Be enrolled in a major in the College of Arts and Sciences or, health science, or a related applied health science oragricultural science major. (___)2. Reflect and focus on previous year academic experiences. Make strategic plans for successful degreecompletion and confirm post-undergraduate degree plans (graduate school, professional school, employment,etc.) (___)3. Achieve an academic status of “good standing” by successfully completing a minimum of 24 credit hours during theacademic year and by achieving a minimum B overall grade point average.(___)*4. Continue to take advantage of academic support services sponsored/co-sponsored by NUCLEUS. (___)5. Check your email daily for correspondence from the NUCLEUS program staff. (___)6. Attend and participate in a minimum of three NUCLEUS sponsored/co-sponsored academic/pre-professionalevents each academic semester. (See list of Participation Opportunties on NUCLEUS website) (___)7. Meet a minimum of twice this academic year with NUCLEUS staff for degree completion progress, careerdevelopment and post-baccalaureate plans. (___)8. Participate in a minimum of two Career Services sponsored-events this academic year for career development.(___)9. Meet with your faculty advisor at least once this academic year to ensure you are on track for successfuldegree completion. Confirm degree checkout with Undergraduate Academic Services (for CAS studentsonly). (___)10. Continue to meet regularly with each of your course instructors during office hours or by scheduled appointments toensure your success in each course. (___)11. Complete a recommendation letter portfolio. (___)12. Complete academic opportunities such as undergraduate research, service learning, field experience etc. (___)13. Complete final drafts of personal statement and update resume. (___)14. Confirm plans to take pre-graduate/pre-professional entrance exams (GRE, MCAT, LSAT etc.) if applicable.(___)15. Complete and submit applications for graduate/professional programs and/or employment. (____)16. Commit to “give back” to NUCLEUS by becoming a peer tutor, peer mentor, participating in student panelsor by initiating & facilitating group study sessions. (___)*Program minimum GPA requirement is 2.75

17. Participate in at least one outreach program activity. Participation may include tutoring, mentoring withinNUCLEUS, serving on a college or student panel, Discovery Days, Blue and Gold Days or participating in otherNUCLEUS or University outreach programs. (___)18. Continue to participate in student affiliate national organization related to your major, career, or professionalinterest. (___)19. Submit a copy of all acceptance letters from graduate/professional/employment opportunities to NUCLEUSStaff. (___)20. For the continual improvement of the NUCLEUS program, participate in and/or complete all evaluationactivities that are sponsored or co-sponsored by NUCLEUS, such as providing feedback on surveys, focusgroups or interview sessions when applicable.
