a) After 30 days and within one year the
domiciliary Births on payment of
prescribed for the Rs.10/- getting
permission of the chief Registrar subject
to receive requisition and others papers
from parents as follows.
i) Affidavit
ii) Reporting Form No.5 of the
respective Primary Health Center.
iii) Birth Form No.1 (available in the
office)
b) After one year the domiciliary Births
registered as per order of magistrate of
the I class or Presidency magistrate with
the payment of prescribed fee of Rs.20/-
subject to receive the following:
i) Requisition Letter
ii) Count order
iii) Reporting Form No.5 of the
respective Primary Health Center.
Not necessary. The duty of the medical officer In-change
of Institutions to give information to the Registration
within 21 days of the events occurred.
Subject to availability of correct particulars in the
application and availability of records the birth
certificates will be issued as detailed below:
