7. INSTRUCTIONS TO THE CANDIDATES FORFILLING THE APPLICATION FORM FOR CET-NBE EXAMINATION DECEMBER 20111. There are 3 forms.a. Computer Readable Scanable Formb. Non-Scanable Form.c. Specimen Formd. Check List2. Kindly fill your data in specimen sheet before you fill the Scannable & NonScannable forms. (Specimen sheet has to be retained by the candidate)3. Medical college and State code to be filled in Col. 16 of the applicationform4. Only Non-Scanable specimen forms and the Check List have to besubmitted along with annexure documents.5. The scannable form must be submitted in a separate envelope Nodocuments / drafts have to be enclosed with the scannable form. Thisenvelope should then be put in the bigger envelope alongwith the Nonscannable form and other documents.6.
