Pleasecomplete it carefully <strong>and</strong> completely, <strong>and</strong> mail itto the address listed on this page.Steps to apply• Print your full name EXACTLY as it appearson your passport• Be sure to check off the program <strong>and</strong>INDICATE the QUARTER <strong>and</strong> YEAR you planto attend• Tell us if you will be applying for an F-1student visa• If you are transferring from another schoolor institution, please complete that sectioncarefully• To apply for an F-1 student visa, you willneed documentation from the InternationalEnglish <strong>and</strong> Professional Programs verifyingyou have been admitted to a course of study.To receive this acceptance documentation,you must prove you have the funds tosupport yourself while in the United Statesby sending us a financial statement in U.S.dollars from your bank or a letter fromyour sponsor. If you submit a financialstatement from a third party (e.g., a relativeor employer), you must also provide a letterfrom the statement holder, which states thathe/she is willing to cover your tuition <strong>and</strong>living expenses while you take part in ourprogram.ESTIMATED MINIMUM EXPENSES*(tuition <strong>and</strong> living expenses)Intensive English Program (10-week) $6,800English for Science <strong>and</strong>Technology Program $3,500Communication <strong>and</strong>Culture Program $3,300English for Legal Professionals $3,500Professional Studies Program (varies) $9,300* All fees are subject to change without notice.2.
