Empfehlung <strong>der</strong> Kommission für Kr<strong>an</strong>kenhaushygiene und Infektionsprävention <strong>bei</strong>m Robert Koch-Institut (RKI)<br />
Recommendation of the Commission for Hospital <strong>Hygiene</strong> <strong>an</strong>d Infection Prevention at the Robert Koch Institute (RKI)<br />
den. Vor Probenahme ist eine hygienische Händedesinfektion<br />
durchzuführen. Nach Probenahme k<strong>an</strong>n eine<br />
erneute <strong>Auf</strong>bereitung <strong>der</strong> untersuchten Endoskope<br />
erfor<strong>der</strong>lich sein (z.B. Nachspülung und Trocknung <strong>der</strong><br />
K<strong>an</strong>äle). Die Vorgaben des untersuchenden Labors<br />
sind zu beachten.<br />
Durchspülung von Endoskopk<strong>an</strong>älen:<br />
• Durchspülflüssigkeit: 20 ml pro K<strong>an</strong>al, steril in geeignetem<br />
Laborgefäß auff<strong>an</strong>gen.<br />
• Von den zugänglichen K<strong>an</strong>älen (Instrumentierk<strong>an</strong>al<br />
und Luft/Wasser-K<strong>an</strong>al) ist <strong>die</strong> Untersuchung mindestens<br />
eines K<strong>an</strong>als obligat durchzuführen, wo<strong>bei</strong><br />
eine Untersuchung des Instrumentierk<strong>an</strong>als bevorzugt<br />
werden sollte.<br />
• Eine Untersuchung des Luft/Wasser-K<strong>an</strong>als wird<br />
empfohlen, ggf. parallel zu <strong>der</strong> obligaten Untersuchung<br />
einer aus dem Optikspülsystem (Flasche und<br />
Anschlussschlauch, vorbereitet wie zur Patientenuntersuchung)<br />
entnommenen Flüssigkeitsprobe.<br />
• Eine Untersuchung des Absaugk<strong>an</strong>als mit Ansaugung<br />
<strong>der</strong> Durchspülflüssigkeit in ein am Gerätestecker<br />
zwischengeschaltetes Tracheal-Absaugset ist<br />
optional.<br />
• Flüssigkeitsproben sind zügig ins Labor zu tr<strong>an</strong>sportieren<br />
und zu verar<strong>bei</strong>ten. Ist ein längerer Tr<strong>an</strong>sport<br />
zu erwarten, wird Kühlung <strong>der</strong> Proben empfohlen.<br />
Abstriche von hinsichtlich <strong>der</strong> <strong>Auf</strong>bereitung kritischen<br />
Endoskopstellen:<br />
• Sterilen Tupfer mit physiologischer NaCl-Lösung<br />
befeuchten.<br />
• Oberfläche des zu untersuchenden Bereichs abstreichen.<br />
• Tupfer in geeignetes Medium geben, zügig ins Labor<br />
tr<strong>an</strong>sportieren und verar<strong>bei</strong>ten.<br />
Zur Durchführung <strong>der</strong> Endoskopprüfungen sind nur<br />
Labore mit hygienischmikrobiologischer Erfahrung zu<br />
beauftragen, <strong>die</strong> <strong>die</strong> Befunde bewerten und Beratung<br />
<strong>bei</strong> Mängeln durchführen.<br />
Hygienisch-mikrobiologische <strong>Anfor<strong>der</strong>ungen</strong> <strong>an</strong><br />
Endoskopprüfungen (Bewertung <strong>der</strong> Untersuchungsergebnisse)<br />
• Kein Nachweis von Escherichia coli, <strong>an</strong><strong>der</strong>en Enterobacteriaceae<br />
o<strong>der</strong> Enterokokken als Indikatoren für<br />
m<strong>an</strong>gelhafte Reinigung o<strong>der</strong> Desinfektion.<br />
• Kein Nachweis von Pseudomonas aeruginosa, <strong>an</strong><strong>der</strong>en<br />
Pseudomonaden o<strong>der</strong> Nonfermentern als Indikatoren<br />
für m<strong>an</strong>gelhafte Schlussspülung o<strong>der</strong> Trocknung.<br />
• Kein Nachweis von hygienerelev<strong>an</strong>ten Erregern wie<br />
Staphylococcus aureus als Indikatoren für z. B.<br />
eine Endoskopkontamination nach <strong>Auf</strong>bereitung <strong>bei</strong><br />
