EFEITOS DA DUPLA TAREFA NA MARCHA DE PACIENTES ATÁXICOS<br />
Introdução: São escassas <strong>na</strong> literatura informações que relacionem<br />
a dificul<strong>da</strong><strong>de</strong> apresenta<strong>da</strong> por indivíduos atáxicos<br />
durante a realização <strong>de</strong> ações que <strong>de</strong>man<strong>da</strong>m associação<br />
entre estruturas cognitivas com as estruturas músculoesqueléticas,<br />
configurando <strong>tarefa</strong>s <strong>dupla</strong>s. O estudo <strong>de</strong>ssa<br />
associação é <strong>de</strong> fun<strong>da</strong>mental importância nos <strong>pacientes</strong> atáxicos<br />
tendo-se em vista o papel do cerebelo no armaze<strong>na</strong>mento<br />
<strong>da</strong> memória motora e <strong>na</strong> possibili<strong>da</strong><strong>de</strong> <strong>de</strong> correções<br />
e ajustes durante a execução <strong>de</strong> um ato motor. Objetivo:<br />
Investigar a interferência motora-cognitiva em <strong>pacientes</strong><br />
atáxicos com realização <strong>de</strong> <strong>tarefa</strong>s <strong>dupla</strong>s durante a realização<br />
<strong>da</strong> <strong>marcha</strong>. Método: Foram realizados 4 testes que<br />
envolvem as seguintes ativi<strong>da</strong><strong>de</strong>s: <strong>marcha</strong> simples, <strong>marcha</strong><br />
com <strong>de</strong>man<strong>da</strong> cognitiva, <strong>marcha</strong> com <strong>de</strong>man<strong>da</strong> efetora e<br />
<strong>marcha</strong> com <strong>de</strong>man<strong>da</strong> combi<strong>na</strong><strong>da</strong> cognitivo-motora, avaliando-se<br />
como variável <strong>de</strong>pen<strong>de</strong>nte a veloci<strong>da</strong><strong>de</strong> média e<br />
número <strong>de</strong> passos durante um percurso <strong>de</strong> 10 metros. Resultados:<br />
Verificou-se que o <strong>de</strong>sempenho <strong>na</strong>s 4 <strong>tarefa</strong>s propostas,<br />
relacio<strong>na</strong><strong>da</strong>s ao número <strong>de</strong> passos, apresentou-se<br />
comprometido no grupo experimental (GE) quando comparado<br />
ao grupo-controle (GC) sendo que as <strong>tarefa</strong>s que<br />
envolveram <strong>dupla</strong> <strong>tarefa</strong> tanto motora, cognitiva, quanto<br />
cognitivo-motora associa<strong>da</strong>s impactaram ain<strong>da</strong> mais sobre<br />
o GE <strong>de</strong> forma negativa. Na comparação do GC e GE ao<br />
que se refere à veloci<strong>da</strong><strong>de</strong> média <strong>de</strong> <strong>marcha</strong>, verificou-se<br />
o comprometimento <strong>de</strong> 3 <strong>tarefa</strong>s no GE, com exceção <strong>da</strong><br />
<strong>tarefa</strong> com <strong>de</strong>man<strong>da</strong> cognitivo-motora (p=0,924), sendo o<br />
<strong>de</strong>sempenho <strong>da</strong> <strong>tarefa</strong> <strong>de</strong> <strong>de</strong>man<strong>da</strong> cognitiva o mais comprometido<br />
no GE. Conclusão: Conclui-se que a realização<br />
<strong>de</strong> <strong>dupla</strong> <strong>tarefa</strong> para o paciente com ataxia encontra-se<br />
comprometi<strong>da</strong>, sendo que tanto as ativi<strong>da</strong><strong>de</strong>s que associam<br />
como <strong>tarefa</strong>s secundárias o caráter cognitivo quanto motor,<br />
contribuem para um pior <strong>de</strong>sempenho <strong>na</strong> <strong>marcha</strong> <strong>de</strong>sses<br />
<strong>pacientes</strong>. De forma especial, o acoplamento <strong>de</strong> <strong>tarefa</strong> cognitiva<br />
à <strong>marcha</strong> foi a condição que mais comprometeu a<br />
realização <strong>da</strong> mesma.<br />

Introduction: Scarce <strong>da</strong>ta are available in literature that relate<br />
the difficult showed by people with ataxia during performance<br />
of actions that require association between cognitive<br />
structures and musculoskeletal functions, configuring dual<br />
task. Studying this association is fun<strong>da</strong>mentally important<br />
in patients with ataxia according to the role of cerebellum<br />
in motor memory storage and the possibility of correction<br />
and a<strong>da</strong>ptations during a motor act performance. Purpose:<br />
Investigating the motor-cognitive interference in patients<br />
with ataxia during gait performance of dual tasks. Method:<br />
Four tests involving the following activities were done: simple<br />
gait, gait with cognitive <strong>de</strong>mand, gait with effecter <strong>de</strong>mand<br />
and gait with combined motor and cognitive <strong>de</strong>mand,<br />
assessing as <strong>de</strong>pen<strong>de</strong>nt variables gait speed and number of<br />
steps during a ten meters distance. Results: According to<br />
the number of steps, it was verified that the performance<br />
in the four suggested tasks was <strong>da</strong>maged in experimental<br />
group (EG) when compared to control group (CG), once<br />
tasks that involved dual task (motor, cognitive as well as<br />
cognitive-motor associated) caused impact in EG in a very<br />
negative way. Comparing CG to EG referring to gait speed,<br />
it was verified <strong>de</strong>triment in three tasks in EG, except the<br />
cognitive-motor <strong>de</strong>mand task (p=0,924), once the performance<br />
in the cognitive <strong>de</strong>mand task was the most impaired<br />
in EG. Conclusion: Patients with cerebellar ataxia showed significantly<br />
<strong>de</strong>triment in performing dual tasks, mainly when<br />
cognitive tasks were requested as secon<strong>da</strong>ry task.<br />

