by 2 independent researchers from 01/01/<strong>2020</strong>–01/08/<strong>2020</strong> to identify <strong>al</strong>l publications on point-of-care <strong>ultrasound</strong> in COVID-19 adult patients,<br />

using English language restriction, and the following MeSH query: ((“lung” AND “<strong>ultrasound</strong>”) OR “echocardiography” OR “Focused cardiac<br />

<strong>ultrasound</strong>” OR “point-of-care <strong>ultrasound</strong>” OR “venous <strong>ultrasound</strong>”) AND (“COVID-19” OR “SARS-CoV2”). Non-pertinent findings were discarded. The<br />

references of relevant papers were hand-searched <strong>for</strong> missed papers. Duplicates were removed. An addition<strong>al</strong> search of pre-print publications was<br />

made through ResearchGate, preprint online repositories and soci<strong>al</strong> medias<br />

[26]. Similar results have been reported in undifferentiated<br />

shock [46]. PoCUS is recommended as a first-line<br />

diagnostic test <strong>for</strong> investigating respiratory failure and/<br />

or hypotension [22, 47]. PoCUS may raise suspicions of<br />

f<strong>al</strong>sely negative RT-PCR and/or <strong>al</strong>ternate diagnoses [48].<br />

Recognition of comorbidities (chronic RV or LV dysfunction)<br />

and COVID-19-associated complications (DVT and<br />

RV failure) may influence patient disposition, and PoCUS<br />

can change their management [40].<br />

We present a conceptu<strong>al</strong> framework <strong>for</strong> triage of respiratory<br />

failure [Addition<strong>al</strong> file 7]. Without more data,<br />

triage protocols cannot be developed that are univers<strong>al</strong>ly<br />

applicable.<br />

Recommendations<br />

1 We suggest using PoCUS, and especi<strong>al</strong>ly LUS (presence<br />

of h<strong>et</strong>erogeneous B-line clusters, pleur<strong>al</strong> line<br />

irregularities, subpleur<strong>al</strong> consolidations), and appropriately<br />

integrate the in<strong>for</strong>mation with clinic<strong>al</strong> assessment<br />

to diagnose COVID-19 pneumonia (LQE II-B,<br />

Very Good Agreement).
