To be transferred to inpatient rehabilitation, the following conditions must applya. The patient should be medically stable (see protocol for “Transfer to inpatientrehabilitation – minimal medical requirements and patient stability”)b. The patient should be able to tolerate at least three hours of multidisciplinaryrehabilitation a dayc. An appropriate discharge plan and destination after the rehabilitation programmust be establishedd. The patient and the patient’s family must agree to the rehabilitation program andlocatione. Payment issues must be resolved4. If patient does not qualify for rehabilitation in current condition, the PM&R team will keepthe patient on the consult list and intermittently follow progress to help defineappropriateness of transfer.a. Issues of medical condition will be communicated frequently by the trauma teamto the rehabilitation teamb. Recommendations made by the rehabilitation team will be implemented (unlesscontraindicated) by the trauma team
