Managing your health & wellness:

At CCAD, your health and wellness are important parts of your success. Let’s

be sure you’re ready to stay healthy and thrive as a student! To ensure that you

have all of the necessary information regarding a health issue, please complete

this information as it applies to you. You should not submit this to CCAD. Please

keep it with you as a personal record.

Important reminders & things to consider:

• Have at least two emergency contacts in your phone.

• Keep your insurance card accessible, along with a valid state ID and/or a

copy of your birth certificate.

• Let all of your health care providers know you are attending college.

Schedule your yearly check-up appointments, transfer your pharmacy

needs, and discuss prescription refill options.

• Does your insurance offer a virtual doctor for when you can’t make it to

your primary care doctor? Most insurance companies now offer virtual

access to medical professionals for non-emergency needs. Be sure to

check it out and download the app!

• If you do not have health insurance, contact studentaffairs@ccad.edu to

understand potential options.