INTRodução<br />
Funções relacio<strong>na</strong><strong>da</strong>s ao controle postural, à coor<strong>de</strong><strong>na</strong>ção<br />
motora, aos movimentos oculares e o<br />
controle <strong>da</strong> fala são realiza<strong>da</strong>s por diversas áreas do<br />
Sistema Nervoso, tais como o cerebelo, os tratos<br />
espinocerebelares, a substância negra, a ponte e as<br />
áreas corticais. Lesões em uma ou mais <strong>de</strong>ssas regiões<br />
po<strong>de</strong>m levar a algum tipo <strong>de</strong> ataxia, sendo a mais<br />
conheci<strong>da</strong> relacio<strong>na</strong><strong>da</strong> a lesões cerebelares (Klockgether<br />
et al. 1 2000).<br />
Durante dois séculos <strong>de</strong> estudos relacio<strong>na</strong>dos ao<br />
cerebelo e às doenças cerebelares, enfatizou-se exclusivamente<br />
a função cerebelar <strong>de</strong> regular o movimento<br />
e a coor<strong>de</strong><strong>na</strong>ção. Porém, os mecanismos do<br />
cerebelo apesar <strong>de</strong> <strong>de</strong>votos a ativi<strong>da</strong><strong>de</strong> motora, são<br />
também ativos em <strong>tarefa</strong>s emocio<strong>na</strong>is e cognitivas<br />
(Leroi et al. 2 2002).<br />
Segundo Bugalho et al. 3 (2006), o cerebelo não<br />
atua somente em funções tradicio<strong>na</strong>lmente conheci<strong>da</strong>s<br />
como efetoras, mas também po<strong>de</strong>m participar em<br />
funções diversas, por meio <strong>de</strong> vias e circuitos próprios,<br />
como no controle <strong>de</strong> funções cognitivas, contribuindo<br />
com elementos específicos para um resultado<br />
comportamental que é a combi<strong>na</strong>ção <strong>de</strong> várias<br />
operações.<br />
Para Gordon 4 (2007), estudos neurológicos por<br />
imagem têm confirmado a i<strong>de</strong>ia <strong>de</strong> que o cerebelo<br />
entra em ativi<strong>da</strong><strong>de</strong> em processos cognitivos como os<br />
<strong>de</strong> organização espacial, visual e <strong>de</strong> memória <strong>de</strong> movimento<br />
tanto em curto quanto em longo-prazo. Nesse<br />
sentido, disfunções relacio<strong>na</strong><strong>da</strong>s a essas funções após<br />
lesões cerebelares po<strong>de</strong>m ser relacio<strong>na</strong><strong>da</strong>s à <strong>de</strong>sconexão<br />
<strong>de</strong> projeções cérebro-cerebelares, <strong>de</strong>senca<strong>de</strong>ando<br />
repercussões motoras, e também em funções<br />
cognitivas e <strong>de</strong> aprendizagem motora.<br />
As disfunções relacio<strong>na</strong><strong>da</strong>s com o cerebelo <strong>de</strong>pen<strong>de</strong>rão<br />
<strong>da</strong> topografia <strong>de</strong> lesão cerebelar; o cerebelo é<br />
conectado com o sistema reticular (responsável pelo<br />
nível <strong>de</strong> ativação), com o hipotálamo (cuja função<br />
mais específica relacio<strong>na</strong>-se ao sistema autonômico<br />
e expressão emocio<strong>na</strong>l), com o sistema límbico (relacio<strong>na</strong>do<br />
à experiência e expressão emocio<strong>na</strong>l), e com<br />
áreas <strong>de</strong> associações paralímbicas e neocorticais críticas<br />
para regulação <strong>de</strong> funções superiores (incluindo<br />
as dimensões cognitivas) (Kan<strong>de</strong>l et al. 5 2000; Lent 6<br />
2001).<br />
Segundo Bugalho et al. 3 (2006), após a análise<br />

