may be a factor in determining the worth of theenterprise.h. Meritorious service. Praiseworthyexecution of duties over a period of time.Service differs from achievement in that serviceconcerns a period of time, while achievementconcerns an enterprise having definite beginningand end, but not necessarily connected with aspecific period of time.i. Officer. The word “officer” means“commissioned or warrant officer”.j. Non-commissioned officer. The word“non-commissioned officer” (NCO) shall includethe grades Sergeant (E-5) through Sergeant (E-9) and certain designated “specialists”.k. Anniversary year. That period of timecommencing with the Date of Entry into theOHMR (Appointment date, see orders) and tothe subse- quent anniversary date, but notbefore 01 Jan 83, except as noted herein.1-4. Unit Awards2-1. Decorations.a. Medal of Valor. The Medal of Valormay be presented to any member whodistinguishes himself by courageous conduct atthe risk of his life, above and beyond the call ofduty, while in the service of the State. Thisaward shall be made only upon clear, convincingand incontestable proof by an affidavit of at leastone eyewitness having personal knowledge ofthe act or deed.b. Distinguished Service Medal. TheDistinguished Service Medal may be presentedto any member who distinguishes himself byexceptional meritorious service to the State in aduty of great responsibility thereby furthering theinterest of and promoting the security andwelfare of the State.c. Medal of Merit. The Medal of Meritmay be presented to any member any personwho, by unselfish and untiring activities inconnection with the <strong>Ohio</strong> <strong>Military</strong> <strong>Reserve</strong>, hasrendered a distinct service in furthering theinterest of and in promoting the security andwelfare of the State.).d. Purple Cross Medal. The PurpleCross Medal may be awarded to any personwho, during an authorized Aid to Civil Authoritiesmission, suffers physical injury, the magnitude ofwhich requires professional medical attention oftwo occasions (i.e., treatment and follow-uptreatment or inspection) in the course of theirduties and as a result of their duties which aredirectly in fulfillment of the mission. Theinterpretation of "as a result of their duties whichare directly in fulfillment of the mission" meansdirect activities in support of civilians, not OHMRsupport activities, and may be illustrated by twoexamples:(1) Two persons, while on an authorizedACA mission, are injured. In both cases, theinjury was severe enough to require twophysician visits. The first, who was struck by acar while directing traffic is eligible for the awardwhile the other, struck by a car while crossingthe street to go to lunch, is not.(2) Two persons, while on an authorizedACA mission, are injured. The first, who suffersa laceration while clearing tornado debris wouldbe eligible for the award while the second,suffering a laceration while unloading a truck toset up the unit's camp site, is not. Although bothrequired stitches and a follow-up visit forinspection and removal, the second individual'sactivities do not meet the "duties in directfulfillment" requirement.e. Lifesaving Medal. The LifesavingMedal may be awarded to any member of theOHMR who directly renders active assistance toanother individual at any time in the form ofrescue, first aid, CPR, or other aid, withoutwhich the victim would have most likely died orhave suffered grievous bodily harm. Personswho are civilian firefighters, police, medicalpersonnel or other similar emergencyresponders are excluded from receiving thisaward when the action is performed inconnection with their emergency responderresponsibilities.2-2. Awards.a. Commendation Award. TheCommendation Award may be awarded to anycommissioned officer, warrant officer or noncommissionedofficer who has distinguishedhimself by meritorious achievement or servicewhich, while lesser than that required for theaward Medal of Merit, must nevertheless havebeen of service to the OHMR and accomplishedwith distinction. It will be awarded for exemplarybehavior, efficiency and fidelity while serving asa member of the OHMR. The Commander’srecommendation must be based on his ownpersonal knowledge of the individual and hisofficial record of military achievement or civilianmission support.b. Achievement Award. TheAchievement Award may be awarded toindividuals who have distinguished themselvesby meritorious achievement or service, whichwhile lesser than that required for theCommendation Award, must nevertheless havebeen of service to the OHMR and accomplishedwith distinction that reflects the qualities andvalues of the OHMR. This award is notpresented to general officers.c. Search and Rescue Award. TheSearch and Rescue Award may be presented toany member of the OHMR who participates inan actual emergency search and rescue missionas a member of the OHMR, either under trainingduty orders or state active duty orders. Personswho are civilian firefighters, police, medicalpersonnel or other similar emergencyresponders are excluded from receiving thisaward when the action is performed inconnection with their emergency responderresponsibilities.d. Community Service Award. TheCommunity Service Award may be presented toany member who has made significantcontributions to the quality of life in a community,either over a period of time or in some singlesubstantial manner, through their personalinvolvement, dedication and effort.e. Aid to Civil Authority Award. The Aidto Civil Authority Award may be presented to anymember of the OHMR who participates in an Aid


