OR(ii) Transfer Certificate/SchoolLeaving Certificate showingapplicant's date <strong>of</strong> birth, issued by thelast school attended by the applicant.In case <strong>of</strong> Govt. schools the transfercertificate/schoolleaving certificate should besigned by the Principal while in thecase <strong>of</strong> Private schools it should alsobe countersigned bythe competent education authority <strong>of</strong>State/or District Education Officer.Attested copy <strong>of</strong> the Certificate <strong>of</strong>Matriculation/Secondary Examinationhaving a record <strong>of</strong> the Date <strong>of</strong> Birth.(No other pro<strong>of</strong> <strong>of</strong> age will be acceptedfor Sr. Secondary admission).Note: (i) In case <strong>of</strong> Orphan/Juvenile/Street Children, the Medical Legal Certificate may be provided as pro<strong>of</strong> <strong>of</strong> Date<strong>of</strong> Birth.(ii)Date <strong>of</strong> birth once given in the Admission Form will not be changed. Please check and state the correct date<strong>of</strong> birth.(b) Minimum • Certificate <strong>of</strong> Class Copy <strong>of</strong> the CertificateEducational VIII passed, or <strong>of</strong> the SecondaryQualifications • Self Certificate (as mentioned at Examination passed from athe back <strong>of</strong> Admission Form) recognised Board <strong>of</strong>Secondary Education dulyattested by a Gazetted OfficerNote: (i) Admission Forms received without supporting documents will be rejected and fee will not be refunded.(ii) If the Secondary level examination is not passed from a recognised Board, admission at Sr. Secondary level will not beconsidered. List <strong>of</strong> recognised Boards is given at Appendix 'C'.(iii) If you possess a High School/Secondary Certificate from any Foreign Board/University and want to take admissionto Senior Secondary course <strong>of</strong> NIOS, you will be required to attach a certificate from the concerned Embassy to theeffect that the examination passed by you is equivalent to the Indian High School/Secondary Examination.(iv) Address Pro<strong>of</strong> is not mandatory for admission.(v) Only a maximum <strong>of</strong> three (03) admissions will be allowed on a given address.2.5 Re-admission for Ex-NIOS StudentsRe-admission is notallowed to thestudents <strong>of</strong> NIOSwhose admission isstill valid.The students <strong>of</strong> NIOS at Secondary/Senior Secondary level who have completedtheir validity period <strong>of</strong> five years <strong>of</strong> admission, but could not complete the course, areeligible to take Re-admission with NIOS. The credit <strong>of</strong> subjects passed (maximum <strong>of</strong>four subjects) in the previous admission will be transferred on submission <strong>of</strong> originalmarksheet for seeking fresh admission, if these subjects are passed by the learnersthrough NIOS during the last ten years (i.e., if passed during 2003 or later). Thelearner is required to pay TOC fee as per details in Table-6A by mentioning Code No. "9901 <strong>National</strong><strong>Institute</strong> <strong>of</strong> <strong>Open</strong> <strong>Schooling</strong> (<strong>National</strong> <strong>Open</strong> School)".
