OR(ii) Transfer Certificate/SchoolLeaving Certificate showingapplicant's date of birth, issued by thelast school attended by the applicant.In case of Govt. schools the transfercertificate/schoolleaving certificate should besigned by the Principal while in thecase of Private schools it should alsobe countersigned bythe competent education authority ofState/or District Education Officer.Attested copy of the Certificate ofMatriculation/Secondary Examinationhaving a record of the Date of Birth.(No other proof of age will be acceptedfor Sr. Secondary admission).Note: (i) In case of Orphan/Juvenile/Street Children, the Medical Legal Certificate may be provided as proof of Dateof Birth.(ii)Date of birth once given in the Admission Form will not be changed. Please check and state the correct dateof birth.(b) Minimum • Certificate of Class Copy of the CertificateEducational VIII passed, or of the SecondaryQualifications • Self Certificate (as mentioned at Examination passed from athe back of Admission Form) recognised Board ofSecondary Education dulyattested by a Gazetted OfficerNote: (i) Admission Forms received without supporting documents will be rejected and fee will not be refunded.(ii) If the Secondary level examination is not passed from a recognised Board, admission at Sr. Secondary level will not beconsidered. List of recognised Boards is given at Appendix 'C'.(iii) If you possess a High School/Secondary Certificate from any Foreign Board/University and want to take admissionto Senior Secondary course of NIOS, you will be required to attach a certificate from the concerned Embassy to theeffect that the examination passed by you is equivalent to the Indian High School/Secondary Examination.(iv) Address Proof is not mandatory for admission.(v) Only a maximum of three (03) admissions will be allowed on a given address.2.5 Re-admission for Ex-NIOS StudentsRe-admission is notallowed to thestudents of NIOSwhose admission isstill valid.The students of NIOS at Secondary/Senior Secondary level who have completedtheir validity period of five years of admission, but could not complete the course, areeligible to take Re-admission with NIOS. The credit of subjects passed (maximum offour subjects) in the previous admission will be transferred on submission of originalmarksheet for seeking fresh admission, if these subjects are passed by the learnersthrough NIOS during the last ten years (i.e., if passed during 2003 or later). Thelearner is required to pay TOC fee as per details in Table-6A by mentioning Code No. "9901 NationalInstitute of Open Schooling (National Open School)".
