffing and management firm<br />

in Wisconsin. It is physician-owned and operated by<br />

board-certified emergency medicine specialists. Infinity<br />

provides emergency medicine direction and physician<br />

coverage at 12 major hospitals in Wisconsin and two in<br />

Illinois, including <strong>St</strong>. <strong>Nicholas</strong> <strong>Hospital</strong> sister hospitals,<br />

<strong>St</strong>. Vincent and <strong>St</strong>. Mary, in Green Bay. All Infinity<br />

physicians are residency trained and board certified in<br />

Prevea Health is a partnership between Prevea Health and<br />

provide education and social activities for individuals with<br />

diabetes. Speakers and activities help people cope with<br />

various aspects of their condition, while opportunities for<br />

sharing and questions assure participants that they are not<br />

Grief is a feeling of distress caused by separation, or loss...<br />

the loss of a loved one...the loss of a job...loss of a lifelong<br />

dream...and more. Our grief group is not limited to<br />

anyone for any reason, but more inclusive of anyone who<br />

Together We Live with Cancer (TLC)<br />

Together we Live with Cancer (TLC) is a “Supportive<br />

Community” that provides cancer survivors, their families,<br />

friends, and caregivers with health and wellness opportunities<br />

for the body, mind, heart and spirit. TLC offers Group<br />

Survivorship Sessions that meet twice a month, a quarterly<br />

Supportive Newsletter, and a variety of Survivor Activities<br />

throughout the year. All TLC events and activities are open<br />

to anyone experiencing cancer in their life (survivors, family,<br />

friends and caregivers) throughout the many stages of the<br />

disease (newly diagnosed, in treatment, or months and years<br />

after diagnosis). TLC is sponsored by Matthews Oncology<br />
