AUGUST 2011-JULY 2012 ISE Candidate Orientation Information: GeneralThe on-site Candidate Orientation complements the <strong>in</strong>formation conta<strong>in</strong>ed <strong>in</strong> this document. If you have latexallergies, please <strong>in</strong>form staff at check-<strong>in</strong>.The NCCTO staff serves as the Candidate advocate to assist with any problems that may arise. There will not beany on-site tra<strong>in</strong><strong>in</strong>g or familiarization with the simulated arm. As with CSE, Candidates will be expected toproperly perform the skills based on the <strong>in</strong>formation <strong>in</strong> this guide and the evaluation form.Candidates are expected to wear pr<strong>of</strong>essional attire. Use “bus<strong>in</strong>ess casual” as a reference, which the <strong>National</strong><strong>Board</strong> <strong>in</strong>terprets as exclud<strong>in</strong>g jeans, T-shirts, garments that could be viewed to be immodest (e.g. tank tops),tennis shoes, and/or flip flops. Candidates must br<strong>in</strong>g and wear white lab coats.Cell phones must rema<strong>in</strong> turned <strong>of</strong>f dur<strong>in</strong>g the ISE and left with personal belong<strong>in</strong>gs <strong>in</strong> the Candidate’sLocker.No notes or other written materials may be taken <strong>in</strong>to the exam<strong>in</strong>ation room, <strong>in</strong>clud<strong>in</strong>g materials thathave been downloaded from the <strong>National</strong> <strong>Board</strong> website. All notes and written materials must also be left<strong>in</strong> the Candidate’s locker. Candidates may not refer to notes or written materials dur<strong>in</strong>g the exam<strong>in</strong>ation.Additionally, s<strong>in</strong>ce NCCTO is video record<strong>in</strong>g skills, it is important that you know that a candidate determ<strong>in</strong>ed to havefalsified data/f<strong>in</strong>d<strong>in</strong>gs dur<strong>in</strong>g the exam<strong>in</strong>ation or abandoned a patient dur<strong>in</strong>g a skill may receive a score <strong>of</strong> zero for thatskill, station or entire exam<strong>in</strong>ation.It is likely that observer(s) may be present <strong>in</strong> the exam<strong>in</strong>ation room. Observers are not there to watch a specificCandidate or Standardized Patient (SP); <strong>in</strong>stead, they are there to watch the ISE and the exam<strong>in</strong>ation process.Observers are <strong>in</strong>structed not to converse or <strong>in</strong>teract with Candidates or SP <strong>in</strong> the exam<strong>in</strong>ation rooms. Observersare not part <strong>of</strong> the evaluation process.The ISE is comprised <strong>of</strong> 3 dist<strong>in</strong>ct procedures that must be performed <strong>in</strong> order: preparation <strong>of</strong> both medications (simulated fluoresce<strong>in</strong> sodium and simulated ep<strong>in</strong>ephr<strong>in</strong>e), perform<strong>in</strong>g an <strong>in</strong>travenous <strong>in</strong>jection for fluoresce<strong>in</strong> angiography, and perform<strong>in</strong>g an <strong>in</strong>tramuscular <strong>in</strong>jection <strong>of</strong> ep<strong>in</strong>ephr<strong>in</strong>e.Given the nature and criticality <strong>of</strong> the <strong>in</strong>jections to be performed at this ISE, Candidates should assume that:○ an appropriate written Informed Consent has been discussed with and signed by the Patientprior to perform<strong>in</strong>g the <strong>in</strong>jections.○ the Patient’s identity has been verified us<strong>in</strong>g 2 methods such as name, date <strong>of</strong> birth, last 4 digits <strong>of</strong>the SSN, and/or address prior to perform<strong>in</strong>g the <strong>in</strong>jections.○ the nature <strong>of</strong> the <strong>in</strong>jection procedure and the <strong>in</strong>jection site have been verified by ask<strong>in</strong>g the Patientto verbalize his/her understand<strong>in</strong>g <strong>of</strong> these issues.○ the Patient is male and has been found to have a normal BP measurement dur<strong>in</strong>g the pre-procedurework-up.Five m<strong>in</strong>utes are allotted for the Candidate to be <strong>in</strong> the exam room prior to the start <strong>of</strong> the exam. Candidatesmay familiarize themselves with equipment <strong>in</strong> the exam room dur<strong>in</strong>g any rema<strong>in</strong><strong>in</strong>g rotation time after arriv<strong>in</strong>gat the station. No cl<strong>in</strong>ical procedures should be performed before the station <strong>of</strong>ficially beg<strong>in</strong>s, as <strong>in</strong>dicated bythe announcement. Thus, dur<strong>in</strong>g the five m<strong>in</strong>ute <strong>in</strong>terval, Candidates may adjust the arm, sharps conta<strong>in</strong>er, andother equipment as desired but <strong>in</strong>jections may not be performed.The total test<strong>in</strong>g time at the ISE is <strong>in</strong>corporated <strong>in</strong>to Station 4. Candidates are responsible for monitor<strong>in</strong>g theirtime. SP will not rem<strong>in</strong>d Candidates <strong>of</strong> the rema<strong>in</strong><strong>in</strong>g time at the station. If time expires before a Candidatecompletes the station, the items not performed will be scored as <strong>in</strong>correct.1
