Dear Readers,<br />
<strong>April</strong> 2nd marks World Autism Awareness Day, which was suggested by Qatar<br />
and approved and applied by the United Nations to the world. In this special<br />
issue, we celebrate that important recognition with interviews from the Shafallah<br />
Center for persons with disabilities, specialists in the field of autism and Down<br />
syndrome, and parents of children who have broken the barriers of isolation live<br />
life as normally as we live it.<br />
We follow up on the latest in healthcare with the Director of Public Health in<br />
Qatar and share their successes and challenges. We also have the latest news<br />
about the growing Zika virus and the search for a cure for Parkinson’s disease.<br />
We aim to provide you with the latest in Qatar regarding health, so we are<br />
pleased to present to you, in this issue, editorial pieces about health and<br />
wellbeing written in collaboration with officials and experts.<br />
As mental health is very important, so is physical health. March was dedicated<br />
to raise awareness about the kidney and how it is a vital organ to our body. We<br />
shed some light on the various diseases and prevention methods in Qatar by<br />
talking to local expert Dr. Hasan Al Malki.<br />
Of course, no issue is complete without fitness and exercise, we have some tips<br />
on how to improve your vitality and how to maintain healthy weight.<br />

A CLOSER LOOK AT THE CHALLANGES AND<br />
SUCCESS OF THE NATIONAL HEALTH STRATAGY<br />
Health in Qatar is a big topic these days. From strategies launched by the Ministry of Health, to<br />
achievements for the public. Health and Life sat with His Excellency Doctor Sheikh Mohammed bin<br />
Hamad bin Jassim Al Thani, Director of Public Health, to find out how healthcare is addressed in Qatar.<br />

To begin, Your Excellency, what<br />
has been accomplished by Qatar’s<br />
health prevention strategy?<br />
The preventative health strategy is the third<br />
goal of Qatar’s National Health Strategy.<br />
The strategy aims to tackle issues, the most<br />
important being traffic accidents, physical<br />
nutrition, smoking, environment, and<br />
public health. All of the targets related to<br />
those issues were more than 80 percent<br />
reached last year. This year, we are working<br />
towards better goals and achieving more<br />
of our objectives.<br />
What are the biggest challenges you<br />
face in dealing with public health?<br />
The biggest challenges we face are<br />
placing the concept of public health and<br />
preventative health at the forefront of<br />
people’s minds and demonstrating its<br />
importance, because people don’t know<br />
that the best treatment is prevention.<br />
The largest death toll comes from traffic<br />
accidents and is mostly centered around<br />
Qataris. One in every nine road accidents<br />
is fatal, which shows us how severe the<br />
issue is. We are working on a strategy<br />
that includes creating laws and multiple<br />
awareness campaigns, and what we see is<br />
that the accidents don’t increase, but they<br />
don’t decrease either. The aim is to work<br />
together to teach the laws of the road and<br />
aim for better health. Another public health<br />
concern in Qatar is disease prevention,<br />
especially diabetes and blood pressure.<br />
We would like to see an increase in health<br />
awareness in every part of the country.<br />
Are chronic illnesses presently<br />
more dangerous in Qatar than<br />
contagious diseases?<br />
Seventy percent of all deaths in Qatar are<br />
attributed to chronic diseases. That very<br />
high number can be seen as a blessing<br />
when we consider the fact that we can<br />
now prevent contagious diseases like<br />
AIDS, hepatitis, and tuberculosis (TB).<br />
Fatal diseases like Ebola are basically<br />

non-existent in Qatar, because of a strong<br />
preventive system and a program of<br />
vaccinations that is 95 percent effective<br />
and prevents diseases from spreading.<br />
We’re also currently focusing on heart<br />
diseases, cancer, and diabetes. The<br />
Ministry of Health works on promoting<br />
change in the community. Multiple<br />
strategies for various diseases are being<br />
put in place right now that may have<br />
an effect on the country’s statistics. For<br />
example, diabetes cases have decreased.<br />
As for cancer, the Ministry focuses on early<br />
detection, because this leads to better<br />
chances of survival, and increases the<br />
acceptance of treatment in Qatar instead<br />
of receiving it outside the country.<br />
What are some of the most common<br />
contagious diseases?<br />
Campaigns against contagious diseases<br />
are very strong here in Qatar. We work<br />
constantly on campaigns for these and<br />
other widespread diseases as well. As you<br />
may know, TB is more prevalent than AIDS,<br />
because TB is airborne. However, AIDS is<br />
transferable from one human to another<br />
through sexual interaction. So dealing<br />
with one disease is different to dealing<br />
with another. We have regular check ups<br />
and border controls to stop disease from

entering the country. We always encourage<br />
people to get regular check ups and get<br />
vaccinated to protect themselves from<br />
diseases like hepatitis B and C. Constant<br />
vaccination helps with prevention of the<br />
disease and helps eradicate it.<br />
The Ministry conducts many<br />
campaigns on the dangers of smoking<br />
in the country. Talk to us about this<br />
topic; how successful is the Ministry<br />
in prohibiting smoking in public?<br />
The percentage of smokers in Qatar<br />
is around 13 percent, which is very low<br />
compared to the rest of the world. For<br />
Qataris the figure is around 16 percent,<br />
with most smokers being men. Caring<br />
about this issue is a must, however, when<br />
the percentage is this low it is very hard to<br />
come up with drastic measures to combat<br />
the issue. Bringing the numbers down<br />
is easy, but for that last group that were<br />
not convinced you must design a special<br />
message. As for smoking in public and<br />
prohibiting it, inspectors working at the<br />
Ministry of Health are monitoring and<br />
giving out appropriate fines to those who<br />
violate the law. Another challenge here<br />
is that cigarettes are cheaper than any<br />
other nation, so we need to raise taxes on<br />
tobacco bought here. We welcome Katara’s<br />
contributions to making their location a<br />
smoke-free zone, and we welcome others<br />
to participate in this initiative from a health<br />
perspective and from the perspective of<br />
raising awareness.<br />
Vaccinations are a great way<br />
of preventing diseases. What is<br />
happening with vaccinations now<br />
in the country, and what is new in<br />
this field?<br />
Qatar is leading the world with its take<br />
on vaccination. Vaccination is the key to<br />
disease prevention, and our focus is to<br />
provide not only the necessary vaccines,<br />
but those which are optional, for free. All<br />
vaccinations are of the highest quality<br />
and ensure the best results. We also<br />
monitor and track vaccines to ensure<br />

that people are always up to date with<br />
their vaccinations. For example, we do<br />
not accept children in schools without<br />
a vaccination card. To make it easier for<br />
the public, we use new technologies for<br />
administering vaccines, for example we can<br />
combine 6 different vaccines in one needle<br />
to ease the pain. The electronic app offers<br />
reminders of annual vaccinations and alerts<br />
you if you miss one. We aim to change all<br />
programs to electronic versions so we can<br />
communicate with the population if any<br />
disease has broken out and vaccines needs<br />
to be altered.<br />
In conclusion, what are you expecting<br />
for the Qatar National Vision and<br />
Qatar’s future health?<br />
The Department of Public Health is working<br />
on launching their strategies of the years<br />
2017 to 2022. Within a couple of months,<br />
the strategy will be ready for the end of<br />
<strong>2016</strong>. We aim to deliver a service that is<br />
unique, preventative and sensible, and to<br />
be ready for anything that comes our way.

50% of Global Population<br />
Short-sighted by 2050<br />
Half the world’s population will be myopic by 2050, according to a study<br />
published in the journal Ophthalmology in February <strong>2016</strong>. This significant<br />
increase has been attributed to environmental factors; spending less time<br />
outdoors and more time glued to a screen, be it for work or play, has a<br />
damaging impact on eyesight. The study’s findings shine a light on a major<br />
public health concern, with the authors encouraging the development of<br />
comprehensive eye care services and measures to protect children from<br />
short-sightedness. “Strategies may include increased time outdoors and<br />
reduced time spent on near based activities including electronic devices<br />
that require constant focusing up close,” said co-author Professor Kovin<br />
Naidoo. Indeed, reducing phone use, internet browsing and social media<br />
checking in favour of more natural activities such as using your eyes to<br />
look out towards the horizon, watch your children play or speak to a friend<br />
face to face can only benefit your eyesight.<br />
World Parkinson’s Day <strong>2016</strong> –<br />
New Hope for Sufferers?<br />
Each year, the 11th <strong>April</strong> marks World Parkinson’s Day,<br />
which aims to raise awareness and promote a deeper<br />
understanding of this neurological condition and how<br />
it affects those who suffer from it. Parkinson’s disease<br />
is caused by dopamine-generating brain cells dying,<br />
which in turn affects motor function. The early stages of<br />
the disease are generally characterized by movement<br />
related symptoms such as shaking, difficulty walking and<br />
rigidity. Later, mental and behavioral symptoms become<br />
apparent, with depression the most<br />
common psychiatric symptom and<br />
dementia often occurring in the<br />
advanced stages of the disease.<br />
The World Health Organization<br />
estimates that Parkinson’s Disease<br />
cases will more than double<br />
by 2030, but there is hope on<br />
the horizon. Scientists from the<br />
Swedish pharmaceutical company<br />
BioArctic Neuroscience, in<br />
collaboration with Uppsala<br />
University and other Swedish<br />
universities and the Karolinska<br />
Institutet, have recently developed a biomarker assay<br />
(analytic procedure) for the disease.<br />
Zika Virus Link to Rare Birth Defect?<br />
Zika Virus is a relatively new and rapidly expanding mosquito-borne<br />
disease, first identified in humans in 1952. Spread by the Aedes mosquito,<br />
it causes fever, skin rashes, conjunctivitis, muscle pain, nausea and<br />
headaches. The symptoms are usually mild and last up to 7 days, however<br />
recently a link has been suggested between the virus and birth defects,<br />
particularly microcephaly, a neurodevelopmental disorder. A study<br />
published in the Lancet Infectious Diseases on 17th February confirms<br />
that the Zika virus can cross the placental barrier and potentially cause<br />
microcephaly, though experts urge caution, stating that although the<br />
evidence strengthens the theory, the link isn’t yet definitely proven. “This<br />
study cannot determine whether the Zika virus identified in these two<br />
cases was the cause of microcephaly in the babies. Until we understand<br />
the biological mechanism linking Zika virus to microcephaly we cannot<br />
be certain that one causes the other, and further research is urgently<br />
needed,” says Dr. Ana de Filippis, lead author of the study.<br />

THE BEST AND HEALTHY WAYS<br />
TO MAINTAIN SAFE AND USABLE<br />
DRINKING WATER<br />
Qatar General Electricity and Water Corporation (KAHRAMAA)<br />
works on providing the highest service standards of electricity and water<br />
to subscribers, thus working to meet its obligations towards securing<br />
Qatar’s requirements for electricity and water at an exemplary level of<br />
efficiency, while at the same time complying with the highest local and<br />
international standards of health, safety and environment. In line with<br />
Kahramaa’s social responsibility to educate the public with the best and<br />
healthiest ways to keep drinking water safe and useable, Kahramaa invites<br />
its subscribers to install a water tank big enough to last the population for<br />
a period of at least 48 hours. The tank can be either a ground tank or a<br />

There are multiple types of tanks used in the State of<br />
Qatar, including: fiberglass tanks, iron tanks, plastic<br />
tanks (rotamold) and concrete tanks, all of which should<br />
comply with the following public health requirements:<br />
» The tank material should match the national<br />
standards specifications.<br />
» The tank must be fixed on higher placement above<br />
ground, and it should be clean and away from<br />
sources of pollution. It should be easily accessible<br />
for maintenance and cleaning.<br />
» The tank must be sealed to prevent the entry of<br />
dust and other contaminants.<br />
» The tank should contain a faucet to remove<br />
sediments when washed and sterilized.<br />
» The internal and external floors and walls of the<br />
tank should be smooth and easy to clean.<br />
» The tank should be fully isolated from water as<br />
well as thermal insulation.<br />
» Include a float to regulate the amount of water in<br />
the tank<br />
» Filters should be placed on the water flowing into<br />
the tank.<br />
» Tanks made of steel must be coated with<br />
weatherproof materials of high quality and free of<br />
toxic compounds steel (officially licensed for use<br />
with food), that does not react with the water or<br />
affect the health of the consumer.<br />
» The tanks should be cleaned and purified twice to<br />
three times a year.<br />
» Regular inspection should be carried out on the<br />
tank and its location to ensure their structural safety<br />
and health standards.<br />
» Tanks should be filled with water from the public<br />
water network or from a known source that has<br />
been proven fit for human use based on<br />
laboratory tests.<br />

Autism itself is a wide term that<br />
encompasses different symptoms,<br />
from mild to severe, which is why<br />
it is more accurately called Autism<br />
Spectrum Disorder (ASD).<br />

this diverse condition. We<br />

understand the condition<br />
and look at best practices for<br />

Autism Spectrum Disorder (ASD) is characterized by early onset socialization and communication difficulties,<br />
associated with restricted interests and behavioural rigidities. This condition is among the least understood developmental<br />
disorders of childhood. There is growing body of research on ASD, making it critically important that paediatricians, family<br />
physicians, child psychiatrists, general psychiatrists, neurologists, clinicians, psychologists, teachers and family members<br />
are well informed about these conditions. Recent epidemiological studies show that 1 in 68 children have autism.<br />

The Diagnostic and Statistical Manual for Mental<br />
Disorders (DSM Edition 5) defines Autistic<br />
Spectrum Disorder using two major categories<br />
of symptoms, communication and behaviour:<br />
A<br />
Persistent deficits in social<br />
communication and social<br />
interaction, Including: Reduced<br />
sharing of interests or emotions; lack of<br />
nonverbal communication; abnormalities<br />
in eye contact and body language; limited<br />
understanding and use of gestures; lack of<br />
facial expressions; problems in developing,<br />
maintaining, and understanding relationships;<br />
difficulties in imaginative play or making friends;<br />
absence of interest in peers.<br />
B<br />
Restricted, repetitive patterns<br />
of behaviour, interests, or<br />
activities, I ncluding at least two of<br />
the following: repetitive motor movements<br />
or phrases; insistence on sameness; inflexible<br />
adherence to routines; ritualized patterns<br />
of behaviour; difficulties with transitions;<br />
rigid thinking patterns; fixated interests<br />
that are abnormal in intensity or focus;<br />
unusual reactivity to sensory aspects of<br />
the environment (e.g. indifference to pain/<br />
temperature, adverse response to specific<br />
sounds or textures, excessive smelling or<br />
touching of objects, fascination with lights<br />
or movement).<br />
To meet the definition criteria symptoms must:<br />
Be present in the early developmental<br />
period (but may not become fully manifest<br />
until later)<br />
Cause significant impairment in social or<br />
occupational functioning.<br />
Not be attributable to another medical<br />
or neurological condition (e.g. intellectual<br />
disability, developmental delay, or another<br />
mental disorder).<br />
Common signs of the disorder are displayed in early childhood, typically<br />
before the age of three. Children will usually display some of these<br />
characteristics with a huge variation in severity:<br />
√ Reduced eye contact with familiar people.<br />

Children in the spectrum develop differently; some find developing<br />
cognitive, motor, language and social skills harder than others.<br />
Learning and memory can be inconsistent and unpredictable because<br />
understanding and interacting is often difficult.<br />
In mild cases, this might be seen as social clumsiness because they<br />
miss verbal and non-verbal communication cues. Acute symptoms make<br />
it seem like they are simply not interested in others, locked in a world<br />
of their own. Children may lack play and conversation skills, or seem<br />
stubborn and obsessive about specific interests. Intonation can often be<br />
incorrect and a symptom called echolalia, repeating specific words or<br />
phrases, is common. Empathy may be weaker or stronger than people<br />
outside the spectrum, making socialization difficult. At the less functional<br />
end, children may not be able to speak at all.<br />
There may be sensory issues, which also affects how they react<br />
to sudden loud noises and smells, as well as variations in lighting or<br />
temperature. Changes in their environment, such as loud noises, smells<br />
or lights, can cause a sensory overload and cause emotional melt-downs,<br />
especially if changes are sudden.<br />

What Causes<br />
AUTISM?<br />
There is much scientific debate about the<br />
apparent causes of autism and the increased<br />
number of cases in Qatar and around the<br />
world. We investigate the possible causes and<br />
latest research into this condition.<br />
A recent study published in the Journal of Autism and<br />
Developmental Disorders (2015) revealed that the costs<br />
associated with Autism Spectrum Disorder (ASD) are set<br />
to double over the next 10 years, potentially reaching $1<br />
trillion in the US alone by 2025. The study revealed that<br />
"The current costs of ASD are more than double the combined<br />
costs of stroke and hypertension and on a par with the<br />
costs of diabetes."<br />
The root causes of autism spectrum disorders are definitely<br />
complex, and it is more than likely that many factors come<br />
into play. Genetics play a role in development of the brain but<br />
epigenetic research is clearly demonstrating that the immediate<br />
environment has a huge impact and actually results in genes<br />
being switched on and off — affecting every aspect of our DNA,<br />
disease states and lives in general.<br />
There is no single cause of autism, and scientists are<br />
uncovering a selection of factors which increase the risk of the<br />
disorder. Boys appear to be 4 times more susceptible than girls<br />
and all races and social groups are equally affected.<br />

Individuals under the autism spectrum have brains that<br />
develop differently. Specific chemicals in the brain (creatine,<br />
choline and N-acetylaspartate) cause the junctions (synapses)<br />
between nerve cells to mature abnormally.<br />
Scientific Research<br />
The latest research into possible causes has uncovered a<br />
wide variety of factors, most of which relate to the neo-natal<br />
environment and genetic make-up:<br />
Flu during pregnancy doubles the risk of autism, say<br />
researchers from the University of Aarhus, Denmark, and a<br />
persistent fever of more than one week triples it.<br />
Antibiotic use raises the risk slightly; although 98% of<br />
those who contract such illnesses and use antibiotics during<br />
pregnancy go on to have healthy babies, without autism.<br />
A father's age has also been implicated, since older men<br />
pass on more genetic mutations than older mothers.<br />
The California Institute of Technology also points to a<br />
woman's immune system, with over-activity contributing to<br />
autism-like behaviors in mice.<br />
A pregnant woman's exposure to particulate matter and<br />
nitrogen dioxide in pollution may be linked to autism<br />
development, report the University of Southern California<br />
and the Children’s Hospital Los Angeles.<br />
Prenatal Exposure to Toxins<br />
During certain stages of development, the embryo is more<br />
prone to the effects of environmental toxins. Early exposure<br />
to harmful chemicals have already been linked to autism and<br />
studies continue to show the damaging effects of prenatal<br />
exposure to toxins.<br />
Chemicals can affect a developing fetus directly or<br />
through the genome by changing the DNA expression<br />
(how the DNA is ‘read’ and proteins are made), this is called<br />
epigenetics. Epigenetic factors can have positive effects,<br />
like taking folic acid to protect the baby’s central nervous<br />
system development. They can also have negative effect,<br />
like the impact of toxins on neural development.
Prenatal Exposure to Toxins<br />
Mount Sinai Hospital, founded in 1852, is one of the<br />
oldest and largest teaching hospitals in the United<br />
States. Their “Autism and Learning Disabilities<br />
Discovery and Prevention Project” launched in May<br />
2010 to discover the environmental causes of autism<br />
and other learning disabilities, and to turn these<br />
scientific discoveries into evidence-based strategies<br />
for autism prevention. Their recent workshop entitled<br />
"Exploring the Environmental Causes of Autism<br />
and Learning Disabilities" produced a preliminary<br />
list, based on a small number of studies, of the most<br />
widely used chemicals found in the environment that<br />
are possibly causing developmental neurotoxicity.<br />

