Medical Requirements: The following are the medical requirement for <strong>New</strong> <strong>Mexico</strong> <strong>Military</strong><br />
<strong>Institute</strong>. All should be completed prior to matriculation. If you have any questions, please call<br />
Marshall Infirmary at (575) 624-8235 after 1 August 2012.<br />
Physical Exam: The physical must be legible and complete with dates and signatures. Incomplete<br />
physicals will not be accepted. All physicals must be less than one year old.<br />
Health and Consent Form (HCF): The HCF must be filled out completely. Every question should<br />
be answered. Any question answered YES will need to have an explanation. If you have had<br />
Chicken Pox please mark the appropriate box and have proper documentation from your doctor’s<br />
office of when the illness occurred. Sign and date the back of the form with current phone numbers<br />
(home and cell) and mailing address.<br />
Insurance Cards: Please provide a clear, enlarged copy of your insurance card. Copies of both<br />
FRONT and BACK are needed. Please make sure it is legible. If you can’t read it, we can’t read it.<br />
A copy of widely accepted insurances will be provided to you on a separate document. If you do not<br />
have any of these insurances, it may be beneficial to you to purchase the schools insurance policy to<br />
ensure that all medical bills are paid accordingly. Please note that foreign insurances and<br />
Medicaid from other states are routinely denied insurances. Please check with your states<br />
Medicaid program to see about receiving NM Medicaid. If you choose to use a foreign<br />
insurance, you will be responsible for submitting your medical claims.<br />
Immunizations: Please be aware of the required immunizations to attend <strong>NMMI</strong>. Requirements<br />
vary from state to state and country to country. Please review the immunization schedule for the state<br />
of <strong>New</strong> <strong>Mexico</strong> to ensure that you have all of the appropriate immunizations prior to matriculation.<br />
Please provide a clear legible copy of your immunization record. Most states have electronic<br />
records. Check to see if your state can provide you with a copy of your electronic records.<br />

MMR – Documentation of two doses Menactra Meningococcal<br />
HEP A – Documentation of two doses HPV (females only)<br />
HEP B – Documentation of at least three doses Influenza (annually)<br />
OPV or IPV – Documentation of at least three doses<br />
Tetanus or Tdap – Documentation of dose in the last ten years<br />
Varicella – Documentation of immunization or of having illness<br />