to Civil Authorities (ACA) mission at the directionof Governor.f. Good Conduct Award. This may bepresented to any enlisted person who hasdemonstrated fidelity through faithful and exactperformance of duty, efficiency through capacityto produce desired results, and whose behaviorhas been such as to deserve emulation. Theminimum period of service for consideration forthis award is not less than three (3) years ofcontinuous active enlisted service subsequent to23 May 1949.g. Longevity Service Award. This maybe awarded to any officer for honorable andsatisfactory in the OHMR for a continuous periodof three (3) years. The years will be onanniversary rather than a calendar year basissubsequent to 23 May 1949.h. Soldier of the Year Award. TheSoldier of the Year Award will be presentedannually to a single enlisted ornoncommissioned officer member of the OHMR.The individual will be selected from candidatesrecommended by the Command Sergeant Majorof each brigade and Corps HHD. A four personboard, appointed by the Corps CSM, andconsisting of NCOs in the grade of E-8 or E-9will interview the candidates, each scoring themin six categories. The candidate with the highesttotal score will be selected as the most qualified.Presentation of the award will be made duringAnnual Training. Criteria and points for theaward are:Awards and decorations 5Physical fitness5 <strong>Military</strong> education10Duty performance 15<strong>Military</strong> bearing, self-confidence 20and professionalismPossible Total 70i. Honor Graduate Award. The HonorGraduate Award may be presented to not morethan one person in a graduating class of BELT,PLDC, 31B10, BNCOC, ANCOC and BOC. Theindividual will be selected by the chief courseinstructor and will be determined by thestudent’s performance, scoring, attitude, militarybearing, professionalism, and assistance toothers. There is no requirement that this awardbe made in every graduating class if suitablecandidates are not represented. The graduatingclass must be composed of at least six persons;in classes with less than six graduates a honorgraduate will not be selected. This award maybe awarded to any person who can provideproof of having achieved Honor Graduate statusin any course listed above completed while amember of the OHMR. This award may beawarded in addition to any other award for thesame course of instruction.j. Officer Training Graduate Award.(1) The Officer Training Graduate Awardmay be awarded to any member of the OHMRwho, while a member of the OHMR, hassuccessfully completed an officer professionaldevelopment course. Officer professionaldevelopment courses are:(a) Basic Level – Basic Officer Course(BOC),(b) Level 2 – Basic Branch OfficerCourse (BBOC)(c) Level 3 – Advanced Branch OfficerCourse (ABOC)(d) Level 4 – Command & General Staff(C&GS)(e) Level 5 – War College (WC)(2) These courses may be completedeither as a resident student at an OHMR orOHARNG school or through correspondence viathe Army Institute for Professional Development,or through similar attendance or correspondenceprograms at another military component school.(3) A numeral device shall be affixed toribbon to indicate the completion of specificlevels of subsequent courses. The basic level(BOC) is indicated by the plain ribbon; the basicbranch level (BBOC) by the numeral 2; theadvanced branch level (ABOC) by the numeral3; the Command & General Staff level (C&GS)by the numeral 4; and the War College level bythe numeral 5.(4) Receipt of this award precludes theawarding of any other award for the samecourse of study, except the Honor GraduateAward and Federal Service School Award.(5) Members who have completedofficer professional development courses prior tojoining the OHMR will not be awarded this awardfor those courses. However, upon completion ofthe next appropriate level of training while anOHMR member, soldiers may be awarded thecommensurate award.k. NCO Training Graduate Award.(1) The NCO Training Graduate Awardmay be awarded to any member of the OHMRwho, while a member of the OHMR, hassuccessfully completed an NCO professionaldevelopment course. NCO professionaldevelopment courses are:(a) Basic Level - Primary LeadershipDevelopment Course (PLDC),(b) Level 2 - Basic Non CommissionedOfficer course (BNCOC)(c) Level 3 - Advanced NonCommissioned Officer Course (ANCOC)(d) Level 4 - Command Sergeant MajorCourse


