Guidelines for Filling out the GBV Reporting Form
Step 1: Enter the name of the county from which the reports originate
Step 2: Enter the name of the district
Step 3: Enter the name of the Organization/Agency providing the information
Step 4: Enter to reporting month. (Note: the month runs from the 1st to the last day. For example: June 1 – 30)
Step 5: Enter the year (ex. 2007)
Step 6: Enter the information for each type of incident, one at a time. For example, fill out all information
for Attempted Rape before going on to Rape, etc.
Step 7: Enter the total number for each column
Other Support
Step 1: Enter before each support item the number of beneficiaries, followed by the kind of materials
provided. For cash support, enter cash amount and reason for support. Description of skills training
should include type of skills training and training duration.
Type of Activity
Step 1: Enter information for each activity, one at a time. Additional information and/or achievements to
be shared with the Task Force should be written in the box below the table. Please be as brief as
possible.
For the profile of the audience, enter the sector/role of all persons that make up the group for
whom the activity was performed
In the column labeled “Subject Covered”, enter the subjects discussed or covered during each
activity. For IEC materials, provide brief information on the subject of the materials.
Cases Received from referral: Enter the number of referrals received at your facility from other facilities.
This is intended to capture the number of referrals coming to the organization/agency.
Cases Referred to Other Organizations/Agencies: Enter the name(s) of the organization and number of
referrals sent to other organizations/agencies. This is intended to counter-check the exchange of referrals.
Definitions
1. Gender-based Violence (GBV): An umbrella term for any harm that is perpetrated against a person’s
will, and that results from power inequities that are based on gender roles.
2. New Case: a survivor coming to your organization/agency for the first time to report an incident. Note
that the survivor is not the case. It is the incident that is the case. Also a survivor may come back
more than once for services related to different incidents.
3. Follow-up: a survivor returning to your organization/agency for the second, third, fourth time, etc.
These visits are intended to check up on their status or condition.
4. Referral: a survivor transferred to another organization/agency for further examination. In most cases,
transfers are based on the type of services rendered. For example, the police will transfer a survivor
to the hospital for medical services or to the court for legal services.
5. Survivor: a person who has experienced violence or other abuse.
6. Perpetrator: a person, group of persons, or institution that directly inflicts or otherwise supports
violence or other abuse inflicted on another.
7. Minor: according to the United Nations Convention on the Rights of the Child, a minor is a person
under the age of 18.
8. Rape: an incident in which:
a) a person intentionally penetrates the vagina, anus, mouth or any other opening of another person (male or
female) with his penis, without the victim’s consent;
b) he or she intentionally penetrates the vagina or anus of another person with a foreign object or with any
part of the body other than the penis, without the victim’s consent; and/or
c) the victim is less than 18 years old, provided the actor is eighteen years or above
9. Attempted rape: any attempt to perpetrate a non-consensual act of sexual intercourse.
10. Incest: Sexual abuse occurring within the family. Although most often perpetrated by a father,
stepfather, uncle, brother, or other male family member, incest may also perpetuated by a female
relative.
