The Conoco Phillips Asthma Swim ProgramThe development and production of the “shortwind” resources for Indigenous people.National Heart FoundationThe National Heart Foundation <strong>NT</strong> is primarily responsible for implementing national projects inthe <strong>NT</strong> but they have worked collaboratively with <strong>Health</strong>y Living to develop the <strong>NT</strong>DH&CS fundedCardiac Rehabilitation. The National Heart Foundation is linked in via an MOU to provideresources for this service. In Central Australia the NHF is involved in Community developmentactivities.In 2004 the Heart Foundation <strong>NT</strong> was funded for 0.2 FTE for an Indigenous nutrition worker butthis ceased in 2006. In 2007 a full time worker was employed to work on the national Childhood<strong>Health</strong>y Weight program.Cancer Council of the Northern TerritoryThe Cancer Council of the Northern Territory receives funding from the Northern Territorygovernment in addition to funding received from the Australian government and revenue raisedthrough fund raising. There have not been any specific services implemented to address the<strong>PCD</strong>S, but general health promotion information is available to the general public to promotegood health.Arthritis and Osteoporosis FoundationThe Arthritis and Osteoporosis Foundation has been the sponsor of self management trainingboth for people with Arthritis and Osteoporosis but for chronic disease self managementgenerally and provides support for physical activity to arthritis sufferers.One major difficulty for health providers in the <strong>NT</strong> is that organisational structures often do notallow productive relationships between hospital and community services. NGOs often provide auseful link between acute services in <strong>NT</strong> hospital and community-based services because NGOeducators are able to see people in hospital and follow them up in the community. For example:<strong>Health</strong>y Living <strong>NT</strong> provides rehabilitation and maintenance services in the community in additionto their hospital services, which results in continuity between these services. In Alice Springs in2004, HL<strong>NT</strong> provided a four-week exercise program, individual consultation with the CardiacEducator and the Nutrition Educator, cardiac education, and rehabilitation services to 72 patients,of whom 42 per cent had received prior inpatient education.
