2011 Risk Events and Mitigation
• Fire at No. 2 HTU Fractionator Overhead Condensers on 2011 July 02
– Auto-ignition of a mixture of light hydrocarbon which leaked from plugs and
header expansion joints on overhead coolers following a loss of power on
the plant
– All major recommendations following the incident investigation have been
implemented
– Learnings from incident shared with all stakeholders
• Loss of Power and Flooding Incidents
– Two incidents of loss of power in the last year
– Continued discussions with service provider on reliability
– Upgrade of emergency power equipment in progress and installation of
satellite diesel units for basic emergency systems
– Some drainage work done but major study is to be initiated along with
dredging of main river course
• No Extreme Weather Conditions Experienced
– Decision taken to lease Salt Water Reverse Osmosis (SWRO) systems in the
short term
– A permanent SWRO Plant is to be constructed