Methylmercury<br />
Embryos are 5 to 10 times more sensitive to<br />
methylmercury toxicity than adults. This metal is<br />
often found in fish. Many ocean fish contain high<br />
levels of selenium that binds to methylmercury. This<br />
creates a compound that the body can't absorb.<br />
Certain species of fish that are high in this toxin, like<br />
swordfish and grouper, should be avoided.<br />
Organophosphate Pesticides<br />
Babies that have high levels of this toxin in their<br />
umbilical cord blood have brains that are structurally<br />
different. The extent of this effect is unknown;<br />
parents and couples planning children should limit<br />
their exposure to pesticides by choosing organic<br />
foods whenever possible.<br />
Automotive Exhaust Fumes<br />
Car exhaust fumes are loaded with toxins, such as<br />
formaldehyde, benzene, carbon monoxide. Lead is<br />
also a component of exhaust that causes neurological<br />
problems. Researchers are uncovering links between<br />
air pollution and a wide variety of diseases including<br />
a strong link to diabetes. Some studies have already<br />
highlighted the danger of air pollution in the Middle<br />
East but more research is needed to understand the<br />
long term impacts and find solutions. Seeking fresh<br />
air by the sea, reducing time in heavy traffic and<br />
consuming lots of fresh vegetables to support the<br />
body’s natural detox and antioxidant processes will<br />
reduce risk.<br />

Polycyclic Aromatic Hydrocarbons (PAHs)<br />
Over 100 different chemicals form a group of<br />
toxins called PAHs. They are formed during the<br />
incomplete burning of coal, oil, gas, garbage, and<br />
other organic substances like tobacco and meat.<br />
They are considered carcinogenic, but they are still<br />
used in some medicines. Reduce your exposure to<br />
all kinds of smoke, avoid charring meat and minimize<br />
your use of plastic for food storage<br />
Brominated Flame Retardants (BFRs)<br />
BFRs are used extensively in electronics, clothes,<br />
and furniture. If you are purchasing new soft<br />
furnishings or clothes, allow time to air in a ventilated<br />
space before use or request retardant free options<br />
(without Scotchguard).<br />
Per-fluorinated Compounds (PFCs)<br />
Most people are exposed to PFCs through Teflon<br />
non-stick pans. Grease resistant food packaging,<br />
like pizza boxes and microwave popcorn bags, also<br />
use PFCs. The production, use and disposal of this<br />
toxin is unregulated but you can keep yourself safe<br />
by using ceramic or steel cookware and avoiding<br />
fast foods — which also lack the nutrients that help<br />
protect you from damage!<br />
What Can You Do?<br />
Concerned parents-to-be and pregnant women<br />
should take action to reduce their exposure to<br />
harmful toxins:<br />
√ General awareness during shopping can<br />
really help - always read labels.<br />
√ Try to buy organic fruits and vegetables and<br />
avoid chemical laden processed and<br />
packaged food.<br />
√ Replace plastic lunch boxes and food storage<br />
containers with ones made of glass.<br />
√ When buying toys for your children, choose<br />
toys that are certified to be free from<br />
harmful compounds.<br />

labels, learning more and becoming a<br />

wrong and not wait to see if the child outgrows any<br />
problems. Learning as much as possible about Autism<br />
Spectrum Disorder helps parents become experts on<br />
their child and the most appropriate treatment options<br />
Treatment Starts with Acceptance<br />
2Acceptance is key. Parents who learn their child's common<br />
stress triggers can also find the best ways to support them and<br />
keep them calm. Troubleshooting problems and preventing<br />
difficult situations by continuously experimenting and<br />
looking for new approaches is key. Being aware of a child's<br />
unique sensory needs reveals trigger-points for disruptive<br />
behavior that might easily be prevented. Celebrating<br />
successes and growth really supports children with autism<br />
and their families. Developing an individual treatment<br />
plan with experts and physicians will support a child’s<br />
rapid growth, and many challenges can be overcome.<br />
A Good Autism Treatment Plan Will:<br />
> Focus on early intervention and heavily<br />
involve the parents.<br />

> Build on the child's interests — actively engaging<br />
attention by mixing structured activities and play.<br />
> Offer a predictable schedule and be consistent across<br />
different environments and caregivers.<br />
> Teach tasks in a series of simple, achievable goals that<br />

> Reward positive behavior with things that appeal to<br />
the child's preferences and special interests.<br />
> Address the physical and emotional needs of<br />
everyone in the family.<br />
> Include different forms of therapy: speech-language,<br />
play-based therapy, behavior therapy, physical<br />

The interdisciplinary assessment of ASD needs to be<br />
comprehensive and include input from parents, school,<br />
pediatrician and observation of the child. The treatment<br />
plan revolves around the needs and strengths of the child<br />
and the family. The successful interventions are child<br />
and family centered and usually include speech therapy,<br />
occupational therapy, appropriate educational programs,<br />
behavioral interventions, and parents and care givers<br />
education and support.<br />

Communication<br />
Children who are unable to talk or dislike doing so should<br />
be encouraged with nonverbal ways to connect. Facial<br />
expressions, body language and touch are all different<br />
ways to communicate. Parents and caregivers may need<br />
to learn the language in which the child is communicating<br />
using sounds and expressions.<br />
Consistency<br />
Creating consistent rules and routines will reinforce<br />
learning — these need to be applied across different<br />
settings. Parents should find out what other members<br />
of their child-care team are doing, including teachers,<br />
therapists or extended family members. Healthcare,<br />
education and social services all contribute to supporting<br />
a child’s success.<br />
Routine<br />
Individuals with autism prefer predictability and routine.<br />
Forcing them to accept change or behave exactly like<br />
other people can inhibit their development and parents can<br />
wrongly misinterpret certain behaviors as disobedience,<br />
creating social stigma that bad parenting is at fault. Raising<br />
public awareness is key to supporting these unique<br />
individuals and their families.<br />
Safe Space and Self Care<br />
7Parents are advised to make room for the child’s<br />
requirements by creating a safe place in the home where<br />
they can be themselves. Having set boundaries helps<br />
children with autism feel safe, secure and accepted.<br />
Studies have shown that, without adequate support, the<br />
health of caregivers can suffer and this greatly impacts the<br />
whole family.<br />

Support and Treatment in Qatar<br />
There are recently exciting advances happening in this area in Qatar.<br />
The National Autism Working Group was created last year under the auspices of Ministry of Public Health. This<br />
group has stakeholders from around the country including child psychiatrists, developmental paediatricians, speech<br />
therapists, occupational therapists, psychologists, schools, administrators and, most importantly, family members.<br />
This group is charged with creating a<br />
National Autism Plan which is based on<br />
six pillars: awareness, screening, diagnosis,<br />
interventions, school services and transition<br />
to adult hood. We are privileged to have<br />
the World Health Organization (WHO)<br />
collaborating with this group in finalizing<br />
the plan.The 3rd World Innovation Summit<br />
for Health (WISH) Conference will be held<br />
in November of this year in Doha. WISH’s<br />
Autism Forum is extremely important in<br />
advancing some of the most pressing issues<br />
regarding autism. The major goal of the<br />
Forum is to identify the service gaps for<br />
children and families locally and globally,<br />
and suggest solutions to different issues<br />
at national and international level. Autism<br />
experts from five continents will participate<br />
along with the WISH Team.<br />
Qatar Biomedical Research Institute<br />
(QBRI) is currently undertaking<br />
the first autism prevalence study in<br />
Qatar. This exciting study will pave<br />

the way in identifying the service needs in<br />
the country.<br />
A number of schools and centers in the<br />
public and private sector are playing an<br />
important role in serving children with autism,<br />
like the Shafallah Center and the Child<br />
Development Center. Qatar Foundation<br />
(QF) is planning to open state of the art<br />
schools this year for children with autism,<br />
which will serve a major need in this area in<br />
the country.<br />
All these initiatives are steps in the right<br />
direction. It is extremely important that<br />
development of future services should be<br />
child and family centred, keeping in mind<br />
children and families are the heart and soul of<br />
successful programs.<br />

Past, Present and<br />
Future of Autism<br />
It is more than possible that there has<br />
always been a spectrum when it comes<br />
to brain development. High functioning<br />
individuals have continued to play key<br />
roles throughout history and industry,<br />
using the persistence, determination,<br />
curiosity and intense focus that are often<br />
seen as gifts that come directly from<br />
their condition.<br />
Each individual with autism has their<br />
own unique challenges and strengths.<br />
A significant number of people under<br />
the Autism Spectrum have high IQs and<br />
specific talents. Creative talents including<br />
art or music and genius like skills in<br />
mathematics, computing and chess have<br />
been seen in individuals with autism<br />
whose abilities and skills can dramatically<br />
exceed the rest of the population. The<br />
corporate world is starting to take notice,<br />
and companies are seeking out people<br />
on the spectrum for their expertise and<br />
ability to focus on certain kinds of tasks,<br />
for example those requiring meticulous<br />
precision. This echoes the message of<br />
the growing neuro-diversity movement<br />
who celebrate the huge diversity of<br />
human skills and abilities, including adults<br />
who do not wish to be “cured” of their<br />
neurological condition.<br />
The vast differences throughout the<br />
spectrum and the improvement in<br />
diagnosis methods coupled with how little<br />
is really known or understood, amplifies<br />
the challenges autism represents to<br />
the medical community and society<br />

CHILDREN IN QATAR<br />
Qatar gives considerable attention to those with disabilities in order to integrate them into society, and<br />
make their lives easier to live. Being as Qatar suggested to the United Nations that 2nd <strong>April</strong> be World<br />
Autism Day, and were approved and implemented, The Shafallah center is the tangible evidence of this<br />
attention. Founded in 1999, it is one of the few centers in the world that takes care not only of people with<br />
disabilities, including mental disabilities, cerebral palsy and rare conditions, but also of autistic and Down<br />
syndrome children. The center seeks mainly to integrate students in the community naturally, and provide<br />
them with the tools necessary to do so.<br />

To discover more about the Shafallah Center and its distinctive role in<br />
society, we spoke with Mr. Mohammed Bader Al-Sada,<br />
Executive Director of Shafallah:<br />
Tell us a little about Shafallah;<br />
how many students can the center<br />
accommodate?<br />
Shafallah Center cares for about 640<br />
students starting from the age of four, 60<br />
percent of which are Qataris. The center<br />
offers premium services and perhaps one<br />
indicator of this is that the center has almost<br />
760 employees. Last year, the behavior<br />
analysis program, prepared by specialists<br />
from the center, was adopted at Shafallah.<br />
This program has been accredited and is<br />
the first of its kind in the Arab world, and<br />
in the Arabic language. Accordingly, the<br />
Shafallah Center has been certified to<br />
train other centers in this service, which is<br />
something that we take much pride in.<br />
You offer a variety of educational<br />
programs for students in the center;<br />
what system of education do you follow?<br />
The educational program in Shafallah<br />
Center is divided into three phases:<br />
Phase I is called Early Childhood and starts<br />
from the age of three to six years, which is<br />
similar to kindergarten. Medical specialists<br />
work on training and rehabilitating the<br />
students. After this stage, the students<br />
are divided according to their condition:<br />
those who can integrate attend a normal<br />
school with their peers and are supervised<br />
by the Integration Unit in the Ministry<br />
of Education.<br />
Phase II deals with those who are unable<br />
to integrate and continue in the center. They<br />
learn essential life skills, such as how to eat<br />
and how to hold pen and paper. We also<br />
work with those who can comprehend basic<br />
education, such as letters and sentences.<br />
This stage continues until the ages of 13 or<br />
14 years, after which the students’ potential<br />
is evaluated and, if they are ready, they are<br />
integrated into regular schools.<br />
Phase III includes students after the age of<br />
14, where they enter in another department<br />
called Rehabilitation and Job Training, and<br />
continue until they are 19, 20 or 21 years old,<br />
depending on their capabilities. At this stage,<br />
the students qualify and train on practical<br />
life skills that can help them enter the labor<br />
market and make a living in the future.<br />
How successful has Shafallah Center<br />
been in integrating students into the<br />
labor market?<br />
In the past year, 194 students of varying<br />
abilities graduated and 114 of them have<br />
become employees in the various sectors of<br />
the State. Our main objective is integration.<br />
Whoever cannot be integrated into the<br />
work environment, we continue working<br />
with at home, to break the barrier and<br />
isolation between them and society, starting<br />
with close family and relatives. Modern<br />
equipment helps us in this area, where<br />
we teach students who cannot express<br />
themselves how to communicate with the<br />
community around them. This helps students<br />

We are proud that SHAFALLAH is<br />
certified to train other centers.<br />
with limited movement to communicate<br />
with their eyes and express what they want<br />
through pictures. Some students have very<br />
advanced mental abilities, and their disability<br />
does not prevent them from learning. Using<br />
these devices, they are now able to write full<br />
sentences using their eyes only, by focusing<br />
on the characters on the keyboard. This<br />
enables students to communicate almost<br />
naturally with the environment.<br />
Some of our students have very high<br />
capabilities, but prefer isolation or are afraid<br />
of people.. We worked with those students<br />
by creating a new experience in cooperation<br />
with Qatar Airways, I would like to extend<br />
my gratitude for their support and especially<br />
the company’s CEO Mr. Akbar Al Baker.<br />
Basically what the students did was work on<br />
packing the complementary bags that are<br />
handed out in first class. Twenty-five students<br />
participated in this experiment for a year;<br />
they impressed the inspectors so much<br />
that they were hired by Qatar Airways.<br />
The students succeeded in filling 35,000<br />
bags perfectly.<br />
How do you take care of student<br />
health in the center?<br />
The center coordinates directly with the<br />
Ministry of Health and its accredited<br />
doctors. In addition, we have a Psychological<br />
Service Department because students,<br />
like all people, are exposed to emotional<br />
or mental difficulties. This service works<br />
to help resolve these problems and liaise<br />
with parents.<br />
Qatar provides a variety of services for<br />
people with disabilities, despite the country’s<br />
population being small. Under the guidance<br />
of His Highness the Emir, all services are<br />
provided free of charge, not only central<br />
services but also additional services such as<br />
meals for children, transportation and other<br />
services needed by the students.

of Psychological Services and Family Support at the Shafallah Center:<br />
What are the most important things<br />
that fall under the responsibility of<br />
your administration and what are its<br />
most notable achievements?<br />
The basic services that we offer in our<br />
department are divided into three sections:<br />
The Social Services Department provides<br />
a link between the parents and the center,<br />
following up on the child at every stage.<br />
It also regulates and organizes activities<br />
for the students. That way, the students<br />
integrate the skills they learn in their daily<br />
activities. This allows the student to feel<br />
normal and integrate with society though<br />
activities like going to supermarkets<br />
and malls.<br />
The department also creates community<br />
awareness of the students’ capabilities and<br />
potential, demonstrating that they can do<br />
just as well as many other children.<br />
The Family Counseling Division provides<br />
counseling services individually and<br />
collectively, to parents and students. It also<br />
offers support services to people who have<br />
similar problems, enabling them to meet<br />
with each other under the supervision of<br />
specialists, to exchange experiences and<br />
provide support. The Family Counselling<br />
Division also conducts outreach activities,<br />
either through lectures or in partnership<br />
with other groups.<br />
The Psychological Services and<br />
Evaluations Division evaluates and<br />
diagnoses any student wishing to enroll in<br />
Shafallah, to ensure that the appropriate<br />
service is provided. This includes social,<br />
medical and psychological evaluations,<br />
as well as physical therapy evaluations,<br />
occupational therapy and evaluation,<br />
assessment of speech therapy and more.<br />
In addition to the evaluations mentioned,<br />
we carry out autism diagnoses to<br />
determine whether a student has autism<br />
characteristics. This is done by trained<br />
professionals using the latest methods,<br />
such as the Autism Diagnostic Observation<br />
Schedule (ADOS).<br />
What are the biggest challenges you<br />
face in the center?<br />
The biggest challenge is to find specialists<br />
trained on the latest scientific methods and<br />
able to deal with the students. The second<br />
challenge is the waiting list. Another<br />
challenge is the large number of laws that<br />
regulate the use of modern tools. However,<br />
we were recently able to get approval for<br />
Shafallah to conduct behavior analysis<br />
training, and also to train specialists.<br />

Can you tell us about behavioral<br />
analysis and the success of using<br />
modern tools to communicate<br />
with students?<br />
Behavioral analysis is a new program<br />
being used with the students at the center;<br />
it covers several aspects:<br />
• Firstly, some students’ behaviors<br />
are undesirable, and often the student<br />
cannot express why he or she behaves<br />
in this way. We use behavior analysis<br />
methods to understand the reasons<br />
behind the behaviors and develop plans<br />
to teach students alternative and more<br />
positive behaviors.<br />
•The second using modern tools in<br />
teaching, for example, developing a<br />
speech evaluation and essential skills<br />
evaluation to those who are autistic.<br />
• We use applied scientific methods<br />
to determine the students’ strengths<br />
and weaknesses and come up with a<br />
personalized plan of action.<br />

