At the time of registration, please submit a legible copy of the latest Child Study Team Evaluation and IEP Conference Report. This information is essential to plan your child’s participation in our program. If you do not have a clear copy of these papers, please request that your child’s school send them to you. FEES Early Registration: February 6–March 16 Program: Residents—$1,265 Nonresidents—$3,800 Transportation: Residents only—$250 Nonresidents—Not eligible General Registration: March 16–May 4 Program: Residents—$1,340 Nonresidents—$3,875 Transportation: Residents only—$300 Nonresidents—Not eligible A late fee of $75 will be charged for all resident and nonresident registrations received after 4:30 p.m. on Friday, May 4. Registration and transportation fees must be paid in full at time of registration. To register in the program, a registration form must be secured from the Department of Recreation, or downloaded from our website at www. specialneedsebnj.com, filled out completely, and returned with the appropriate fee. Registration will NOT be permitted and fees will NOT be accepted unless the appropriate registration form is submitted at the same time. REGISTRATION 1. All fees must be paid in full for registration to be complete. 2. All forms must be completely filled out before a request for registration will be processed. 3. The registration fee must accompany the submission of the registration materials. This money will be held as a deposit until notification of acceptance into the program. If we are unable to accept the participant, all money sent in with the registration materials will be returned. 4. The Department reserves the right to evaluate registration requests and will determine if a registrant can be accepted into the program. 5. Admission to one program does not guarantee acceptance in future programs. 6. Please do not have the medical form completed until you have received notification of your child’s acceptance into the program. Tuesday Evening Bowling at Brunswick Zone Carolier* Our ‘teams’ are formed according to age and ability to maximize each participants’ enjoyment of the activity. Our staff provides assistance to each individual as is needed. We neither foster nor encourage competition, but emphasize each participant’s uniqueness and assist them to develop and improve their skills and abilities. Throughout the program, the staff encourages the development of social interaction. Pizza will be served each evening at the conclusion of bowling and is included in the registration fee. Thursdays at Daisy (T.A.D.)* Recreational activities will be offered to youth and young adults with special needs. A variety of activities will be offered including social games and activities. The emphasis will be on providing opportunities for social growth and peer interaction. Our activities are planned to promote the concept of “education through recreation.” As in all of our programs for people with special needs, our staff will provide assistance to each participant sufficient to make their participation a positive experience. This program is funded by the Township of East Brunswick, The Daisy Association, and through a grant from the State of New Jersey, Dept. of Community Affairs. Counselor-In-Training Daisy Recreation Program Through a planned and supervised program, the Counselors-in-Training (CITs) will gain experience in working with the special needs participants of the Daisy Recreation Program. Program activities provide an extremely worthwhile and productive experience. Supervision and evaluation of the CITs will take place throughout the program. As paid staff positions become available in subsequent seasons, those CITs who have demonstrated superior ability may be considered for employment. Registration forms for the CIT Program are available at the Department of Recreation and must be filled out and signed by both parent and registrant before registration is complete. Both registration forms and fees must be submitted at the same time. All potential CITs must contact Janice Pennington, Supervisor at 732-390-6797 (Voice/TTY) before registering. Friday Nite Live for People with Special Needs Friday Nite Live is a recreational, social, and educational program for people with special needs. For 21 years, this program has afforded persons who are less able to independently travel, the opportunity to participate in a variety of community and statewide recreational activities. Our staff provides assistance to participants, allowing them to function in an independent and safe manner. Spring: Our seven week program will include Bingo, a spring fling dance, Gypsy at Playhouse 22, bowling, and karaoke. Summer: A seven week program of fun including Bingo, bowling, a mystery play, karaoke, summer jam dance, and EBYACP musical production. Previous participants will receive a registration packet in the mail. New applicants should call Fanny Korman, C.T.R.S., Supervisor, Friday Nite Live at 732-390-6797 (Voice/TTY) for information. On occasion, a program may be changed to another evening due to availability of programming and/or facilities. A late fee of $25 will be assessed to all registrations received after the registration deadline.
