MEDICAL PLANToll Brothers offers you comprehensive Medical and Prescription Drug coverage. You have a choice of coveragelevels within the medical plan so you can decide which one is best for you and your family: employee, employee+ child(ren), employee + spouse and family.Please refer to the Medical plan inserts located in the back of this benefit brochure for coverage and rate options.Important note: If you do not make a Medical election when you are first eligible, you will not have Medicalcoverage and will not be able to enroll until the next annual Open Enrollment period, unless you experience aqualified life status change.Your coverage will automatically renew at current levels if you do not make changes during Open Enrollment.SPOUSAL SURCHARGEYou have the opportunity to cover your spouse under theMedical plans offered by Toll Brothers. However, if your spousehas the ability to elect Medical coverage with his/her employer,and has voluntarily elected not to join his/her employer’s plan,a spousal surcharge of $175 per month will be added to yourregular payroll contributions for Medical coverage.The spousal surcharge will not apply if:■■Your spouse is not employed■■Your spouse is self-employedImportant note: If you have elected medicalcoverage that includes coverage for yourspouse, and one of the conditions listedapplies, you will be required to complete andsign an affidavit each Open Enrollment. Also,supporting documentation will need to besubmitted to have the spousal surchargewaived. If this is not completed, the spousalsurcharge will be added to your payrollcontributions.■■Your spouse is not eligible for Medical coverage through his/her employer■■Your spouse is required to pay the full cost of health coverage through his/her employer■■Your spouse is employed by Toll Brothers6