We spoke with Fahad’s mother:<br />
What are the difficulties faced by children<br />
with autism in Qatar?<br />
Finding a suitable center for an autistic child is<br />
difficult. And the lack of specialists is considered<br />
a big problem, especially in the private centers.<br />
There have been situations where a child has been<br />
beaten or tied down because the centers do not<br />
know how to deal with them. The only center where<br />
you can truly feel your child is safe is Shafallah.<br />
Swimming is a good hobby as it allows the child to<br />
exert his energy, but there are no clubs for autistic<br />
children due to lack of specialists. They began to<br />
establish centers and clubs with pools for people<br />
with special needs such as Al Gharafa, but this<br />
is subject to the availability of a specialist for this<br />
activity. The other problem occurs in the summer, as<br />
there are no summer centers.<br />
How has Fahad’s condition improved?<br />
The first thing I noticed was a change in his behavior.<br />
Before attending Shafallah, he had a private tutor<br />
working with him. Now, with the help of Shafallah,<br />
Fahad has improved greatly.<br />
What are some of the most memorable<br />
moments with your child?<br />
The best times where when he overcame some<br />
undesirable behaviors. When I see Fahad achieve<br />
something new or learn something, I thank God for it.<br />
But I am afraid of the future; I would love to integrate<br />
him in a normal school, but that can’t happen due to<br />
his hyperactivity.<br />
Would you like to say something to the<br />
community?<br />
I wish the community would be more conscious,<br />
and that parents would be more observant of their<br />
children, especially in their infant years — that’s<br />
when autistic characteristics appear. Prompt<br />
action is key. A child should not be deprived from<br />
the joy of simple everyday things; let him play<br />
with his siblings like a normal child. Society must<br />
understand that autistic children are not crazy.<br />
We also spoke with Abdulrahman’s mother:<br />
What does Qatar offer those<br />
who are autistic?<br />
First off, I consider Abdulrahman<br />
lucky that he wasn’t on the waiting<br />
list long. However, it is sad to see that<br />
there are other children, in more<br />
difficulty than my son, who are still<br />
on the waiting list.<br />
As for all the Autism Unit<br />
employees in Shafallah, they are<br />
very cooperative and apply the<br />
latest scientific development. The<br />
center is only missing one thing: the<br />
summer period, which lasts almost 3<br />
months. The children have nowhere<br />
to go because there are no other<br />
clubs suitable for autistic children.<br />
How has Abdulrahman<br />
improved?<br />
Abdulrahman<br />
improved<br />
tremendously in the first two years<br />
at Shafallah. But I can’t say that the<br />
condition has improved much. This<br />
is because he has a minor autistic<br />
level, so he discovers things faster<br />
than other children with autism.<br />
Can you describe the most<br />
beautiful moments with<br />
your child?<br />
With Abdulrahman, I’ve shared<br />
many sweet moments. First, when<br />
they told me that he is able to be<br />
integrated in a normal school.<br />
The second best moment was<br />
unforgettable, when Abdulrahman<br />
participated in an international<br />
swimming competition for normal<br />
youth and won a gold medal. This<br />
was the first time Abdulrahman had<br />
ever been in the ocean. He received<br />
his training in the pool. He also won<br />
first place in a normal school’s arts<br />
and crafts competition.<br />
What would you like society<br />
to know?<br />
I have something to say to our<br />
beloved Government and<br />
something to society. I wish that<br />
you would accept our children<br />
and understand them, for they are<br />
different but not retarded! Be there<br />
to support them and be their anchor<br />
– because after God, they have no<br />
one else but us. The community<br />
needs to understand the nature<br />
of our children, because it is not<br />
a disability, it is an illness. Science<br />
has not yet proven that autism is in<br />
fact a disability.<br />
I say to our beloved Government,<br />
please provide services for our<br />
children, because the earlier the<br />
intervention, the more lives saved.<br />
However, when it is too late the<br />
situation become very hard to<br />
handle. Autism comes as a shock to<br />
the parents — one that will change<br />
their entire home life — so you either<br />
absorb that shock and save your<br />
child or you live in denial and wreck<br />
your home all together.<br />

Sugars Cause Decay<br />
Although candies, cookies and<br />
desserts are obvious foods to keep<br />
to a minimum to protect your teeth<br />
from decay, any form of sugar can<br />
cause caries. The Journal of Public<br />
Health Nutrition recommends<br />
limiting intake of sugars to less than<br />
2 percent of total calories to reduce<br />
the risk of tooth decay.<br />
Consumption of sugar sweetened<br />
beverages causes the most damage,<br />
but even the natural sugars present<br />
in fruit can contribute to the loss of<br />
tooth enamel. Rather than limiting<br />
your intake of fruit, you should aim<br />
to have fruit with your meal. If you<br />
do decide to have fruit or another<br />
sweet snack, it’s a good idea to rinse<br />
your teeth and mouth afterwards.<br />
Make Vitamin C a Priority<br />
As low dietary intakes of vitamin C<br />
are linked to gum disease, ensuring<br />
a good intake of foods rich in vitamin<br />
C is essential for the health of your<br />
gums. Vitamin C plays an important<br />
role in collagen production, a protein<br />
that is essential for tissue growth and<br />
repair. Additionally, vitamin C is an<br />
antioxidant, allowing it to protect your<br />
gums from damaging free radicals,<br />
which you come into contact with daily.<br />
Smoking increases your exposure<br />
to free radicals, which explains why<br />
smokers have higher vitamin C<br />
requirements. While some of the foods<br />
that are a good source of vitamin C<br />
are acidic, such as citrus fruits, kiwis,<br />
tomatoes and berries, there are plenty<br />
of less acidic options for vitamin C; try<br />
green leafy vegetables, green peas and<br />

want to include acidic foods and<br />

vegetable sticks, yoghurt or nuts are<br />

Don't Forget Folate<br />
It isn't just pregnant women who need<br />
to include plenty of folate in their diet,<br />
as this B vitamin protects your gums as<br />

folate improves the symptoms of gum<br />
disease and relieves inflamed gums.<br />

include leafy greens, beans, whole<br />

your orthodontist is also encouraged,<br />
so that you can consult them should any<br />
issues arise, and make sure you’re giving<br />
your teeth the best possible care.<br />
Orthodontic Laser Treatment<br />
A lot of time and effort are demanded from<br />
a patient seeking orthodontic treatment.<br />
However, dentists hope to reduce the<br />
period of time, and amount of pain that<br />
patients have to go through by using laser<br />
treatments. “Using laser treatment can<br />
increase the rate of orthodontic tooth<br />
movement by up to 40 times, and also<br />
successfully eliminate the pain experienced<br />

No larger than the size of a pear, the<br />
kidney is a complicated organ working<br />
silently and expecting nothing in return.<br />
Your blood flows through it an amazing<br />
36 times every day – filtering about<br />
1300 liters of blood!<br />
Due to its importance, the world<br />
celebrates International Kidney Day on<br />
the 10th of March. This year, the aim was<br />
“kidney disease prevention in children.”<br />
We sat down with Dr. Hasan Al Malki,<br />
Head of Kidney Diseases Department<br />
at Hamad Medical Corporation.<br />
The most common reasons<br />
for chronic kidney failure are<br />
diabetes and high blood pressure.<br />
The kidney plays a huge role in our<br />
body, can you tell us more about the<br />
function of the kidney?<br />
The kidney is very important in the human<br />
body; it has multiple roles. It removes<br />
excess fluids from the body and eliminates<br />
toxins produced during metabolic function.<br />
It also clears away any medicines or other<br />
toxic substances in your system. It maintains<br />
correct blood pressure and helps regulate<br />
acids and minerals in the bloodstream.<br />
Other functions of the kidney include<br />
hormone regulation and production of red<br />
blood cells. It also activates vitamin D, which<br />
helps absorb calcium and phosphorous,<br />
vital in supporting bone density in the<br />
human body.<br />
Kidney disease is very dangerous<br />
as it affects many people and at<br />
different ages. What is chronic<br />
kidney disease and what causes it?<br />
Kidney disease is divided into categories:<br />
Acute failure or acute illness, which usually<br />
happens fairly quickly and improves<br />
really quickly as well. However, if the<br />
damage is chronic, the functions of the<br />
kidney are affected long term with little<br />
expected improvement.<br />
As for chronic kidney failure, there are<br />
multiple reasons why it occurs, the most<br />
common being diabetes and high blood<br />
pressure. The disease can be divided into<br />
5 stages depending on the functions of the<br />
kidney that are affected – ranging from 90<br />
percent use in the first stage to complete<br />
kidney failure in the fifth stage.<br />
Other possible causes of kidney disease<br />
are hereditary kidney cysts, kidney stones,<br />
chronic kidney infections in the lining<br />
of the kidney, imbalance in the immune<br />
system and genetic diseases. Sometimes,<br />
the kidney can be affected by repeated<br />
bacterial infections, which in time can lead<br />
to malfunction of the organ. Kidney stones<br />
are also another likely cause of problems<br />
as they cause blockages in the organ.<br />
Nearly 10 percent of the world’s<br />
population suffers from chronic<br />
kidney disease, how widespread<br />
is it in Qatar?<br />
Kidney disease is widespread across<br />
the globe. In most countries, the<br />
spread is between 10-13 percent and<br />
is distributed at all stages from 1-5. It<br />
is wide spread in Qatar at nearly the<br />
same percentage.<br />
Who is more likely to be affected by<br />
kidney failure?<br />
People who have diabetes, high blood<br />
pressure, and those with history of dialysis<br />
and kidney transplant for themselves or<br />
their family. The people more likely to be<br />
affected by kidney failure should frequently<br />
visit their doctor and find out as much as<br />
possible about the disease.<br />
Individuals over 50 years of age should<br />
also have regular check-ups to ensure that<br />
they don’t have any signs of the diseases.<br />
Of course, those patients who have kidney<br />
cysts or stones or have reoccurring urine<br />
infections should also see a doctor on a<br />
regular basis.<br />
What causes kidney stones?<br />
Kidney stones are caused by a variety of<br />
reasons including a genetic predisposition.<br />
Another reason is that people do not drink<br />

enough liquids, and this can also cause<br />
kidney infections. Some types of bacteria<br />
can also cause kidney stones. The high<br />
temperatures in Qatar cause dehydration<br />
and people tend not to drink enough<br />
liquids to keep themselves hydrated, which<br />
in time leads to concentration in the urine<br />
and that leads to kidney stones.<br />
What is a transplant and what does<br />
it require of the patient? Is there<br />
difficulty finding ideal donors?<br />
Those who have kidney disease in the<br />
last phases, with less than 18 percent<br />
functionality, have a significant risk of<br />
kidney failure within 6 months, even if they<br />
start dialysis straight away. So we advise<br />
that they start looking for donors from the<br />
family to increase the chances of finding<br />
a match to the tissue — this makes the<br />
operation smoother and faster.<br />
Regarding kidney transplants, how<br />
damaged must the kidney be before<br />
the patient requires a transplant?<br />
Any patient with kidney functionality that<br />
drops below 18 percent must go through<br />
tests for transplant. Then they must<br />
receive official consultation with their<br />
doctor about the operation.<br />
What percentage in functionality<br />
is needed for us to survive<br />
kidney failure?<br />
The lowest percentage of functionality<br />
needed for a human body to survive<br />
is 18 percent.<br />
How many kidney transplants<br />
have been conducted yearly in<br />
Hamad hospital?<br />
The transplant program started in the<br />
late 80s, more than 25 years ago. In the<br />
last couple of years transplant operations<br />
ranged from 5 – 10 per year and in 2015 we<br />
had 18 successful transplant operations, and<br />
that’s a new record.<br />
How many are on the waiting list to<br />
receive a transplant?<br />
Our transplant waiting list is reviewed on a<br />
monthly basis and currently has about 80<br />
patients.<br />
It might be hard to diagnose kidney<br />
disease accurately before it becomes<br />
too advanced. However, how can we<br />
avoid chronic kidney disease?<br />
Avoiding the dangers of getting chronic<br />
kidney disease starts with simple steps<br />
like eating healthy, exercising, maintaining<br />
an ideal weight, drinking more liquids,<br />
avoiding smoking, and not taking any<br />
medication without a medical prescription.<br />
Patients who have a family history of kidney<br />
diseases must check with their doctor on a<br />
yearly basis at least. Patients who have a<br />
high risk of kidney disease like those who<br />
have diabetes, blood pressure, kidney<br />
infections, kidney stones, and kidney cysts<br />
require constant care from their physician.<br />
How much does the transplant<br />
operation usually cost?<br />
Transplant operations in Qatar are free by<br />
an Emir decree. The transplant operation<br />
and the preparation for it, as well as all the<br />
care after it is all free to everyone in Qatar.<br />
Any last advice about kidney disease<br />
and ways to avoid it?<br />
My advice is for people to maintain<br />
an ideal weight, eat healthily, exercise<br />
regularly, and to those who have a high risk<br />
of kidney diseases, to check regularly with<br />
their doctor.<br />

World Down Syndrome Day has been officially observed on 21st March by the United Nations<br />
since 2012. It is a day which helps to raise awareness of the condition and looks at the role<br />
people with Down syndrome play in our communities. To mark the event, Health & Life spoke<br />
to Aisha Ahmadi, Lecturer at Qatar University’s Department of Clinical Mental Health<br />
Counselling Psychology, to find out more about Down syndrome.<br />
Extra Chromosome<br />
Down syndrome, also known as Trisomy<br />
21, is caused by a genetic abnormality.<br />
Each cell present in the human body<br />
contains 23 pairs of chromosomes (or 46<br />
chromosomes). People with the condition<br />
are born with an extra 21st chromosome,<br />
and thus have 47 chromosomes in<br />
cells instead of 46. Although Down<br />
syndrome has a single cause (the extra<br />
21st chromosome), it affects people<br />
differently. Each individual has their own<br />
abilities, strengths, weaknesses and<br />
unique personality.<br />
Characteristics<br />
Individuals with the condition have<br />
unique cognitive, behavioral and physical<br />
characteristics. However, many share<br />
certain features, and some of the most<br />
common include:<br />
> Low muscle tone<br />
> Upward slanting eyes<br />
> Wide short hands with short fingers<br />
> A single deep crease across the palm<br />
of the hand<br />
> Small ears<br />
> Large or protruding tongue<br />
> Slow learning<br />
> Short attention span<br />
> Late language and speech<br />
development<br />
> Impulsive behavior<br />
Learning Potential<br />
While it may take children with Down<br />
syndrome longer to develop certain skills,<br />
the common misconception that they<br />
have a pre-determined ability to learn is<br />
simply not true. In fact, it is impossible to<br />
predict the degree of mental cognition at<br />
birth, which is why it is important that their<br />
learning potential is supported with early<br />
intervention, a good education and plenty<br />
of positive encouragement.<br />
Health Conditions<br />
Down syndrome is associated with several<br />
health conditions.<br />
> Heart problems are one of the earliest<br />
concerns, and need to be investigated<br />
even before birth. An ultrasound during<br />
pregnancy will predict any problems that<br />
could occur during gestation or delivery.<br />
> Hearing and vision problems in newborns<br />
with Down syndrome might lead to blindness<br />
and deafness, and should therefore be<br />
investigated by specialists.<br />
> Sleeping disorders are another common<br />
health concern. Obstructive sleep apnea<br />
has been noticed in many individuals with<br />
Down syndrome. The principle symptoms<br />
to look out for are: snoring, waking up many<br />
times at night, breathing difficulties and/or<br />
pauses in breathing while sleeping and being<br />
sleepy during the daytime.<br />
> The special characteristics distinctive to<br />
individuals with Down Syndrome make them<br />
more prone to issues such as depression,<br />
anxiety and obsessive compulsive<br />
behaviors.<br />
Despite the fact these health concerns,<br />
and thanks to the developments in medicine<br />
today, individuals with Down syndrome have<br />
much longer life expectancy than in the past.<br />

Risk Factors and Testing<br />
According to the World Health<br />
Organization, 10 to 11 children with Down<br />
syndrome are born for every 10,000<br />
live births worldwide. Some parents are<br />
at higher risk of having a baby with the<br />
condition:<br />
> Women aged 40 years old<br />
and above.<br />
> Parents who already have a<br />
Down syndrome child.<br />
> Parents who are carriers of<br />
the genetic translocation for<br />
Down syndrome.<br />
If you are concerned about your risk, it<br />
is possible to test for the condition with<br />
an amniocentesis, a procedure in which<br />
amniotic fluid is removed from the sac<br />
surrounding the fetus between the 15th<br />
and 18th week of pregnancy. This fluid<br />
is then sent for analysis to determine<br />
whether the unborn child has Down<br />
syndrome. An amniocentesis can help<br />
parents to plan the care for their baby<br />
and ensure they give their child the best<br />
possible start in life.<br />
It’s important to remember that<br />
Down syndrome affects each person<br />
differently. Individuals with the condition<br />
are each unique and have their own<br />
abilities, talents, thoughts and interests.<br />

الظهرية، قد تجد نفسك ال تحن لتناول وجبة<br />
خفيفة بعد الظهر!<br />
حرق السعرات الحرارية مع المشي<br />
كمية السعرات الحرارية التي تحرق أثناء املي<br />
تعتمد عىل سرعة املي، والتضاريس. املي<br />
بمتوسط سرعة 3-4 ميال يف الساعة، يحرق من<br />
يزن 75KG 90 سعرة حرارية لكل ميل يف املي، يف<br />
حني أن من يزن 100KG سوف يحرق 120 سعرة<br />
حرارية. ال يهم إذا كنت تمي أبطأ من ذلك، فمع<br />
مرور الوقت سوف تتحسن لياقتك، وسوف تكون<br />
قادراً عىل زيادة سرعتك.<br />