(2) These courses may be completedeither as a resident student at an OHMR orOHARNG school or through correspondence viathe Army Institute for Professional Development,or through similar attendance or correspondenceprograms at another military component school.(3) A numeral device shall be affixed toribbon to indicate the completion of specificlevels of subsequent courses. The basic level(PLDC) is indicated by the plain ribbon; thebasic course level (BNCOC) by the numeral 2;the advanced course (ANCOC) by the numeral3; and the Command Sergeant Major course bythe numeral 4.(4) Receipt of this award precludes theawarding of any other award for the samecourse of study, except the Honor GraduateAward and Federal Service School Award.(5) Consistent with US Army TRADOCpolicy, the First Sergeant Course is notconsidered an NCO professional developmentcourse. The NCO Training Graduate Award maynot be awarded for completion of the FirstSergeant Course.(6) Members who have completed NCOprofessional development courses prior tojoining the OHMR will not be awarded this awardfor those courses. However, upon completion ofthe next appropriate level of training while anOHMR member, soldiers may be awarded thecommensurate award.l. Basic Entry Level Training CompletionAward. The Basic Entry Level TrainingCompletion Award may be presented to anymember of the OHMR upon satisfactorycompletion of the OHMR basic entry leveltraining requirements. Persons for whom basicentry level training requirements are waived dueto previous experience or training will not beawarded this award.m. Physical Fitness Award. The PhysicalFitness Award may be presented to any memberof the OHMR who:(1) maintains a height/weight level of10% less or lower than the maximum allowablelevel for their age group as specified in OHMRRegulation 40-1 for a period of one year and(2) maintains a hip/waist ratio of .95 formales and .80 for females, or less, determinedas specified in OHMR Regulation 40-1, for aperiod of one year and(3) scores 220 or higher on the ArmyPhysical Fitness Test (APFT) during the year.Points (1) and (2) above will be verified by aquarterly weigh-in and measurement performedby a responsible NCO or officer in the soldier’schain of command. Point c will be verified by aresponsible NCO or officer in the soldier’s chainof command by testing at any point during thesoldier’s year. Verification documents must besubmitted with the recommendation for award.n. Federal Service School Award. TheFederal Service School Award may be awardedto any member for the successful completion ofany course accomplished as a resident studentof at least four (4) days and any U.S. ArmedForces or National Guard Service School, thecontents of which will aid the training or fullmobilization mission of the OHMR in general,including aid to civil authorities. While notmandatory because there are exceptions, thecourse should be related to the individual’sgrade, branch and primary or secondary MOS.Evidence of completion of such a course, with aminimum of twelve credit hours, is mandatoryand it must be in the form of an official diplomaor certificate from the service school. Thisaward may be awarded in addition to either theOfficer Training Graduate or the NCO TrainingGraduate provided all requirements of bothawards are met.o. <strong>Military</strong> Indoctrination Award. The<strong>Military</strong> Indoctrination Award may be awarded toany member who successfully completes anymilitary indoctrination course including entrylevel courses appropriate for the individual’sgrade, branch and primary or secondary MOS.This will be accomplished by correspondence oras a resident student of at least two (2) days atany U.S. Armed Forces, National Guard or <strong>Ohio</strong><strong>Military</strong> <strong>Reserve</strong> Service School or comparableeducational institution. Evidence of completionof such a course with a minimum of 15 credithours is mandatory and must be in the form ofan official diploma, certificate or a letter from anofficer or instructor of the institution ororganization offering the course. In no case willthe <strong>Military</strong> Indoctrination Award be awardedwith the Officer Training Graduate Award or theNCO Training Graduate Award for the samecourse of study.p. Emergency Service Training Award.The Emergency Service Training Award may beawarded to any member who has satisfactorilycompleted any course dealing with military orcivilian emergencies by correspondence or as aresident student in a U.S. Armed Forces,National Guard, <strong>Ohio</strong> <strong>Military</strong> <strong>Reserve</strong> ServiceSchool or civilian school program. This includes,but is not limited to, areas of planning,coordination, management, or execution and, forexample, it includes such areas as CPRcourses, American Red Cross First Aid courses,Radiological Monitoring, Riot Control, etc.Evidence of completion of such a course with aminimum of 15 credit hours is mandatory andmust be in the form of an official diploma,certificate or letter from and officer or instructor