topográfica cerebelar, é possível relacio<strong>na</strong>r como<br />
consequências diretas <strong>da</strong> ataxia tanto alterações relacio<strong>na</strong><strong>da</strong>s<br />
a sintomas e si<strong>na</strong>is motores como o <strong>de</strong>sequilíbrio<br />
<strong>da</strong> <strong>marcha</strong>, incoor<strong>de</strong><strong>na</strong>ção <strong>de</strong> movimento<br />
dos membros, alteração dos movimentos oculares<br />
e a disartria, como alterações em áreas <strong>de</strong> controle<br />
<strong>de</strong> diferentes funções cognitivas, quais sejam: <strong>de</strong> memória<br />
motora e aprendizagem. Assim, <strong>pacientes</strong> com<br />
lesões cerebelares, como os atáxicos, apresentam<br />
disfunções temporais em <strong>tarefa</strong>s que envolvem <strong>de</strong>s<strong>de</strong><br />
um simples movimento <strong>de</strong> <strong>de</strong>do a um piscar <strong>de</strong> olhos<br />
após condicio<strong>na</strong>mento (Spencer et al. 7 2005).<br />
Dentre as alterações mais comuns para os <strong>pacientes</strong><br />
com doenças cerebelares, a <strong>marcha</strong> é a mais<br />
recorrente, sendo <strong>de</strong>scrita como lenta, com base<br />
alarga<strong>da</strong> e apresentando irregulari<strong>da</strong><strong>de</strong>s <strong>de</strong> tempo e<br />
amplitu<strong>de</strong> <strong>de</strong> passos. O balanço po<strong>de</strong> ser unidirecio<strong>na</strong>l<br />
em lesões do cerebelo vestibular, lateralizado em<br />
lesões <strong>de</strong> hemisférios, e predomi<strong>na</strong>ntemente ânteroposterior<br />
em caso <strong>de</strong> atrofia do lobo anterior. Além<br />
do <strong>de</strong>sequilíbrio, a disfunção <strong>da</strong> cinemática do tronco<br />
e <strong>de</strong> membros e a incoor<strong>de</strong><strong>na</strong>ção entre os membros<br />
po<strong>de</strong>m ser responsáveis pelos distúrbios <strong>da</strong> <strong>marcha</strong><br />
(Ebersbach et al. 8 1999).<br />
Dessa forma, são escassas <strong>na</strong> literatura informações<br />
que relacionem a dificul<strong>da</strong><strong>de</strong> apresenta<strong>da</strong> por<br />
indivíduos atáxicos durante a realização <strong>de</strong> ações que<br />
<strong>de</strong>man<strong>da</strong>m mais estruturas cognitivas, como maior<br />
<strong>de</strong>man<strong>da</strong> atencio<strong>na</strong>l, como no caso <strong>da</strong> realização <strong>de</strong><br />
<strong>dupla</strong> <strong>tarefa</strong>, também <strong>de</strong>nomi<strong>na</strong><strong>da</strong> <strong>de</strong> <strong>tarefa</strong>s concorrentes.<br />
A <strong>dupla</strong> <strong>tarefa</strong> é um método que tem sido utilizado<br />
para <strong>de</strong>termi<strong>na</strong>r a <strong>de</strong>man<strong>da</strong> atencio<strong>na</strong>l <strong>de</strong> <strong>tarefa</strong>s particulares<br />
(Wright e Kemp 9 1991). Seu <strong>de</strong>sempenho,<br />
conhecido também como <strong>de</strong>sempenho simultâneo,<br />
envolve a execução <strong>de</strong> uma <strong>tarefa</strong> primária, que é o<br />
foco principal <strong>de</strong> atenção, e uma <strong>tarefa</strong> secundária,<br />
executa<strong>da</strong> ao mesmo tempo (Teixeira e Alouche 10<br />
2007).<br />
Segundo Schmidt e Wrisberg 11 (2001), a capaci<strong>da</strong><strong>de</strong><br />
<strong>de</strong> atenção no ser humano, além <strong>de</strong> ser limita<strong>da</strong>,<br />
parece apresentar-se <strong>de</strong> forma seria<strong>da</strong> em relação à<br />
<strong>na</strong>tureza <strong>da</strong> <strong>tarefa</strong> executa<strong>da</strong>, pois normalmente focaliza-se<br />
primeiro a atenção em uma ativi<strong>da</strong><strong>de</strong> para<br />
<strong>de</strong>pois haver possibili<strong>da</strong><strong>de</strong> <strong>de</strong> direcio<strong>na</strong>r a atenção<br />
para duas <strong>tarefa</strong>s ao mesmo tempo. Tentar processar<br />
qualquer combi<strong>na</strong>ção <strong>de</strong>ssas mo<strong>da</strong>li<strong>da</strong><strong>de</strong>s <strong>de</strong> infor-

mação, simultaneamente durante os estágios iniciais<br />
<strong>da</strong> execução <strong>da</strong> <strong>marcha</strong> <strong>de</strong> uma habili<strong>da</strong><strong>de</strong> motora,<br />
tor<strong>na</strong>-se uma <strong>tarefa</strong> complexa.<br />
Para Shumway-Cook e Woollacott 12 (2003), durante<br />
a <strong>marcha</strong>, ao se percorrer um ambiente complexo<br />
ou cheio <strong>de</strong> objetos, informações sensoriais são<br />
solicita<strong>da</strong>s para aju<strong>da</strong>r no controle e <strong>na</strong> a<strong>da</strong>ptação do<br />
an<strong>da</strong>r. Nesse sentido, o comportamento locomotor<br />
inclui também a capaci<strong>da</strong><strong>de</strong> <strong>de</strong> iniciar e termi<strong>na</strong>r a<br />
locomoção, ajustar e a<strong>da</strong>ptar o an<strong>da</strong>r <strong>de</strong> maneira a<br />
evitar obstáculos e alterar a veloci<strong>da</strong><strong>de</strong> e a direção<br />
<strong>de</strong> acordo com o ambiente. Além <strong>de</strong>ssa <strong>de</strong>man<strong>da</strong>, a<br />
<strong>marcha</strong> geralmente contextualiza-se como <strong>tarefa</strong> primária,<br />
já que quando se a<strong>na</strong>lisa o an<strong>da</strong>r <strong>na</strong>s ativi<strong>da</strong><strong>de</strong>s<br />
diárias, este sempre está associado a uma <strong>tarefa</strong> secundária,<br />
como falar, carregar objetos ou associado<br />
a processos mentais internos. Esse acoplamento <strong>de</strong><br />
<strong>tarefa</strong> primária e secundária é aprendido e adquirido<br />
ao longo <strong>da</strong> vi<strong>da</strong> com a prática e a experiência (Bond<br />
e Morris 13 2000).<br />
Durante o processo <strong>de</strong> aquisição <strong>de</strong> uma habili<strong>da</strong><strong>de</strong><br />
motora, o córtex é totalmente exigido enquanto o<br />
processo <strong>de</strong> aquisição se consoli<strong>da</strong>. Após a automatização,<br />
os movimentos passam a ocorrer <strong>na</strong> área subcortical<br />
(núcleos <strong>da</strong> base, cerebelo e tálamo) <strong>de</strong>ixando<br />
a área cortical livre para processar informações mais<br />
complexas. Ou seja, a automatização tor<strong>na</strong> possível a<br />
execução <strong>de</strong> <strong>dupla</strong>s <strong>tarefa</strong>s (Bond e Morris 13 2000).<br />
Em circunstâncias normais, a realização concomitante<br />
<strong>de</strong> <strong>tarefa</strong>s motoras e cognitivas é comum e,<br />
nessas situações, as ativi<strong>da</strong><strong>de</strong>s motoras são <strong>de</strong>sempenha<strong>da</strong>s<br />
automaticamente, ou seja, não requerem<br />
recursos atencio<strong>na</strong>is conscientes. Esse estágio autônomo<br />
<strong>da</strong> aprendizagem <strong>de</strong> uma habili<strong>da</strong><strong>de</strong> motora é<br />
alcançado a partir <strong>de</strong> um processo no qual a prática e<br />
sua variabili<strong>da</strong><strong>de</strong> levam à formação <strong>de</strong> programas <strong>de</strong><br />
ação (Cookburn et al. 14 2003).<br />
Segundo Teixeira e Alouche 10 (2007), quando duas<br />
<strong>tarefa</strong>s são executa<strong>da</strong>s ao mesmo tempo, exigindo<br />
alto grau <strong>de</strong> processamento <strong>de</strong> informações, o <strong>de</strong>sempenho<br />
<strong>de</strong> uma ou <strong>de</strong> ambas é diminuído. Um prejuízo<br />
do <strong>de</strong>sempenho <strong>da</strong> <strong>tarefa</strong> primária <strong>na</strong> execução<br />
<strong>da</strong> <strong>dupla</strong> <strong>tarefa</strong> indica que não há automatização <strong>de</strong>ssa<br />
<strong>tarefa</strong> primária e essa piora no <strong>de</strong>sempenho é consequência<br />
<strong>da</strong> ativi<strong>da</strong><strong>de</strong> <strong>dupla</strong>. Esse prejuízo <strong>na</strong> <strong>tarefa</strong><br />
primária e/ou <strong>na</strong> <strong>tarefa</strong> secundária ocorre, porque<br />
ambas as <strong>tarefa</strong>s competem por <strong>de</strong>man<strong>da</strong>s similares<br />