للتمتع بها عند نصف الطريق أو<br />
بعد االنتهاء من املي. املكسرات<br />

يوافق اليوم العالمي للتوعية بمتالزمة الداون 21 مارس كما حددته األمم المتحدة منذ عام 2012. وهو يوم<br />
مخصص لرفع مستوى الوعي عن المتالزمة ودور المصابين بها في مجتمعاتنا. بهذه المناسبة، تحدثت مجلة الصحة<br />
والحياة مع عائشة األحمدي، من قسم العلوم النفسية في جامعة قطر للتعرف على متالزمة داون.<br />
القدرة على التعلم<br />
الفكرة الخاطئة املأخوذة عن أطفال متالزمة<br />
داون هي أن لديهم قدرة محددة مسبقاً عىل<br />
التعلم وهي ببساطة ليست صحيحة بالرغم<br />
من أنهم قد يستغرقون وقتا أطول لتطوير<br />
مهارات معينة. يف الواقع، إنه من املستحيل<br />
التنبؤ بدرجة اإلدراك العقيل عند الوالدة، ولهذا<br />
السبب من املهم نعلم أن قدراتهم عىل التعلم<br />
تعزز مع التدخل املبكر، والنظام التعليمي<br />
الجيد والكثري من التشجيع اإليجابي.<br />
المشاكل الصحية<br />
من املتعارف عليه أن هناك بعض األمراض<br />
الصحية املرتبطة بمتالزمة داون.<br />
> أمراض القلب أحد األمراض التي يتوجب<br />
تحريها حتى قبل الوالدة. صور املوجات فوق<br />
الصوتية للقلب خالل فرتة الحمل تمكننا من<br />
التنبؤ بأي مشاكل يف القلب قد تحدث خالل<br />
فرتة الحمل أو عند الوالدة. أمراض القلب قد<br />
تظهر بعد الوالدة، لذلك يجب االنتباه إىل أي<br />
أعراض قد تتجسد الحقاَ.<br />
> مشاكل السمع والنظر عند حديثي<br />
الوالدة املصابني بمتالزمة داون قد تؤدي إىل<br />
فقدان القدرة عىل السمع والرؤية. ولتفادي<br />
هذه املشكلة يجب أن يتم فحص النظر والسمع<br />
عند أطفال متالزمة داون باستمرار عىل أيدي<br />
متخصصني.<br />
> مشاكل النوم هي أيضاً من املشاكل<br />
الصحية األخرى التي يعاين منها املصابون<br />
بمتالزمة داون. توقف التنفس أثناء النوم<br />
لوحظ عند العديد من املصابني بمتالزمة داون.<br />
األعراض التي يجب االنتباه إليها هي الشخري،<br />
واالستيقاظ املتكرر من النوم ليال، وصعوبة<br />
يف التنفس أثناء النوم، وتوقف التنفس أثناء<br />
النوم، والشعور بالنعاس<br />
خالل النهار.<br />
> هناك حاجة أكرب للعناية بالصحة النفسية<br />
املستمرة. واملشكالت النفسية التي قد تظهر<br />
عليهم تشمل القلق، االكتئاب، السلوك<br />

كروموزوم اضافي<br />
متالزمة داون هي اضطراب جيني، املعروف<br />
برتسومي 21. Trisomy 21 وأغلب املصابني<br />
بها يملكون 47 كروموزوما يف الخلية الواحدة<br />
بدال من 46 كروموزوما. الكروموزوم اإلضايف<br />
هو كروموزوم 21 الذي قد يتواجد بشكل جزيئ<br />
أو كيل. يؤثر الكروموزوم اإلضايف يف كل فرد<br />
بشكل مختلف، حيث يصبح كل فرد متمتعاً<br />
بمجموعته الخاصة من القدرات ونقاط الضعف<br />
والتميز يف الشخصية.<br />
السمات المرتبطة بمتالزمة داون<br />
األفراد املصابون بمتالزمة داون لديهم سمات<br />
معرفية، وسلوكية وجسدية مختلفة. ولكن<br />
يشرتك الكثري منهم يف مجموعة من<br />
الصفات، أشهرها:<br />
> انخفاض قوة العضالت<br />
> ميل العينني لألعىل<br />
> أيدي عريضة قصرية مع قصر<br />
ا ألصابع<br />
> خط واحد عميق يف راحة اليد<br />
> أذن صغرية<br />
> لسان كبري<br />
> بطء يف التعلم<br />
> قصر الفرتة التي يستطيعون<br />
الرتكيز فيها<br />
> التأخر يف النمو اللغوي والقدرة<br />
عىل الكالم<br />
> السلوك املتهور.<br />
القهري، فقدان بعض املهارات التي كانوا<br />
يملكونها يف السابق.<br />
هناك مشاكل صحية أخرى ترتبط بمتالزمة<br />
داون لكن يجب أن ننوه بأنه مع التقدم يف<br />
الطب، أصبحت توقعات الحياة عند املصابني<br />
بمتالزمة داون أطول بكثري مما كانت<br />
عليه يف السابق.<br />
العوامل المسببة واالختبار<br />
ووفقا ملنظمة الصحة العاملية، يولد 11-10<br />
طفالً يعانون من متالزمة داون لكل 10,000<br />
والدة حية يف جميع أنحاء العالم. بعض<br />
اآلباء واألمهات هم أكرث عرضة إلنجاب طفل<br />
يعاين من هذه الحالة:<br />
النساء الذين ترتاوح أعمارهم بني<br />
> 40 سنة وما فوق.<br />
اآلباء الذين لديهم بالفعل طفل<br />
> متالزمة داون.<br />
اآلباء واألمهات الذين يحملون<br />
> األسباب الوراثية ملتالزمة داون.<br />
إذا كنت تشعر بالقلق إزاء تلك املخاطر، فمن<br />
املمكن اختبار الحالة عن طريق إجراء معني<br />
يتم فيه أخذ عينة من السائل الذي يحيط<br />
بالجنني بني األسبوع ال15 وال18 من الحمل.<br />
يتم إرسال هذا السائل للتحليل لتحديد<br />
ما إذا كان الطفل الذي لم يولد بعد لديه<br />
متالزمة داون. هذا االجراء يمكن أن يساعد<br />
اآلباء واألمهات عىل تخطيط الرعاية للطفل،<br />
وضمان أنها سوف تعطي أبنائهم أفضل بداية<br />
ممكنة يف الحياة.<br />
من املهم أن نتذكر أن متالزمة داون تؤثر<br />
عىل كل شخص بشكل مختلف. كل فرد من<br />
األفراد الذين يعانون من هذه الحالة فريد من<br />
نوعه وله قدراته ومواهبه وأفكاره واهتماماته<br />
الخاصة. يوم متالزمة داون يحتفل بالعديد<br />
من جوانب الحالة، والدور الذي تلعبه يف<br />
حياة األفراد واملجتمعات لدينا.

في مؤسسة حمد الطبية في<br />

ويعترب املؤتمر منرباً يتم من خالله تبادل الخربات<br />
واملعارف واستعراض آخر املستجدات العلمية<br />
والطبية يف مجاالت أمراض الغدد الصماء<br />
والسمنة وهشاشة العظام والسكري.<br />
وقال الدكتور محمود عيل زرعي، استشاري أول<br />
ورئيس قسم الغدد الصماء والسكري والتمثيل<br />
الغذايئ بإدارة الطب الباطني بمؤسسة حمد<br />
الطبية ورئيس اللجنة املنظمة للمؤتمر:" يعدّ<br />
هذا املؤتمر الحدث الطبي األهم لهذا العام الذي<br />
يجمع هذا العدد الضخم من الكوادر الطبية<br />
املهتمة بطب وأمراض الغدد الصماء والبحوث<br />
العلمية والطبية ذات العالقة، ولقد سَ عِ دْنا<br />
باستضافة العديد من املشاركني يف املؤتمر من<br />
مختلف أنحاء العالم من أطباء أخصائيني وأطباء<br />
عامّني وأطباء تحت التدريب."<br />
ونظراً الرتفاع معدالت السمنة والسكري<br />
واألمراض املرتبطة بها يف قطر فقد قامت<br />
مؤسسة حمد الطبية بتطوير أساليب تشخيص<br />
ومعالجة هذه األمراض وتوفري أفضل مستويات<br />
الرعاية الصحية للمرىض الذين يعانون من هذه<br />
األمراض وذلك عرب املركز الوطني لعالج السكري<br />
يف كل من مستشفى حمد العام ومستشفى<br />
الوكرة والتي تم اعتمادها من قبل اللجنة<br />
الدولية املشرتكة )JCI( مؤخراً ضمن برنامج<br />
املراكز األكاديمية الصحية.<br />

للبالغني بمؤسسة حمد الطبية، والرئيس<br />
اإلكلينييك للمركز الوطني لعالج السكري يف<br />
مستشفى الوكرة: " تطبق فروع املركز الوطني<br />
لعالج السكري منهج الرعاية الصحية املقدمة من<br />
فرق متعددة التخصصات الطبية، ويعدّ ذلك<br />
جزءاً من االسرتاتيجية الوطنية للسكري والهادفة<br />
إىل الربط بني كافة مرافق الرعاية الصحية يف<br />
دولة قطر من أجل توحيد الجهود يف سبيل<br />
تطوير وتحسني الخدمات املقدمة ملرىض السكري.<br />
وقد وُجهت الدعوة إىل متحدثني محليني<br />
ودوليني بارزين لتبادل املعارف والخربات الواسعة<br />
مع الحضور واملشاركني يف املؤتمر وذلك من خالل<br />
جلسات بحث ومناقشات تمّ تخصيصها لبحث<br />
معالجة مرىض السكري باإلنسولني، ومعالجة<br />
اضطرابات الغدة الدرقية، وغريها من املواضيع<br />
الطبية املرتبطة بأمراض الغدد الصماء.

البكترييا التي تسبب الحى وتزيد منها. وطبعاً<br />
يف جوّنا الحار هذا بعض املرىض ال يشربون<br />
ما يكفي من سوائل مما يؤدي مع الوقت إىل<br />
الركز يف كمية البول األمر الذي يؤدي<br />
إلىالحى.<br />
بالنسبة ملوضوع زراعة الكىل ، ما هي نسبة<br />
التلف يف الكىل ليحتاج الفرد لعملية الزراعة؟<br />
أي مريض تقل وظائف الكىل لديه عن ١٨٪<br />
يخضع لفحوصات الزراعة ومن بعدها يتم<br />
التنسيق مع الدكتور املعالج لعمل الزراعة.<br />
ما هي نسبة أداء الوظائف يف الكىل التي ال<br />
يتمكن اإلنسان العيش من غريها ؟<br />
أقل نسبة يحتاج لها اإلنسان لتبديل الكىل هي<br />
أقل من ٨٪<br />
ما هي عملية الزراعة وماذا تتطلب من<br />
املريض؟ هل هناك صعوبات يف العثور عىل<br />
متربعن مناسبن؟<br />
املرىض املصابون بمرض الكىل يف املراحل األخرية<br />
بنسبة أقل من ١٨% يكون لديهم احتمال<br />
لتدهور الكىل يف خالل ٦ شهور لو بدأوا<br />
بالغسيل مباشرة. لذلك ننصحهم يف البحث<br />
عن متربع من األهل لتعظيم فرصة تطابق<br />
أو تقارب األنسجة مما يجعل العملية أسهل<br />
وأسرع.<br />
قد يكون من الصعب جدً ا تشخيص أمراض<br />
الكىل بدقة قبل وصولها إىل مراحل متأخرة<br />
، ولكن كيف يمكن الحد من مخاطر اإلصابة<br />
بمرض الكىل املزمن؟<br />
الحد من املخاطر تبدأ بخطوات سهلة وهي<br />
األكل الصحي، والرياضة، واملحافظة عىل<br />
الوزن املثايل، وشرب السوائل الكافية،<br />
واالبتعاد عن التدخني، وعدم أخذ األدوية<br />
إال باالستشارة الطبية. املرىض الذين يعانون<br />
من تاريخ عائيل بأمراض الكىل يجب عليهم<br />
مراجعة طبيبهم بصورة سنوية عىل األقل.<br />
املرىض الذين لديهم خطورة عالية ملرض الكىل<br />
مثال كاملرىض بالسكري والضغط وااللتهابات<br />
والحى وأكياس الكىل يحتاجون متابعة<br />
مستمرة مع طبيبهم املعالج.<br />
ماذا تكلف إجراءات زراعة الكىل؟<br />

هل لديك نصيحة أخرية بشأن أمراض الكىل<br />
وتفاديها؟<br />
أنصح الجميع باملحافظة عىل الوزن املثايل،<br />
األكل الصحي، الرياضة، والناس الذين لديهم<br />
خطورة أكرب ألمراض الكىل يجب عليهم متابعة<br />
طبيب معالج عىل شكل مستمر.<br />

الصحة والحياة: الكلية معجزة من<br />
معجزات الله يف خلق اإلنسان، هي<br />
مصفاة عاقلة، تتيح لك أن تأكل ما تشاء<br />
، وهي تأخذ من املواد الذائبة يف الطعام<br />
كالسكر واألماح واملعادن والربوتينات<br />
نسبا ثابتة وتطرح الباقي للخارج.<br />
والكلية التي ال يزيد حجمها عن حبة<br />
الكمرثى جهاز بالغ التعقيد تعمل يف صمت<br />
وبال مقابل وبال أجر ، يدخل إليها الدم ستا<br />
وثالثني مرة يف اليوم ، وتقوم الكليتان بتصفية<br />

من أمراض الكىل" ،وبهذه املناسبة كان<br />
ملجلة الصحة والحياة هذا الحوار مع<br />

األسباب األكثر شيوعاً<br />
ألمراض الكلى المزمنة<br />
هي مرض السكري<br />
وضغط الدم المرتفع<br />
تؤدي الكىل دورا هاما وبالغ التعقيد<br />
يف جسم اإلنسان هل تعرفنا عىل<br />
هذا الدور ووظيفة الكىل يف الجسم؟<br />
الكىل من األعضاء املهمة يف جسم اإلنسان، لها<br />
الكثري من الوظائف، ومنها التخلص من السوائل<br />
الزائدة يف الجسم، والتخلص من سموم الجسم<br />
وأي فضالت من عملية األيض metabolism<br />
يف جسم اإلنسان، والتخلص من األدوية وأي<br />
سموم أخرى يف الجسم. وتساعد الكىل عىل<br />
انتظام ضغط الدم، وتساعد عىل انتظام نسب<br />
أحماض وأمالح الدم. ومن الوظائف األخرى<br />
التي تقوم بها الكىل هي إبراز هرمون الدم،<br />
الذي بدوره يساعد عىل انتظام الهيموغلوبني<br />
.Hemoglobin وكذلك إبراز الفيتامني د الناشط<br />
الذي يساعد عىل انتظام الكالسيوم والفسفور<br />
ويحافظ عىل العظام يف جسم اإلنسان.<br />
أمراض الكىل من الحاالت الخطرية<br />
التي تؤثر عىل صحة وحياة الكثري من<br />
األشخاص من مختلف األعمار ، وسؤالنا<br />
ما هو مرض الكىل املزمن وما هي أسبابه؟<br />
أمراض الكىل تنقسم إىل نوعني: الفشل<br />
الحاد أو املرض الحاد، ويف غالب األحيان<br />
تحدث هذه األمراض بصورة سريعة ومع<br />
الوقت تتحسن بطريقة سريعة. ولكن إذا<br />
كان الضرر مزمناً يف الكىل تتأثر الوظائف<br />
عىل املدى البعيد من دون أي تحسن متوقع.<br />
أما أسباب الفشل الكلوي املزمن طبعاً فهي<br />
كثرية أهمهما مرض السكري ومرض ارتفاع<br />
ضغط الدم، وينقسم إىل ٥ مراحل اعتماداً عىل<br />
وظائف الكىل – تتدرج من 90% يف املرحلة األوىل<br />
إىل الفشل الكلوي النهايئ يف املرحلة الخامسة.<br />
أما األسباب األخرى فهي أكياس الكىل<br />
الوراثية وحى الكىل والتهابات الكىل املزمنة يف<br />
نسيج الكىل واالختالل يف جهاز املناعة واألمراض<br />
الوراثية وأحياناً تحدث اإلصابة بعد االلتهابات<br />
البكتريية املتكررة حيث يمكن أن تؤدي مع الوقت<br />
لخلل يف وظائف الكىل. كما أن حى الكىل<br />
سبب مهم آخر خصوصاً يف حالة االنسداد.<br />
يعاين ما يصل إىل 10 يف املائة من سكان<br />
العالم من أمراض الكىل املزمنة ، فما مدى<br />
انتشار املرض )مرض الكىل املزمن( يف قطر؟<br />
مرض الكىل منتشر يف كل املجتمعات يف<br />
كل العالم يف أغلب الدول يكون االنتشار<br />
ما بني ١٠-١٣٪ بمعنى أنه متواجد يف كل<br />
مجتمع بنسبة تراوح بني – ١٠ ١٣٪ ويكون<br />
موزعاً عىل املراحل األوىل إىل الخامسة. وهو<br />
منتشر يف الدوحة بنفس النسبة تقريباً.<br />
من هم األشخاص األكر عرضة<br />
لتلف الكىل؟<br />
األكر عرضة هم الذين نتوقع منهم أن<br />
يراجعوا الطبيب الكتشاف املرض يف صورة<br />
مبكرة وهم طبعا الذين يعانون من مرض<br />
السكر وارتفاع ضغط الدم والذين عندهم<br />
تاريخ لغسيل أو زراعة يف العائلة. وجدير<br />
بالذكر أن املرىض فوق ال٥٠ يف العمر يستمرون<br />
مع طبيهم الخاص يف املركز الصحي وطبعا<br />
املرىض املصابون بأكياس كىل أو حى يف<br />
الكىل واملرىض املصابون بالتهابات متكررة يف<br />
البول يحتاجون لطبيب خاص بصورة دورية.<br />
ما سبب اإلصابة بحصاوي الكىل؟<br />
الحى لها أسباب كثرية منها أسباب وراثية<br />
يعني يف الجينات ومنها قلة شرب السوائل ومنها<br />
التهابات الكىل. فهناك بعض أنواع البكترييا<br />

تقويم األسنان<br />
عندما يتعلق األمر بتقويم األسنان، الكثير يربط تطبيقه بالجماليات. ولكن إلستخدام أسناننا بشكل<br />
مريح، نحن بحاجة إلنسجام األسنان مع العضالت والعظام والمفاصل. في كثير من األحيان، يسعى<br />
الناس إلصالح بعض األسنان المعوجة ويكتشفون أن لديهم مشاكل أكثر خطورة، مثل محاذاة<br />
الفك الغير صحيحة، بعد زيارة طبيب تقويم األسنان. إذا تجاهلت، تؤدي هذه المشاكل إلى قضايا<br />