of the institution or organization offering thecourse.q. Volunteer Service Award. TheVolunteer Service Award may be awarded to anymember who has completed a minimum of 100hours of volunteer service to the OHMR duringhis anniversary year, exclusive of attendance atdrills, staff meetings, ceremonies, and parades.For those personnel who achieve a minimum of500 hours, the Service Award will have a goldoak leaf attached.r. <strong>Military</strong> Readiness Award. The<strong>Military</strong> Readiness may be awarded to anymember for 100% attendance at all assignedunit drills, plus annual training, within theanniversary year.s. Recruiting Achievement Award. TheRecruiting Achievement Award may be awardedto any member who recruits five (5) newenlistees or officers within any twelve (12) monthperiod. Certification of the enlistees andappointments will accompany recommendationsfor the award and a list indicating the names anddates of those enlisted or appointed is required.Re-enlistments do not qualify in meeting thecriteria for this award.t. <strong>Military</strong> Proficiency Award. The<strong>Military</strong> Proficiency Award may be awarded toany member who has attained a military rating ofexcellent or superior in the performance of hisduties as well as displaying a high level ofproficiency. The minimum time required is oneyear (12 months).u. Small Group Instructors TrainingCourse Graduate Award. The SGITC GraduateAward will be awarded to anyone who, while amember of the OHMR, satisfactorily completedthe Small Group Instructors Training Course.2-3. Unit Awards.a. Outstanding Unit Citation Award (withGold Frame). This award is intended to fosterunit morale and to provide incentive and espritde corps by recognizing those units which meeteither of the following criteria, or thoseorganizations which meet the criteria inparagraph (2):(1) Service: Maintain an average unittraining assembly attendance of 90% or betterfor eight (8) or more of the twelve (12) months ofthe training year; and to have achieved a ratingof satisfactory on the last IG report with nounsatisfactory rating in any sub-areas thereof.(2) Achievement: Any unit ororganization which has distinguished itselfthrough performance of a specific act, theperformance of which must have beenaccomplished in a manner that clearly exceedsthat which is normally expected.(3) Award and Wear. The OutstandingUnit Citation Ribbon (OUCA) is awarded to unitsor organizations as well as the members ofthose groups.(a) When a unit or organization isawarded the OUCA, all current members of thatunit or organization are permanently awardedthe award. The award order will be maintainedin their personnel file and they may continue towear the award regardless of future unit ororganizational assignments.(b) Persons who were not assigned tothe unit or organization when the award wasmade, but are subsequently assigned to the unitor organization, may wear the award only aslong as they are so assigned. No record of theaward will appear in their personnel file. Upontransfer out of the unit or organization, they mayno longer wear the award.b. Commanding General’s MeritoriousUnit Service Award (with Gold Frame). Thisaward may be awarded at the direction of theCommanding General to any unit or organizationwhich the CG determines has made a significantcontribution, above and beyond the norm, to thesuccess of the OHMR. This award will include agold frame and will be worn over the right breastpocket.(1) Award and Wear. The CommandingGeneral's Meritorious Unit Service Award(MUSA) is awarded to members of units ororganizations. When a unit or organization'smembers are awarded the MUSA, all currentmembers of that unit or organization arepermanently awarded the award. The awardorder will be maintained in their personnel fileand they may continue to wear the awardregardless of future unit or organizationalassignments.(2) Persons who were not assigned tothe unit or organization when the award wasmade, but are subsequently assigned to the unitor organization, may not wear the award.2-4. Qualification.a. For members to discharge theoperational mission of the OHMR in the event offull mobilization by the Governor, instruction ortraining will be required for a multitude ofsituations and specialties (MOS), some of whichmay be beyond current experience orimagination. Therefore, any doubt as to theapplicability of the training to qualify for theabove ribbons should be resolved in favor of therecipient. However, HQOHMR will closelymonitor all ribbons awarded to preclude obviousdeviations from the intent herein and provideguidance for future awards of these trainingribbons.