para o seu processamento.<br />
Assim, uma interferência motora-cognitiva, durante<br />
uma <strong>dupla</strong> <strong>tarefa</strong>, po<strong>de</strong> ser um importante indicador<br />
do estado funcio<strong>na</strong>l em que se encontra um paciente<br />
durante uma doença ou um período <strong>de</strong> reabilitação.<br />
Após uma lesão cerebral, essa alteração po<strong>de</strong> surgir,<br />
e ativi<strong>da</strong><strong>de</strong>s anteriormente automatiza<strong>da</strong>s passam a<br />
requerer maior <strong>de</strong>man<strong>da</strong> atencio<strong>na</strong>l, implicando em<br />
prejuízo durante essas <strong>tarefa</strong>s concorrentes (Teixeira<br />
e Alouche 10 2007).<br />
Para Cookburn et al. 14 (2003), com o processo <strong>de</strong><br />
automatização, tor<strong>na</strong>-se <strong>de</strong>snecessário apresentar<br />
to<strong>da</strong> a atenção <strong>de</strong>sti<strong>na</strong><strong>da</strong> à execução do movimento<br />
primário (no caso, a <strong>marcha</strong>), po<strong>de</strong>ndo, assim, realizarem-se<br />
outras funções secundárias <strong>de</strong> caráter cognitivo<br />
ou motor. Segundo Camicioli et al. 15 (1998) e<br />
Bond e Morris 13 (2000), no paciente neurológico essa<br />
capaci<strong>da</strong><strong>de</strong> <strong>de</strong> realização <strong>de</strong> <strong>dupla</strong>s <strong>tarefa</strong>s encontrase<br />
prejudica<strong>da</strong> principalmente no paciente com lesão<br />
cerebral, porém lesões cerebelares não foram muito<br />
estu<strong>da</strong><strong>da</strong>s sob essa perspectiva. Assim, não há indícios<br />
concretos <strong>de</strong> que, após lesões cerebelares, a capaci<strong>da</strong><strong>de</strong><br />
<strong>de</strong> realização <strong>de</strong> <strong>dupla</strong>s <strong>tarefa</strong>s esteja prejudica<strong>da</strong>.<br />
Portanto, o objetivo <strong>de</strong>ste estudo é investigar a interferência<br />
motora-cognitiva no <strong>de</strong>sempenho <strong>da</strong> <strong>marcha</strong><br />
em <strong>pacientes</strong> atáxicos com realização <strong>de</strong> <strong>tarefa</strong>s<br />
concorrentes.<br />
MéTodos<br />
local<br />
O estudo foi realizado <strong>na</strong> clínica <strong>de</strong> fisioterapia do<br />
Centro Universitário FMU, local on<strong>de</strong> os <strong>pacientes</strong><br />
foram selecio<strong>na</strong>dos.<br />
casuística<br />
Foram selecio<strong>na</strong>dos 6 <strong>pacientes</strong> atáxicos para<br />
constituir o grupo experimental (GE), com média <strong>de</strong><br />
tempo <strong>de</strong> lesão <strong>de</strong> 5,26 anos (DP=3,88), i<strong>da</strong><strong>de</strong> média<br />
<strong>de</strong> 40 anos, que apresentassem ataxia cerebelar, além<br />
<strong>de</strong> ortostatismo e <strong>marcha</strong> in<strong>de</strong>pen<strong>de</strong>nte, com ou sem<br />
uso <strong>de</strong> auxiliares externos. A casuística foi composta<br />
por 3 mulheres e 3 homens.<br />
Os critérios <strong>de</strong> exclusão foram <strong>pacientes</strong> com alteração<br />
visual (como diplopia ou hemianospsia), alteração<br />
cognitiva (com Mini Exame do Estado Mental<br />
acima <strong>de</strong> 23 pontos), afasia <strong>de</strong> compreensão, instabi-

