Required Documents for Funeral ApplicationsIntroductionEligibility for OW and ODSP has already been established for ongoingclients at the time of their death. Some additional verification ofdocuments may be required. The CPU CW will determine what isnecessary in order to make a decision.For Non SARs, the CPU CW is required to determine if the deceasedwould have been eligible to receive OW assistance at the time of theirdeath.PolicyThe following documents are required (if not already in the possessionof OW) on behalf of the deceased in order to determine eligibility forfuneral services. Original documents of the deceased are not kept byOW.• Last Will• Birth Certificate, or• Immigration Documents (proof of status in Canada, passport)• Social Insurance Number• Health Card• Verification of Current Bank Account(s)• Earnings/Income Verification• Assets such as RRSPs, Bonds, Investments• Life insurance policies• Prepaid funeral policy• Verification of shelter expenses – Non SARS only• Name of Funeral Home and Cemetery• Important: CPU CW’s must inquire if there is a last Will for thedeceased. The last Will indicates whom the deceased has named asExecutor estate.In all cases where an Executor has been named, only the Executorcan make decisions for the funeral arrangements. CPU CW’s shallonly deal with the Executor in these circumstances.FileStandardsOriginal copies are not retained and/or kept in the master file.All original identification must be returned to the next of kin.All photocopies must be stamped “Certified True Copy”.Continued on next page6