. Any questions relating to thequalifications for any of the above decorations orawards should be directed informally to theAssistant Chief of Staff, Personnel andAdministration (G1), HQOHMR. Differences ofopinion or disputes relating to the abovedecorations or awards will be resolved for theCommander, OHMR, by the Chief of Staff or hisDeputy and that decision will be final.Chapter 3Recommendations and Approval Authority3-1. General. It is the responsibility andprivilege of every individual having personalknowledge of an act, achievement or servicebelieved to warrant a decoration or an award tosubmit a formal recommendation for appropriateconsideration. No decoration or award may beworn unless approved and an order for its awardhas been issued by the proper authority.Persons violating this provision will bedisciplined according to the violation, up to andincluding involuntary separation.3-2. Approval authority.a. The Adjutant General of <strong>Ohio</strong> or hisdesignated representative is the approvingauthority for the following decorations:(1) Medal of Valor(2) Distinguished Service Medalb. The Commander, OHMR, is theapproving authority for the following awards:(1) Medal of Merit(2) Purple Cross Medal(3) Lifesaving Medal(4) Commendation Award(5) Outstanding Unit Citation Award(6) CG Meritorious Unit Service Awardc. Battalion commanders are theapproving authority within their commands, andthe Chief of Staff is the approving authority forHHD, 4 th CSSB, for all awards not listed inparagraphs 3-2a and 3-2b above.3-3. Recommendation for decoration.a. Recommendations of all decorationswill be on OHMR Form 638(RECOMMENDATION FOR DECORATION),documentation attached and be forwardedthrough command channels to HQOHMR, whereit will be presented to an Awards Board createdherein for appropriate recommendation to theCommander, OHMR, for completion of the finalapproval process. While each intermediatecommand will either recommend “approval” or“disapproval” over his signature he does nothave authority to disapprove decorations orawards and this authority will not be delegated.Letters of transmittal will not be prepared as theydefeat the purpose of the Form 638.b. A separate recommendation will besubmitted for each proposed award of adecoration and only one proposed recipient willbe named in the OHMR Form 638.c. All recommendations fordecorations must be received at least ninety (90)days prior to the desired date of presentation.3-4. Decorations Board. An Awards Board offield grade officers will be assembled as neededat the direction of the G1 to review allrecommendations for decorations prior to therecommendation being forwarded to theCommander, OHMR for the final approvalprocess. While the orders appointing such aboard may contain a number of officers, aquorum shall require only three (3) officers. Inunusual circumstances, the board may summonindividuals, including the recipient, to appear inperson before the board. The oral deliberationsof the Awards Board are confidential and exceptfor their written “approval” or “disapproval”, theywill not be in writing or disclosed.3-5. Recommendation for award.a. All recommendations for awardswill be prepared in memorandum format and acopy of the recommendation memorandum willbe placed in the temporary section of theindividual’s local personnel records (MPRJ). Aseparate recommendation will be submitted foreach proposed award (ribbon) but multiplerecipients may be named on eachmemorandum.b. All recommendations for awardsmust be received at least thirty (30) days prior tothe desired date of presentation.3-6. Processing and announcement.a. Recommendations will beforwarded through channels to the brigade S-1.b. The S-1 will immediately forwardthose recommendations requiring higherauthority (refer to paragraphs 3-2a and 3-2babove) to OHMR-G1.c. Brigade level awards will becertified by the brigade S-1 and submitted to thebrigade commander for approval.d. Award recommendations formembers of HHD, Corps, will be forwarded toOHMR-G1 for certification and forwarding toeither higher authority or the Chief of Staff, asrequired in paragraph 3-2 above.e. After approval, the originalrecommendation will be retained by the approvalauthority. The approving authority will issuepermanent orders, forwarding a copy to therecipient's unit personnel manager. The


