Launch of the LotuMoui Pasefika Grant<br />
Thursday 23rd November 2.15pm (Main arena)<br />
As part of <strong>Counties</strong> <strong>Manukau</strong> District Health Board’s commitment to improving health outcomes for<br />
Pacific peoples in the District, CMDHB has developed the Paseifka LotuMoui Grant for Pacific church<br />
based health projects. This Grant aims to provide opportunities for Pacific Churches to develop and<br />
implement health promotion and disease prevention programmes that will support their<br />
congregations to live healthier and active lifestyles.<br />
To apply you must meet the following criteria:<br />
• A church organisation<br />
• Your church must be situated in the <strong>Counties</strong> <strong>Manukau</strong> district<br />
• You must have an Incorporated Trust or Charitable society status<br />
• The Majority of your congregation is of Pacific descent<br />
There are two levels of funding available at the amounts of $3,000 and $5,000.<br />
To be eligible for the Grant you must supply the following information with your Application<br />
Form:<br />
• Confirmation of your GST number<br />
• Evidence of your legal status and infrastructure<br />
• <strong>Programme</strong> Plan (no more than three pages)<br />
Application forms will be distributed for the Grant at the end of Day Two.<br />
Housekeeping<br />
Name Cards<br />
You have been issued an identification card. Delegates must wear their identification cards at all<br />
times.<br />
Please make sure you attend the workshop that you have registered to attend as all workshop<br />
spaces have been filled (this information is displayed on your name tag).<br />
Youth Space<br />
There is a marquee set up outside for young people. All lunches for the youth delegates will be<br />
served in the Youth Space.<br />
Staff<br />
If you require any assistance during the two days, staff members will be on hand to support you. They<br />
can be easily identified by their name tags.<br />
Time Keeping<br />
<strong>We</strong> would ask that you please keep to the times outlined in the programme as much as possible,<br />
thank you.<br />
<strong>We</strong> hope you enjoy your two-day experience at LotuMoui and look forward to hearing about your<br />
visions and aspirations for improving the health and wellness of Pacific peoples in <strong>Counties</strong>-<strong>Manukau</strong>.<br />