li<strong>da</strong><strong>de</strong> clínica e ataxia por etiologia sensitiva ou vestibular.<br />
Para constituir o grupo-controle (GC) foram recrutados<br />
12 sujeitos saudáveis com i<strong>da</strong><strong>de</strong> entre 40 e<br />
65 anos sem dores músculoesqueléticas, ou doenças<br />
reumatológicas. Em relação ao gênero <strong>da</strong> casuística,<br />
foi composta por 61,1% do sexo feminino e 38,9% do<br />
masculino.<br />
O estudo foi aprovado pelo Comitê Ético interno<br />
<strong>da</strong> Instituição, respeitados os aspectos éticos concernentes<br />
à Resolução <strong>de</strong> nº 196 <strong>de</strong> 10 <strong>de</strong> outubro <strong>de</strong><br />
1996, que <strong>de</strong>limita diretrizes e normas regulamentadoras<br />
<strong>de</strong> pesquisas envolvendo seres humanos. A coleta<br />
<strong>de</strong> <strong>da</strong>dos iniciou-se após assi<strong>na</strong>tura <strong>de</strong> termo <strong>de</strong><br />
Consentimento Livre e Esclarecido contendo explicações<br />
<strong>de</strong>talha<strong>da</strong>s sobre o estudo e sua fi<strong>na</strong>li<strong>da</strong><strong>de</strong>.<br />
MATeRIAIs<br />
Os materiais utilizados foram uma calculadora<br />
convencio<strong>na</strong>l, um jaleco (avental) com bolsos bilaterais,<br />
uma bola <strong>de</strong> ping-pong e um pedômetro Tech<br />
Line.<br />
Delineamento<br />
Após a seleção dos indivíduos, foi realiza<strong>da</strong> a avaliação<br />
<strong>da</strong> veloci<strong>da</strong><strong>de</strong> <strong>da</strong> <strong>marcha</strong> <strong>de</strong> forma auto-selecio<strong>na</strong><strong>da</strong><br />
em uma plataforma <strong>de</strong> 10 metros, sem nenhuma<br />
<strong>tarefa</strong> concorrente (Tarefa 1). O método <strong>de</strong> avaliação<br />
foi utilizar a veloci<strong>da</strong><strong>de</strong> como uma medi<strong>da</strong> isola<strong>da</strong> do<br />
an<strong>da</strong>r funcio<strong>na</strong>l, uma vez que esta é simples, rápi<strong>da</strong><br />
e composta <strong>da</strong>s variáveis temporal e espacial. É importante<br />
a possibili<strong>da</strong><strong>de</strong> <strong>de</strong> calcular a veloci<strong>da</strong><strong>de</strong> <strong>de</strong><br />
an<strong>da</strong>r auto-selecio<strong>na</strong><strong>da</strong> pelo paciente, in<strong>de</strong>pen<strong>de</strong>ntemente<br />
do tipo <strong>de</strong> superfície, uma vez que esse cálculo<br />
representa uma pontuação cumulativa <strong>da</strong> quali<strong>da</strong><strong>de</strong><br />
<strong>da</strong> confiança e <strong>da</strong> capaci<strong>da</strong><strong>de</strong> exibi<strong>da</strong>s pelo paciente<br />
durante o an<strong>da</strong>r.<br />
Sequencialmente foi avalia<strong>da</strong> a <strong>marcha</strong> no mesmo<br />
espaço <strong>de</strong> 10 metros com a ativi<strong>da</strong><strong>de</strong> concorrente<br />
<strong>de</strong> <strong>de</strong>man<strong>da</strong> cognitiva (Tarefa 2), <strong>na</strong> qual o paciente<br />
<strong>de</strong>veria memorizar uma sequência <strong>de</strong> 5 números por<br />
1 minuto e repeti-la verbalmente enquanto realizava<br />
a <strong>marcha</strong>.<br />
Após essa etapa, avaliou-se a <strong>marcha</strong> com a ativi<strong>da</strong><strong>de</strong><br />
concorrente <strong>de</strong> <strong>de</strong>man<strong>da</strong> motora (Tarefa 3).<br />
Para isso, enquanto o paciente realizava a <strong>marcha</strong> <strong>de</strong><br />
10 metros, <strong>de</strong>via pegar uma bola <strong>de</strong> ping-pong situa<strong>da</strong><br />