ecommendation filed in the temporary sectionof the MPRJ will be removed and attached to thepermanent order which will be filed in thepermanent section of the MPRJ.f. If a recommendation isdisapproved, the recommendation will beendorsed with the reason and/or authority fordisapproval and returned to the individual's unitpersonnel manager. The recommendation filedin the temporary section of the MPRJ will beremoved, destroyed, and the disapprovalendorsed recommendation will be filed in thepermanent section of the MPRJ.g. Quarterly, all approval authoritieswill forward to OHMR-G1 copies of allrecommendations which have been acted upon,all relative documentation and all awardannouncement orders issued during thepreceding quarter.Chapter 4Presentation of Decorations, Awards andUnit Awards4-1. Decorations. The Medal of Valor shall bepresented by the Governor and the Award ofMerit may be presented by the Governor or theAdjutant General or a General Officerdesignated by him and the Commander of theOHMR in the name of the Governor. Alldecorations will be presented at an appropriateceremony or will be forwarded with ribbon andcertificates to the individual’s unit commanderwith a letter of transmittal.4-2. Awards and Unit Awards. All awards andunit awards may be presented by theCommander, OHMR, Chief of Staff, or BattalionCommanders, in the name of the Commander,OHMR. Any officer may present awards forwhich he has final approval authority. All awardswill be presented at an appropriate ceremony orwill be forwarded with ribbon to the individual’sunit personnel manager.4-3. Issuance of medal, ribbon or device.a. Decorations. One medal and oneribbon, or one device, will be issued with eachdecoration order from stocks maintained by theaward authority.b. Awards. One ribbon or one devicewill be issued with each award order from stocksmaintained by the award authority.4-4. Procurement of medals, ribbons anddevices.a. Medals. Full size medals fordecorations only will be ordered by the G4Section and maintained in stock to meet theneeds anticipated by the G1 Section.b. Ribbons. Ribbons for decorationsand awards will be ordered by the G4 Sectionand maintained in stock to meet the needsanticipated by the G1 Section. Quantities ofribbons anticipated to be needed each quarterby local award authorities will be ordered fromOHMR-G1 by memorandum or voice message.Local award authorities are not authorized toissue or sell replacement or duplicate ribbons;all supplemental ribbons must be purchased byindividuals through the OHMR-G1 office or itsdesignate.c. Medals for awards. Medals forawards are authorized for wear in accordancewith the provisions of Regulation 670-1, Wearand Appearance of Uniforms and Insignia, andits counterpart, AR 670-1. Medals for awardswill not be procured for or sold to personnelexcept at the direction of the Commander,OHMR.d. Miniature medals. Miniaturemedals for decorations and awards areauthorized for wear in accordance with theprovisions of Regulation 670-1, Wear andAppearance of Uniforms and Insignia, and itscounterpart, AR 670-1. Miniature medals for willnot be procured for or sold to personnel exceptat the direction of the Commander, OHMR.e. Devices. Devices will be orderedby the G4 Section and maintained in stock tomeet the needs anticipated by the G1 Section.Quantities of device anticipated to be neededeach quarter by local award authorities will beordered from OHMR-G1 by memorandum orvoice message. Local award authorities are notauthorized to issue or sell replacement orduplicate devices; all supplemental ribbons mustbe purchased by individuals through the OHMR-G1 office or its designate or commercial outlets.Chapter 5Miscellaneous Awards and Honors5-1. Shoulder Cords.a. Shoulder cords of military pattern(braided and without pendants) of the followingcolors will be awarded to personnel who havemet the following criteria:(1) Green/Golden Yellow - completion ofthe US Army 31B10 School, Phase l, in priorservice; as a resident student or a through aUSARF school. Prior to 01 October 2011. Thiscourse is not available by correspondence.Completion of BELT is also required unlessindividual has completed such training duringprior service and requirement has been waived


in accordance with OHMR policy. Individualmust also hold a valid <strong>Ohio</strong> driver's license.(2) Crimson/Yellow - completion ofOHMR 31B10 requirements as specified in SOP204.00 through attendance at an OHMR 31Bschool prior to 01 October 2010. It will also beawarded going forward from 01 October 2010for completion of OHMR 38C10 <strong>Military</strong>Civil Support Specialist. Completion of BELT isalso required unless individual has completedsuch training during prior service andrequirement has been waived in accordancewith OHMR policy. Individual must also hold avalid <strong>Ohio</strong> driver's license.b. Documentation of the completion ofthe training and eligibility requirements specifiedabove which is filed in the <strong>Military</strong> PersonnelRecords Jacket (MPRJ) or Individual TrainingRecord Jacket (ITRJ) constitutes authority towear the appropriate cord.c. Shoulder cords are readily availableon the market through various uniform andmilitary outfitters. Individuals will acquire cordsat their own expense.5-2. QRST (Quick Response Security TrainingTeam) tab. This tab will be awarded to personsmeeting the requirements