التشخيص املبكر<br />
من املهم الحصول عىل استشارة مبكرة ألن<br />
العظام لدى الطفل ال تزال يف مرحلة النمو،<br />
لذلك يستجيب لعالجات تقويم األسنان أسرع<br />
من البالغني. تويص الجمعية األمريكية لتقويم<br />
األسنان بدأ تلقي اإلستشارات حول تقويم<br />
األسنان يف حوايل سن السابعة. هذا العمر يعد<br />
مهماً ألن األضراس والقواطع تبدأ بالظهور<br />
مما قد يفتح الباب لعدد من املشاكل. من<br />
خالل إتخاذ أشعة متكاملة لألسنان، يمكن<br />
للمختصني تحديد أي إنحراف وإمكانية حدوث<br />
ذلك يف املستقبل. أهم يشء يف هذا العمر هو<br />
مراقبة نمط نمو الفكني ملساعدة األخصائيني يف<br />
تحديد ما إذا كان الطفل يحتاج إىل متابعة كل<br />
٦ أو ١٢ شهر أو يتطلب معالجة فورية.<br />
أي تقويم مناسب لك؟<br />
إذا كنت أنت أو طفلك بحاجة للتقويم وتخى<br />
من عدم الراحة أو تكره كيف يبدو شكله، ال<br />
حاجة للقلق بعد اليوم! قطع تقويم األسنان<br />
شوطاً طويالً منذ القطع املعدنية التقليدية.<br />
التقويم امللون أصبح أكرث شعبية لدى األطفال.<br />
التقويم الشفاف املصنوع من السراميك يناسب<br />
الذين يشعرون بالقلق من املظاهر ألنه يكون غري<br />
مرىْي متناغم مع لون األسنان. التقويم الداخيل<br />
يثبت عىل الجانب الداخيل لألسنان. للتغلب<br />
عىل عدم الراحة للسان، يكون هذا التقويم<br />
رقيقاً جداً إىل ١.٥ ملم مع تصميم سلس للمزيد<br />
من الراحة. ولكن هذا التقويم أكرث كلفة.<br />
الرعاية الاحقة والنظافة العامة<br />
إتباع نظام صارم لتنظيف الفم هو مفتاح نجاح<br />
العالج. متلقو التقويم يجب عليهم تنظيف<br />
أسنانهم بعد كل وجبة، ويصل التنظيف إىل<br />
خمس مرات يف اليوم بفرشاة األسنان الخاصة<br />
للتقويم وفرشاة ناعمة. وقالت الدكتورة نادين<br />
عبد السالم، أخصائية تقويم األسنان يف مركز<br />
داماس لألسنان، "فرشاة تقويم األسنان لديها<br />
شعر رقيق يكون يف جميع الجهات للوصول<br />
للمناطق يف جهاز التقويم التي ال تمكن<br />
الفرشاة العادية الوصول إليها." إستخدام<br />
غسول الفم مرتني يومياً مهم جداً للتأكد من<br />
نظافة الفم والحفاظ عىل أفضل حالة تنظيف.<br />
يجب التأكد من نظافة فم الذين يضعون<br />
تقويم األسنان، ألن قلة الرعاية قد تؤدي إىل<br />
أمراض اللثة، وتراكم البالك والعديد من<br />
املشاكل األخرى أثناء العالج. ابقي عىل اتصال<br />
وثيق مع أخصايئ تقويم األسنان، يف حني<br />
كانت لديك أي مشاكل أو تخوفات، وتأكد<br />
من أنك تعطي أسنانك أفضل رعاية ممكنة.<br />
تقويم األسنان بالليزر<br />
يتطلب عالج التقويم الكثري من الجهد والوقت<br />
من الذين يتلقون العالج. ولكن يأمل أطباء<br />
األسنان تقليل الفرتة الزمنية، وااأللم املصاحب<br />
للعالج التقويمي من خالل تقويم األسنان<br />
مع الليزر. قالت الدكتورة نادين عبد السالم،<br />
"إستخدام العالج بالليزر يزيد معدل حركة<br />
األسنان بنسبة تصل إىل ٤٠ ضعف، ويقيض عىل<br />
األلم املصاحب للعالج بنجاح." لكن لألسف هذا<br />
العالج املتقدم ليس متاحاً يف قطر حالياً ولكن<br />
سيكون متاحاً يف مركز داماس يف وقت قريب.<br />

كم من الوزن تفقده<br />

يعتقد الناس أن فور استهالكهم<br />
للمنتج سيفقدون عشرة أو خمس<br />
عشر كيلوغراماً. ولكن كل جسم<br />
فريد من نوعه ويتفاعل مع املنتج<br />
بطريقة مختلفة. النظام الغذايئ<br />
ونمط الحياة مهم أيضاً. ال يمكنك<br />
توقع فقدان الوزن إذا كان طعامك<br />
عبارة عن وجبات كبرية من األرز أو<br />

الطعام اليسء. ال يمكنني ضمان<br />
مقدار الوزن الذي ستخسره، لكن<br />
أؤكد مئة باملئة أنك ستفقد الوزن.<br />
تنويه: يف حني أن منتج فات برنرز )حرق الدهون( يمكنه أن يعزز<br />

السكريات تسبب التآكل يف األسنان<br />
عىل الرغم من وضوح العالقة بني أكل<br />
الحلوى والكعك والحلويات والتآكل يف<br />
األسنان، وألن أي شكل من أشكال السكر<br />
يمكن أن يسبب تسوس األسنان، فإن مجلة<br />
الصحة العامة والتغذية تنصح بالحد من<br />
تناول السكريات إىل أقل من 2% من مجموع<br />
السعرات الحرارية للحد من مخاطر التسوس.<br />
استهالك املشروبات املحالة بالسكر يسبب<br />
ضرراً أسوأ، كما أن السكر الذي يوجد يف<br />
الفاكهة الطبيعية يساهم أيضا يف فقدان<br />
مينا األسنان. وبدالً من تقليص تناول<br />
الفاكهة حاول أن تتناولها مع طعامك<br />
أو وجبتك. إذا قررت تناول الفاكهة<br />
أو تصبرية محالة، فإن املضمضة بعد<br />
تناول السكريات فكرة مفيدة ورائعة.<br />
اجعل للفيتامن يس أولوية<br />
ترتبط األنظمة الغذائية التي تفتقر لفيتامني<br />
يس بأمراض اللثة، وضمان كمية من<br />
األطعمة الغنية بالفيتامني يس ضروري<br />
لصحة اللثة. ويلعب فيتامني يس دورا<br />
مهما يف إنتاج الكوالجني ،Collagen وهو<br />
الربوتني األسايس لنمو األنسجة وإصالحها.<br />
باإلضافة إىل ذلك، يعمل فيتامني يس<br />

األسنان بقدر الضرر الذي تسببه األطعمة<br />

أسوأ ما يف األطعمة واملشروبات الحمضية<br />
هو الكوال والعصائر الحامضة. ولكن<br />
إذا كنت تريد تناول هذه االصناف من<br />
الحمضيات فادمجها مع عنصر أكرث قلوية،<br />
كالخضروات أو الزبادي أو املكسرات..<br />
ال تنس حمض الفوليك<br />
ال تنطبق احتياجات النظام الغذايئ لحمض<br />
الفوليك عىل الحوامل فقط، بل لجميع<br />
الناس. فهذا الفيتامني من عائلة B يحمي<br />
اللثة. وتشري البحوث إىل أن حمض الفوليك<br />
يخفف التهابات اللثة وأعراضها. وتشمل<br />
قائمة أفضل مصادر لحمض الفوليك كال<br />
من األعشاب الورقية الخضراء والفاصوليا<br />
والحبوب الكاملة وحبوب اإلفطار.<br />
تأكد من تناول كمية جيدة<br />
من الكالسيوم والفوسفور<br />
الكالسيوم والفوسفور كالهما مهمان<br />
إلعادة التمعدن remineralization<br />
يف األسنان، للمساعدة يف مكافحة تآكل<br />
األسنان. إىل جانب منتجات األلبان، التوفو<br />
والفول والخضروات الورقية واملكسرات<br />
والبذور، كلها توفر الكالسيوم بكميات<br />
مفيدة. حاول تناول ما بني 3-2 حصص<br />
من األطعمة الغنية بالربوتني يومياً مثل<br />
اللحوم واألسماك ومنتجات األلبان<br />
واملكسرات والبذور، للحصول عىل ما<br />
يكفي من الفوسفور يف نظامك الغذايئ.<br />
حاجتك ألسنانك تستمر طول العمر<br />
صحة أسنانك وقوتها تكمن يف اتباع نظام<br />
غني بالفوائد الغذائية وتجنب اإلفراط<br />

ما هي الصعوبات التي يواجهها األطفال<br />
الذين يعانون التوحد يف قطر؟<br />
هناك صعوبة يف إيجاد مركز مناسب للطفل الذي<br />
يعاين من التوحد. وعدم وجود أخصائيني عىل<br />
مستوى يعترب مشكلة كبرية خصوصاً يف املراكز<br />
الخاصة، وحدثت أكرث من مشكلة تعدي بالضرب أو<br />
بالحبس عىل الطفل ألنهم ال يعرفون كيف يتعاملون<br />
معهم. واملركز الوحيد الذي تأمنني عىل طفلك فيه<br />
هو الشفلح.<br />
السباحة من الهوايات الجيدة وتسمح للطفل بإفراز<br />
طاقته الحركية، ولكن ال توجد نوادي ألطفال التوحد<br />
بسبب قلة األخصائيني. وقد بدأوا بإنشاء مراكز ونوادِ<br />
فيها سباحة لذوي االحتياجات منها نادي الغرافة<br />
ولكن بشرط توافر أخصايئ للسباحة. واملشكلة األخرى<br />
تحدث يف الصيف لعدم تواجد مراكز صيفية.<br />
كيف تحسنت حالة ولدك فهد؟<br />
أول يشء الحظته هو التعديل السلويك ولكن قبل أن<br />
يدخل فهد للشفلح كان لديه مدرّسة خاصة أجنبية<br />
تعمل عىل تصحيح سلوكه، ومع الشفلح تقدم فهد<br />
بنسبة كبرية، ويف السنة الثانية تجاوزنا مرحلة كبرية<br />
يف تعديل السلوك.<br />
ما هي أحىل لحظة مررت فيها مع طفلك؟<br />
أحىل األوقات مع فهد كانت عند تجاوزه عدة حاالت<br />
غري مرغوبة. كل ما أرى فهد يحقق شيئا جديدا أو<br />
يتعلم أقول الحمد الله هناك نتيجة. ولكن أخاف من<br />
املستقبل، وأرغب يف دمجه يف مدرسة عادية، ولكن لم<br />
يتم ذلك بسبب إفراطه يف الحركة.<br />
يشء تقولينه للمجتمع؟<br />
أتمنى أن يكون املجتمع واع أكرث وأتمنى أن ينتبه األهل<br />
عىل أبنائهم خصوصاً يف عمر ٢-٣ سنوات – يف هذه<br />
املرحلة التي تتضح فيها حالة التوحد. البداية السليمة<br />
هامة وهي مهمة تقع عىل عاتق البيت. ال يجب حرمان<br />
الطفل من البيت، واللعب مع إخوانه والخروج<br />
واللعب كأنه إنسان طبيعي. يجب أن يعرف املجتمع<br />
أن أطفال التوحد ليسوا مجانني.<br />
ماذا تقدم قطر ملن يعاين من<br />
ا لتو حد ؟<br />
أوال، أعترب ولدي عبد الرحمن محظوظاً<br />
ألنه لم يبق عىل قائمة االنتظار طويالً.<br />
ولكن املؤلم أن هناك أطفاال كرث يف حالة<br />
صعبة ما زالوا عىل قائمة االنتظار.<br />
وبالنسبة لكل املوظفني يف وحدة<br />
التوحد يف الشفلح فهم متعاونون جداً،<br />
ويطبقون أحدث الربامج لديهم عىل<br />
الطالب.<br />
ينقصنا يشء واحد وهو فرتة الصيف<br />
حيث يفتقد الشفلح الربامج الصيفية<br />
وهي فرتة طويلة تقارب ٣ أشهر. وهنا<br />
ال بد من اللجوء إىل املراكز الخاصة<br />
يف الفرتات الصباحية أو املسائية وهي<br />
لألسف ال تفيد الطفل وفيها استغالل.<br />
كيف تحسنت حالة عبد الرحمن؟<br />
عبد الرحمن تحسن كثرياً يف أول سنتني<br />
يف الشفلح – ولكن ال أستطيع القول إن<br />
حالته تحسنت بدرجة فائقة ألن درجته<br />
يف التوحد من خفيفة إىل متوسطة –<br />
ولذلك يكتشف هذه املهارات بسرعة.<br />
أجمل لحظة مع طفلك؟<br />
مع عبد الرحمن هناك لحظات حلوة<br />
كثرية. أولها قبل سنة عندما قالوا يل<br />
إن عبد الرحمن قابل للدمج – طبعاً<br />
كانت لحظة جميلة. اللحظة الثانية –<br />
لن أنساها عندما شارك عبد الرحمن يف<br />
بطولة دولية للسباحة املفتوحة للناس<br />
العاديني وفاز بامليدالية الذهبية. كانت<br />
سباحة حرة يف املياه العميقة وكانت<br />
أول مرة لعبد الرحمن يف البحر حيث<br />
كان تدريبه يف املسبح. واستمر نجاحه<br />
حيث حصل بعد سنتني عىل املركز األول<br />
يف مسابقة فنية للمدارس العادية.<br />
يشء تقولينه للمجتمع؟<br />
كلمة للحكومة الرشيدة وكلمة<br />
للمجتمع: أتمنى أن تقبلوا أوالدنا<br />
وأن تفهموهم ألنهم مختلفون وليس<br />
متخلفني! وكونوا لهم عوناً وسنداً –<br />
فال أحد لهم بعد الله وبعدنا – وعىل<br />
املجتمع أن يفهم طبيعة أوالدنا أنها<br />
ليست إعاقة بل هي اضطراب ألن العلم<br />
إىل اآلن لم يقدر عىل تشخيص التوحد<br />
وما هو بالضبط – هل هو مرض أو<br />
إعاقة.<br />
وأقول لحكومتنا الرشيدة أن تجلب<br />
الخدمات ألطفالنا ألن كل ما كان<br />
التدخل مبكراً كل ما أنقذ عددا أكرب.<br />
ولكن يف حالة التأخر يصبح الوضع<br />
مزرياً جداً. ويأيت التوحد كصدمة لألم<br />
واألب – صدمة تغري حياتك كلها يف<br />
البيت – فإما تستوعب الصدمة وتنقذ<br />
طفلك ويصل لرب األمان أو إنك تعيش<br />
وتنكر وتخرب البيت بأجمعه.
بصفتك مدير إدارة الخدمات النفسية<br />
والدعم األسري يف مركز الشفلح، ما هي<br />
أهم األشياء التي تقع تحت مسؤولية<br />
إدارتكم وما هي أهم انجازاتها؟<br />
الخدمات األساسية التي نقدمها يف إدارة<br />
الخدمات النفسية والدعم األسري يف مركز<br />
الشفلح تنقسم إىل 3 أقسام:<br />
القسم األول: قسم الخدمات االجتماعية: وهو<br />
حلقة الوصل بني املركز واألهل ومعني بمتابعة<br />
الطفل مع األهل يف كل املراحل وكل املجاالت،<br />
ويشرف القسم وينظم النشاطات الطالبية<br />
الداخلية أو الخارجية التي تساعد يف عملية<br />
تعميم املهارات. فاملهارة التي يتعلمها الطالب يف<br />
املركز يجب أن تعمم بالخارج وإال سوف تمحى.<br />
كما تساعد النشاطات أيضاً يف عملية دمج<br />
الطالب اجتماعياً يف املجتمع مثل الذهاب ألماكن<br />
قد تكون سوبر ماركت، أو املوالت.<br />
• توعية املجتمع بقدرات وإمكانيات هؤالء<br />
الطالب، وأنهم يستطيعون القيام بمهارات<br />
وأشياء كثرية مثلهم مثل الطالب اآلخرين.<br />
القسم الثاين: قسم اإلرشاد األسري:<br />
ويقدم خدمات إرشادية فردية وجماعية،<br />
لألهل، وللطالب<br />
• تقديم خدمات الدعم لألهل الذين عندهم<br />
مشاكل متشابهة، ليجتمعوا مع بعضهم البعض<br />
تحت اشراف األخصائيني، لتبادل الخربات، مما<br />
يعطي الدعم لآلباء واألمهات اآلخرين.<br />
• القيام بنشاطات التوعية، سواء بتقديم<br />
املحاضرات أو باملشاركة مع جماعات أصدقاء<br />
الوقاية من األمراض<br />
مقالة خاصة بمناسبة اليوم العالمي للتوحد<br />
وكان ملجلة الصحة والحياة هذا اللقاء مع الدكتور محمد األطرش<br />
مدير إدارة الخدمات النفسية والدعم األسري يف مركز الشفلح.<br />
الشفلح حيث نتعاون مع أكرث من 100 مدرسة<br />
لتوعية الطالب من خالل النشاطات املختفة.<br />
القسم الثالث: الخدمات النفسية والتقييم<br />
التقييم والتشخيص ألي طالب يريد االلتحاق<br />
بالشفلح ليك نستطيع تقديم الخدمة املناسبة<br />
مع حالة الطالب. ويتضمن ذلك التقييم<br />
االجتماعي، والتقييم الطبي، والتقييم النفيس،<br />
ومستوى القدرات العقلية، وتقييم العالج<br />
الطبيعي، وتقييم العالج الوظيفي، وتقييم<br />
عالج النطق، وغريها من التقييمات.<br />
• باإلضافة للتقييمات املذكورة، عندنا<br />
أيضاً عملية تشخيص لحالة التوحد لتحديد إذا<br />
كان الطالب عنده سمات التوحد أم ال. ويتم<br />
ذلك بواسطة مختصني مدربني وباستخدام<br />
أحدث األساليب املوجودة بالعالم مثل أدوات<br />
ال ADOS وهي Autism Diagnostic<br />

ما هي أكرب التحديات التي تواجهونها<br />
يف املركز؟<br />
أكرب التحديات هي العثور عىل املختصني املدربني<br />
عىل أحدث األساليب العلمية والقادرين عىل<br />
التعامل مع الطالب، أما التحدي الثاين فهو<br />
قوائم االنتظار والقدرة عىل استيعاب جميع<br />
الطالب. ومن التحديات األخرى، كرثة القوانني<br />
التي تنظم استخدام األدوات الحديثة. ولكن<br />
استطعنا مؤخراً أن نحصل عىل االعتماد<br />
واملوافقة عىل اعتبار مركز الشفلح مركزاً للتعليم<br />
والتدريب املستمر يف مجال تحليل السلوك،<br />
وأيضا يف تدريب األخصائيني.<br />

