ENROLLING AND PAYING FOR BENEFITSAs a new hire, you are eligible for Toll Brothers benefits on the first day of the month following 60 days ofservice (called the “effective date”).After your initial new hire enrollment, you will be able to make subsequent changes to your benefit electionsduring the annual Open Enrollment period.You and Toll Brothers share responsibility for paying for your benefits. The amount you are responsible forpaying depends on the coverage options and levels you elect.Toll Brothers has one enrollment period each year. Open Enrollment takes place during the month of May andyour elections are effective July 1 each year.You will be able to enroll in, or make changes to, the following benefits during the enrollment period:■■Medical and Prescription Drug coverage■■Dental coverage■■Vision coverage■■Health and Dependent Care Flexible Spending Accounts■■Supplemental Life and Accidental Death and Dismemberment (AD&D) Insurance■■Voluntary Short-Term Disability InsuranceMAKING CHANGES TO YOUR BENEFITSIRS rules allow you to pay for certain benefits with pre-tax contributions, but restrict the changes you can make tothem during the year. Unless you have a qualified life status change, you cannot make changes to your benefitsuntil the next open enrollment period.Examples of life status changes include:■■Marriage or Divorce■■Legal separation■■Birth or adoption of a child■■Death of an eligible dependent■■A dependent losing eligibility for coverage(e.g. a child reaches maximum age,or your spouse loses coverage)■■You or your spouse become eligible orineligible for Medicare■■Change to spouse’s benefits during his/her employer’s open enrollment■■A termination or commencement ofemployment by you, your spouse ordependent child(ren)If you experience a qualified life status change, you must contact the <strong>Benefits</strong> Group at 866-947-3378 orbenefits@tollbrothersinc.com within 60 days of the status change. You will be required to submit documentationof the qualified life status change, such as marriage certificate or proof of adoption. Any changes you make to yourbenefits must be consistent with the nature of your qualified life status change.Note: If you do not notify the <strong>Benefits</strong> Group within 60 days of your qualified life status change, you must waituntil the next Open Enrollment period to make changes to your benefits.5
