New Medical Plan Option<br />
MDLive from Blue Cross Blue Shield of Illinois connects you with a doctor from<br />
the comfort of your home or office, using your computer or mobile device. See<br />

You will have a new medical plan option in 2017—the $6,550 Deductible Plan<br />

The deductible amounts for two of our current medical options will increase<br />
due to rising healthcare costs:<br />
• The $350 deductible plan will change to a $400 individual/<br />
$800 family deductible<br />
• The $800 deductible plan will change to a $900 individual/<br />
$1,800 family deductible<br />
Employees who take prescription medication on an ongoing basis will be<br />
required to fill prescriptions through the mail order program after the first two<br />
refills at a retail pharmacy. See page 8 for details.<br />
