SHC Early Discharge Project<br />

The South Health Campus (SHC) Hospitalists worked with multidisciplinary team members to develop and<br />

implement a new discharge model. A number of improvement cycles were conducted to streamline the<br />

admission to discharge pathway to improve timely discharges and mitigate the challenges experienced as a<br />

result of high capacity.<br />

Discharge planning education, launch of Discharge Education Resource Tool Kit and the Patient Discharge by<br />

10am poster occurred and it is anticipated that these activities will result in increased numbers of patients<br />

discharged by 10am.<br />

Glycemic Optimization (GO) Project<br />

The aim of this project is to improve quality of patient care through<br />

glycemic optimization. Working closely with the Diabetes, Obesity and Nutrition<br />

SCN, interventions such as multidisciplinary education resources, revised order<br />

sets, improved diabetes tracking tools, Hospitalist diabetes data collection, and<br />

case studies were developed.<br />

One in five patients admitted to<br />

medical inpatient units have diabetes<br />

Hyperglycemia is associated with<br />

morbidity and mortality<br />

Diabetic patients admitted to the<br />

hospitalist program were spending<br />

35% of their admission days in a<br />

hyperglycemic state<br />

Diabetic patients have a length of stay<br />

two days longer than patients without<br />

diabetes<br />

Basal Bolus Insulin Therapy (BBIT) is<br />

recommended by best practice<br />

guidelines<br />

Preliminary results include the<br />

statistically significant results of an<br />

increase of BBIT ordered from 27% to<br />

40% and a decrease in<br />

hyperglycemia days > 14 from 35% to<br />

33%. Patient’s length of stay has<br />

seen an overall 14% reduction since<br />

the implementation of the GO Project<br />