حدثنا عن التحليل السلويك ومدى<br />
نجاح األساليب الحديثة للتواصل مع<br />
الطاب يف الشفلح؟<br />
التحليل السلويك هو برنامج جديد يستخدم مع<br />
الطالب يف املركز، له عدة شعب أو اتجاهات:<br />
• األوىل: لبعض الطالب سلوكيات غري مرغوبة،<br />
وقد ال يستطيع الطالب أن يعرب ملاذا يقوم بهذا<br />
السلوك، وبالتايل نستخدم أساليب تحليل<br />
السلوك ملعرفة أسباب قيام الطالب بهذا<br />
السلوك، وهذا بسمح لنا بتطوير خطط سلوكية<br />
لتعديل وتعليم الطالب سلوكيات بديلة<br />
للسلوكيات الغري مرغوبة.<br />
• الثانية: هي جانب التعليم باستخدام األساليب<br />
الحديثة يف تعليم وتدريب طالبنا، فعىل سبيل<br />
املثال باالعتماد عىل تحليل السلوك قمنا بتطوير<br />
تقييم السلوك اللفظي واملهارات الوظيفية<br />
لألفراد ذوي التوحد.<br />
• الثالثة: استخدام أساليب تحليل السلوك<br />
العلمية التطبيقية لتحديد جوانب الضعف<br />
وجوانب القوة بدقة والتي تبنى عىل أساسها<br />
الخطة الرتبوية الفردية التي نقوم بتنفيذها،<br />
والتي أثبتت فعاليتها وحققنا فيها نتائج<br />
رائعة جداً.<br />
• الرابعة: تدعم نتائجنا عندما يالحظ األهل تغري<br />
سلوك أطفالهم. يف السابق كانوا يستخدمون<br />
أسلوب تعليم املهارة ويتمنون تعميمها يف<br />
السلوك، ولكن باستخدام أساليب تحليل<br />
السلوك ال نتمنى، بل نتأكد من تعميم السلوك<br />
يف كل مكان يذهب إليه الطالب.
لنا مع القطرية تجربة ناجحة في<br />
مجال الدمج في سوق العمل<br />
يف البداية نود تسليط الضوء عىل سري<br />
العمل يف املركز وعدد الطلبة فيه?<br />
يعتني املركز بحوايل 640 طالبا وطالبة، نسبة<br />
القطريني منهم حوايل 60%. ويقدم املركز<br />
خدمات متميزة فعالً ولعل أحد املؤشرات عىل<br />
ذلك هو أن املركز يضم 760 موظفا وموظفة<br />
تقريبا. ويف العام املايض 2015، تم اعتماد برنامج<br />
تحليل السلوك يف مركز الشفلح، وهو برنامج<br />
تم إعداده من قبل مختصني من املركز. وقد تم<br />
اعتماده وهو األول من نوعه يف الوطن العربي<br />
باللغة العربية، وبذلك أصبح مركز الشفلح<br />
يمتلك شهادة لتدريب املراكز األخرى يف هذه<br />
الخدمة، ونحن نفخر بذلك كثرياً.<br />
تقدمون برامج تعليمية متنوعة لطاب<br />
املركز، فما هو نظام التعليم لديكم؟<br />
ينقسم الربنامج التعليمي يف مركز<br />
الشفلح إىل ثالث مراحل:<br />
املرحلة األوىل: ويبدأ التعليم فيها من عمر<br />
ثالث إىل ست سنوات، وتسمى مرحلة الطفولة<br />
املبكرة وهي مثل الروضة. حيث يعمل أخصائيو<br />
تأهيل طبي نفيس عىل إعطاء الطالب جرعات<br />
تأهيلية وتدريبية. وبعد هذه املرحلة ينقسم<br />
الطالب حسب حالتهم، فالذين يستطيعون<br />
االندماج يلتحقون بمدرسة عادية مع أقرانهم،<br />
ويتم اإلشراف عليهم عن طريق إدارة الدمج<br />
يف وزارة التعليم.<br />
املرحلة الثانية: وهي تتناول الجزء اآلخر من<br />
الطلبة والذي يستمر معنا يف املركز حيث يتلقون<br />
التعليم الوظيفي، مثل كيفية األكل، وكيفية<br />
إمساك القلم، والورقة والتي تعد مهارات<br />
أساسية. ونعمل أيضاً مع من يستطيع تعلم<br />
أساسيات التعليم، مثل الحروف، والجمل.<br />
وتستمر هذه املرحلة حتى سن 13 سنة أو 14<br />
سنة، وبعد ذلك يتم تقييم قدراتهم ودمجهم<br />
إذا كانوا عىل استعداد للدمج يف املدارس العادية.<br />
املرحلة الثالثة: وتشمل الطلبة بعد سن 14<br />
عاما، حيث يدخل الطالب يف إدارة أخرى اسمها<br />
التأهيل والتدريب الوظيفي، ويستمر حتى سن<br />
أو 19سنة، أو 20 سنة أو 21 سنة عىل حسب<br />
قدراته. ويف هذه املرحلة يؤهل الطالب ويدرب<br />
عىل املهارات الحياتية والعملية التي يستطيع أن<br />
يعيش منها يف املستقبل ويتمكن من الدخول إىل<br />
سوق العمل.<br />
عىل ذكر سوق العمل، ما مدى<br />
نجاح املركز يف دمج طابه وإلحاقهم<br />
بسوق العمل؟<br />
خرّجنا يف العام املايض 194 طالبا وطالبة،<br />
بقدرات متفاوتة. وقد أصبح 114 منهم موظفني<br />
يف قطاعات الدولة املختلفة. وهدفنا الرئييس هو<br />
الدمج، ولكن من ال يستطيع االندماج يف بيئة<br />
العمل، نعمل عىل ادماجه يف البيت، وكسر<br />
الحاجز بينه وبني عزلته مع املجتمع البسيط<br />
من حوله، كاألهل واألقارب. وتساعدنا األجهزة<br />
الحديثة يف هذا املجال، حيث ندرب الطالب<br />
الذين ال يستطيعون التعبري عن رأيهم، عىل<br />
كيفية التواصل مع املجتمع. وتساعد هذه<br />
األجهزة الطلبة الذين ال يستطيعون الحركة عىل<br />
الشفلح يمتلك شهادة لتدريب<br />
المراكز الأخرى .<br />
التواصل مع غريهم بالعني والتعبري عما يريدون<br />
من خالل الصور. وبعض الطلبة مستواهم<br />
الذهني متقدم جداً، ولم تعقهم اإلعاقة الكاملة<br />
عن التعلم، فتدربوا عىل هذه األجهزة إىل<br />
أن أصبحوا يستطيعون كتابة الجمل الكاملة<br />
بعيونهم فقط، من خالل الرتكيز عىل األحرف<br />
يف كيبورد ومن خالل ذلك يتكلم الطالب بشكل<br />
شبه طبيعي مع البيئة.<br />
ولدينا طالب عندهم قدرات عالية جدا لكنهم<br />
انعزاليون ويخافون من الناس. وقد دخلنا<br />
مع هذه الفئة يف تجربة جديدة بالتعاون مع<br />
القطرية، والتي أحب أن أوجه لها وملديرها<br />
السيد أكرب الباكر كل الشكر والتقدير. كانت<br />
التجربة عبارة عن نموذج مصغر من عملية<br />
تحضري الشنط التي توضع يف الطائرة يف الدرجة<br />
األوىل وهي من األعمال اليدوية. وقد شارك يف<br />
هذه التجربة 25 طالبا من املركز ملدة سنة كاملة،<br />
ونجح الطالب يف تعبئة 35 ألف شنطة وتغليفها<br />
بإتقان كامل.<br />
كيف تعتنون بالحالة الصحية للطاب<br />
داخل املركز؟<br />
املركز ينسق مباشرة مع وزارة الصحة. وكل<br />
األطباء املوجودين يف املركز معتمدون من وزارة<br />
الصحة. وباإلضافة إىل ذلك، لدينا إدارة خدمات<br />
نفسية ألن الطالب مثل جميع الناس، يتعرضون<br />
لبعض املشاكل، وتعمل هذه اإلدارة عىل حل<br />
هذه املشاكل والتواصل مع األهل.<br />
قطر تهتم بشكل كبري بتوفري خدمات متنوعة<br />
لذوي اإلعاقة رغم صغر عدد السكان. ومع ذلك<br />
وبتوجيهات من القيادة العليا، تقدم جميع<br />
الخدمات مجاناً، ليس فقط الخدمات املركزية<br />
وأيضاً الخدمات املضافة مثل الوجبات املقدمة<br />
لألطفال، واملواصالت وغريها من الخدمات التي<br />
يحتاجها الطالب.<br />

يبدأ من رأس الدولة<br />
الصحة والحياة: تعطي دولتنا العزيزة قطر اهتماما كبيرا لفئة ذوي اإلعاقة بهدف دمجهم في المجتمع وتسهيل سبل<br />
العيش أمامهم، والتفاعل مع مختلف مراحل الحياة ومتطلباتها واحتياجاتها، ويكفينا فخرا أن 2 أبريل وهو اليوم العالمي<br />
للتوحد كان اقتراحا من دولة قطر، ووافقت عليه جميع الدول األعضاء في األمم المتحدة، ويتصدر مركز الشفلح المشهد<br />
كمؤشر ودليل ملموس على هذا االهتمام. تأسس الشفلح عام 1999، وهو من المراكز القليلة في العالم التي ال تعتني<br />
بذوي اإلعاقة وخصوصا الذهنية فقط، بل يعتني أيضاً بالتوحد ومتالزمة داون، واإلعاقات المختلفة مثل الشلل الدماغي<br />
وغيرها من الحاالت النادرة. ويسعى المركز بشكل رئيسي لدمج الطالب بشكل طبيعي في المجتمع، وتزويدهم باألدوات<br />
الالزمة لذلك. ومن هذا المنطلق ولمعرفة المزيد عن مركز الشفلح ودوره المتميز في المجتمع كان لنا هذا التحقيق<br />
والذي بدأناه بحوار مع السيد محمد بدر السادة المدير التنفيذي لمركز الشفلح:<br />

واملستقبل للتوحد<br />
يمكن القول بأن طيف التوحد طاملا كان<br />
موجوداً. وقد لعب البارزون منهم أدواراً<br />
رئيسية عىل مر التاريخ والصناعة، وذلك<br />
باستخدام املثابرة والعزم والفضول والرتكيز<br />
املكثف والتي غالبا ما ينظر إليها عىل أنها<br />
الهدايا التي تأيت مباشرة من حالتهم.<br />
كل شخص لديه مرض التوحد يواجه<br />
تحديات فريدة وقوة خاصة. وهناك عدد<br />
كبري من الناس تحت طيف التوحد يتمتعون<br />
بمعدل ذكاء عال ومواهب معينة. املواهب<br />
اإلبداعية بما يف ذلك الفن أو املوسيقى<br />
والعبقرية مثل املهارة يف الرياضيات،<br />
والحوسبة والشطرنج شوهدت يف األشخاص<br />
الذين يعانون من مرض التوحد وتجاوزت<br />
بشكل كبري قدرات ومهارات بقية السكان.<br />
عالم التوظيف بدأ باالنتباه، والشركات<br />
تبحث عمن هم تحت طيف التوحد لخربتهم<br />
وقدرتهم عىل الرتكيز عىل أنواع معينة من<br />
املهام، عىل سبيل املثال تلك التي تتطلب<br />
دقة فائقة. وهذا يعكس تنامي رسالة<br />
"حركة التنوع العصبي" neuro-diversity<br />
movement التي تحتفل بالتنوع الهائل<br />
من املهارات والقدرات البشرية، بما يف ذلك<br />
الذين ال يرغبون يف "الشفاء" من حالتهم<br />
العصبية.<br />
الفوارق الشاسعة يف جميع أنحاء طيف<br />
التوحد وتحسني طرق التشخيص إىل جانب<br />
الكم القليل من املعروف أو املفهوم عنه،<br />
يضاعف التحديات التي يمثلها التوحد<br />
للمجتمع الطبي واملجتمع ككل.<br />

مقالة خاصة بمناسبة اليوم العالمي للتوحد<br />
الدعم والعاج يف قطر<br />
حدث يف اآلونة األخرية تقدم مثري يف هذا املجال يف قطر. فقد تم إنشاء برنامج وطني ملرىض التوحد العام املايض تحت رعاية<br />
وزارة الصحة العامة. هذه املجموعة من املهتمني من جميع أنحاء البالد بما يف ذلك األطباء النفسيني، وأطباء عالج النطق<br />
والعالج الوظيفي، وعلماء النفس، واملدارس، واإلداريني، واألهم أفراد األسرة.<br />
هذه املجموعة مسؤولة عن خلق خطة التوحد<br />
الوطنية التي تقوم عىل ست ركائز وهي:<br />
الوعي، والفحص والتشخيص والتدخالت<br />
والخدمات املدرسية واالنتقال إىل مرحلة الكبار<br />
العمرية. ونحن محظوظون بأن تعمل منظمة<br />
الصحة العاملية )WHO( بالتعاون مع هذه<br />
املجموعة يف وضع اللمسات األخرية<br />
عىل الخطة.<br />
سيتم عقد مؤتمر القمة العاملي لالبتكار يف<br />
الصحة )WISH( يف تشرين الثاين من هذا<br />
العام يف الدوحة. منتدى التوحد التابع ل<br />
WISH له أهمية كبرية يف تقدم بعض من<br />
أهم القضايا امللحة املتعلقة بالتوحد. الهدف<br />
الرئييس للمنتدى هو التعرف عىل الثغرات يف<br />
خدمة األطفال واألسر محلياً وعاملياً، واقرتاح<br />
الحلول ملختلف القضايا عىل الصعيدين<br />
الوطني والدويل. وسيعمل خرباء التوحد من<br />
القارات الخمس املشاركة جنبا إىل جنب مع<br />
فريق .WISH<br />
معهد البحوث الطبية الحيوية قطر<br />
)QBRI( يقوم حاليا بإعداد أول دراسة عن<br />
انتشار مرض التوحد يف قطر. هذه الدراسة<br />
املثرية تمهد الطريق لتحديد احتياجات<br />
الخدمات يف البالد.<br />
ويلعب عدد من املدارس واملراكز يف<br />
القطاعني العام والخاص دورا هاما يف تقديم<br />
الخدمات لألطفال املصابني بالتوحد، مثل<br />
مركز الشفلح ومركز تنمية الطفل. وتخطط<br />
مؤسسة قطر )QF( لفتح مدارس فنية عالية<br />
املستوى هذا العام لألطفال املصابني بالتوحد،<br />
والتي سوف تخدم حاجة كبرية يف هذا املجال<br />
يف البالد.<br />
كل هذه املبادرات خطوات يف االتجاه<br />
الصحيح. ومن املهم للغاية أن تركز عملية<br />
تطوير الخدمات يف املستقبل عىل الطفل<br />
واألسرة، مع األخذ يف االعتبار أن األطفال<br />
واألسر هم قلب وروح الربامج الناجحة.<br />

لتشخيص<br />
التشخيص املبكر أمر مهم: هو الخطوة األوىل يف فهم<br />
نقاط القوة لدى الطفل واملجاالت التي تحتاج إىل تحسني.<br />
سيكون بإمكان األهل اتخاذ قرارات أفضل إذا توفرت لديهم<br />
معلومات أكرث. يجب عىل األهل الحصول عىل املساعدة يف<br />
أقرب وقت ممكن حني االشتباه ويجب عليهم أال ينتظروا<br />
ملعرفة ما إذا كان الطفل سيتخطى املشاكل. إن تعلم وفهم<br />
أكرب قدر ممكن عن اضطراب طيف التوحد يساعد األهل<br />
عىل أن يكونوا خرباء يف حالة أطفالهم والتعرف عىل خيارات<br />
العالج األكرث مالئمة.<br />
يبدأ العاج بالتقبل<br />
القبول هو الحل. األهل الذين يتعلمون مسببات الضغط<br />
النفيس يف طفلهم يمكنهم أيضاً اكتشاف أفضل الطرق<br />
لتقديم الدعم لهم والحفاظ عىل هدوئهم. معرفة املشاكل<br />
وحلها ومنع املواقف الصعبة عن طريق تجارب مستمرة<br />
والبحث عن حلول جديدة هي مفتاح الحل. إدراك احتياجات<br />
الطفل الحسية الفريدة يكشف لألهل عن مسببات السلوك<br />
املضطرب ويكون من السهل منعه. االحتفال بنجاح ونمو<br />
األطفال املصابني بالتوحد وأسرهم يدعمهم حقاً. ووضع<br />
خطة عالج فردية مع الخرباء واألطباء تدعم نمو الطفل<br />
السريع، ويمكن من التغلب عىل العديد من التحديات.<br />
الخطة الجيدة لعاج التوحد ينبغي أن :<br />
يحتاج تقييم اضطراب طيف التوحد اختصاصات<br />
متعددة وأن يكون شامالً ويتضمن مداخالت من<br />
األهل، والمدرسة، وطبيب األطفال مع المراقبة<br />
للطفل. خطة العالج تتمحور حول احتياجات ونقاط<br />
القوة في الطفل واألسرة. التدخالت الناجحة<br />
تتمركز حول الطفل واألسرة وعادة ما تشمل<br />
عالج النطق والعالج المهني والبرامج التعليمية<br />
المناسبة، والتدخالت السلوكية، واألهل ومقدمي<br />
الرعاية والدعم.<br />

تضمن كافة أشكال العالج: النطق واللغة، والعالج<br />
القائم عىل اللعب، والعالج السلويك، والعالج<br />
الطبيعي والعالج املهني.<br />
التواصل<br />
األطفال الذين ال يستطيعون التحدث أو ال يروق لهم ذلك<br />
ينبغي تشجيعهم بطرق غري لفظية للتواصل. تعابري الوجه<br />
ولغة الجسد واللمس جميعها وسائل مختلفة للتواصل.<br />
األهل ومقدمو الرعاية قد يحتاجون ملعرفة اللغة التي<br />
يتواصلون بها مع الطفل باستخدام األصوات والتعبريات.<br />
لتناسق<br />
إنشاء قواعد وإجراءات متسقة سوف يعزز التعلم – ويجب<br />
أن تطبق يف جميع بيئات الطفل. يجب عىل األهل معرفة<br />
ما يقوم به أعضاء فريق رعاية الطفل، بما يف ذلك املعلمني<br />
واملعالجني أو أفراد األسرة املمتدة. الرعاية الصحية والتعليم<br />
والخدمات االجتماعية كلها عوامل تساهم يف<br />
دعم نجاح الطفل.<br />
الروتن<br />
األفراد الذين يعانون من التوحد يفضلون القدرة عىل التنبؤ<br />
والروتني. إجبارهم عىل قبول التغيري أو التصرف تماماً مثل<br />
غريهم من الناس يمكن أن يمنع تطورهم ويمكن لألهل<br />
إساءة فهم السلوكيات وكأنها عصياناً لألوامر وتساعد عىل<br />
خلق وصمة اجتماعية مرتبطة بالرتبية األسرية السيئة. رفع<br />
مستوى الوعي العام هو املفتاح لدعم هؤالء األفراد الفريدين<br />
من نوعهم وأسرهم.<br />
البيئة اآلمنة والعناية الذاتية<br />
من األفضل أن يفسح اآلباء واألمهات املجال ملتطلبات الطفل<br />
من خالل خلق مكان آمن يف املنزل حيث يمكن للطفل أن<br />
يأخذ راحته. تعيني الحدود يساعد األطفال الذين يعانون من<br />
التوحد بالشعور باألمان والطمأنينة والقبول. وقد أظهرت<br />
الدراسات أنه بدون الدعم الكايف، فإن صحة مقدمي الرعاية<br />
يمكن أن تتأثر وهذا يؤثر بشكل كبري عىل جميع أفراد األسرة.<br />

