ICT ICT-Based ICT -Based Telemedicine elemedicine System System De Development De elopment (Indonesia) (Indonesia) Project Budget Grant Awarded: US$ 26,992 Supplementary Grants Four sets of PCs from the Department of Electrical Engineering ITB. Program adopted by Bandung Health Office and Community Health Centers. Project Objectives Develop, implement, install and conduct a trial run of eight medical stations for community health centers; and a station for each referral hospital, health office, and the test laboratory. The Primary Community Health Center (Puskesmas) medical stations were located in selected under-served urban and rural areas in Bandung and its suburbs. These stations were used as a base for establishing a pilot network of Internet-based Telemedicine Establish a Pilot network of Puskesmas Internet-based Telemedicine. The system will be used as a case study in preparing the above-mentioned ICT policy recommendation, and proving the validity of usage of the above mentioned Internet-based Puskesmas Telemedicine for selected simple but critical Clinical-data transfers. It is also expected that the system can deliver trial integrated-package of “grassroots” telemedicine services, including limited teleconsultation, simple telediagnostic, telecoordination and teleeducation. Enhance day-to-day Primary Community healthcare delivery, especially for the critically under-served Mother- Child health-care in Puskesmas through the application of Internet-based Telemedicine for Primary Community Health-care. ICT Policy Reform Project Purpose Develop a recommendation to reform the ICT policy in Indonesia, specifically on the social-equity and communitysustainability of the use of Internet networking for social and community application, with emphasis on healthcare related activities. This includes recommendations on infrastructure reform. A shortage of human resources, healthcare facilities and financial resources cause these problems. Being a developing country, the physical-infrastructure development in Indonesia, especially in the field of communication and transportation, is one serious constraint affecting the efficiency of national Primary Community Healthcare delivery. Internet-based Telemedicine system, with its relatively low development and operating costs, could alleviate the current system inefficiencies. More than 7,600 Community Health Centers (Puskesmas) serve more than half of the total population in Indonesia. Due to a shortage of resources, the primary health care system operates on a “referral system” that relies on communication and transportation infrastructure. In the city of Bandung, 70 Community Health Centres serve about 5,000 patients per day. The fundamental problem the project team faced was overcoming the relatively high Maternal Mortality Rate (MMR)and Children Mortality Rate (CMR) in Primary Community Health Care. Through this project, the team plans to continuously decrease the MMR and CMR, while also benefiting other patients of the Telemedicine for Primary Community Healthcare including: under-served communities in rural and urban areas and all the major players in the delivery of Primary Community Healthcare. The team’s general objective is to develop a recommendation to reform the ICT policy in Indonesia to encourage the use of social and community Internet networking, with an emphasis on healthcare related activities. This recommendation also includes infrastructure reform. The development project uses existing Internet technology to further develop PC-based medical stations and field-tests in 12 Puskemas stations. In the process, we developed a recommendation to reform the Internet Communications Technology (ITC) policy in Indonesia. Support from other parties came in form of four sets of PCs from the Department of Electrical Engineering Institut Teknologi Bandung (ITB) and the adoption of the program by the Bandung Health Office and other Community Health Centers. Project Method The system prototype consists of eight Puskesmas. The project team first developed, implemented, installed and tested the pilot network in the eight stations. Then, the team expanded the network to include one health office and one referral hospital. The network delivers an integrated-package of “grass-roots” telemedicine services, which include limited teleconsultation, simple telediagnostic, telecoordination and tele-education Next, the project team implemented a pilot Digital Healthcare Infrastructure using available Internet technology and linked all major components of the Primary Community Healthcare delivery system (Community Health Centers, Referral Health Office, Referral Hospital). This upgrade strengthened the Health Referral system that had been hampered by insufficient infrastructure. And it also enhanced day-to-day Primary Community healthcare delivery, especially for critically under-served Mother-Child health-care in Primary Community Health.
