PRESCRIPTION DRUG PLANYou will automatically receive Prescription Drug coverage when you enroll in a Medical plan through Toll Brothers.Please refer to the Prescription Drug coverage inserts located in the back of this benefit brochure forspecific information.When you purchase a prescription drug, you will be charged a copay. Your copay will depend on the type ofprescription (generic, preferred, non-preferred) and on how the prescription is filled (retail or mail-order). Beloware some important terms that you should know:■■Participating Retail Pharmacies: You can get up to a 30-day supply of most medications when you purchaseprescription drugs at a participating pharmacy.■■Mail-Order Pharmacy: You have the option of filling prescriptions via mail order for your maintenancemedications. The mail-order program is an affordable and convenient way to fill a three-month supply ofprescription drugs for less than the same copay that you would pay for a three-month supply through yourretail pharmacy.■■Generic: These are drugs that are chemically identical versions of brand-name drugs. They are approvedby the Food and Drug Administration. Because they have the same therapeutic value, but cost less thanbrand- name drugs, you can save money without sacrificing quality.■■Preferred: These are brand-name drugs that are on your Medical plan’s formulary. They are typically moreexpensive than generic, but less expensive than non-preferred brand-name drugs.■■Non-Preferred: These are brand-name drugs that are not on your Medical plan’s formulary. They are typicallythe most expensive drugs.7
DENTAL & VISIONDENTAL PLANSToll Brothers offers Dental plan options toemployees. Plans cover preventive care, basicservices and major restorative care for you andyour covered dependents.VISION PLANToll Brothers offers a Vision plan to employees.The plan covers annual exams, glasses and contactlenses for you and your dependents.Important note: If you do not make a Vision orDental election when you are first eligible, youwill not have coverage and will not be able toenroll until the next annual Open Enrollmentperiod, unless you experience a qualified lifestatus change.Your coverage will automatically renew atcurrent levels if you do not make changesduring Open Enrollment.Please refer to the enclosed plan inserts in the back ofthis benefit brochure for specific information.8