اضطراب طيف التوحد هو تشخيص شاق ويجلب<br />
معه خشية من المستقبل المجهول. التشخيص في<br />
الوقت المناسب والدعم الكافي لألطفال المصابين<br />
بالتوحد وأسرهم أمر ضروري للحصول على أفضل<br />
النتائج العالجية.<br />

مستشفى ماونت سايناي ،Mount Sinai التي تأسست عام<br />
١٨٥٢، هي واحدة من أقدم وأكرب املستشفيات التعليمية يف<br />
الواليات املتحدة. ومشروعهم "مشروع استكشاف التوحد<br />
وصعوبات التعلم والوقاية منها" الذي دشن يف مايو عام<br />
٢٠١٠ للكشف عن األسباب البيئية للتوحد وصعوبات التعلم<br />
األخرى، وتحويل هذه االستكشافات العلمية إىل اسرتاتيجيات<br />
مبنية عىل أدلة للوقاية من التوحد. ورشة العمل األخرية كانت<br />
بعنوان "استكشاف األسباب البيئية للتوحد وصعوبات التعلم"<br />
وأصدرت الئحة أولية، بناء عىل عدد قليل من الدراسات، عن<br />
املواد الكيميائية األكرث استخداماً يف البيئة التي ربما تسبب<br />
التسمم النمايئ العصبي. developmental<br />
neurotoxicity<br />
ميثيل الزئبق Methylmercury<br />
األجنة تكون أكرث من ٥ إىل ١٠ مرات حساسية للتسمم<br />
من ميثيل الزئبق من البالغني. وغالباً ما توجد هذه املعادن<br />
يف األسماك. وتحتوي العديد من أسماك املحيط عىل<br />
مستويات عالية من السيلينيوم الذي يرتبط بميثيل الزئبق.<br />
وهذا يخلق مركبا ال يمكن للجسم امتصاصه. بعض أنواع<br />
األسماك تحتوي عىل نسبة عالية من هذه املادة السامة،<br />
مثل سمك السيف وسمك القشر، وينبغي تجنبها.<br />
مبيدات الفوسفات العضوية<br />
تتسبب املستويات العالية من هذه املادة السامة يف دم<br />
الحبل السري يف تشكيل العقول بشكل مختلف. لكن<br />
مدى هذا التأثري غري معروف، أما الذين يخططون إلنجاب<br />
األطفال فيجب عليهم الحد من تعرضهم للمبيدات<br />
الحشرية عن طريق اختيار األغذية العضوية قدر اإلمكان.<br />
عوادم السيارات<br />
الباحثون يكتشفون صلة بني تلوث الهواء ومجموعة<br />
واسعة من األمراض بما يف ذلك وجود صلة قوية بمرض<br />
السكري. وقد أبرزت بعض الدراسات خطر تلوث الهواء<br />
يف منطقة الشرق األوسط، ولكن املزيد من البحوث<br />
يجب أن تقدم لفهم التأثريات طويلة املدى وإيجاد حلول<br />
لها. وينبغي البحث عن الهواء النقي عند الشاطئ،<br />
والتقليل من الوقوف يف زحمة السري واستهالك الكثري<br />
من الخضروات الطازجة لدعم التخلص من السموم يف<br />

السكري. وقد أبرزت بعض الدراسات خطر تلوث الهواء يف<br />

تقدم لفهم التأثريات طويلة املدى وإيجاد حلول لها. وينبغي<br />
البحث عن الهواء النقي عند الشاطئ، والتقليل من<br />

مثبطات اللهب املربومة Brominated<br />
Flame Retardants<br />
تستخدم عىل نطاق واسع يف مجال اإللكرتونيات، واملالبس،<br />
واألثاث. إذا كنت ستشرتي أثاثا أو مالبس جديدة لينة،<br />
أتح الوقت للتهوية قبل استخدامها أو اطلب خيارات بدون<br />
املثبطات )بدون .)Scotchguard<br />
مركبات فلوريد العضوية Per-fluorinated<br />
)Compounds (PFCs<br />
يتعرض معظم الناس إىل تلك املركبات من خالل استخدام<br />
التفلون يف القاليات الغري الصقة. مواد التغليف املقاومة<br />
للشحوم، كعلب البيتزا وأكياس فشار امليكروويف، تستخدم<br />
أيضاً هذه املركبات. إنتاج واستخدام، والتخلص من هذه<br />
املادة السامة غري منظم ولكن يمكنك أن تحمي نفسك<br />
باستخدام أدوات املطبخ املصنوعة من السرياميك أو الفوالذ،<br />
وتجنب الوجبات السريعة – التي تفتقر للمواد الغذائية التي<br />
تساعد عىل حمايتك من الضرر!<br />

السموم البيئية<br />
ما الذي تستطيع القيام به؟<br />
األهايل القلقون والحوامل يجب عليهم اتخاذ اإلجراءات للحد<br />
من تعرضهم للسموم الضارة:<br />
الوعي العام أثناء التسوق يمكن أن يساعد – قراءة العالمات<br />
عىل األغذية دائماً.<br />
محاولة شراء الفواكه والخضروات العضوية وتجنب األطعمة<br />
املصنعة واملعلبة التي تمتئل بالكيماويات.<br />
استبدال علب الغداء وحاويات تخزين املواد الغذائية املصنوعة<br />
من البالستيك بتلك املصنوعة من الزجاج.<br />
عند شراء األلعاب لألطفال، اخرت األلعاب املعتمدة بخلوها من<br />
املركبات الضارة.<br />
احم نفسك وعائلتك من خال قراءة امللصقات، وتعلم أكر<br />
وكن مستهلكاً واعياً.<br />

البحث العلمي<br />
كشفت أحدث األبحاث عن األسباب املحتملة، مجموعة<br />
متنوعة من العوامل، معظمها تتعلق ببيئة الوالدة وعوامل<br />
الوراثة:<br />
يقول الباحثون من جامعة آرهوس Aarhus يف الدنمارك<br />
إن اإلنفلونزا أثناء الحمل تضاعف خطر اإلصابة بالتوحد،<br />
والحمى املستمرة ألكرث من أسبوع واحد تضاعف الخطر<br />
ثالثة أضعاف.<br />
ما الذي يسبب<br />
التوحد؟<br />
الكثير من الجدل العلمي يتمحور حول<br />
األسباب الواضحة للتوحد وزيادة عدد حاالته<br />
في قطر وحول العالم. وهنا سنحاول أن<br />
نتحقق من األسباب المحتملة وأحدث األخبار<br />
حول هذا االضطراب.<br />
كشفت دراسة حديثة نشرت يف مجلة التوحد واالضطرابات النمائية<br />
Journal of Autism and Developmental Disorders يف ٢٠١٥ أن<br />
التكاليف املرتبطة باضطراب طيف التوحد سوف تتضاعف خالل السنوات<br />
العشر املقبلة، وربما تصل إىل تريليون دوالر أمرييك يف الواليات املتحدة<br />
وحدها بحلول عام ٢٠٢٥. وكشفت الدراسة أن "التكاليف الحالية للتوحد<br />
هي أكرث من ضعف تكاليف التعامل مع السكتات الدماغية وارتفاع ضغط<br />
الدم وعىل قدم املساواة مع تكاليف مرض السكري."<br />
األسباب الجذرية الضطراب طيف التوحد معقدة بالتأكيد، ومن<br />
املحتمل جداً أن العديد من العوامل لها تأثري مشرتك. الوراثة تلعب دوراً<br />
يف تطور الدماغ ولكن األبحاث الجينية epigenetic research تدل<br />
بشكل واضح عىل أن البيئة املحيطة لها تأثري كبري وتؤثر فعالً<br />
عىل الجينات .DNA<br />
ليس هناك سبب واحد للتوحد، ويكشف العلماء عن مجموعة من<br />
العوامل التي تزيد من خطر اإلصابة باالضطراب. ويبدو أن الذكور أكرث<br />
عرضة من البنات بأربع مرات وتكون احتماالت اإلصابة متكافئة يف جميع<br />
األجناس والفئات االجتماعية.<br />
تتطور عقول أطفال التوحد بشكل مختلف. املواد الكيميائية املحددة يف<br />
الدماغ )الكرياتني، الكولني و )N-acetylaspartate تسبب املشاكل يف<br />
الخاليا العصبية وتمنعها من النضج بشكل غري طبيعي.<br />

استخدام املضادات الحيوية يزيد من مخاطر اإلصابة<br />
قليالً، عىل الرغم من أن ٩٨٪ ممن يستخدمون املضادات<br />
الحيوية أثناء الحمل يرزقون بأطفال أصحاء من<br />
دون توحد.<br />
سن الوالد يعترب عامالً، فكرب سن الرجال يمرر الطفرات<br />
الجينية أكرث من األمهات األكرب سناً.<br />
يشري معهد كاليفورنيا للتكنولوجيا أيضاً إىل الجهاز املناعي<br />
يف املرأة، املفرط يف النشاط، والذي يسبب سلوكيات<br />
مماثلة للتوحد لدى الفران املستخدمة يف االختبار.<br />
تعرض املرأة الحامل لألتربة وثاين أكسيد النيرتوجني يف<br />
التلوث قد يرتبط بالتوحد، بحسب تقرير جامعة جنوب<br />
كاليفورنيا ومستشفى األطفال يف لوس أنجلس.<br />
التعرض قبل الوالدة للسموم<br />
خالل مراحل معينة من النمو، يكون الجنني أكرث عرضة ألثر<br />
السموم البيئية. وقد تم ربط التعرض املبكر للمواد الكيميائية<br />
الضارة بالتوحد وتتواىل الدراسات بإثبات اآلثار الضارة لتعرض<br />
الجنني للسموم.<br />
املواد الكيميائية يمكن أن تؤثر عىل الجنني مباشرة أو من خالل<br />
الgenome عن طريق تغيري تعابري الحمض النووي )كيف<br />
"يقرأ" الحمض النووي وكيف يصنع الربوتني(، وهذا يسمى<br />
علم التخلق .Epigenetics يمكن لعوامل جينية أن يكون لها<br />
آثار إيجابية، مثل تناول حمض الفوليك لحماية تطوير النظام<br />
العصبي املركزي يف الطفل. يمكن أيضاً أن يكون لها تأثري سلبي،<br />
مثل تأثري السموم عىل التطور العصبي.
يتميز اضطراب طيف التوحد بصعوبات اجتماعية مبكرة وصعوبة يف التواصل، ويرتبط أيضاً باهتمامات محدودة والجمود<br />
السلويك. ويعترب اضطراب طيف التوحد من بني اضطرابات النمو األخرى ملرحلة الطفولة األقل وضوحاً. وهناك مجموعة<br />
متزايدة من البحوث املهتمة بالتوحد، مما يجعل األمر مهماً للغاية لألطباء وخصوصاً أطباء األطفال واألسرة، وعىل األطباء<br />

اضطراب طيف التوحد باستخدام فئتني مهمتني من<br />
األعراض، التواصل والسلوك هما :<br />
القصور املستمر يف التواصل<br />
والتفاعل االجتماعي، بما يف<br />
ذلك: تبادل محدود يف االهتمامات أو<br />
العواطف. قصور يف التواصل الغري لفظي. مشاكل<br />
يف التواصل بالعني ولغة الجسد. فهم محدود<br />
واستخدام اإليماءات. عدم وجود تعابري الوجه.<br />
مشاكل يف فهم وتطوير العالقات والحفاظ عليها .<br />
صعوبات يف اللعب الخيايل أو تكوين صداقات، غياب<br />
االهتمام باألقران.<br />
السلوكيات واملصالح أو األنشطة<br />
املحدودة واملتكررة، من بينها اثنان عىل<br />
األقل مما ييل: تكرار الحركات أو العبارات.<br />
اإلصرار عىل التماثل. االلتزام الغري مرن للروتني.<br />
نمط شكلية السلوك. صعوبات يف التأقلم. نمط<br />
التفكري املتجمد. اهتمامات مركزة بشكل غري طبيعي.<br />
تفاعل غري عادي مع البيئة املحيطة )الالمباالة لأللم<br />
أو درجة الحرارة، االستجابة السلبية لألصوات أو<br />
ملواد محددة، الشم املفرط أو ملس األشياء، االنبهار<br />
باألضواء أو الحركة.(<br />
لي تعترب األعراض مشخصة<br />
للتوحد، يجب أن تكون:<br />
حاضرة يف فرتة التطور املبكر )ولكن قد ال<br />
تصبح ظاهرة بشكل كامل حتى وقت الحق(<br />
تسبب ضعفاً ملحوظاً يف األداء<br />
االجتماعي أو املهني.<br />
ال تعزى إىل حالة أخرى طبية أو عصبية<br />
)مثل اإلعاقة الذهنية، تأخر يف النمو، أو<br />
اضطراب عقيل آخر(.<br />
أعراض التوحد<br />
تظهر العالمات الشائعة لهذا االضطراب يف مرحلة الطفولة املبكرة، عادة قبل سن<br />
الثالثة. وتظهر بعض األعراض عادة يف األطفال ولكن باختالف كبري يف حدتها:<br />
تخفيف تواصل العني مع الناس املعروفني للطفل.<br />
√ انخفاض رد الفعل يف تعابري الوجه، كرد االبتسامة.<br />
√ انخفاض استجابة متبادلة للعواطف.<br />
√ زيادة يف كمية التكرار.<br />
√ انخفاض يف الرد عىل اسمهم أو عىل األصوات املألوفة.<br />
√ انخفاض باستخدام اإليماءات، كالتأشري.<br />
√ استخدام األصوات لجذب االهتمام أو استخدام أصوات األطفال.<br />
√ تأخر كبري يف اللغة.<br />
√ صعوبة يف طلب املساعدة.<br />
√ زيادة الحساسية لألصوات، والضوء واللمس.<br />
يجب عىل اآلباء القلقني طلب املشورة الطبية فوراً عند مالحظتهم أن<br />
أطفالهم يظهرون عدة أعراض أو عالمات من هذه القائمة، ألن الكشف املبكر<br />
يحسن نتائج العالج.<br />
أطفال التوحد يتطورون بطريقة مختلفة، حيث تجد أن بعضهم ينمي<br />
املعرفة والحركة واللغة واملهارات االجتماعية أكرث من غريهم. وتكون طريقة<br />
التعلم والحفظ غري متناسقة وغري متوقعة ألن االستيعاب والتفاعل غالباً ما<br />
يكون صعباً.<br />
يف الحاالت املعتدلة من التوحد، يمكن النظر إليها كحماقة اجتماعية ألنه<br />
يؤثر عىل االتصال اللفظي والغري لفظي. األعراض الحادة تجعلهم يبدون<br />
وكأنهم غري مهتمني باآلخرين، ومنعزلني يف عاملهم الخاص. قد يفتقر األطفال<br />
ملهارات اللعب واملحادثة، أو يبدون وكأنهم عنيدون أو مهووسون باهتمامات<br />
معينة. قد يكرر أطفال التوحد الكلمات أو عبارات معينة. قد يكون التعاطف<br />
عندهم أضعف أو أقوى من غريهم، مما يصعب التواصل االجتماعي. عند الحد<br />
األدىن للطيف الوظيفي، قد ال يكون الطفل قادراً عىل التحدث عىل اإلطالق.<br />
قد تكون هناك قضايا حسية، تؤثر عىل تفاعل األطفال من سماع األصوات<br />
العالية، والروائح، وكذلك االختالفات يف اإلضاءة أو درجة الحرارة. التغريات<br />
يف بيئتهم املحيطة، مثل سماع األصوات العالية، والروائح، واألضواء، يمكن<br />
أن تسبب حمال حسيا زائداً وتسبب اضطرابات يف العاطفة، وخاصة إذا كانت<br />

التوحد؟<br />
من الصعب جداً تعريف التوحد وفهمه. وال<br />
تزال البحوث العلمية تحاول فهم هذه الحالة<br />
المعقدة. لكن مصطلح التوحد نفسه واسع<br />
ويشمل أعراضاً مختلفة، تتراوح من المعتدلة<br />
إلى الحادة، لهذا تُسمى تلك األعراض بشكل<br />

إضافة إىل قانونية عملهم بشكل رسمي وطبقا للقوانني املعتمدة بالدولة<br />

خلوهم من كافة األمراض املعدية، مع إثبات حصولهم عىل التطعيمات<br />
الضرورية ضد تلك األمراض من السلطات الصحية املختصة فقط..<br />
الضوابط الصحية الواجب اتباعها لتنظيف وتعقيم<br />
خزانات مياه الشرب يف املنازل:<br />
نظراً ألهمية سالمة خزانات مياه الشرب بصفة دائمة وحرصاً عىل الصحة العامة<br />
فإنه يتم تنظيف خزانات مياه الشرب عند استخدام الخزان ألول مرة )الخزانات<br />
الجديدة(. وعند تلوث الخزان أو ركود املياه نتيجة عدم االستخدام لفرتات متباعدة<br />
أو بعد العودة من اإلجازات، كما بينت املؤسسة الطريقة املثىل للتنظيف، وطالبت<br />
بالتأكد من سالمة العوامة املثبتة يف الخزان.<br />
مالحظات:<br />
• يجب عىل العاملني يف هذا الحقل االلتزام التام<br />
باستخدام وسائل السالمة الذاتية وعدم<br />
استخدام أية أدوات كهربائية أو كيميائية )غري<br />
مركبات الكلور( أو غازات أثناء عملهم<br />
• االمتناع عن االكل والشرب أثناء العمل<br />
• عدم السماح لألطفال باالقرتاب من أدوات<br />