em um bolso, sendo que havia bolsos bilateralmente,<br />
e transferi-la para o outro. Por fim, foi avalia<strong>da</strong> a ativi<strong>da</strong><strong>de</strong><br />
concorrente <strong>de</strong> <strong>de</strong>man<strong>da</strong>s cognitiva e motora<br />
associa<strong>da</strong>s <strong>na</strong>s quais o paciente, durante a <strong>marcha</strong>, <strong>de</strong>via<br />
repetir verbalmente a mesma sequência numérica<br />
vista anteriormente e ao mesmo tempo digitá-la em<br />
uma calculadora (Tarefa 4).<br />
Como variáveis avalia<strong>da</strong>s <strong>na</strong>s 4 <strong>tarefa</strong>s, foram consi<strong>de</strong>rados:<br />
números <strong>de</strong> passos e veloci<strong>da</strong><strong>de</strong> média,<br />
tanto <strong>na</strong> avaliação do GC quanto no GE. Essas medi<strong>da</strong>s<br />
foram feitas por meio <strong>de</strong> pedômetro e cronômetro<br />
digital.<br />
análise estatística<br />
Na realização <strong>da</strong> análise estatística foram utilizados<br />
os Softwares: SPSS V11.5, Minitab 14 e Excel XP.<br />
Para este trabalho foram utilizados os testes não<br />
paramétricos U <strong>de</strong> Mann-Whitney, Friedman e Wilcoxon.<br />
E, <strong>na</strong> complementação <strong>da</strong> a<strong>na</strong>lise <strong>de</strong>scritiva,<br />
fez-se uso <strong>da</strong> técnica <strong>de</strong> Intervalo <strong>de</strong> Confiança para<br />
média. Sendo assim, a análise dos <strong>da</strong>dos contemplou<br />
avaliação intergrupo (Grupos: GC x GE) e intragrupo<br />
(Tarefas: 1x2x3x4).<br />
Foi <strong>de</strong>finido para este trabalho um nível <strong>de</strong> significância<br />
<strong>de</strong> 0,05 (5%). Todos os intervalos <strong>de</strong> confiança<br />
construídos ao longo do trabalho obtiveram 95% <strong>de</strong><br />
confiança estatística.<br />
Ressalta-se que foram utilizados testes e técnicas<br />
estatísticas não paramétricas porque as condições<br />
como a normali<strong>da</strong><strong>de</strong> e a homogenei<strong>da</strong><strong>de</strong> <strong>da</strong>s variâncias<br />
não foram encontra<strong>da</strong>s nesse conjunto <strong>de</strong> <strong>da</strong>dos.<br />
ResuLTAdos<br />
Os grupos foram compostos segundo as seguintes<br />
características <strong>de</strong>mográficas: 6 sujeitos atáxicos constituiram<br />
o grupo experimental (GE), com média <strong>de</strong><br />
tempo <strong>de</strong> lesão <strong>de</strong> 5,26 anos (DP=3,88); i<strong>da</strong><strong>de</strong> média<br />
<strong>de</strong> 53,3 (DP=4,2). Em relação ao gênero, a casuística,<br />
foi composta por 3 mulheres e 3 homens. Já o<br />
grupo- controle (GC) foi composto por 12 sujeitos<br />
saudáveis com i<strong>da</strong><strong>de</strong> média <strong>de</strong> 50,8 (3,9). Em relação<br />
ao gênero, 61,1% foram do sexo feminino e 38,9% do<br />
masculino.<br />
Na Tabela 1 apresenta-se a análise intergrupo, referente<br />
à comparação do GC e GE, quanto ao <strong>de</strong>sempenho<br />
durante as 4 <strong>tarefa</strong>s relacio<strong>na</strong><strong>da</strong>s à variável

A Tabela 2 apresenta a comparação do GE e GC<br />
no que se refere a variável veloci<strong>da</strong><strong>de</strong> <strong>de</strong> <strong>marcha</strong> durante<br />
as execuções <strong>da</strong>s 4 <strong>tarefa</strong>s.<br />
Nota-se que existe diferença estatisticamente significante<br />
entre o GC e GE tanto para o número <strong>de</strong><br />
passos quanto para a veloci<strong>da</strong><strong>de</strong> <strong>de</strong> <strong>marcha</strong> durante<br />
a execução <strong>da</strong>s 4 <strong>tarefa</strong>s, exceto no que se refere à<br />
veloci<strong>da</strong><strong>de</strong> <strong>de</strong> <strong>marcha</strong> durante a execução <strong>da</strong> <strong>tarefa</strong><br />
com <strong>de</strong>man<strong>da</strong> cognitivo-motora (p=0,924).<br />
A seguir, ilustra-se a análise intragrupo para ca<strong>da</strong><br />
variável mensura<strong>da</strong>.<br />
Com base nos <strong>da</strong>dos <strong>da</strong> Figura 1, consi<strong>de</strong>randose<br />
que quanto menor o número <strong>de</strong> passos melhor a<br />
execução <strong>da</strong> <strong>tarefa</strong> realiza<strong>da</strong> (Balasubramanian et al. 16<br />
2007), po<strong>de</strong>-se observar que no GC as <strong>tarefa</strong>s <strong>de</strong> melhor<br />
<strong>de</strong>sempenho foram as <strong>da</strong> <strong>marcha</strong> (Tarefa 1) e<br />

e motora (Tarefa 3) isola<strong>da</strong>mente. No GE, a or<strong>de</strong>m<br />
relativa ao <strong>de</strong>sempenho <strong>na</strong>s <strong>tarefa</strong>s foi a <strong>marcha</strong> segui<strong>da</strong><br />
pelas <strong>de</strong>mais com mesmo valor.<br />
Consi<strong>de</strong>rando-se que quanto maior a veloci<strong>da</strong><strong>de</strong><br />
média melhor a realização <strong>da</strong> <strong>tarefa</strong> (Von Schroe<strong>de</strong>r<br />
et al. 17 1995), <strong>na</strong> Tabela 3, po<strong>de</strong>-se afirmar que para o<br />
GC a melhor performance ocorre <strong>na</strong> <strong>tarefa</strong> <strong>de</strong> <strong>marcha</strong>,<br />
segui<strong>da</strong> pela cognitiva e, por último, pela motora<br />
e cognitivo-motora, com os mesmos valores. Já para<br />
o GE, a <strong>tarefa</strong> <strong>de</strong> <strong>marcha</strong> apresenta melhor valor, segui<strong>da</strong><br />
pela motora, cognitivo-motora e, por último, a<br />
<strong>tarefa</strong> cognitiva.<br />
Discussão<br />
O alvo <strong>de</strong>ste estudo é investigar os efeitos <strong>da</strong> du-

Nota-se, neste estudo, em relação à veloci<strong>da</strong><strong>de</strong><br />
média, que to<strong>da</strong>s as <strong>tarefa</strong>s para o grupo <strong>de</strong> <strong>pacientes</strong><br />
atáxicos (exceto a cognitivo-motora) apresentaram<br />
diferenças estatisticamente significantes com relação<br />
ao grupo-controle (GC) apresentando maiores valores<br />
em relação ao grupo experimental (GE).<br />
Nesse contexto, para Titianova et al. 18 (2005), a<br />