An announcement will play that <strong>of</strong>ficially beg<strong>in</strong>s each station. The purpose <strong>of</strong> this formal beg<strong>in</strong>n<strong>in</strong>g is tostandardize the amount <strong>of</strong> exam<strong>in</strong><strong>in</strong>g time for all Candidates. Therefore, <strong>Exam<strong>in</strong>ers</strong> and/or SPs have been<strong>in</strong>structed to ignore any Candidate procedures that precede the announcement, <strong>in</strong>clud<strong>in</strong>g general station itemssuch as greet<strong>in</strong>g the SP. Candidates who beg<strong>in</strong> before the announcement start<strong>in</strong>g the station must repeat anytask performed <strong>in</strong> order to be scored. Similarly, no notes are to be written before the announcement, s<strong>in</strong>ce<strong>Exam<strong>in</strong>ers</strong> and/or SPs may be unable to dist<strong>in</strong>guish notes created <strong>in</strong> the room from notes <strong>in</strong>appropriatelybrought <strong>in</strong>to the room. Writ<strong>in</strong>g notes <strong>in</strong> the room before the announcement start<strong>in</strong>g the Station/cycle maybe cause for disqualification from, dismissal from, and/or failure <strong>of</strong> the exam<strong>in</strong>ation. Candidates shouldrema<strong>in</strong> <strong>in</strong> the exam<strong>in</strong>ation room until the announcement <strong>in</strong>dicat<strong>in</strong>g the end <strong>of</strong> that station.The SP will answer questions such as where an equipment switch, room light<strong>in</strong>g control, or cl<strong>in</strong>ical material islocated. However, questions such as how to use the equipment or cl<strong>in</strong>ical material are not appropriate and willnot be answered by the SP. Candidates may ask the SP “where” questions about the equipment and/or roomlight<strong>in</strong>g control throughout the stations dur<strong>in</strong>g the allocated station time. No additional exam<strong>in</strong>ation time willbe provided for any questions and answers.Candidates may reread the <strong>in</strong>structions posted <strong>in</strong> the exam<strong>in</strong>ation room before the station beg<strong>in</strong>s. The SP handsthe Candidate a sheet <strong>of</strong> blank paper upon enter<strong>in</strong>g the room after the announcement that beg<strong>in</strong>s the station.This paper is for record<strong>in</strong>g notes or data and is not scored. At the conclusion <strong>of</strong> each station, Candidates mustleave beh<strong>in</strong>d any notes or data recorded dur<strong>in</strong>g the station.The skill evaluation form conta<strong>in</strong>s the criteria that <strong>Exam<strong>in</strong>ers</strong> use to assess Candidate performance. The criteriaare <strong>in</strong> the form <strong>of</strong> yes-no checklists, and the numbered performance items are sequenced <strong>in</strong> the order <strong>in</strong> whichthey should be optimally conducted. Candidates must perform the procedures <strong>in</strong> the follow<strong>in</strong>g order:preparation <strong>of</strong> both medications (fluoresce<strong>in</strong> and ep<strong>in</strong>ephr<strong>in</strong>e), perform<strong>in</strong>g an <strong>in</strong>travenous <strong>in</strong>jection forfluoresce<strong>in</strong> angiography, and perform<strong>in</strong>g an <strong>in</strong>tramuscular <strong>in</strong>jection <strong>of</strong> ep<strong>in</strong>ephr<strong>in</strong>e.Candidates may alter the sequenc<strong>in</strong>g <strong>of</strong> the numbered items performed with<strong>in</strong> a procedure without loss <strong>of</strong> creditbut are strongly discouraged from do<strong>in</strong>g so. For example donn<strong>in</strong>g <strong>of</strong> protective gloves could occur earlieralthough this sequence may <strong>in</strong>terfere with the Candidate’s dexterity. However, certa<strong>in</strong> items must be performed<strong>in</strong> order to receive full credit (e.g., application <strong>of</strong> the tourniquet prior to <strong>in</strong>jection.) Thus, follow<strong>in</strong>g thesequence as <strong>in</strong>dicated on the evaluation form helps ensure that Candidates will not forget to perform anyrequired item and also facilitates Exam<strong>in</strong>er scor<strong>in</strong>g.Candidates are randomly matched with <strong>Exam<strong>in</strong>ers</strong>.SPs may appear to be neutral. Candidates should not regard this as a personal dislike or an <strong>in</strong>dication <strong>of</strong>performance quality. SPs are <strong>in</strong>structed to conduct the exam<strong>in</strong>ation <strong>in</strong> a personally neutral manner to promoteuniform, equal treatment <strong>of</strong> Candidates. The SPs’ detachment produces a more objective, impartial evaluation.SPs are allowed to say very little other than what has been scripted.SPs are responsible for ensur<strong>in</strong>g safety. If a SP believes that the techniques or procedures used by a Candidatejeopardize safety, the SP has the responsibility to <strong>in</strong>tervene or stop the <strong>in</strong>jection procedure be<strong>in</strong>g assessed. If theSP <strong>in</strong>tervenes, the Candidate will receive no credit for that item. If the SP stops the procedure, the Candidatewill receive no credit for that item and any subsequent items <strong>in</strong> that procedure (e.g., Candidate was stopped onitem 33, the Candidate would receive no credit for items 33-45 but would be allowed to perform items 46-62).Note that this differs from CSE; <strong>in</strong> CSE, if a Candidate is stopped the Candidate will receive no credit for thatitem or the rema<strong>in</strong><strong>in</strong>g items on that skill.Any open wound on a Candidate's f<strong>in</strong>ger or hand must be covered dur<strong>in</strong>g the exam<strong>in</strong>ation. This applies to preexist<strong>in</strong>gwounds and those that may develop dur<strong>in</strong>g the exam<strong>in</strong>ation. In the <strong>in</strong>stance <strong>of</strong> the latter, a band-aid andgloves will be provided to protect the Candidate.2