للمحافظة على مياه الشرب<br />
آمنة وصالحة لالستخدام<br />
تتمثل رسالة املؤسسة العامة القطرية للكهرباء واملاء "كهرماء" يف توفري خدمة<br />
الكهرباء واملاء للمشرتكني بجودة عالية، وتعمل املؤسسة يف هذا السياق عىل<br />
تحقيق أهدافها املتمثلة يف الوفاء بالتزامها نحو توفري احتياجات دولة قطر<br />
من الكهرباء واملاء بدرجة عالية من الكفاءة وااللتزام باملعايري املحلية والدولية<br />
الخاصة بالصحة والسالمة والبيئة. وتحرص كهرماء، يف إطار مسؤوليتها<br />
تجاه املجتمع عىل توعية جمهور املشرتكني بأفضل الطرق الصحية للمحافظة<br />
عىل مياه الشرب آمنة وصالحة لالستخدام، كما تدعو كهرماء جمهور<br />
مشرتكيها إىل توفري خزان مياه يكفي حاجة السكان ملدة 48 ساعة عىل األقل،<br />
وهي إما أرضية أو علوية.<br />
كما أن هناك عدد من أنواع الخزانات املنتشرة يف دولة قطر،<br />
منها: خزانات األلياف الزجاجية، خزانات الصاج، خزانات<br />
البالستيك )الروتامولد(، الخزانات الخرسانية، ويجب أن<br />
تتوفر يف تلك الخزانات اشرتاطات صحية عامة وهي:<br />
» أن تكون املادة املصنوع منها الخزان مطابقة ملواصفات<br />
هيئة املواصفات واملقاييس القطرية.<br />
» أن يثبت الخزان عىل أرضية مرتفعة عن سطح االرض.<br />
وأن يكون موقعه نظيفاً وبعيداً عن مصادر التلوث.<br />
ويف مكان يسهل الوصول إليه عند الصيانة والنظافة.<br />
» أن يكون الخزان محكم الغلق ملنع دخول األتربة<br />
وامللوثات األخرى.<br />
» أن يحتوي الخزان عىل صنبور إلزالة الرواسب عند<br />
غسله وتعقيمه.<br />
» أن تكون أرضية وجدران الخزان الداخلية والخارجية<br />
ملساء وسهلة التنظيف.<br />
» أن يكون الخزان معزوالً عزالً مائياً كامالً<br />
وكذلك عزالً حرارياً.<br />
» توفر عوامة للتحكم يف كمية مياه الخزان.<br />
» أن توضع مصفيات )فلرت( عىل املاء الوارد إىل الخزان.<br />
» الخزانات املصنوعة من الحديد يجب أن تطىل<br />
بمواد مانعة للصدأ عالية الجودة وخالية من<br />
املركبات السامة )مرخص باستخدامها<br />
رسميا للمواد الغذائية( وأال تؤثر علىخواص املاء أو<br />
صحة املستهلك.<br />
» تنظف الخزانات وتطهر من مرتني إىل ثالث<br />
مرات يف العام.<br />
» يجرى كشف دوري عىل الخزانات واملنطقة التي توجد<br />
فيها للتأكد من سالمتها إنشائياً وصحياً.<br />
» يتم ملء الخزانات بمياه من الشبكة العامة أو من<br />
مصدر معروف تثبت صالحيته لالستعمال اآلدمي بناءً<br />

نصف سكان العالم سيكونون قصريي النظر بحلول عام ٢٠٥٠، وفقاً لدراسة نشرت<br />
يف مجلة طب العيون journal Ophthalmology يف فرباير ٢٠١٦. هذه الزيادة<br />
الكبرية قد تعزى إىل عوامل بيئية. قضاء وقت أقل يف الهواء الطلق واملزيد من الوقت<br />

الباركنسون – ٢٠١٦ أمل جديد<br />
للمرضى؟<br />
الحادي عشر من أبريل من كل عام هو اليوم العاملي ملرض الباركنسون،<br />
الذي يهدف إىل رفع الوعي وتعزيز فهم العالم لهذه الحالة العصبية<br />
وكيفية تأثريها عىل الذين يعانون منها.<br />
يحدث مرض الباركنسون بسبب تلف خاليا الدماغ التي تولد الدوبامني<br />
،dopamine والتي بدورها تؤثر عىل وظيفة الحركة يف الجسم. وتتميز<br />
املراحل املبكرة من املرض عموماً بأعراض مرتبطة بحركة الجسم مثل<br />
الهز، والصعوبة يف املي وصالبة الجسم يف الحركة. والحقاً، األعراض<br />
النفسية والسلوكية تصبح واضحة، مع<br />
اإلكتئاب وهو أكرث األعراض النفسية شيوعاً<br />
وغالباً ما يتطور للخرف يف املراحل املتقدمة<br />
من املرض.<br />
تقدر منظمة الصحة العاملية أن<br />
حاالت مرض الباركنسون ستضاعف<br />
أكرث بمرتني بحلول عام ٢٠٣٠، ولكن<br />
هناك أمل. علماء من شركة أدوية<br />
سويدية BioArctic املختصة بعلم<br />
األعصاب، بتعاون مع جامعة أوبساال<br />
Uppsala وغريها من الجامعات السويدية<br />
ومعهد كارولينسكا ،Karolinska طوروا مؤخراً<br />
فحص العالمات البيولوجية )اإلجراء التحلييل( لهذا املرض.<br />
وتستخدم املؤشرات الحيوية Biomarkers عىل نحو متزايد يف الطب<br />
للتشخيص ويف تحديد املآالت. البحث قد يؤدي إىل تشخيص مسبق<br />
وأسرع وعالجات أكرث فعالية. لوجه.<br />
ما صلة فيروس زيكا بعيب خلقي؟<br />
فريوس زيكا هو مرض جديد نسبياً ينقله البعوض. أول مرة أصيب به البشر عام<br />
١٩٥٢. ينتقل عن طريق بعوض أيديس، ويسبب الحمى، والطفح الجلدي، والتهاب<br />
امللتحمة، وآالم يف العضالت، والغثيان، والصداع. تكون األعراض عادة خفيفة<br />
وتستمر حتى سبعة أيام، ولكن يف اآلونة األخرية، إقرتح وجود صلة بني الفريوس<br />
وعيوب الوالدة، وال سيما صغر الرأس، وإضطراب النمو العصبي.<br />
تؤكد دراسة نشرت يف مجلة النسيت لألمراض املعدية Lancet Infectious<br />
Diseases يف ١٧ فرباير أن فريوس زيكا يمكن أن يعرب حاجز املشيمة وربما يسبب<br />
صغر الرأس، عىل الرغم من أن الخرباء يحثون عىل توخي الحذر، مشريين إىل أنه<br />
عىل الرغم من أن األدلة تقوي الناحية النظرية، فإن الصلة لم تثبت بالتأكيد. تقول<br />
الدكتورة آنا دي فيليبيس، املؤلف الرئييس للدراسة، "هذه الدراسة ال يمكنها تحديد<br />

ما هي األمراض االنتقالية األكر<br />
انتشارا يف الباد؟<br />
نحن نتلقى بانتظام إشعارات حول 67<br />
مرضاً انتقالياً يتوجب اإلبالغ عنه، ونقوم<br />
بمتابعة التوجهات العاملية السائدة لهذه<br />
األمراض. باإلضافة إىل أنه لدينا نظام كشف<br />
وفحص عىل مداخل الدولة. كما نشجع<br />
عىل التطعيم ونتبع آليات الكشف املبكر عىل<br />
بعض األمراض مثل التهاب الكبد الوبايئ ممّا<br />
جعل حاالت االصابة بهذا املرض منخفضة<br />
يف دولة قطر، حسب آخر الدراسات امليدانية.<br />
ولقد سجلنا حاالت ملرض السلّ أكرث من<br />
مرض االيدز يف الدولة وتبقى جميعها أمراض<br />
قليلة االنتشار لدينا مقارنة بالدول األخرى.<br />

أظهرت نتائج املسح العاملي الستهالك<br />
التبغ يف قطر الذي قمنا بتنفيذه سنة<br />
2014 بالتعاون مع منظمة الصحة العاملية<br />
واملجلس األعىل للتعليم ووزارة التخطيط<br />
التنموي واإلحصاء أن حوايل 16% يستهلكون<br />
التبغ بأنواعه املختلفة وهي نسبة قليلة<br />
مقارنة مع دول العالم. كما أظهرت النتائج<br />
أن النسبة أعىل لدى الذكور. ونحن نعمل<br />
عىل التصدي لهذه الظاهرة من خالل تعزيز<br />
الحمالت التوعية ومنع أي إعالنات ترويجية<br />
ملنتجات التبغ يف دولة قطر وتكثيف نشاط<br />
مفتي الوزارة وتغريم املخالفني والعمل عىل<br />
رفع أسعار منتجات التبغ من خالل فرض<br />
دفع الضرائب والزيادة فيها. وال يفوتني أن<br />
أتقدّ م باسم الوزارة بالشكر إىل قيادات كتارا<br />
التخاذهم قرار حظر التدخني يف مرافق الحي<br />
الثقايف ونشجع القطاعات األخرى التي تعمل<br />
عىل زيادة الوعي بالصحة واالهتمام<br />
باملظهر العام.<br />
التطعيم من وسائل الوقاية الناجحة<br />
فما هو وضع التطعيم يف الدولة وما هو<br />
الجديد يف هذا املجال؟<br />
التطعيم أحد أنجح وسائل الوقاية الصحية<br />
وأكرثها اقتصادًا يف التكلفة ودولة قطر<br />
رائدة وسبّاقة يف هذا املجال حيث يتمّ تقديم<br />
مختلف التطعيمات مجانا للجميع. كما<br />
تمّ إعداد جدول للتطعيم بما يتوافق مع<br />
توجهات منظمة الصحة العاملية. وتمّ اتخاذ<br />
إجراءات صارمة بحيث ال يقبل األطفال<br />
يف املدارس بدون دفرت التطعيم. ولقد تمّ<br />
دمج 6 تطعيمات يف إبرة واحدة للتخفيف<br />
عىل املواطنني. كما أنّ هناك نظام الكرتوين<br />
للتذكري بمواعيد تطعيم األطفال.<br />
وباملناسبة فإنّ هناك توجه عام لتحويل<br />
جميع برامج الصحة إىل نظام الكرتوين<br />
ذيك وموحد للتنسيق بني جميع الجهات<br />
املعنية لربط املعلومات واملزيد من التنسيق<br />
بخصوص اإلنذار املبكر إزاء أي طارئ أو انتشار<br />
ألي مرض ال قدّ ر الله<br />
ختاما، ما هي تطلعاتكم لرؤية قطر<br />
ومستقبل قطر الصحي؟<br />
نعمل حاليا عىل إعداد اسرتاتيجية الصحة<br />
العامة 2022-2017 ونعمل عىل توفري املوارد<br />
الالزمة لها وإطالقها وتنفيذها ومتابعة<br />
إنجازها كما نطمح إىل تقديم خدمات صحية<br />
وقائية وعالجية متميزة وشاملة ومتكاملة<br />
بما يتماىش مع احتياجنا وبما يشهده املجال<br />
من تطور واستعداد ألي طارئ تماشيا مع<br />
رؤية الوزارة ومع رؤية قطر 2030.<br />

بداية نريد تعريف القراء عمّ ا تحقّق<br />
بخصوص الصحة الوقائية يف قطر؟<br />
الرعاية الصحية الوقائية هي ثالث أهداف<br />
االسرتاتيجية الوطنية للصحة <strong>2016</strong>-2011<br />
وتأيت مباشرة بعد أهداف العمل عىل إرساء<br />
نظام رعاية صحية شامل ومتكامل. وهي<br />
تمثل االنتقال من الرتكيز عىل عالج األمراض<br />
إىل الرتكيز عىل الوقاية منها. وتشتمل عىل<br />
عدة مشاريع من بينها التغذية والنشاط<br />
البدين، اإلقالع عن التدخني، األمراض<br />
السارية، الصحة املهنية، السالمة املرورية،<br />
الصحة البيئية... ولقد تمّ تنفيذ حوايل 80%<br />
من هذه املشاريع ونعمل عىل تحقيق املزيد من<br />
اإلنجازات املخطط لها.<br />
ال شك أنّ العمل عىل أرض الواقع يظهر<br />
صعوبات وتحديات، فما هي أكرب التحديات<br />
التي تواجهكم يف مجاالت الصحة العامة؟<br />
من أكرب التحديات التي نواجهها ونتعامل<br />
معها هي كيفية وضع الصحة العامة - والتي<br />
هي علم الوقاية من األمراض - يف مكانها<br />
الصحيح وإبراز أهميتها وأهمية االستثمار<br />
فيها. وعىل سبيل املثال فإنّ الوفيات<br />
واإلصابات الناتجة عن الحوادث املرورية يف<br />
قطر خاصة بني صفوف الشباب تمثّل خسارة<br />
يف اإلنتاج تتكبدها املجموعة الوطنية عن كل<br />
شاب قتيل أو مصاب. وتفيد الدراسات لدينا<br />
أن إصابة من ضمن عشر إصابات تؤدي إىل<br />
الوفاة. ولذلك عملنا إىل جانب شركائنا يف<br />
الدولة عىل الجانب الوقايئ إلرساء ثقافة<br />
الوقاية من خالل حمالت التوعية بأهمية<br />
السالمة املرورية ووضع القوانني املرورية<br />
املالئمة. وهكذا فإنّ الدراسات يف الدولة تشري<br />
حاليا إىل أنّ حوادث املرور ال تزيد ولكن يجب<br />
أيضا ان نالحظ أنها ال تقلّ وهذا يتطلب منّا<br />
املزيد من العمل الجماعي والدؤوب.<br />
ومن بني التحديات أيضا التي نواجهها،<br />
األمراض غري السارية املعروفة أيضا باألمراض<br />
املزمنة مثل مرض السكري ومرض ارتفاع<br />
ضغط الدم والتي تؤثر عىل مختلف املستويات<br />
الصحية واالجتماعية واالقتصادية وهي مكلفة<br />
للدولة وتمثل عبئا متعدّد األوجه. وهذا ما<br />
جعلنا نفكر مع شركائنا يف القطاعني العام<br />
والخاص يف املزيد من تحسني نظامنا الصحي<br />
ومواصلة عملية التغيري التي بدأناها من أجل<br />
االستجابة ملتطلبات الوقاية والعالج. ويف هذا<br />
٧٠٪ من الوفيات في<br />
قطر سببها األمراض<br />
المزمنة وهي نسبة عالية<br />
اإلطار نحن نسعى إىل دمج الصحة يف جميع<br />
السياسات بالتنسيق مع زمالئنا يف مختلف<br />
الجهات املعنية بالدولة. وهو خيار منظمة<br />
الصحة العاملية والذي من شأنه تحقيق مردود<br />
إيجابي عىل الصحة وعىل جودة الحياة.<br />
تتصدى األجهزة الطبية وبرامجها املختلفة<br />
لكافة أنواع األمراض. ولكن هل تمثل<br />
األمراض املزمنة يف قطر خطورة أكرب من<br />
األمراض االنتقالية؟<br />
نحن نبذل جهودا حثيثة ومتواصلة للقضاء<br />
عىل األمراض االنتقالية أو السارية مثل<br />
التهاب الكبد الوبايئ واإليدز والسل وغري ذلك<br />
من األمراض. وللعلم فإن بعض األمراض<br />
االنتقالية القاتلة مثل "اإليبوال" ال توجد يف<br />
قطر. وتتمثّل مجهودات وزارة الصحة العامة<br />
يف اتخاذ اإلجراءات الوقائية الناجحة وعمل<br />
التطعيم اإلجباري الفعّ ال والذي وصل إىل<br />
نسبة %95.ما يحد من انتشار األمراض<br />
يف الدولة.<br />
أمّا بالنسبة إىل األمراض املزمنة مثل أمراض<br />
القلب والسرطان والسكري فإنّ وزارة الصحة<br />
العامة تواصل العمل عىل تفعيل آليات<br />
الكشف املبكر لهذه األمراض. ولذلك فإنّ<br />
نسبة اإلصابة بمرض السكري أصبح أقلّ من<br />
السابق. ونعمل اآلن عىل الحمالت التوعوية<br />
للكشف املبكر عىل مرض السرطان حيث كلّما<br />
كان اكتشاف اإلصابة مبكرا كلّما ارتفعت نسبة<br />
الشفاء. وهو ما سوف يجعل املرىض يتقبلون<br />
أكرث العالج داخل قطر ويستغنون عن<br />
العالج بالخارج.

القراء الاعزاء،<br />
إن اليوم العالمي للتوعية بالتوحد يصادف الثاني من أبريل – الذي<br />
كان اقتراحاً من دولة قطر، ووافقت عليه جميع الدول في الأمم<br />
المتحدة. وفي هذا العدد الخاص، نحتفل بهذه المبادرة المهمة<br />
بمقابلة مع الرئيس التنفيذي في مركز الشفلح للأشخاص ذوي<br />
الإعاقة، وأخصائيين في مجال التوحد ومتلازمة داون، وأهالي<br />
أطفال التوحد الذين كسروا حاجز انعزالهم ليعيشوا الحياة كما<br />
نعيشها نحن.<br />
كما تابعنا أخبار الصحة العامة في قطر مع مدير إدارة الصحة<br />
العامة وتعرفنا على أهم الإنجازات والتحديات لإدارة الصحة في<br />
الدولة. ولدينا آخر الأخبار عن فيروس الزيكا المنتشر حالياً في<br />
العالم وآخر ما في الأبحاث عن مرض الباركنسون.<br />
وبما أننا نطمح لتقديم أحدث الأخبار عن الصحة في قطر، يسعدنا<br />
أن نقدم لكم في هذا العدد بعض المقالات التحريرية من الجهات<br />
الرسمية حول الصحة والرفاهية.<br />
كما أن الصحة النفسية مهمة، وكذلك الصحة البدنية. وقد كان شهر<br />
مارس مكرساً للتوعية بالكلى ووظائفها وأهميتها للإنسان. نسلط<br />
الضوء على أمراض الكلى والوقاية منها في قطر مع المختص<br />
الدكتور حسن المالكي.<br />
وطبعاً، لا يكتمل العدد من غير إستكشاف نواحي اللياقة البدنية،<br />
فلدينا عدة نصائح لتحسين الحيوية والمحافظة على الوزن الصحي.<br />