veloci<strong>da</strong><strong>de</strong> <strong>da</strong> <strong>marcha</strong> afeta parâmetros espaciais e<br />
temporais tanto em sujeitos saudáveis quanto em <strong>pacientes</strong>,<br />
sugerindo que o déficit <strong>da</strong> <strong>marcha</strong> po<strong>de</strong>ria<br />
ser classificado em relação à sua veloci<strong>da</strong><strong>de</strong>. Portanto,<br />
a<strong>na</strong>lisando os resultados acima, po<strong>de</strong>-se indicar que o<br />
GE apresentou uma <strong>marcha</strong> mais comprometi<strong>da</strong> em<br />
relação ao GC, exceto quando utiliza<strong>da</strong>s <strong>tarefa</strong>s cognitivo-motoras.<br />
Segundo Ebersbach et al. 8 (1999), <strong>pacientes</strong> com

doenças cerebelares apresentam alterações <strong>na</strong> <strong>marcha</strong>,<br />
com diminuição <strong>de</strong> veloci<strong>da</strong><strong>de</strong>, irregulari<strong>da</strong><strong>de</strong>s <strong>de</strong><br />
tempo e amplitu<strong>de</strong> <strong>de</strong> passos. O fato <strong>de</strong> o cerebelo<br />
participar <strong>de</strong> múltiplas funções cerebrais (Bugalho et<br />
al. 3 2006), bem como do controle <strong>de</strong> funções cognitivas<br />
e sobre o sistema reticular responsável pelo nível<br />
<strong>de</strong> ativação (Kan<strong>de</strong>l et al. 5 2000; Lent 6 2001) justifica<br />
o seu comprometimento não somente em <strong>tarefa</strong>s<br />
motoras, mas também <strong>de</strong> caráter cognitivo. Essa<br />
afirmação corrobora os achados do presente estudo<br />
<strong>na</strong> medi<strong>da</strong> em que houve diminuição significativa <strong>da</strong><br />
veloci<strong>da</strong><strong>de</strong> média no GE quando comparado ao GC<br />
nessa situação.<br />
Roerdink et al. 19 (2006) realizaram um estudo com<br />
<strong>pacientes</strong> pós AVE em que se verificou relação entre<br />
a realização <strong>da</strong> <strong>tarefa</strong> cognitiva com a <strong>de</strong>terioração do<br />
controle postural (mensurado pelo <strong>de</strong>slocamento do<br />
centro <strong>de</strong> gravi<strong>da</strong><strong>de</strong>). Detectou-se que com a realização<br />
<strong>de</strong> <strong>tarefa</strong> cognitiva houve diminuição <strong>da</strong> estabili<strong>da</strong><strong>de</strong><br />
local e, assim, do controle postural. Esses resultados<br />
po<strong>de</strong>m ser somados aos do estudo presente,<br />
pois <strong>pacientes</strong> atáxicos apresentam <strong>de</strong>sequilíbrio e incoor<strong>de</strong><strong>na</strong>ção<br />
entre os membros, com diminuição do<br />
controle postural, sendo essa per<strong>da</strong> agrava<strong>da</strong> quando<br />
associa<strong>da</strong> à realização <strong>de</strong> <strong>tarefa</strong>s concorrentes, implicando<br />
em <strong>de</strong>terioração <strong>da</strong> veloci<strong>da</strong><strong>de</strong> e número <strong>de</strong><br />
passos.<br />
Segundo Bond e Morris 13 (2000), a automatização<br />
<strong>de</strong> uma <strong>tarefa</strong> tor<strong>na</strong> possível executá-las simultaneamente,<br />
entretanto, os <strong>pacientes</strong> com ataxia não apresentam<br />
disponibili<strong>da</strong><strong>de</strong> total para realizar uma segun<strong>da</strong><br />
<strong>tarefa</strong> concomitante à <strong>marcha</strong>, mesmo esta sendo<br />
uma habili<strong>da</strong><strong>de</strong> extremamente pratica<strong>da</strong> e inerente<br />
ao ser humano. Assim, a simples ativi<strong>da</strong><strong>de</strong> <strong>de</strong> an<strong>da</strong>r<br />
<strong>de</strong>man<strong>da</strong> muita atenção, provavelmente em função<br />
do <strong>de</strong>sequilíbrio, não favorecendo a disponibili<strong>da</strong><strong>de</strong><br />
<strong>de</strong> atenção necessária para a combi<strong>na</strong>ção <strong>de</strong> uma segun<strong>da</strong><br />
<strong>tarefa</strong>.<br />
A<strong>na</strong>lisando os resultados do presente estudo, a<br />
única <strong>tarefa</strong> que não apresentou diferença estatisticamente<br />
significante em relação à veloci<strong>da</strong><strong>de</strong> média<br />
foi quando associa<strong>da</strong> a <strong>de</strong>man<strong>da</strong>s cognitivo-motoras.<br />
Para Cockburn et al. 14 (2003), ativi<strong>da</strong><strong>de</strong>s motoras e<br />
cognitivas, quando combi<strong>na</strong><strong>da</strong>s, <strong>de</strong>man<strong>da</strong>m maior<br />
acio<strong>na</strong>mento e conexão cerebral, aumentando a complexi<strong>da</strong><strong>de</strong><br />
<strong>da</strong> <strong>tarefa</strong>, sendo os efeitos <strong>da</strong> <strong>tarefa</strong> concorrente<br />
mais evi<strong>de</strong>ntes. Essa po<strong>de</strong> ser a razão pela<br />

