TO PROVIE COORDINATING SERVICES FOR TH CITY OF JERSEY CITYDEPARTMENT OF HEALTH AN HUM SERVICES MUCIPAL DRUG ALLIACEPROGRANOW, THEREFORE, BE IT RESOLVED, by the Municipal Council of the City of Jersey City that:1. Subject to such modifications as' may be deemed necessar or appropriate byCorporation Counsel, the Mayor or Business Adnnistrator is authorized to execute theattached agreement with Paul Steffens, LCADC of PRS Consultants, Inc. to provideservices as the Municipal Drug AllianceProgram Coordinator; .2. The total amount ofthe contract shall not exceed $60,000 and the term ofthe contractshall be for one year effective from January 1, 2012 thru December 31, 2012;3.. The award ofthis contract shall be subject to the condition that the contractor providesatisfactory evidence of compliance with the Affrmative Action Amendments to theLaw Against Discrimination, N.J.S.A. 10:5-31 ~ ~.;4. Upon certification by an offcial or employee ofthe City authorized to attest that thecontractor has complied with the specifications in all respects, and requirements.of thecontract met, then payment to the contractor shall be made in accordance with theprovisions of the Local Fiscal Mfairs Law, N.J.S.A. 40A:5-1 ~~.; and5. The Business Entity Disclosure Certification, Chapter 271 Political ContrbutionDisclosure Certification, Certfication of Compliance with the City's Contractor Pay-to-Play Reform Ordinance, and DetermationofValue Certification attached hereto andincorporated herein by reference shall be placed on fie with this resolutionI, ~~, Donna Mauer, Chief Financial Officer, hereby certify that fuds in theamount of $60,000.00 are available in the MuncipalDrug Alliance Account No.02-213-40-260-312 forthe payment of this resolution.