m<strong>an</strong>gelhafter Lagerung o<strong>der</strong> unzureichen<strong>der</strong> Händehygiene<br />
des Personals.<br />
• Bei Endoskopen, <strong>die</strong> zu Untersuchungen in mikrobiell<br />
nicht besiedelten Bereichen des oberen Gastrointestinaltraktes<br />
o<strong>der</strong> Respirationstraktes verwendet<br />
werden (z. B. Bronchoskope, Seitblickduodenosbe<br />
disinfected hygienically. After sampling, renewed<br />
reprocessing of the examined endoscopes might be<br />
necessary (e.g. rerinsing <strong>an</strong>d drying ch<strong>an</strong>nels). The<br />
specifications of the examining laboratory must be followed.<br />
Flushing Endoscope Ch<strong>an</strong>nels:<br />
• Flushing liquid: 20 ml per ch<strong>an</strong>nel, recovered in a<br />
sterile m<strong>an</strong>ner in <strong>an</strong> adequate laboratory flask.<br />
• At least one ch<strong>an</strong>nel of all accessible ch<strong>an</strong>nels (instrument<br />
ch<strong>an</strong>nel <strong>an</strong>d air/water ch<strong>an</strong>nel) must be examined<br />
at all costs, however, preference should be given<br />
to the examination of the instrument ch<strong>an</strong>nel.<br />
• An examination of the air/water ch<strong>an</strong>nel is recommended,<br />
where appropriate, alongside the compulsory<br />
examination of a liquid sample taken from<br />
the lens rinsing system (bottle <strong>an</strong>d connecting hose,<br />
prepared in the same way as for patient examinations).<br />
• The suction ch<strong>an</strong>nel c<strong>an</strong> optionally be examined by<br />
sucking flushing liquid into a tracheal suction set<br />
which is interlinked at the connector.<br />
• Liquid samples must be tr<strong>an</strong>sported quickly to the<br />
laboratory <strong>an</strong>d processed. Cooling the samples is<br />
recommended if longer tr<strong>an</strong>sportation time is to be<br />
expected.<br />
Swabs of endoscope points which are critical with<br />
regard to reprocessing:<br />
• Moisten sterile swab with physiological NaCl-solution.<br />
• Take a swab of the surface of the area to be examined.<br />
• Place swab in adequate medium, tr<strong>an</strong>sport to the<br />
laboratory promptly <strong>an</strong>d process.<br />
Only laboratories which have hygienic-microbiological<br />
experience that evaluate the findings <strong>an</strong>d act as a consult<strong>an</strong>t<br />
in case of deficiencies must be commissioned<br />
with endoscope tests.<br />
Hygienic-Microbiological Requirements of Endoscope<br />
Tests (Evaluation of Test Results)<br />
• No detection of Escherichia coli, other enterobacteriaceae<br />
or enterococci as indicators for insufficient<br />
cle<strong>an</strong>ing or disinfection.<br />
• No detection of Pseudomonas aeruginosa, other<br />
pseudomonads or nonfermenters as indicators for<br />
insufficient final rinsing or drying.<br />
• No detection of hygiene-relev<strong>an</strong>t pathogens, such<br />
as Staphylococcus aureus, as indicators for, for<br />
example, of contamination of the endoscope after<br />
reprocessing in case of inadequate storage or inadequate<br />
h<strong>an</strong>d hygiene of staff.<br />
• No detection of virid<strong>an</strong>s streptococci as <strong>an</strong> indicator<br />
for contaminations with pharyngeal flora in case of<br />
endoscopes, which are used for examining microbially<br />
uncontaminated areas of the upper gastrointestinal<br />
tract or respiratory tract (e.g. bronchoscopes or<br />