of OHMR SOP 405.00or its successor. It will be embroidered in thetraditional military police colors; the backgroundwill be green with the lettering and border goldenyellow. Lettering will read QRST TEAM. Thisaward is only authorized for persons whocompleted training as stated above prior to 01October 2010. (see Figure 2).Figure 2.5-3. Berets.a. Although the wear of berets (eitherdark blue or olive drab only) as an award hasbeen permitted as a matter of practice in thepast and officially under OHMR Policy 98-02since 10 Oct 98, there is currently no Brigadewidestandard of criteria for earning or wearingof the beret. Until such a standard is devised,members of the OHMR will not wear any beretwith the OHMR uniform.b. The wear with the OHMR uniform ofany beret awarded while serving in the U.S.Armed Forces, or any other organization, isprohibited.5-4. Letters of Commendation/Achievement.Local commanders and Chief of Staff sectionsmay, and are encouraged to, recognizeindividual acts not meriting a decoration or anaward through the issuance, publication andpresentation of appropriate Letters ofCommendation or Achievement over thepersonal signature of the Commander or Chiefof Staff. The original will be given to theindividual, one copy will be forwarded toHQOHMR, and one copy will be placed in theindividual’s personnel records jacket.5-5. Certificates of Appreciation. (<strong>Military</strong>) AnOHMR Certificate of Appreciation has beenestablished to recognize individual acts notwarranting decorations or awards or to expressappreciation to individuals or organizations foroutstanding services performed. Certificates willnormally be signed by the Commander or Chiefof Staff of local OHMR units and will be includedas an enclosure to the correspondencedescribed in Paragraph 5-4 above.5-6. OHMR Golden Crest Award. (Civilian)a. May be awarded to any organizationand/or first-line supervisor who hasdemonstrated exceptional support of the OHMRmember on their employment in connection withthe OHMR Employers Support Program.b. Where first-line supervisors do notmeet all the prerequisites enumerated above, aCertificate of Appreciation may be awarded inlieu thereof for this recognition of the faithful andcooperation in the release of employees forOHMR duty and/or training. c. The OHMRGolden Crest Award and the Certificate ofAppreciation will be signed by the Commander,OHMR.d. The presentation will be made by theCommander, OHMR.5-7. Academy Instructor Badge.a. The OHMR Academy InstructorBadge (AIB) is authorized for wear by individualswho are assigned to the Academy as eitherpermanent staff or on a temporary assignmentfor two years as an instructor.b. Individuals must have completedPrimary Leadership Development Course as anenlisted/NCO or Basic Officers Course as anOfficer and must have completed Small GroupInstructors Training Course.c. After completion of a 24 monthcontinuous (no break in service) dutyassignment as an instructor at the Academy, thebadge will become authorized for permanentwear on a soldiers uniform.d. The AIB is to be worn on the rightbreast pocket of the class A, B, and C uniforms.A bright brass pin on metal award will be wornon the class A and B uniforms and a subduedcloth patch will be worn on the class C uniform.The badge will be 2-3/4 inches wide by 2-1/2inches tall.


5-7. Wearing of Decorations, Awards andBadges.a. Only decorations, awards and badgesawarded by the United States Armed Forces, theArmed Forces of a friendly ally, the NationalGuard, the State of <strong>Ohio</strong>, another State, the CivilAir Patrol, the US Coast Guard Auxiliary, theState Guard Association of the United States(SGAUS), or the <strong>Ohio</strong> <strong>Military</strong> <strong>Reserve</strong>Association (OMRA), may be worn on theuniform by members of the OHMR.b. If worn, the following order ofprecedence will apply: Unites States ArmedForces, the armed forces of a friendly ally,National Guard, State of <strong>Ohio</strong>, <strong>Ohio</strong> <strong>Military</strong><strong>Reserve</strong>, other States, the Civil Air Patrol, theUS Coast Guard Auxiliary, and SGAUS, OMRA.c. No other decorations, medals, badgesor ribbons may be worn on the uniform bymembers of the OHMR. An exception may bemade for temporary identification or participantbadges at public events, if required.Chapter 6Auditing Awards6-1. Auditing of awards.a. It is the responsibility of the ACS ofPersonnel and Administration (G1), to randomlyaudit awards on a regular basis to ensurecompliance with all current standards forpromotion. A report of audit findings will besubmitted to the Commander, OHMR not lessthan annually.b. Awards found to have been approvedin contradiction to standards, or without sufficientdocumentation, will be revoked by G1 and theperson authorizing the award(s) subject toadministrative disciplinary action.
