detecteerd wordt. Daarnaast biedt de<br />
vroege piek en snelle kinetiek van H-<br />
FABP meerwaarde in de directe postoperative<br />
periode na cardiochirurgie,<br />
omdat het mogelijk wordt om nieuwe<br />
ischemische events te onderscheiden<br />
van chirurgische schade. Op dit gebied<br />
is H-FABP superieur aan cTn. Met<br />
name de recente ontwikkeling van een<br />
‘point of care test’ <strong>voor</strong> de bepaling<br />
van H-FABP zal er<strong>voor</strong> zorgen dat de<br />
diagnose van perioperatieve myocardischemie<br />
aanzienlijk versneld wordt.<br />
De toepasbaarheid van de H-FABP<br />
sneltest in de perioperatieve periode is<br />
dan ook onderwerp van onderzoek op<br />
de afdeling Anesthesiologie van het St.<br />
Antonius Ziekenhuis in Nieuwegein.<br />
Naast de perioperatieve fase kunnen<br />
nieuwere biomarkers een belangrijke<br />
rol spelen bij preoperatieve cardiale<br />
risicostratificatie. Met name <strong>voor</strong><br />
NT-proBNP groeit het bewijs dat het<br />
kan bijdragen in het identificeren van<br />
chirurgische patiënten met een verhoogd<br />
risico op cardiale complicaties.<br />
De bepaling van cardiale biomarkers<br />
in de preoperatieve fase kan gebruikt<br />
worden om patiënten te identificeren<br />
die gebaat zijn bij verdere cardiologische<br />
behandeling, zodat de kans op<br />
een cardiale complicatie in de (nabije)<br />
toekomst geminimaliseerd wordt.<br />
