I have read and understood the Information Sheet for this study.3. The nature and possible effects of the study have been explained to me.4. I understand that the study involves discussing my thoughts and experiences ofparenting as a teenager in a group situation. The session will last approximately 1 ½hours and that I can review the recorded information at any time during the session.].5. I understand that participation involves the risk(s) that I may feel distressed whendiscussing my circumstances and that I can leave the session at anytime, choose tospeak privately with the facilitator or my service provider. I can also contact anexperienced youth counsellor if I wish.6. I understand that all research data will be securely stored on the 3p Consultingpremises for five years from the publication of the study results, and will then bedestroyed7. Any questions that I have asked have been answered to my satisfaction.8. I understand that the researcher(s) will maintain confidentiality and that anyinformation I supply to the researcher(s) will be used only for the purposes of theresearch.9. I understand that the results of the study will be published so that I cannot beidentified as a participant10.