qual a <strong>tarefa</strong> cognitivo-motora prejudicou <strong>de</strong> maneira<br />
semelhante a veloci<strong>da</strong><strong>de</strong> média tanto do GC quanto<br />
do GE.<br />
Além disso, quando se a<strong>na</strong>lisa a característica e a<br />
<strong>na</strong>tureza <strong>da</strong>s <strong>tarefa</strong>s propostas, po<strong>de</strong>-se dizer que a<br />
<strong>tarefa</strong> concorrente <strong>de</strong> <strong>de</strong>man<strong>da</strong> cognitiva e motora<br />
associa<strong>da</strong> possivelmente prejudicou o controle visual<br />
durante a <strong>marcha</strong>. Nesse sentido, o equilíbrio foi,<br />
então, <strong>de</strong>safiado, o que po<strong>de</strong> explicar o pior <strong>de</strong>sempenho<br />
dos <strong>pacientes</strong> atáxicos em relação ao grupocontrole.<br />
Em relação ao número <strong>de</strong> passos, em to<strong>da</strong>s as<br />
<strong>tarefa</strong>s o GC apresentou menor valor, com importante<br />
diferença estatisticamente significante, quando<br />
comparado ao GE. Segundo Camicioli et al. 15 (1998),<br />
indivíduos saudáveis apresentam dificul<strong>da</strong><strong>de</strong>s ao realizar<br />
a <strong>dupla</strong> <strong>tarefa</strong> durante a <strong>marcha</strong> no que se refere<br />
à quanti<strong>da</strong><strong>de</strong> <strong>de</strong> passos, mas quando comparados à<br />
<strong>pacientes</strong> neurológicos esse prejuízo é muito menor.<br />
Em um estudo realizado por O´Shea et al. 20 (2002),<br />
observou-se que <strong>pacientes</strong> com doença <strong>de</strong> Parkinson<br />
apresentam menor número <strong>de</strong> passos e <strong>de</strong> veloci<strong>da</strong><strong>de</strong><br />
quando executam <strong>dupla</strong>-<strong>tarefa</strong>, fato este que se<br />
opõe a este estudo quando comparados GC e GE.<br />
Isso po<strong>de</strong> indicar que <strong>tarefa</strong>s motoras e cognitivas<br />
po<strong>de</strong>m afetar <strong>de</strong> forma diferente <strong>pacientes</strong> atáxicos e<br />
com doença <strong>de</strong> Parkinson, sendo uma provável causa<br />
a topografia lesio<strong>na</strong>l.<br />
Ao a<strong>na</strong>lisar os <strong>da</strong>dos no que se refere ao grupo<br />
<strong>de</strong> sujeitos atáxicos, ain<strong>da</strong> com relação à <strong>na</strong>tureza <strong>da</strong>s<br />
<strong>tarefa</strong>s propostas como secundárias à execução <strong>da</strong><br />
<strong>marcha</strong>, po<strong>de</strong>-se afirmar que, quanto ao número <strong>de</strong><br />
passos, os sujeitos com ataxia foram perturbados <strong>de</strong><br />
forma similar. Ou seja, acoplar ativi<strong>da</strong><strong>de</strong> <strong>de</strong> memória<br />
à <strong>marcha</strong> impacta com a mesma magnitu<strong>de</strong> que acoplar<br />
<strong>tarefa</strong> <strong>de</strong> coor<strong>de</strong><strong>na</strong>ção entre mãos e <strong>tarefa</strong>s <strong>de</strong><br />
memorização associa<strong>da</strong>s à ativi<strong>da</strong><strong>de</strong> manual. Porém,<br />
ao consi<strong>de</strong>rar a veloci<strong>da</strong><strong>de</strong> média, o fato <strong>de</strong> acoplar<br />
ativi<strong>da</strong><strong>de</strong> cognitiva à execução <strong>da</strong> <strong>marcha</strong> repercute<br />
com magnitu<strong>de</strong> mais acentua<strong>da</strong> do que associar a<br />
<strong>marcha</strong> à ativi<strong>da</strong><strong>de</strong> <strong>de</strong> coor<strong>de</strong><strong>na</strong>ção entre membros<br />
superiores. Esse achado salienta a gran<strong>de</strong> importância<br />
do cerebelo em funções cognitivas e não somente<br />
como receptor e comparador <strong>de</strong> aferências provenientes<br />
<strong>de</strong> diversas regiões do sistema nervoso.<br />
O limitado número <strong>de</strong> sujeitos <strong>na</strong> amostra e a heterogenei<strong>da</strong><strong>de</strong><br />

refere a algumas extrapolações dos achados do presente<br />
estudo para a terapêutica com <strong>pacientes</strong> cerebelares.<br />
Porém, o impacto negativo <strong>da</strong> <strong>tarefa</strong> secundária<br />
cognitiva associa<strong>da</strong> à <strong>marcha</strong> <strong>de</strong>ve ser avaliado<br />
pelo fisioterapeuta como roti<strong>na</strong> <strong>na</strong> prática avaliativa<br />
<strong>da</strong> <strong>marcha</strong> <strong>de</strong>stes <strong>pacientes</strong>. Uma sistematização <strong>de</strong><br />
avaliações quantitativas <strong>da</strong> <strong>marcha</strong> acoplando <strong>tarefa</strong>s<br />
secundárias diferencia<strong>da</strong>s, tais como motora, cognitiva<br />
e cognitivo-motora <strong>de</strong>ve ser implanta<strong>da</strong> como<br />
roti<strong>na</strong> tanto <strong>na</strong> avaliação, quanto no trei<strong>na</strong>mento <strong>da</strong><br />
<strong>marcha</strong> <strong>de</strong>sses <strong>pacientes</strong>.<br />
São necessários, assim, novos estudos que permi-<br />

tam a comparação entre diferentes doenças neurológicas<br />
no que se refere à realização <strong>da</strong> <strong>dupla</strong> <strong>tarefa</strong><br />
para que possam ser <strong>de</strong>senvolvidos programas <strong>de</strong> reabilitação<br />
mais específicos e eficientes.<br />
CoNCLusão<br />
Conclui-se que a realização <strong>de</strong> <strong>dupla</strong> <strong>tarefa</strong> para<br />
o paciente com ataxia encontra-se comprometi<strong>da</strong>,<br />
sendo que as <strong>tarefa</strong>s que associam caráter cognitivo<br />
e motor separa<strong>da</strong>mente contribuem para um pior <strong>de</strong>sempenho<br />
<strong>na</strong> <strong>marcha</strong> <strong>de</strong>sses <strong>pacientes</strong>.
